외측웨지인솔이 대퇴절단자의 무릎내전모멘트에 미치는 영향 (Biomechanical Effect on Knee Adduction Moment by Lateral Wedge Insole in Transfemoral Amputee)

  • The prevalence of knee osteoarthritis was higher people with lower limb amputation. This was identified that transfemoral amputees have a greater external knee adduction moment than ablebodied subjects by biomechanical studies. Therefore, they need rehabilitative intervention for prevention and reduction of knee osteoarthritis. The purpose of this study was to determine the effect of lateral wedge insole used in the treatment of knee osteoarthritis. This study was participated in fourteen unilateral transfemoral amputees and we were analyzed the difference gait variables between without lateral wedge insole and with $5^{\circ}$ and $10^{\circ}$ lateral wedge insole during gait. Our results showed that step length ratio was more symmetrical and, hip adduction and ankle inversion angle were more close to normal value, and knee adduction moment was decreased as the wedge angle increases. We proposed that these data would be utilized conservative treatment of knee osteoarthritis in lower limb amputees.

회외족의 Wedge Insole 각에 따른 보행 시 접지 시간, 접지면적 및 족저압력의 비교 (The Effect of the Wedge Insole Angle of Supinated Group on Foot Contact Time, Foot Contact Area and Foot Pressure)

  • This study was conducted on male college students with supinated foot to measure the foot pressure by having them wear three kinds of wedge insoles ($0^{\circ}$, $3.5^{\circ}$, $7^{\circ}$). Foot contact time, foot contact area, peak pressure and mean pressure were measured using a foot pressure distribution measuring instrument. And the surface of the foot sole was divided into 10 areas. Regarding foot contact time, there was no statistically significant difference by showing $0.69{\pm}0.004$ seconds at $3.5^{\circ}$ and $0.68{\pm}0.006$ seconds at $0^{\circ}$ and $7^{\circ}$. Regarding the foot contact area, it appeared broad in the inside area of the foot according to wedge insole, and there was statistically significant difference in the area 1 of the rear foot(p< .01) and the area 3 of the middle foot(p< .05). The peak pressure by foot area decreased in the outside of the foot according to wedge insole, while increasing in the inside of the foot. Among the areas, there was statistically significant in the area 2 of the rear foot (p< .01) and the area 3 of the middle foot (p< .05). Regarding the mean pressure by foot area, the pressure roughly increased in the inside area of the foot according to wedge insole, while decreasing in the outside of the foot.

트레드밀 걷기 동안 인솔형태가 하퇴 근육의 근 활성에 미치는 영향 (The Effects of the Insole Types on Lower Leg Muscle Activity during Treadmill Walking)

  • Purpose : We investigated the effects of the insole types on lower leg muscle activation during treadmill walking. The three insole types investigated for this study were normal insole, medial wedge insole, and viscoheel. Methods : Participants were assigned into three groups. People with foot transformation were excluded from this study. Each participant walked for ten minutes. The first day we applied a normal insole. On the second day, a medial wedge insole was applied. Finally, on the last day a viscoheel was applied. After walking on a treadmill for ten minutes, we measured muscle activation in lower leg muscles (gastrocnemius and tibialis anterior). Surface electromyography (EMG) was used to measure muscle activity. The data were analyzed using one-way analysis of variance (ANOVA) with repeated measures to determine the statistical significance. Results : The results of this study were summarized as follows. While walking on the treadmill, root mean square (RMS) values of the gastrocnemius when the viscoheel was applied were significantly lower than the other insole types. There was no significant difference for the RMS values for the tibialis anterior using viscoheel. The normal insole and viscoheel insole were significantly different in a post hoc analysis. However, there was no significant difference for normal insole and medial wedge insole. Conclusion : Using a viscoheel insole decreases muscle activity of the lower leg. Therefore, in conclusion, the viscoheel insole type reduces the load on the lower leg during walking.

키높이 인솔두께에 따른 족부의 생체역학적 특성변화에 대한 연구 (A Study on Changes in Biomechanical Characteristics of the Foot with Respect to Wedge-type Insole Thickness)

  • Recently, functional insoles of wedge-type it is for the young to raise their height inserted between insole and heel cause foot pain and disease. Additionally, these have a problem with stability and excessively load-bearing during gait like high-heel shoes. In this study, we compared the changes in biomechanical characteristics of foot with different insole thickness then we will utilize for the development of the insole with the purpose of relieving the pain and disease. Subjects(male, n = 6) measured COP(center of pressure) and PCP(peak contact pressure) on the treadmill(140cm/s) using F-scan system and different insole thickness(0~50 mm) between sole and plantar surface during gait. Also, we computed changes of stresses at the foot using finite element model with various insole thickness during toe-off phase. COP moved anterior and medial direction and, PCP was increased at medial forefoot surface, $1^{st}$ and $2^{nd}$ metatarsophalangeal, ($9%{\uparrow}$) with thicker insoles and it was show sensitive increment as the insole thickness was increased from 40 mm to 50 mm. Change of the stress at the soft-tissue of plantar surface, $1^{st}$ metatarsal head represents rapid growth($36%{\uparrow}$). Also, lateral moments were increased over the 100% near the $1^{st}$ metatarsal as the insole thickness was increased from 0 mm to 30 mm. And it is show sensitive increment as the insole thickness changed 10 mm to 20 mm. As a result, it was expected that use of excessively thick insoles might cause unwanted foot pain at the forefoot region. Therefore, insole thickness under 30 mm was selected.

Effects of Combined Wedge on Angle and Moment of Ankle and Knee Joint During Gait in Patients With Genu Varus

  • The purpose of this study was to investigate the effects of combined wedge on the range of motion in ankle and knee joint, ankle eversion moment and knee adduction moment, and center of pressure excursion of foot for genu varus among adult men during gait. This study was carried out with 10 adult men for genu varus in a motion analysis laboratory in J university. The subjects of the experiment were measured above 5cm width between the knees on contact of both medial malleolus of ankle while standing. The width of their knees in neutral position was measured without the inversion or eversion of the subtalar joint by the investigator. The subjects of the experiment were ten who were conducted randomly for standard insole, insole with $10^{\circ}$ lateral on rear foot wedge, insole at $10^{\circ}$lateral on rear foot and $5^{\circ}$ medial on fore foot wedge. Before and after intervention, changes on the range of motion in ankle and knee joint, ankle eversion moment and knee adduction moment, and center of pressure excursion were measured. In order to compare analyses among groups; repeated one-way ANOVA and $Scheff{\acute{e}}$ post hoc test were used. As a result, combined wedge group was significantly decreased compared to control wedge group in terms of knee varus angle in mid-stance(p<.05). Combined wedge group was significantly decreased compared to lateral wedge group in terms of ankle eversion moment in whole stance(p<.05). Combined wedge group was significantly decreased compared to lateral wedge group in terms of knee adduction moment in whole stance(p<.05). Combined wedge group was significantly decreased compared to lateral wedge in terms of center of pressure excursion in whole stance(p<.05). The results of this study suggest that combined wedge for genu varus decreased ankle eversion moment and knee adduction moment upon center of pressure excursion. We hypothesize that combined wedge may also be effective in the protection excessive ankle pronation.

밀착성 외측 쐐기 스트랩 깔창이 내반슬 골관절염 환자의 대퇴경골각에 미치는 영향 (The Effects of Femorotibial Angle of a Contact Lateral-Wedged Insole with Strapping in Patients with Varus Deformity Osteoarthritis of the Knee)

  • The purpose are to assess the efficacy of a contact lateral-wedged insole with strapping on the femorotibial angle in patients with varus deformity osteoarthritis of the knee after treatment period. 25 outpatients with knee osteoarthritis (OA) were randomized to be treated with a contact lateral-wedge insole with strapping. Standing radiographs were used to analyze the femorotibial angle for each subject. The result of repeated two-way ANOVA's reveled that a contact lateral-wedged insole with strapping produced significantly differences in the femorotibial angle between groups after treatment period (P<0.05). and repeated one-way ANOVA's reveled that it produced significantly differences in the femorotibial angle between experimental groups after treatment period (P<0.05). We suggest that these results may be beneficial for manufacturing foot orthotic devices, such as wedged insoles, to control medial and later compartment forces in the knee varus-valgus deformity.

내반슬 골관절염 환자에 대한 밀착형 외측 쐐기 스트랩 발받침이 한국형 WOMAC에 미치는 영향에 관한 연구 (Study on Effect of KWOMAC of a Contact Lateral-Wedged Insole with Strapping in Patients with Varus Deformity Osteoarthritis of the Knee)

  • The purpose are to demonstrate the pain, stiffness, and physical function by the Korean Western Ontario MacMaster University(KWOMAC) score of the patients with varus deformity osteoarthritis of the knee after treatment period. Twenty outpatients with knee osteoarthritis(OA) were randomized and divided into the control(n=10) and wedge(n=10) groups. The remission scores of the KWOMAC index of severity for knee OA were compared between the groups. The result of repeated two-way ANOVA's revealed that a contact lateral-wedged insole with strapping produced significantly differences in the pain and physical function score between groups after treatment period(P<0.05), but it was not significant differences in the stiffness score between groups(P>0.05). These results suggest that the pain relief and improvement in function reported by patients with osteoarthritis while using lateral-wedged insole with strapping may be achieved by a reduction in external varus moment and medial compartment load.
