The Domestic Trends of Osteoarthritis-related Research on Each Constituent Herb of ChondroT - Focused on the Journal of Korean Medicine (ChondroT 구성약물의 골관절염 관련 국내 연구 동향 - 한의학 학술지를 중심으로)

  • Objectives The purpose of this study is to analyze the osteoarthritis-related effect of each constituent herb of ChondroT, the herbal medicine that used for osteoarthritis. Methods We searched osteoarthritis-related studies on each constituent herb of ChondroT via five Korean Medicine web databases. We classified these studies by each constituent herb (Ostericii Radix, Angelica Gigas Nakai, Clematidis Radix, Lonicerae Flos, Phellodendri Cortex) and researched the osteoarthritis-related effect. Results We collected 18 studies (two studies of Ostericii Radix, five studies of Angelica Gigas Nakai, three studies of Clematidis Radix, six studies of Lonicerae Flos, and two studies of Phellodendri Cortex). All of these studies were composed of in vivo and in vitro, and there were six pharmacopuncture experimental study. All experiments showed the significant effects on specific inflammatory mediators. Conclusions These results suggest that ChondroT will be able to apply to the treatment of osteoarthritis through continuous research and development.

  • Web-based Enterprise Intranet System is developed Object-oriented programming language and data management is constructed using RDBMS. Between Object-oriented programming language and RDBMS bring about "Object-Relational Impedance Mismatch" due to heterogeneous paradigm. To solve this kinds of problems commonly use the ORM Framework that it is converting data between incompatible type systems in databases and object-oriented programming languages, uses complex mapping metadata. It causes difficult to develop and maintain because of inflexible in changes. This paper proposed persistence framework that solve the existing complexity of ORM framework and has more flexible in evolution of database table. This persistence framework is unnecessary meta information that connecting with entity of database table and the objects, it offers users convenience of maintenance and it allows far more flexible and affordable systems to be built because of automatically code generation. The result of testing based on the proposed persistence framework with Hibernate, iBATIS, It is similar response time with iBATIS and it has more about three times faster than Hibernate. But iBATIS has problems of mass data processing.

The Domestic Trends of Traditional Korean Medicine Treatments on Frozen Shoulder (동결견의 한의학적 치료에 대한 국내 연구 동향)

  • Objectives : The purpose of this review is to investigate the trends of traditional korean medicine treatments on frozen shoulder. Methods : We investigated the studies on traditional korean medicine treatments for frozen shoulder via searching 6 Korean web databases. As a result, 32 research papers were found to be analyzed according to their published year, the titles of journals, published institution, the types of study, the number of cases, the types of treatments, the instruments for assessment and ethical approvals. Results : 32 papers were published since 1989. The studies on traditional korean medicine treatments about frozen shoulder were mainly published in The journal of korean acupuncture & moxibustion medicine. 10 case series, 7 reviews, 6 non-randomized controlled trials, 5 randomized controlled trials and 4 case reports had been under research. In most of the research, the number of the cases were not enough. In 15 cases of the studies, various traditional korean medicine treatments were used to treat the symptoms. In 8 cases of the studies, one method was used to treat the symptoms. Range of motion(ROM) and visual analogue scale(VAS) were used as primary assessments. Among the 25 clinical studies, 5 of them were accepted by institutional review board(IRB). Conclusion : In this study, we analyzed the trends of traditional korean medicine treatments on frozen shoulder. Reviewing the domestic trends of studies on traditional korean medicine treatments for frozen shoulder and examining the strong and weak points of those treatments are essential for the future studies.

Correlation between the Head Forward Posture and the Site of Herniation of Single Level Cervical Intervertebral Disc (두부 전방 정도와 경추 추간판 탈출증 발생 부위의 상관성 분석)

  • Objectives : The present study examines the domestic trend of Chuna treatments on lumbar spinal disorder in Korea. Methods : We investigated the studies on Chuna treatments for lumbar spinal disorder via searching 10 Korean web databases. As a result, 63 research papers were found to be analyzed according to their published year, the titles of journals, the types of study, the techniques of Chuna, the instruments for assessment, the Chuna technique and the number of the treatment trials by the cases of lumbar spinal disorder and ethical approvals. Results : The number of the research papers published tends to increase every year. The studies on Chuna treatments were mainly published in The Journal of Korea CHUNA Manual Medicine for Spine & Nerves. The most frequently adopted technique of Chuna in the examined studies was Cox flexion & distraction technique. Visual analogue scale(VAS), oswestry disability questionnaire(ODI) were used as primary means of assessments. The ethical problems of the examined studies needed to be improved. Conclusion : Reviewing the domestic trend of studies on Chuna treatments for lumbar spinal disorder and examining the strong and weak points of those treatments are essential for the future studies. It is anticipated that this review benefits the future in-depth study on the treatments for Chuna in Korean medicine.

Daoyin Exercise Therapy for Low Back Pain : A Systematic Review (요통에 대한 도인운동요법의 효과 : 체계적 문헌 고찰)

  • Objectives: To determine the evidence supporting the effectiveness and safety of Daoyin exercise therapy for treating low back pain. Methods: We searched 11 electronic databases(Pubmed, Web of Science, EMBASE, Cochrane Library, CAJ, KISTI, KISS, NDSL, KMBASE, RISS, and DBpia) and two related journals,till April 2018 for randomized controlled trials(RCTs) related to testDaoyin exercise therapy for low back pain. Results: The inclusion criteria were fulfilled by eight RCTs. The meta-analysis of the eight studies showed positive results when Daoyin exercise therapy was used for treating low back pain. Conclusions: Based on the meta-analysis, we found indications for the favorable impact of Daoyin exercise therapy on the treatment of low back pain. However, our systematic review has limited evidence to support this theory because of the low quality of original articles. Hence, additional well-designed RCTs should be encouraged.

Effects of Lumbar Mobilization for Lower Limb Strength in Healthy Individuals: A Protocol for Systematic Review and Meta-Analysis

  • Background: The effect of mobilization on lumbar back pain has been fully described in several clinical aspects, but evidence for muscle strength would be still less clear. Objective: To assess the effect of lumbar mobilization on lower limb strength in healthy individuals. Methods and Analysis: Healthy people aged 18-65 will be included regardless of race or sex. Original peer-reviewed primary reporting randomized controlled trials (RCTs) will be included. Electronic databases, such as MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, Web of Science, Pedro, CINAHL, ClinicalTrials.gov will be searched from inception until July 30. Only studies published in English will be included in this review. Two reviewers will complete the screening for eligibility independently, and the other two reviewers will also complete the risks of data extraction and bias assessment independently. Lower Limb strength will be assessed as primary outcome, and particular intervention or participant characteristics will be assessed as the secondary outcomes. Meta-analysis will be conducted using Review Manager 5.3.3, and evidence level will be assessed using the method for Grading of Recommendations Assessment, Development and Evaluation. Outcomes will be presented as the weighted mean difference or standardized mean difference with 95% CI. If I2 ≤ 50%, P>.1, the fixed effect model will be used, otherwise, random-effects model will be used. Ethics and dissemination: This review might not be necessary ethical approval because it does not require individual patient's data; these findings will be published in conference presentations or peer-reviewed journal articles. Development of a Novel Subunit Vaccine Targeting Fusobacterium nucleatum FomA Porin Based on In Silico Analysis

  • Selecting an appropriate antigen with optimal immunogenicity and physicochemical properties is a pivotal factor to develop a protein based subunit vaccine. Despite rapid progress in modern molecular cloning and recombinant protein technology, there remains a huge challenge for purifying and using protein antigens rich in hydrophobic domains, such as membrane associated proteins. To overcome current limitations using hydrophobic proteins as vaccine antigens, we adopted in silico analyses which included bioinformatic prediction and sequence-based protein 3D structure modeling, to develop a novel periodontitis subunit vaccine against the outer membrane protein FomA of Fusobacterium nucleatum. To generate an optimal antigen candidate, we predicted hydrophilicity and B cell epitope parameter by querying to web-based databases, and designed a truncated FomA (tFomA) candidate with better solubility and preserved B cell epitopes. The truncated recombinant protein was engineered to expose epitopes on the surface through simulating amino acid sequence-based 3D folding in aqueous environment. The recombinant tFomA was further expressed and purified, and its immunological properties were evaluated. In the mice intranasal vaccination study, tFomA significantly induced antigen-specific IgG and sIgA responses in both systemic and oral-mucosal compartments, respectively. Our results testify that intelligent in silico designing of antigens provide amenable vaccine epitopes from hard-to-manufacture hydrophobic domain rich microbial antigens.

  • Data mining refers to a set of techniques for discovering implicit and useful knowledge from large database. Many studies on data mining have been pursued and some of them have involved issues of temporal data mining for discovering knowledge from temporal database, such as sequential pattern, similar time sequence, cyclic and temporal association rules, etc. However, all of the works treat problems for discovering temporal pattern from data which are stamped with time points and do not consider problems for discovering knowledge from temporal interval data. For example, there are many examples of temporal interval data that it can discover useful knowledge from. These include patient histories, purchaser histories, web log, and so on. Allen introduces relationships between intervals and operators for reasoning about relations between intervals. We present a new data mining technique that can discover temporal relation rules in temporal interval data by using the Allen's theory. In this paper, we present two new algorithms for discovering algorithm for generating temporal relation rules, discovers rules from temporal interval data. This technique can discover more useful knowledge in compared with conventional data mining techniques.

An Efficient Technique for Evaluating Queries with Multiple Regular Path Expressions (다중 정규 경로 질의 처리를 위한 효율적 기법)

  • As XML has become an emerging standard for information exchange on the World Wide Web, it has gained attention in database communities to extract information from XML seen as a database model. XML queries are based on regular path queries, which find objects reachable by given regular expressions. To answer many kinds of user queries, it is necessary to evaluate queries that have multiple regular path expressions. However, previous work such as query rewriting and query optimization in the frame work of semistructured data has dealt with a single regular expression. For queries that have multiple regular expressions we suggest a two phase optimizing technique: 1. query rewriting using views by finding the mappings from the view's body to the query's body and 2. for rewritten queries, evaluating each query conjunct and combining them. We show that our rewriting algorithm is sound and our query evaluation technique is more efficient than the previous work on optimizing semistructured queries.

Influence of autoclave sterilization procedures on the cyclic fatigue resistance of heat-treated nickel-titanium instruments: a systematic review

  • Objectives: This systematic review evaluated the influence of autoclave sterilization procedures on the cyclic fatigue resistance of heat-treated nickel-titanium (NiTi) instruments. Materials and Methods: A systematic search without restrictions was conducted in the following electronic databases: PubMed, Scopus, Web of Science, ScienceDirect, Cochrane, and Open Grey. The hand search was also performed in the main endodontic journals. The eligible studies were submitted to the methodological assessment and data extraction. Results: From 203 abstracts, a total of 10 articles matched the eligible criteria. After reading the full articles, 2 were excluded because of the absence of the heat-treated instruments in the experimental design and 3 due to the lack of a control group using heat-treated instruments without autoclave sterilization. From the 5 included studies, 1 presented a low risk of bias, 3 presented moderate and 1 high risk. It was observed heterogeneous findings in the included studies, with autoclave sterilization cycles increasing, decreasing or not affecting the cyclic fatigue life of heat-treated NiTi instruments. However, the retrieved studies evaluating the cyclic fatigue resistance of endodontic instruments presented different protocols and assessing outcomes, this variability makes the findings less comparable within and also between groups and preclude the establishment of an unbiased scientific evidence base. Conclusions: Considering the little scientific evidence and considerable risk of bias, it is still possible to conclude that autoclave sterilization procedures appear to influence the cyclic fatigue resistance of heat-treated NiTi instruments.
