정유 및 석유화학플랜트 중대사고 전조신호 평가지표 개발에 관한 연구 (A Study on the Development of Assessment Index for Catastrophic Incident Warning Sign at Refinery and Pertrochemical Plants)

  • 정유 또는 석유화학플랜트에서 폭발 등과 중대사고가 발생할 때 심각한 인명 및 재산피해를 야기시켜 보험시장에 큰 영향을 끼쳐왔다. 일반적으로 정유, 석유화학공장 등 장치산업에서 이러한 사고 발생시 국립과학수사연구원, 사고원인 조사자, 손해보험사 손해사정사 등이 조사하여 손해 및 사고와 가장 인접한 근인 위주로 사고원인을 도출하고 있다. 반면 실제 중대사고로 이어지기까지 문제 및 결함으로 작용한 여러 중대사고 전조신호에 대한 근본원인분석을 실시하여 예방대책을 수립하는 것이 중요하나 그 동안이에 대한 것이 미흡하였다. 이에 본 연구에서는 전 세계에서 발생된 중대사고 사례에 대하여 근본원인분석 방법과 스위스치즈모델 원리를 활용한 기여요소분석법 등을 통하여 도출하였던 미국 화학공정안전센터의 중대사고 전조신호 자체평가 도구의 전조신호 판단기준 항목을 우선적으로 고찰하였다. 여기에 실제 정유 및 석유화학플랜트 내 중대사고 전조신호가 해외재보험사 Loss control engineer 등 Auditor 들에게 어떠한 식으로 권고되어 왔는지 확인하고자 지난 17년간 Loss Control Engineer가 Risk Survey 이후 도출하였던 안전권고사항 약 614개를 분석하였다. 최종적으로 이를 중대사고 전조신호 평가지표로 개발이 용이하도록 정유 및 석유화학플랜트에서의 중대사고 전조신호 판단기준을 유형별로 그룹화한 후 상위 및 하위 항목으로 구분하였다. 또한, 정유 및 석유화학공장 관련 전문가(40명)에게 설문 실시 및 AHP기법을 적용하여 각 항목별 가중치(중요도)를 도출하여 최종 전조신호 판단기준과 항목별 가중치가 적용된 '정유 및 석유화학플랜트에서의 중대사고 전조신호 평가지표'를 개발하였다. 그리고 개발한 지표를 8개의 정유 및 석유화학플랜트에 적용하여 분석하였다. 본 연구에서 개발된 평가지표는 정유 및 석유화학공장 등 장치산업에서의 중대사고 예방을 위하여 추적관리 되어야 할 전조신호 항목 및 요소가 무엇인지 인식하고 취약수준을 평가하는데 도움이 될 수 있고, 관련 사업장 자체 관계자뿐만 아니라 외부 auditor들에게 유용하게 활용되리라 판단된다.

A Study on the Disaster Prevention Design- Based Safety Signs in School Zone

  • The number of accidents in school zone is decreasing than before with the introduction of strengthening traffic safety policy since January 2011, but the danger still exists. The School zone sign is widely known to have much effect in protecting children from risks of traffic accidents, but design improvement is being demanded to improve a sense of safety and legibility of safety signs in School zone due to the lack of understanding on the safety signs in crosswalk and School zone. This study analyzed differences in shape and color of existing safety signs through a case analysis of traffic developed countries as America, England, Japan, and Germany and suggested improvement plans for drivers to clearly perceive the school zone. For improvement methods, this study suggested the importance of delivering definite and unified warning message for school zone to drivers by using indication sign and caution sign together, and to use yellow, a safety color, and to unify the safety sign into triangle shape that symbolizes warning and caution to conform the international standards. Actual design production and experiment through improvement plans are needed in the future, and it is expected to secure safety of children and to provide international standardization of safety signs in school zone.

지역사회 교육홍보사업이 지역주민의 뇌졸중 경고증상 및 골든타임 인지도에 미치는 효과 (Effects of community-based education and advocacy intervention on public awareness about the warning signs of stroke and the golden window of time)

  • Objectives: This study evaluated the effects and relevant factors of community-based education and advocacy intervention on stroke awareness among the general population. Methods: The study enrolled 505 community-dwelling adults aged 19 years or older who completed a survey before and after a community-based intervention held between May 2014 and December 2014. Trained staff collected data about knowledge of five stroke warning signs, emergency response, golden window of time (${\leq}3h$), and demographics. Results: After the intervention, public awareness of all five stroke warning signs increased significantly from 26.5% to 33.9% (p=0.011). Public awareness of the golden window of time also increased significantly from 70.1% in April 2014 to 75.8% in December 2014 (p=0.040). In a multivariate logistic regression analysis, older age, higher education level, and exposure to stroke-related public service announcements or educational materials were significant predictors of knowledge of all five warning signs for stroke. Conclusions: Comprehensive community-based intervention is effective for improving public awareness of the warning signs of stroke and the golden window of time.

IPA를 활용한 제품사용설명서의 작성원칙에 대한 소비자 중요도-만족도 분석 (Consumer Importance-Performance Analysis on Writing Rules of Product User Guide Using IPA)

  • Purpose: This study examined the importance and satisfaction of consumer's perception of the information delivery format of products and services required and recommended by KS A ISO / IEC Guide 13: 2003 and KS A ISO / IEC Guide 37: 2003. Methods: In this study, the importance and satisfaction of clarity, principles of information transfer, how to use expressions and technical terms, and technical terms, warning signs, and durability were investigated. Results: The t-test analysis of the importance and satisfaction of the Writing Rules of the product user guide showed that all items had statistically significant differences. The importance of the writing rules of the product user guide for the use of the product was in the order of warning signs (4.423), how to use expressions and technical terms (4.215), principles of information transfer (4.163), clarity (3.914), and finally durability (3.851). Conclusion: Among the 'explanation,' 'instruction' and 'warning' functions of the Product User Guide, the consumer uses the manual to identify the inherent dangers and misuse of the product. Besides, since the life of the product is prolonged, the Product User Guide also needs to improve the quality of the cover or paper so that it can use for a long time.

Domestic Violence in the Canadian Workplace: Are Coworkers Aware?

  • Background: Domestic violence (DV) is associated with serious consequences for victims, children, and families, and even national economies. An emerging literature demonstrates that DV also has a negative impact on workers and workplaces. Less is known about the extent to which people are aware of coworkers' experiences of DV. Methods: Using data from a pan-Canadian sample of 8,429 men and women, we examine: (1) awareness of coworker DV victimization and perpetration; (2) the warning signs of DV victimization and perpetration recognized by workers; (3) whether DV victims are more likely than nonvictims to recognize DV and its warning signs in the workplace; and (4) the impacts of DV that workers perceive on victims'/perpetrators' ability to work. Results: Nearly 40% of participants believed they had recognized a DV victim and/or perpetrator in the workplace and many reported recognizing more than one warning sign. DV victims were significantly more likely to report recognizing victims and perpetrators in the workplace, and recognized more DV warning signs. Among participants who believed they knew a coworker who had experienced DV, 49.5% thought the DV had affected their coworker's ability to work. For those who knew a coworker perpetrating DV, 37.9% thought their coworker's ability to work was affected by the abusive behavior. Conclusion: Our findings have implications for a coordinated workplace response to DV. Further research is urgently needed to examine how best to address DV in the workplace and improve outcomes for victims, perpetrators, and their coworkers.

A Study on Process Safety Incident Precursors to Prevent Major Process Safety Incidents in the Yeosu Chemical Complex

  • Process safety incidents and loss events can be prevented if we identify and adequately take measures on process safety incident precursors in a timely manner. If we look into and take action against the process safety hazard factors causing the incident in the refinery and petrochemical plant, major process safety incidents can be prevented through eliminating or decreasing hazardous factors. We conducted a survey for the major process safety incident precursor to look specifically into the potential process safety hazardous factors of refineries and petrochemical plants in the Yeosu chemical complex. A self-assessment checklist, which was published by Center for Chemical Process Safety "Recognizing catastrophic incident warning signs in the process industry" on major incidents warning sign, was used for the survey. Through this survey, the major process safety incident leading indicators in the process industry were found by process safety management elements, and each site and/or facility can use these leading indicators for activities for process safety incident prevention. In addition, we proposed action items required to eliminate the root cause of those process safety incident leading indicators.

Production of Fear: The Visual Analysis of Local Lockdown Warning Signs

  • During the Covid-19 pandemic's first term of April-June 2020, the general public throughout Indonesia became familiar with the slang term "local lockdown." This term emerged in response to disorderly implementation of the half-hearted government policy called Pembatasan Sosial Berskala Besar (PSBB). In villages around the country, people started to build portals to restrict "strangers" or "outsiders" from entering their village areas. These portals were also meant to publicly signal the villagers' fear of the spread of the virus. This paper will discuss two things: first, how fear was produced, using frameworks drawn from Giorgio Agamben's notable works State of Exception and Homo Sacer, and how governance reproduces it; and second, how people come to accept the state of emergency and then publicly express their acceptance of the situation. Critical discourse analysis is applied to read government policy and its reception. The research took place at Rempoah, Kedungmalang, and Pabuwaran villages in Banyumas, the southern regency of Central Java, Indonesia. The villagers' responses to the government's policy are visually represented through written warning signs.

  • Objectives : The objective of this study was to assess whether there were sex differences in stroke types, risk factors, warning signs, and stroke complications among patients with first-ever stroke. Methods : Six-hundred seventy six patients with first-ever stroke were recruited at the Department Cardiovascular and Neurologic Diseases (Stroke center) of KyungHee University Oriental Hospital, DongGuk University International Hospital, and Kyungwon University In-cheon Oriental Medical Hospital from September 2005 to June 2007. Patients were hospitalized within 28 days after the onset of stroke. We investigated their stroke types, ischemic stroke subtypes by TOAST classification, risk factors, warning signs, stroke complications, general characteristics such as age, sex, etc. Results : Overall, 347 patients were male and 279 female. Compared with males, female patients were significantly older (mean age 67.3${\pm}$1.1 versus 62.4${\pm}$1.6 years) (P=0.000). We did not find significant sex differences in stroke types or ischemic stroke subtypes by TOAST classification. History of hypertension was significantly more frequent in female than male patients (P=0.000). Among stroke complications, urinary tract infection was significantly more frequent in female than male patients (P=0.003). Among warning signs, blepharospasm was significantly more frequent in female than male patients (P=0.006). Conclusions : Knowledge of sex differences of stroke patients can help us gain better insights on the characteristics of stroke patients. We need further and larger scale research to acquire more concrete conclusions on this theme.

안검경련(眼瞼痙攣)을 중풍 전조증상이라 할 수 있는가? (Does a Blepharospasm mean the Presymptom of Stroke?)

  • Objectives : The purposes of this study were to investigate that a blepharospasm means the presymptom of stroke based on the traditional hypothesis in the oriental medicine and to compare a blepharospasm with warning signs of stroke in the western medicine. Methods : In the time period Oct. 2005 to Oct. 2006, 409 patients with a first-ever stroke admitted in the department of Internal Medicine of Kyungwon University In-cheon Oriental Medical Hospital, Kyunghee University Oriental Medical Hospital, Dongguk Il-san Oriental Medical Hospital were included. Patients were hospitalized within 14 days after the onset of stroke. Stroke patients were interviewed by residents who studied standard operation procedures in Fundamental Study for Standardization and Objectification of Differentiation and Pattern Identification of Syndrome of Oriental Medicine for Stroke. A questionnaire was completed by a question-and-answer form between patients and residents after explanation details to patients and the agreement of patients. Results : Age, care of patients or grandson, diabetes mellitus, and smoking were higher among cerebral infarction group, while smoking was higher among hemorrhage group. Female, young age, hyperlipidemia, hypertension in the family history and fat body in waist-hip ratio were higher among patients undergoing the blepharospasm before stroke onset. And the incidence of blepharospasm was lower in patients who dislike the fast food. Finally, when we compared a blepharospasm with warning signs of stroke in the western medicine, the incidence of blepharospasm in this study were the most frequent. Conclusion : In this study, the incidence of blepharospasm in patients before stroke onset was more frequent than that of warning signs in the western medicine. But more data from prospective cohort studies should be collected to be accepted that the blepharospasm is the presymptom of stroke as like warning signs in the western medicine.

Knowledge about Cervical Cancer Early Warning Signs and Symptoms, Risk Factors and Vaccination among Students at a Medical School in Al-Ahsa, Kingdom of Saudi Arabia

  • Background: Cervical cancer is the second most common cancer among females and also the most preventable. In the literature there is abundant evidence that awareness regarding cervical cancer and its prevention is low in the developing countries. Medical students are the future health professionals and can play an important role in increasing awareness among the general population. To assess the knowledge regarding symptoms, risk factors and prevention of cervical carcinoma among medical students in th Kingdom of Saudi Arabia, the present study was planned. Materials and Methods: This cross-sectional study was conducted using a self-administered questionnaire with students at the College of Medicine, King Faisal University, Al-Ahsa, KSA, from December 2012 to May 2013. Results: The responses of 188 students (males 111, females 77) in their second, third, fourth, and fifth years were recorded and used in the data analysis. The majority of the students were not aware of the early warning signs, symptoms and risk factors. On average, only 43.7% males and 56% of females were aware about the early signs and symptoms whereas 51.4% males and 57.8% females had knowledge about the risk factors of cervical cancers. Some 55% males and 46.8% females were unable to select the correct answer regarding human papilloma virus (HPV) infection as the cause of cervical cancer. Majority of the students (67%) were not aware about the availability of vaccine against HPV. Conclusions: Lack of knowledge regarding early signs and symptoms, risk factors and prevention of cervical cancer was observed in the present study.
