Relationships among Serum Adiponectin, Leptin and Vitamin D Concentrations and the Metabolic Syndrome in Farmers (농업인의 혈중 아디포넥틴, 렙틴 및 비타민 D 농도와 대사증후군 간의 관련성)

  • The purpose of this study was to investigate the relationships among serum adiponectin, leptin and vitamin D concentrations and the metabolic syndrome in Korean farmers. 105 (26 males, 79 females) farmers (39~78 years, mean age $59.4{\pm}9.6$ years) in Gangwon - area were included in this study. Anthropometric measurements and biochemical blood analysis of subjects were carried out. The prevalence of obesity, abdominal obesity, hypertension, diabetes, hypertriglyceridemia, hypercholesterolemia and hyper LDL-cholesterolemia, metabolic syndrome were 51.9%, 65.7%, 49.5%, 15.3%, 17.3%, 13.5%, 11.5% and 40.9%, respectively. Serum adiponectin and leptin levels ($8.90{\mu}g/ml$ and 12.6 ng/ml) of females were significantly higher than those ($6.49{\mu}g/ml$ and 4.88 ng/ml) of males. But there was no significant difference in 25(OH)vitamin D concentration between males (15.4 ng/ml) and females (16.9 ng/ml). In the subjects with metabolic syndrome, the adiponectin levels were significantly lower and leptin levels were significantly higher than those of the subjects without metabolic syndrome. Serum adiponectin level had positive correlations with HDL-cholesterol level (r = 0.325, p < 0.001), but showed negative correlations with triglyceride and fasting blood glucose concentrations, body weight and waist/hip circumference ratio (r = -0.202 ~ -0.317, p < 0.05). Serum leptin and 25(OH)vitamin D concentrations were positively correlated with body fat (kg, %) and BMI, waist and hip circumferences (r = 0.244 ~ 0.682, p < 0.001). The results of this study suggested that adiponectin and leptin levels could be credible indices to predict chronic diseases in farmers. However, further research on vitamin D should be carried out considering another factors.

  • The summarized findings resulted from experiments and investigation are suggested as follows ; The analysis of measurements on the lower limb movements : For this experimentation, data was collected from three hundred and eighty female, age 19 to 23, who answered five lower limb movements(M1~M5). The statistics show that the order of the expansion ratio is gluteal area-length/knee-girth/back-line/knee-depth/thigh-depth/hip-girth, from the highest to the lowest in all movements. When comparing the correlation coefficient of the measurements, the values of the correlation coefficient of the height and the length items are very low, but those of the girth, the breadth, and the depth items are relatively high and those of the waist and the hip items are highest. For more sophisticated analysis, the factor analysis was conducted on the lower limb movements. Four factors were classified on the factor load by the "varimax rotation" method. Each movement shows the most important factor differently, as follows ; the most important factor in M1 is "the shape factor of lower limb below hip-line", that in M2 is "the cross-sectional shape factor", that in M3 is "the size factor of abdominal and loins region", and those in M4 and M5 accord with the interpretation of M3. When the investigation of the estimated function was conducted, in the selectional case of representative items on the slacks construction, it found that it would be better to add abdomen and thigh items as important considerations to waist girth, hip girth and crotch length.

The Association Between Serum Albumin Levels and Metabolic Syndrome in a Rural Population of Korea

  • Objectives: A positive association between serum albumin levels and metabolic syndrome has been reported in observation studies, but it has not been established in the Korean population. The purpose of this study was to evaluate the association between serum albumin levels and the presence of metabolic syndrome among a sample of apparently healthy Korean adults. Methods: This cross-sectional study analyzed data of 3189 community-dwelling people (1189 men and 2000 women) who were aged 40 to 87 years and were living in a rural area in Korea. Serum albumin levels were classified into quartile groups for each sex. Metabolic syndrome was defined according to the National Cholesterol Education Program Adult Treatment Panel III guidelines with an adjusted waist circumference cut-off value ${\geq}90\;cm$ for men and${\geq}85\;cm$ for women). An independent association between serum albumin levels and metabolic syndrome was assessed by multiple logistic regression analysis. Results: Higher serum albumin levels were associated with increased prevalence of metabolic syndrome. The odds ratio (95% confidence interval) of the prevalence of metabolic syndrome for the highest versus the lowest serum albumin quartiles was 2.81 (1.91 to 4.14) in men and 1.96 (1.52 to 2.52) in women, after adjusting for age, smoking status, alcohol consumption, and physical activity. When each metabolic abnormality was analyzed separately, higher serum albumin levels were significantly associated with hypertriglyceridemia and hyperglycemia in both sexes, and with abdominal obesity in men. Conclusions: These results suggest that higher serum albumin levels are positively associated with an increased risk of metabolic syndrome in Korean adults.

The Analysis of Geriatric Nutritional Risk Index (GNRI) for Nutritional Assessment and Health Care in Elderly Women (여자노인의 영양상태 평가 및 건강관리를 위한 Nutritional Risk Index (NRI) 비교 분석)

  • Nutritional assessment for the elderly can identify health status and morbidity. However, development of Nutritional Risk Index (NRI) remains limited for elderly because of difficulties in understanding physiological mechanism of elderly. This study was performed to analyze and develop Nutritional Risk Index for Korean elderly Women (Geriatric Nutritional Risk Index, GNRI). Based on literature review, factors for NRI were identified and indices were assessed by a cross-sectional survey. The survey involved Korean elderly women (${\geq}$60, n = 94) in Gwangju area, and sociodemographics, lifestyle characteristics, health conditions, dietary intakes based on 24h- recall, anthropometric measures (wt, ht, BMI, waist, hip, WHR, body protein, body fat, abdominal fat, and triceps skinfold thickness), and clinical biochemistry parameters (systolic blood pressure, diastolic blood pressure, cholesterol, HDL-cholesterol, triglyceride, total protein, albumin, prealbumin, hemoglobin, hematocrit, fasting blood glucose, HbAlc, ferritin, Zn, Ca, Na, K, Vit E, Vit $B_{12}$, folate, C-reactive protein) were examined relation to nutritional risk index. Based on literature review and data analyses, three NRIs were categorized (NRI I, NRI II, NRI III) and used for further analysis. NRI I was related to having metabolic syndrome, NRI II was related to serum albumin and body weight, and NRI III was related to food habit and health concerns. Abdominal fat (%) of elderly was correlated with each NRIs. NRI II was correlated with nutritional deficiency and higher tendency of inflammatory response, and NRI III was correlated with nutritional status which tend to be lower on aging (protein, folate, Vit $B_{12}$). NRI can serve as a useful tools in assessing health risk and nutritional status. Some modification of items in NRI and validity study are need to apply to Korean elderly.

  • By cluster sampling measurements, passing months of pregnancy according to changes of pregnant women body forms. On the basis of the above mentioned data, sizes of pregnant women clothes were decided. The results are as follows. 1. By cluster sampling measurements, sixty-nine items of apparel were obtained during the three different periods of pregnancy. They included the mean and standard deviation of body form measurement and the minimum and maximum values (Chart 3-4). The results of these surveys made it possible to notice the changes of pregnant womens' physical characteristics, i. e., the increased physical proportions: frontal waist area, umbilicus width, abdominal girth, crotch length, the length of the nipple, and body weight and the decreased proportions: omphalos height, perineum dimension. There was little change in the rear parts of the body. 2. The analized results of principle factors for body form measurements by cluster measurements produced seven major factors for which the proper values were over 1.0. They were: form factor, pregnancy factor, posture factor, breast formation factor, rear body formation factor, and nipple to nipple breadth. 3. In deciding garment sizes of pregnant women, four different sizes were established; small, medium, large, and extra-large according to the third, fifth, eighth, and tenth month of pregnancy. The measurement value of each item was produced by estimate.

Effect of Wearing Compression Clothing on Body Shape (압박의류 착용에 의한 신체변화 연구)

  • The purpose of this study was to determine the effect of compression clothing on the body shape of the human subjects. Thirty seven healthy females being overweighted with local adiposity on the buttocks and legs between the ages of 20's to 50's were used as the human subjects for the study. The selected subjects wore the compression clothing during 60 days by 8 hours a day. At the start(T0), after 30(T30) and after 60 days of treatment(T60), a cutaneous echography was measured on the right trochanter area of each volunteer in order to assess the thickness of the subcutaneous fatty tissue. At the start(T0), after 30(T30) and after 60 days of treatment(T60), the body weight of each volunteer was taken and the instrumental variations of skinfold measurements of the abdominal fold and inner thigh circumference of the waist, hips and thigh were conducted. The results show that the echography and weight loss of subjects were reduced to 8.34% and 2.08% after 60 days, respectively. Moreover, the skinfold of subjects was also reduced. Finally, skin elastometry of subjects increased.

A Study on the Relationship between Working Patterns and Health Conditions and Eating Habits of Workers in the Gyeongnam Area (경남지역 일부 직장인들의 근무형태와 건강상태, 식습관의 관련성 연구)

  • This study was conducted with 375 workers in Changwon to examine the effects of working patterns on dietary habits and health. A self-administered questionnaire was conducted July 22 October 30, 2019. According to the results of the dietary habits and health related factors analysis, non-shift administrative workers showed significantly higher rates of green tea intake (p<0.05), and shift production workers showed significantly higher smoking rates (p<0.001) and waist circumferences (p<0.01). According to the results of the Pearson's correlation coefficient analysis, work hours showed negative correlations with job satisfaction (r=0.22, p<0.01) and positive correlations with perceived stress level (r=0.14, p<0.01). Temporary workers showed negative correlations with feel job satisfaction(r=0.14, p<0.01), perceived stress level (r=0.12, p<0.05), and concern about health (r=0.13, p<0.05). Diabetes showed positive correlations with hypertension (r=0.20, p<0.01), low HDL cholesterolemia (r=0.22, p<0.01), abdominal obesity (r=0.13, p<0.05), and hypertriglyceridemia(r=0.22, p<0.01). Based on these results, this researcher proposes that continuous attention and support of industries and communities are necessary for nutritional education and counseling relative to improving workers' dietary lives as well as disease prevention and control.

Effects of a Exercise Program on Body Composition, Physical Fitness and Lipid Metabolism for Middle-Aged Obese Women (운동프로그램이 중년비만여성의 체구성, 체력 및 지질대사에 미치는 영향)

  • Purpose: This study was to examine the effects of an exercise program for middle-aged obese women. Method: The exercise program combined folk dance and resistance training. The subjects group consisted of 85 middle-aged obese women between 40 and 60 years of age. Three 8 week sessions consisted of a $55-80\%$ maximum heart rate (MHR) exercise for 60-90 minutes a day and 3 times a week from March to November, 2004. Data was collected through a pre- and post-exercise test before and after each session. Data was collected with Inbody, dynamometer and blood. This data was analyzed by descriptive statistics, and a paired t-test with an SPSS/PC(10.0 version) program. Results: There were significant positive changes in body weight, body fat mass, body mass index, percent body fat, muscle strength, muscle endurance, flexibility, and balance quality, but no significant positive changes in skeletal mass, fat free mass, percent abdominal fat(waist-hip ratio), visceral fat area, agility, total cholesterol, high density lipoprotein cholesterol, or triglycerides. Conclusion: This study showed that an exercise program has partially positive effects for middle-aged obese women. The results of this study show that exercise at community health centers should continue for middle-aged obese women's health.

The Effect of L-Carnitine and Isoflavone Supplementation on Weight Reduction and Visceral Fat Accumulation in Overweight Women (과체중 여성에서 L-carnitine과 Isoflavone 보충 섭취가 체중 및 내장지방 감소에 미치는 영향)

  • This study was performed to examine the combined effects of L-carnitine and isoflavone supplementation on weight reduction and body fat distribution in overweight women. Overweight/obese women (body mass index > $23kg/m^2$) who were not diagnosed any type of diseases were included in this study and sixty subjects ($41.1{\pm}1.5$ years, $25.9{\pm}0.3kg/m^2)$ were randomly assigned to a placebo (n=30) or a supplement group (n=30, L-carnitine 300 mg+isoflavone 40 mg/day). We measured anthropometric parameters, abdominal fat distribution by computerizd tomography and blood components before and after the 12 week intervention period. After the 12 weeks of supplementation, subjects in L-carnitine and isoflavone supplement group showed a significant reduction of body weight (p < 0.001), body fat % (p < 0.05), and waist to hip ratio (p < 0.01) whereas placebo group did not show any changes. In a CT-scanned results, total fat area at L4 level was significantly reduced by 8.1% (p < 0.01) with the reduction of visceral fat area (-11.1%, p < 0.001) and subcutaneous fat area (-7.0%, p < 0.05) in the supplement group. The supplementation of L-carnitine and isoflavone showed the significant improvement of HDL-C (p < 0.01) and apoB (p < 0.05) concentrations, however, change values in those markers were not significant compared with those of the placebo group. In addition, a significant increase of adiponectin level (p<0.001) was observed in the supplement group after the intervention. The result of present study demonstrated that supplementation of 300 mg L-carnitine and 40 mg isoflavone per day fur 12 weeks can give beneficial effects on weight reduction and visceral fat accumulation. These potential antiobesity supplement can produce more favorable effects when combined with lifestyle modification.

Dietary Iron Intake and Body Iron Status of Myocardial Infarction Patients in Chunan Area

  • It has been known for some time that elevated body iron could be a risk factor for coronary heart disease. The present study was conducted to determine body iron status and dietary iron intake of patients with myocardial infarction(MI). Seventy five patients from the Chunam area with their first MI history within he past 2 months were recruited. The serum iron concentration, total iron binding capacity(TIBC) and percent transferrin saturation(TS) were selected as indicators of body iron status. Twenty four hour recall was conducted by trained interviewers to asses the dietary intake. Most women (91.3%) showed waist to hip ratio(W/H) greater than 0.85 while 17.3% of men were assessed to have a tendency of abdominal obesity(W/H>0.95). The average BMI of women was 25.80 and that of men was 23.98. The average diet intake of participants was below the recommended dietary allowances (RDA) for most nutrients. He average dietary iron intake was 10.03 mg/day for all subjects while women's iron intake was significantly lower than men's. However, a great proportion of participants (77%) showed a tendency to have normal iron status. About 9% of the participants were assessed as iron deficient and 14% had an iron overload. The mean serum iron concentration was 125 g/dl ranging from 13.3 to 280.6 g/dl. Iron intake from animal sources were significantly associated with body iron status (r=0.257, p=0.026) when TIBC was used as an iron status indicator. When iron status was assessed with TS, it was directly associated with iron intake from animal sources(r=0.278, p=0.05) for he subjects in the normal iron status group. He results of the present study showed that the nutrient intake of Mi patients in Chunan was not quite adequate while iron status was mostly in the normal range. Further studies are needed to investigated whether there is a possible difference in iron metabolism of the MI patients.
