  • This study investigates the relationship between dysphonia severity index(DSI) and voice handicap index(VHI). Seventeen patients with a vocal nodule and twenty patients with laryngopharyngeal reflux(LPR) patients participated in this study. Results showed that there is no significant difference in either DSI or VHI between vocal nodule and LPR patients, with a weak negative correlation between DSI and VHI. Results also showed that there is significant difference only in both MPT and Fhi of all DSI parameters between vocal nodule and LPR patients. These results suggest that voice evaluation should be conducted both objectively in terms of acoustical and aerodynamic parameters and subjectively in terms of GRBAS and VHI.

  • Bowing of the vocal cords may be due to aging. atrophy. bilateral superior laryngeal nerve paralysis. injudicious vocal cord surgery, of an idiopathic cause. The bowing usually produces a dysphonia characterized by breathiness due to air escape : however, it can produce aphonia. This report reviews vocal function after surgical correction of bowing of the vocal cords for diagnosis and management. The vocal function of 13 patients with sulcus vocalis and 12 patients with vocal cord atrophy was evaluated with the use of a test battery of multidimensional evaluation items. The voice was improved postoperatively in most patients. The voice improvement was reflected objectively in maximum phonation time, mean air flow rate during phonation, stroboscopic findings. sound pressure level range and fundamental frequency range of phonation, and results of acoustic analyses of tape-recorded voice. The vocal function after surgical correction of the sulcus vocalis and vocal cord atrophy was improved postoperatively in most patient, but the results were not satisfactory.

  • The origin and growth of laryngology is inseparably linked to the development of endoscopic surgery of the larynx. Phonomicrosurgery is a means of maximally preserving the layered microstructure of the vocal fold, that is, the epithelium and lamina propria. Phonomicrosurgery has developed from convergence of micro laryngoscopic surgical technique theory and the mucosal wave theory of laryngeal sound production. Improvements in technology (i.e., laryngoscopes, handled instruments, and lasers), which in part arise from developments in more frequently performed minimally invasive surgical procedures, will probably facilitate the next generation of procedural innovations. The best methods of optimizing phonosurgical outcomes include making an accurate diagnosis, completing a comprehensive voice evaluation, providing sufficient preoperative therapy, carefully selecting patients to undergo phonomicrosurgical procedures, and requiring sufficient postoperative rest and therapy. Phonomicrosurgery will continue to evolve as a result of the interdependent collaboration of surgeons with voice scientists, speech pathologist, and other voice professionals.

  • The primary goal of therapeutic intervention in dysarthric speakers is to increase the speech intelligibility. Decision of critical features to increase the intelligibility is very important in speech therapy. The purpose of this study is to know the effects of pitch increasing training (PIT) on speech of a subject with Parkinson's disease (PD). The PIT program is focused on increasing pitch while a vowel is sustained with the same loudness. The loudness level is somewhat higher than that of the habitual loudness. A 67-year-old female with PD participated in the study. Speech therapy was conducted for 4 sessions (200 minutes) for one week. Before and after the treatment, acoustic, perceptual and speech naturalness evaluation was peformed for data analysis. Speech and voice satisfaction index (SVSI) was obtained after the treatment. Results showed Improvements in voice quality and speech naturalness. In addition, the patient's satisfaction ratings (SVSI) indicated a positive relationship between improved speech production and their (the patient and care-givers) satisfaction.

  • The purpose of this study was to compare the acoustic voice outcomes of children with cochlear implant to those of children with normal hearing. Participants were 41 children using unilateral cochlear implant (18 males and 23 females), and children with normal hearing from the same age and sex. Mean age of implantation was approximately 3 years old, mean duration of implant use was 4 years in CI group. Acoustic analyses were performed using MDVP of CSL. Speech samples were 3 sustained vowels, /a, i, u/. 9 parameters (F0, Fhi, Flo, Jitter, Shimmer, vF0, vAm, NHR, and SPI) were analyzed. Children with CI did not show the significant differences in those parameters after the vowel /a/ phonation. Meanwhile, there were significantly different results in F0, Fhi, vF0, and SPI after /i, u/ phonation. These results revealed that differences of voice characteristics in children with CI compare to children with NH persist regarding vowel context. It suggests that high vowels would recommend as speech samples for acoustic evaluation. Futhermore perceptual analysis and speech therapy for phonation control would be necessary for children with CI.

  • The term "phonosurgery," coined in the early 1960s, refers to surgical procedures that maintain, restore, or enhance the human voice. Phonosurgery includes phonomicrosurgery (endoscopic microsurgery of the vocal folds), laryngoplastic phonosurgery (open-neck surgery that restructures the cartilaginous framework of the larynx and the soft tissues), laryngeal injection (injection of medications as well as synthetic and organic biologic substances), and reinnervation of the larynx. Phonomicrosurgery is a means of maximally preserving the layered microstructure of the vocal fold, that is, the epithelium and lamina propria. The purpose of the surgery is usually to improve the vibratory characteristics of the layered microstructure of the vocal folds. Phonomicrosurgery has developed from convergence of microlaryngoscopic surgical technique theory and the mucosal wave theory of laryngeal sound production. • Substantial confusion exists regarding the nomenclature of benign vocal fold lesions (BVFL), because there were no pathologically diagnostic findings and deep understanding of pathogenesis in the past. There is no consensus on specific labels for BVFL, nor are the most commonly used terms defined. There is a need for a defined nomenclature for the purpose of improving communication among clinicians and with patients. Furthermore, precise definitions of BVFL will facilitate clinical research of voice disorders and may lead to a better understanding of outcomes for BVFL treatment. Laryngoscope, stroboscope, voice evaluation are used to diagnose BVFL. The objective of this review article was to develop a new paradigm of BVFL nomenclature using patient's history, stroboscope, voice therapy results, and operative findings. Furthermore, precise definitions of BVFL will facilitate clinical research of voice disorders and may lead to a better understanding of outcomes for BVFL treatment.
