성대진동검사 (Analysis of Glottal Vibration)

  • Because the human voice is produced through vibration of the vocal cords during the exhalation of airflow, it is important to observe the vibration pattern of the vocal cords in patients complaining of voice changes. However, it is not easy to observe the actual vibration pattern of the vocal cord because it vibrates so fast that it cannot be observed by the naked eye and it is located deep in the throat. Recently, with advances in instruments, including laryngoscopes and video camera systems, the vibration pattern of the vocal cords can now be observed. However, considering that present video camera systems can detect 30-60 images per second and the vocal cord vibrates at 100-200 and 200-300 times per second in men and women, respectively, it is not possible to record the whole mucosal wave of the vocal cord in real time. To overcome this limitation, a stroboscope, which converts fast movements of the vocal cord into slower images, has been developed. Since then, several instruments were developed to examine vocal vibration pattern. However, each instruments have advantages and disadvantages. Therefore, we should know about these things to apply them in patients with voice problem.

성대진동검사의 최신 지견 (Recent Advances in Examination of Vocal Fold Vibration)

  • Human vocal cords vibrate as quickly as 100-250 times per second, so it is impossible to observe them with normal endoscopic diagnostic equipment. High-speed videolaryngoscopy (HSV) allows the visualization of non-periodic vibratory motion of vocal fold beyond the limitation of videostroboscopy. New developed post-processing methods that converts HSV to two-dimensional videokymography (2D VKG) using U-medical image-processing software can provide quantitative information on vocal fold mucosa vibration. Multifunctional laryngeal examination system is composed of 3 kinds of examinations such as HSV, 2D scanning digital kymography (2D DKG) and line scanning digital kymography (DKG). Evaluation of entire vocal cord vibratory pattern in each cord is possible using 2D DKG and a faster and more reliable quantitative information can be obtained. As this system is used in clinical and research, it is expected to bring much advances to the diagnosis of voice disorders. In this review, I will introduce the principles and advantages on examination of the vocal fold vibration, which is in the spotlight recently, and proceed with the literature review.

전기성문전도(EGG) 시스템의 개발 및 평가 (Implementation and Evaluation of Electroglottograph System)

  • 전기성문전도는 발성시에 성문의 진동이 전기적 임피던스를 이용하여 검출되는 신호이다. 본 연구는 이러한 전기성문전도를 기록하기 위한 장비를 구현하고 음성분석 및 후두질환 진단에 대한 적용생을 평가하고자 하였다. 전기성문전도의 하드웨어는 2 쌍의 링전극, 동조증폭기, 검파기, 저역통과필터, 자동이득조절부 등으로 구성되며, 2.7MHz의 반송파 신호를 이용하고 진폭 변조 방식의 검파를 통해 임피던스 신호를 추출하도록 하였다. 추출된 신호는 PC 사운드 카드의 라인 입력을 통해 샘플링되고 양자화되었다. 검출 신호를 분석하기 위한 파라미터는 패래 시간을(CQ), 개폐 속도율(SQ), 개폐속도지수(SI), 성대진동 주파수(F0), 성대진동 주파수변동지수(Jitter), 성대진동 진폭변동지수(Shimmer) 등을 추출하였다. 전기성문전도를 분석한 결과, F0가 증가할수록 CQ는 커지고, SQ와 SI는 작아지는 경향을 보였으며, 전기성문전도와 음성 선호의 기본주파수가 일치함을 알 수 있었다. CQ, SQ, SI는 정상인과 후두암 환자를 비교한 결과 유의한 차이를 보였다. 이러한 결과는 성대의 운동을 관찰할 수 있는 휴대용 전기성문전도 계측기의 구현이 가능하게 하였고, 성대 기능 이상 검사가 가능함을 시사하였다.

혈압 상승이 성대 진동 및 음성 에너지 크기에 미치는 영향 분석 (Analysis for the Effect of Blood Pressure Increase on Vocal Cord Vibration and Voice Intensity)

  • 건강한 삶의 질이 향상되고 있으나 만성 질환으로 인한 고통은 날로 증가하고 있다. 만성 질환의 주요 요인은 스트레스, 혈압, 비만 등이 있으며 고혈압으로 인한 만성 질환 발병율은 매우 높은 편이다. 따라서 본 논문에서는 혈압 상승에 따른 음성을 분석하여 혈압 상승이 지속적으로 발생되는 현상을 조기에 진단하여 예방하기 위한 방법을 제안하고자 한다. 이를 위해 유산소 운동으로 혈압을 상승시킨 후 음성을 수집하고 음성 분석 기술 중 성대 진동을 측정하는 Pitch와 음성 에너지의 크기를 측정하는 Intensity를 적용하여 혈압 상승에 의해 음성에 미치는 영향을 분석, 연구하였다.

애성환자에서 음향지표인 RAP, PPQ 및 APQ의 유용성 (Significance of Acoustic Parameter - RAP, PPQ, APQ- in Hoarseness)

  • Change of voice, espicially hoarseness show irregular vibration of vocal cord. So, computerized acoustic analysis has presented many acoustic parameters for objective evaluation of voice. We objectively investigated the vocal vibration of normal persons and hoarseness patients in Korea. The RAP(relative average perturbation), PPQ(pitch period perturbation quotient) and APQ(amplitude perturbation quotient) of normal persons were compared with that of hoarseness patients with multidimensional voice program for the possibility of distinguishing the pathologic vocal vibration from normal. Authors agree that RAP, PPQ and APQ showed interesting differences between the normal and the hoarseness patients by the multivariate statistical analysis. In conculusion, relative average perturbation, pitch period perturbation and amplitude perturbation quotient might be meangingful screening parameters distinguishing hoarseness patients from normal.

  • Voice is made by systemic interaction of respiration, vocalization, articulation and resonation. There is no existing multi-channel voice analysis system to assess voice and respiration simultaneously. The most existing systems consist of. vocal fold vibration measurement such as stroboscopy, EGG (electroglottography) or laryngeal electromyography and voice analysis system. Since respiration has close relationship with voice simultaneous analysis of both vocal cord vibration and respiration are essential. In this study, a four channel integrated system are developed for acoustic analysis through microphone, vocal fold vibratory analysis using EGG, and respiratory analysis using two channel RIP (respiratory inductive plethysmography).

  • 이중음성이란 성대진동이 정상이 아닌 유사주기성에 의해 양측 성대진동의 위상차이때문에 발생되는 소리로서 대표적으로 양측성대의 긴장불균형(tension imbalance)시와 양측성대 사이의 높이차이(level difference)시에 발성직전의 성문열림의 정도에 따라 성대진동의 주기가 서로 다르기 때문에 이중음성이 발생한다고 한다. 그러나 실제 이러한 현상은 일측성대 마비환자뿐 아니라 성대의 점막부위에 증가된 질량효과에 의해 이중음성이 나타날 수 있는데 대표적으로 성대내 낭종이라 하겠다. 본 연구에서는 청각적으로 이중음성을 나타낸 일측성대마비, 성대내낭종 및 성대육아종 등 수술을 받았던 19례를 대상으로 음향학적 분석중 음성파형을 이용한 분석을 한 결과 유사주기성 음성파형에 대한 특징들중에서 파형수, peak변이도, 잡음의 정도등이 각질환에 따라 다른 양상을 보여 이중음성의 발생기전이 질환에 따라 다름을 알 수 있었다.

Development of Two-Dimensional Scanning Videokymography for Analysis of Vocal Fold Vibration

  • Objectives : We developed two-dimensional (2D) scanning videokyomography to evaluate the mucosal wave of whole vocal cords in real time to overcome the limit of preexisting stroboscopy and line scanning videokymography which could not evaluate it. Methods : We implemented a continuous light source with high brightness, a high-definition CMOS camera, and capture board for saving the data. We created the software program to analyze the image data from the system. The test of the functionality of the 2D scanning videokymography camera was performed in one of the authors (P.H.J 32 years old male). Vocal cord images were obtained during normal phonation and falsetto phonation. Images were obtained also during cough, diplophonia. Results : The system made it possible to measure objective parameters, including fundamental frequency, amplitude, regularity, mucosal wave, and phase difference, medial and lateral peak, opening versus closing duration related to vocal fold vibration. Simultaneously, it enabled analysis of the whole mucosal wave of the entire vocal fold in real time. 2D scanning videokymography was also effective for evaluating the dynamic status of the vocal fold when the subject phonated aperiodic voice. Conclusion : In conclusion, 2D scanning videokymography can support the analysis of the whole mucosal wave of the entire vocal cord with objective vocal parameters, overcoming the limitations of stroboscopy and previous line scanning videokymography techniques.

제3형 갑상연골성형술에 의한 병적인 고음성 치료 (Type III Thyroplasty for High-Pitched Voice)

  • Thyroplasty type III provides an effective mean of shortening the vocal fold. It is used primarily fer lowering vocal pitch. The indications for this surgery are the male patients with too high a vocal pitch of long duration which has proven resistant to voice therapy and a type of dysphonia characterized by high pitch and a breathy voice, in which the laryngeal findings include a very narrow glottal chink and a small amplitude of vibration, suggesting a stiff vocal cord. In this study, we have experienced two cases of high pitched voice, one is mutational falsetto and other is high pitched, breathy voice with stiff vocal fold. The results were satisfactorily successful with type III thyroplasty. So we report the cases of type III thyroplasty in patients of dysphonia with high pitched voice.

후두 스트로보스코프 검사의 신호 동기화를 위한 진동 검출기의 유용성 (Usefullness of the Vibration Pick-Up in Detection of Pitch for Synchronization of Laryngeal Stroboscopy)

  • Objective and Background: Laryngeal stroboscope is an useful equipment in evaluation of vocal cord vibration and in early detection of mucosal lesion including invasive cancer of the vocal cord. Recently Lee et al. (2006) developed portable stroboscope using voice as synchronization signal. It has been frequently impaired ability to synchronize the flashes even in normal female. Authors tried to investigate various methods including vibration pick-up, microphone, laryngeal microphone, and contact microphone for development of simple and accurate method like electroglottograph signal. The purpose of this study was to estimate wheher the vibration pick-up is available and is consistent with the signal of EGG. Subjects and Methods: Authors compared the signals between EGG and noncontact method such as voice, contact methods including vibration pick-up, laryngeal microphone, and contact microphone in normal twenty adults (male 10 and female 10). The number of peak in one cycle was compared with the number of the peak in EGG, and the percent of phase difference in the peak was compared with EGG Also, authors tried to investigate which site of vibration pick-up was most effective for synchronization of stobo flashes. Three site including anterior neck below the cricoid cartilage, thyroid ala, and suprahyoid region were analysed. Results: Among various methods for synchronization of strobo flashes, vibration pick-up was most effective method in peak detection. And anterior neck below cricoid cartilage was the most available site of the vibration pick-up. Conclusion: Authors suggest that vibration pick-up is most available and effective method for synchronization of strobo flashes.
