환자감시장치를 위한 HL7 V2.5 미들웨어의 개발 (Development of HL7 V 2.5 Middleware for Patient Monitoring Device)

  • A hospital room has multiple patient monitoring devices at the bedside to monitor his or her status. However, vital sign monitors, ventilator and other bedside devices are made by a variety of different manufacturers and often cannot easily interface to the hospital information system. Medical environments incorporate complex and integrated data networks to transfer vast amounts of patient information, such as images, waveforms, and other forms of digital data. Hence, to assure interoperability of images, waveforms and patient data, Health Level Seven (HL7) was developed as an international standard to facilitate the communicating and storing of medical data. In this study, we developed middleware capable of receiving data from mCare 300 vital signs monitoring devices and converting the data to HL7 data format. The HL 7 middleware streamline clinical workflow and support patients. Therefore, clinical expertise are empowered to respond to dynamic healthcare situation as soon as they emerged, and consequently quality of care while helping to reduce the length of a patient's stay in a hospital.

Completeness of Patient Care Report (PCR) by Paramedics

  • This study evaluated the completeness of patient care report (PCR). A retrospective quality analysis was conducted using raw data of 122,140 EMS activity reports prepared by paramedics in Gyeonggi-do from April 1 to May 31, 2021. In all, 67,830 cases of normal transfers were statistically analyzed using IBM SPSS Statistics version 22, and statistical significance was set at p<0.5. The 119EMT_2 certificate was omitted in 50,037 (73.8%) cases, followed by time-related items in 1,227 (1.8%) cases. In the primary assessment of vital signs, systolic blood pressure was omitted and erroneous in 1,218 (1.9%) and 1,129 (1.8%) cases, respectively. In the secondary assessment, the completeness of all vital sign items was approximately 70%. Advanced emergency care and online medical control (OLMC) reporting showed discrepancies in all items. As the severity of the patient's condition increased, the errors in the Patient care report (PCR) also increased, at a significant level (p= .00). Paramedics must be aware of the importance of completing the activity report.

  • 본 논문에서는 센서 네트워크를 이용해서 환자 모니터링 시스템(PVMS : Patient Vital Sign Monitoring System)을 구현하였다. 최근 의료 서비스에 유비쿼터스 컴퓨팅 기술을 적용한 사례들이 늘고 있다. 기존 사례에서는 센서 전지 수명, 이동 통신비, 응급상황 대처 등 개선할 부분이 존재한다. 본 연구에서는 이런 점들을 해결하기 위해 환자의 체온 및 맥박 생체신호를 측정하기 위해 소형센서를 사용하였다. 또한 생체신호 전달을 위해 초저전력 무선통신 노드를 사용하여 언제 어디서나 환자 모니터링이 가능하고 의료진에게 응급상황을 신속하게 전달할 수 있는 웹기반 시스템을 개발하였다. 본 연구의 결과는 병동환자뿐만 아니라 활력증후를 상시로 모니터해야하는 원거리 환자를 위한 의료시스템 구축에도 활용될 수 있을 것이다.

  • 사람으로부터 나오는 신호는 다양하고 그 데이터의 양도 매우 많다. 이러한 신호는 누구에게나 똑같이 발생하는 것이 아니기 때문에 각각의 사람마다 발생하는 신호를 분석하면 그 사람의 건강을 분석할 수 있는 중요한 자료로서 사용되어질 수 있다. 본 시스템은 인체의 손끝은 장부와 연결되어 손끝의 정보를 활용하면 건강을 진단할 수 있다는 한의학적 진단방법을 활용하여 인체 손끝에서 나오는 맥, 온도, 저항을 반사형 포토센서로 측정하여 이를 분석하고 장부의 허실을 판단하여 건강을 진단하는 시스템이다.

퍼지 논리 기반의 건강관리용 팔목 밴드 개발 (Development Of Fuzzy Logic Based Wristband For Healthcare)

  • 고령화 인구 증가에 따른 만성질환자, 노약자 및 독거인의 일상생활에서 위험하거나 불안정한 건강 상태를 관리할 수 있는 요구가 커지고 있다. 최근 무선 통신 기술의 발달로 특정한 장소에 구애받지 않고 언제 어디서나 개인의 건강 상태를 감시할 수 있는 장치가 주목 받고 있다. 본 논문은 생체 신호로 심박수와 체온을 주기적으로 측정하고 밴드 착용자의 나이와 측정된 생체 신호를 토대로 건강관리가 필요함을 알리는 팔목형 건강관리용 밴드를 개발하는 것이 목적이다. 나이, 체온 및 심박수 사이의 정상 범위 관계는 비선형이고 측정된 생체 신호는 오차를 포함 부정확하기 때문에 생체 신호 기반의 건강관리 판정에는 부정확하고 애매한 정보의 처리가 필요하다. 본 논문은 이런 문제를 해결하기 위해 부정확하고 애매한 정보를 처리하는데 적합한 퍼지 논리 기반의 건강관리 판정 시스템을 밴드에 구현한다. 밴드에 연동해 동작하는 건강관리 앱(Apps)도 함께 개발된다. 앱은 측정 데이터를 저장, 관리하며 밴드에서 건강관리가 필요한 신호가 감지되면 앱에 사전 등록된 보호자 또는 간병인에게 해당 정보를 전송한다.

Noninvasive Life Signal Detecting Systems and Their Analyses

  • Wireless life signal detecting system is implemented with using the mechanism of Doppler Effect. This system can measure the respiration and heart rates with the periodic movement of skin and muscle near the heart. The system is consisted of antenna, RF transmitter, receiver, and display part. We did use two operating frequencies at 1.9 ㎓ and 10 ㎓. Firstly, the link budget about detecting system is analyzed and the signal detected from the system is compared with electrocardiogram(ECG) of monitor which is using for patient monitoring in hospital. Secondly, the detection of vital sign is also performed according to the different distances, and including behind the wall.

Communications Protocol Used in the Wireless Token Rings for Bird-to-Bird

  • We developed a multicast communication packet radio protocol using a time-sharing tablet system ("wireless token ring") to achieve the efficient exchange of files among packet radio terminals attached to swans. This paper provides an overview of the system and the protocol of the packet communications. The packet device forming the main part of the transceiver developed is the Texas Instruments CC2500. This device consists of one call-up channel and one data transmission channel and could improve error frame correction using FEC (forward error correction) with 34.8 kbps MSK and receiving power of at least -64 dBm (output 1 dBm at distance of 200 m using 3 dBi antenna). A time-sharing framework was determined for the wireless token ring using call sign ordinals to prevent transmission right loss. Tests using eight stations showed that resend requests with the ARQ (automatic repeat request) system are more frequent for a receiving power supply of -62 dBm or less. A wireless token ring system with fixed transmission times is more effective. This communication protocol is useful in cases in which frequency resources are limited; the energy consumed is not dependent on the transmission environment (preset transmission times); multiple terminals are concentrated in a small area; and information (position data and vital data) is shared among terminals under circumstances in which direct communication between a terminal and the center is not possible. The method allows epidemiological predictions of avian influenza infection routes based on vital data and relationships among individual birds based on the network topology recorded by individual terminals. This communication protocol is also expected to have applications in the formation of multiple in vivo micromachines or terminals that are inserted into living organisms.

Design of Kalman Filter to Estimate Heart Rate Variability from PPG Signal for Mobile Healthcare

  • In the mobile healthcare system, a very important vital sign in analyzing the status of user health is the HRV (heart rate variability). The used signals for measuring the HRV are electrocardiograph and PPG (photoplethysmograph). In extracting the HRV from the PPG signal, an important issue is that extract the exactly HRV from PPG signal distorted from the user's movements. This study suggested a design method of the Kalman filter to solve the problem, and evaluated the performances of a proposed method by PPG signals containing motion artifacts. In the results of experiments that compared with a variable step size adaptive filter proposed in recently, the proposed method showed better performance than an adaptive filter.

Heart Rate Estimation Based on PPG signal and Histogram Filter for Mobile Healthcare

  • The heart rate is the most important vital sign in diagnosing heart status. The simple method to measure the heart rate in the mobile healthcare device is using the PPG signal. In developing the mobile healthcare device using the PPG signal, the most important issue is the inaccuracy of the measured heart rate because the PPG signal is distorted from the user's motions. To improve the problem, this study proposed the new method that is to estimate the heart rate without an additional sensor in real life. The proposed method in this study is using the histogram filter. In order to evaluate the performance of the proposed method, the study compares its results with the moving average method in motion environment. According to the experimental results, the performance of the proposed method was more than 40% better than the performances of the MAF.

Comparison of health literacy and health behaviors between Korean women with and without breast cancer

  • Purpose: This descriptive survey compared health literacy and health behaviors between Korean women with and without breast cancer. Methods: In total, 95 women with and 97 women without breast cancer (age range: 40-69 years) completed the survey. Health literacy and health behavior were analyzed in women with and without breast cancer, using the Newest Vital Sign. Results: Women with breast cancer were less frequently alcohol drinkers (14.7% vs. 47.4%, p < .001) and more frequently exercised (65.3% vs. 49.5%, p = .027), obtained health information (17.9% vs. 8.2%, p = .047), and attended health education programs (10.5% vs. 1.0%, p = .005) than women without cancer. In both groups, women with higher literacy levels outnumbered those with limited literacy. Conclusion: The results indicated that women with breast cancer were more likely to engage in health-promotion activities than women without cancer, and this increased their health literacy levels. The findings could inform interventions involving breast cancer prevention methods.
