Cancer Pain Control for Advanced Cancer Patients by Using Autonomic Nerve Pharmacopuncture

  • Objectives: The purpose of this study is to report a case series of advanced cancer patients whose cancer pain was relieved by using autonomic nerve pharmacopuncture (ANP) treatment. ANP is a subcutaneous injection therapy of mountain ginseng pharmacopuncture (MGP) along the acupoints on the spine (Hua-Tuo-Jia-Ji-Xue; 0.5 cun lateral to the lower border of the spinous processes of vertebrae) to enhance the immune system and to balance autonomic nerve function. Methods: Patients with three different types of cancer (gastric cancer, lung cancer, colon cancer with distant metastases) with cancer pain were treated with ANP. 1 mL of MGP was injected into the bilateral Hua-Tuo-Jia-Ji-Xue on the T1-L5 sites (total 12-20 mL injection) of each patient's dorsum by using the principle of symptom differentiation. During ANP treatment, the visual analogue scale (VAS) for pain was used to assess their levels of cancer pain; also, the dosage and the frequency of analgesic use were measured. Results: The cancer pain levels of all three patients improved with treatment using ANP. The VAS scores of the three patients decreased as the treatment progressed. The dosage and the frequency of analgesics also gradually decreased during the treatment period. Significantly, no related adverse events were found. Conclusion: ANP has shown benefit in controlling cancer pain for the three different types of cancer investigated in this study and in reducing the dosage and the frequency of analgesics. ANP is expected to be beneficial for reducing cancer pain and, thus, to be a promising new treatment for cancer pain.

Pituitary Apoplexy : Surgical Experience with 16 Patients

  • Objective : Pituitary apoplexy, resulting from an acute infarction or hemorrhage mainly in pituitary adenomas, is a rare yet major clinical event with neurological, ophthalmological and hormonal emergent consequences. The authors review our surgical experience with a series of 16 cases of pituitary apoplexy. Methods : The cases of pituitary apoplexy, operated via trans-sphenoidal approach in our hospital between 1998-2005, were retrospectively analyzed in terms of their clinicoradiological features, pathological findings and surgical outcomes. Results : The mean age of patients (9 male and 7 female) was 47.1 years. The average time of presentation after onset of symptom was 8.9 days. Pituitary apoplexy occurred as an initial manifestation of pituitary adenoma in all patients. Headache was the most common presenting symptom (94%). Visual disturbance was found in 56% of patients. Thirty-one percent of the patients had hypopituitarism. On magnetic resonance imaging, this entire catastrophic event accompanied with macroadenoma in a mean size of 22.5 mm. Only four patients needed postoperative hormone replacement therapy. Three of them showed preoperative hypopituitary function, and one patient in normal pituitary function. There was no specific complication in any of these patients. Conclusion : Early trans-sphenoidal decompression with high-dose corticosteroid replacement showed good outcomes of pituitary apoplexy.

Is Titanium Mesh Cage Safe in Surgical Management of Pyogenic Spondylitis?

  • Objective : To report our experience with pyogenic spondylitis treated with anterior radical debridement and insertion of a titanium mesh cage and to demonstrate the effectiveness and safety of the use of a titanium mesh cage in the surgical management of pyogenic spondylitis. Methods : We retrospectively analyzed the clinical characteristics of 19 patients who underwent surgical treatment in our department between January 2004 and December 2008. The average follow-up period was 11.16 months (range, 6-64 months). We evaluated risk factors, cultured organisms, lab data, clinical outcomes, and radiographic results. Surgical techniques for patients with pyogenic spondylitis were anterior radical debridement and reconstruction with titanium mesh cage insertion and screw fixation. All patients received intravenous antibiotics for at least 6 weeks postoperatively, and some patients received oral antibiotics. Results : The infections resolved in all of the patients as noted by normalization of their erythrocyte sedimentation rates and C-reactive protein levels. The mean pain score on a Visual Analog Scale was 7.8 (range, 4-10) before surgery and 2.4 (range, 1-5) after surgery. The Frankel grade was improved by one grade in seven patients. After surgery, the average difference of the angle was improved about $6.96^{\circ}$ in all patients. At the last follow-up, the mean loss of correction was $4.86^{\circ}$. Conclusion : Anterior radical debridement followed by the placement of instrumentation with a titanium mesh cage may be a safe and effective treatment for selected patients with pyogenic spondylitis. This surgical therapy does not lead to recurrent pyogenic spondylitis.

A Case Report of a Right-sided Hemichorea Patient Diagnosed with Chorea Hyperglycemia Basal Ganglia Syndrome : A Combined Western-Korean Medicine Approach (우반신 무도증을 호소하는 Chorea Hyperglycemia Basal Ganglia Syndrome 환자에 대한 증례보고 : 양한방 복합치료적 접근으로)

  • Objectives : The purpose of this report is to introduce a rare case of a patient with Chorea Hyperglycemia Basal Ganglia syndrome (C-H-BG) whose initial medical interventions were started 2 weeks after the onset, and to suggest the possibility of treatment using a combined Western-Korean medicine approach. Methods : A 75-year-old female C-H-BG patient complaining of persistent right-sided hemichorea was treated with a therapy that combined Korean and Western medicine from April 4, 2015 to April 29, 2015. Improvements of symptoms were measured by a motor assessment of Unified Huntington's Disease Rating Scale (UHDRS), Visual Analog Scale (VAS) and the number of involuntary movements. Results : Motor assessment of UHDRS, VAS and the number of involuntary movements all showed a gradually improving tendency during 26 days of admission treatment. However, the patient's hemichorea persisted. Conclusion : Rapid blood sugar control is the most important treatment for C-H-BG, because pathologic changes of basal ganglia seem to become irreversible as time goes by. A combined Western-Korean medicine approach to treating C-H-BG seems effective not only in reducing hemichorea, but also in the management of accompanying symptoms such as muscle pain and general weakness.

Comparison of Clinical and Psychological Characteristics between Self-Reported Bruxism and Clinically Detected Bruxism by Wear Facet on Splint

  • Purpose: Bruxism is commonly considered a major risk factor for temporomandibular disorders (TMD), and the psychosocial factors had been one of the etiologic factor of bruxism. But there are still unsolved issues on the relationship between sleep bruxism and TMD and the etiologic factors of bruxism. This study is aim to evaluate the clinical and psychosocial characteristics according to diagnostic grade of bruxism in TMD patients. Methods: Three hundred subjects were enrolled who were under the stabilization splint therapy for TMD. Recently international consensus proposed a diagnostic grading system of "possible", "probable", and "definite" sleep or awake bruxism for clinical and research purpose. According to their suggestion, we classified these subjects as self-reported bruxism (SRB) and wear facet bruxism (WFB). We investigated the clinical characteristics (sex, age, chief complaint, pain duration, visual analogue scale), sum of tenderness (temporomandibular joint, masticatory muscles, cervical muscles), diagnosis of TMD according to research diagnostic criteria (the Research Diagnostic Criteria for Temporomandibular Disorders, RDC/TMD), headache, subjective sleep quality (Pittsburgh Sleep Quality Index, PSQI), and psychosocial characteristics (Symptom Checklist-90-Revised, SCL-90-R) in enrolled subjects. We compared the clinical and psychosocial characteristics between these bruxism groups. Results: There were no significant correlation between self-reported and WFB (p=0.13). SRB subjects more reported pain as a chief complain than subject who did not report bruxism (p=0.014). The mean score of global PSQI was significantly higher in SRB than in did not report positively subjects (p=0.045). The mean score of anxiety and phobic anxiety was significantly higher in SRB than in did not reported positively subjects (p=0.045, p=0.041). Conclusions: Although bruxism is regarded as risk factor of TMD, this study showed inconsistent result between SRB and clinically detected bruxism by wear facet on slpint. We suggest that the clinician should consider with extreme caution when they assess SRB.

Effect of simultaneous therapy of arthrocentesis and occlusal splints on temporomandibular disorders: anterior disc displacement without reduction

  • Objectives: This study sought to evaluate the effect of simultaneous application of arthrocentesis and occlusal splint. Materials and Methods: A retrospective study of 43 patients (3 males, 40 females) whose symptoms had improved was conducted at the Department of Oral and Maxillofacial Surgery, Dong-A University Hospital between 2008 and 2010. Subjects were divided into three groups: Group A (17 patients with arthrocentesis and occlusal splints simultaneously applied), Group B (13 patients whose symptoms did not improve with occlusal splints, undergoing arthrocentesis after occlusal splint use for 8 weeks), and Group C (13 patients that only used occlusal splints). We compared these groups in maximum comfortable opening (MCO) and the visual analogue scale of pain and noise. Follow-up was performed at 1 week, 1 month, 3 months, and 6 months. Results: The improvement of symptoms was noted in all three groups, but Group A had a quicker improvement than the other groups, in terms of pain reduction and MCO increases. Conclusion: The simultaneous application of arthrocentesis and occlusal splints can reduce patient discomfort more quickly.

  • Objective : The purpose of the study is to examine the construct validity and internal consistency of the Sensory Processing Scale for Children (SPS-C). Method : 117 participants who are aged 3 to 5 years old has been evaluated by the Sensory Processing Scale for Children (SPS-C) for this study and the period is from November 2014 to June 2016. Statistics used for data analysis is factor analysis for construct validity and Cronbach's alpha for internal consistency. Results : In the factor analysis, the items in auditory, visual, and tactile showed a high factor loading, respectively. The items in vestibular and proprioceptive were overlapped. For internal consistency, Cronbach's alpha was .728 for whole scale which is high level. And Cronbach's alpha of each factor was range within .326~.636. Conclusion : The results of the psychometric test in this study evidence that the SPS-C is valid and reliable to be utilized. Further study need to examine various validities and reliabilities as well as standardization.

  • Objectives : The purpose of this study is to report the patients with radial nerve palsy, who inproved by oriental medical treatment Methods & Results : The patients were hospitalized Dept. of Acupunture & Moxibustion, College of Oriental Medicine, Daejeon University from 1st, Mar. 2005 to 31th. Aug. 2005, and treated with Acupuncture and Electrical Stimulation theraphy, Herbal medication, Oriental physical therapy. To evaluate the wrist drop and numbness of fingers, Visual Analogue Scale(VAS) and the measurement of wrist circumstance are used. As the result, symptoms are improved remarkably Conclusion : Patients were treated for 4.1 weeks(average). 6 cases were excellent and 1 case was good.

  • Objectives : The purpose of this case is to report the improvement after treatment about tonic spasm of a transverse myelitis patient. Methods : We treated the patient with acupuncture therapy and herbal medicine from the 19th July 2006 to the 4th October 2006. At first we used Ssanghwa-tang until the 24th July 2006. After that we used Samul-tang until 2nd August. Lastly we used Yukmigihwang-tang until the 4th October 2006. And we used acupuncture at Asi points and used Sheng Ge of Liver from beginning to end. We checked the patient with frequency of tonic spasm and other symptoms. Results : After two months of treatments, frequency of tonic spasm decreased and abdominal muscle rigidity, visual disturbance, ocular discomfort and oppressed feeling in the chest were improved.

The Comparison of Effectiveness between Near Acupuncture Point Needling and Remote Acupuncture Point Needling on Treating Ankle Sprain of Acute Stage (급성기 족관절 염좌의 근위취혈(近位取穴)과 원위취혈(遠位取穴)의 치료 효과 비교 연구)

  • Objective : The purpose of this study is to compare the effects of near acupuncture point needling and remote acupuncture point needling on treating ankle sprain of acute stage. Methods : From March 1st, 2007 to May 30th, 2007, the 50 patients who had visited Iksan oriental medical hospital, Wonkwang university with acute ankle sprain were divided into 2 groups ; one group took near acupunture point needling, and the other group took remote acupunture point needling. Both group had been treated with the same additional rest, ice, compression and elevation(RICE) therapy. To evaluating the efficiency of each treatment, Visual Analog Scale(VAS) and Ankle-Hindfoot Scale(AHS) were applied before 1st treatment and after 3rd treatment. Results : As a result of evaluation by using AHS, the score change comparison between the two groups had no significance after the treatment. As a result of evaluation by using VAS, treatment score of remote acupucture point needling was marked lower than score of near acupucture point needling. Conclusion : Remote acupucture point needling is more effective than near acupucture point needling in controlling the pain of acute ankle sprain.
