Effects of Head Acupuncture Versus Upper and Lower Limbs Acupuncture on Signal Activation of Blood Oxygen Level Dependent(BOLD) fMRI on the Brain and Somatosensory Cortex (두침과 상하지 침자극이 뇌와 뇌의 체성감각피질에 미치는 영향에 대한 fMRI Study)

  • Objectives : To evaluate the effects of Head Acupuncture versus Upper and Lower Limbs Acupuncture on signal activation of Blood Oxygen Level Dependent(BOLD) fMRI on the Brain and Somatosensory Cortex. Subjects and Methods : 10 healthy normal right-handed female volunteer were recruited. The average age of the 10 subjects was 30 years old. The BOLD functional MRI(fMRI) signal characteristics were determined during tactile stimulation was conducted by rubbing 4 acu-points in the right upper and lower limbs($LI_1$, $LI_{10}$, $LV_3$, $ST_{36}$). After stimulation of Head Acupuncture in Sishencong($HN_1$), $GB_{18}$, $GB_9$, $TH_{20}$ of Left versus Upper and Lower Limbs Acupuncture($LI_1$, $LI_{10}$, $LV_3$, $ST_{36}$ of Right) and took off needles. Then the BOLD fMRI signal characteristics were determined at the same manner. Results : 1. When touched with cotton buds(sensory stimulation), left Parietal Lobe, Post-central Gyrus, primary somatosensory cortex(BA 1, 2, 3), and primary motor cortex(BA 4) were mainly activated. When $ST_{36}$ was stimulated, Frontal Lobe, Parietal Lobe, Cerebellum, and Posterior Lobe as well as Inter-Hemispheric displaying a variety of regions. 2. In signal activation before and after Head Acupuncture reaction, it showed signal activation after removing the acupuncture needle and right Somatosensory Association Cortex, Postcentral Gyrus, and Parietal Lobe were more activated. 3. In reactions of before and after Upper and Lower Limb Acupuncture, it also showed signal activation after removing the acupuncture needle and bilateral Occipital Lobe, Lingual Gyrus, visual association cortex, and Cerebellum were activated. 4. After acupuncture stimulation, In Upper and Lower Limb Acupuncture Group, left frontal Lobe, Precentral Gyrus and Bilateral parietal lobe, Postcentral Gyrus and Primary Somatosensory Cortex(BA 2) were activated. In Head Acupuncture Group, which has most similar activation regions, but especially right Pre-Post central Gyrus, Primary Somatosensory Cortex(BA 3), Primary Motor Cortex, frontal Lobe and Parietal Lobe were activated. Conclusions : When sensory stimulation was done with cotton buds on four acup-points($LI_1$, $LI_{10}4, $LV_3$, $ST_{36}$), while bilaterally activated, contralateral sense was more dominant. It showed consistency with cerebral cortex function. When $ST_{36}$ was stimulated Frontal Lobe, Parietal Lobe, Cerebellum, Posterior Lobe as well as Inter-Hemispheric were stimulated. In Head Acupuncture, it showed more contralateral activation after acupuncture. In Upper and Lower Limb Acupuncture, it showed typically contralateral activation and deactivation of limbic system after acupuncture stimulation. Therefore, there were different fMRI BOLD signal activation reaction before and after Head Acupuncture vs Upper and Lower Limb Acupuncture which might be thought to be caused by acu-points' sensitivity and different sensory receptor to response acupuncture stimulation.

Effects of Carbon Dioxide Fertilization on the Quality and Storability of Strawberry 'Maehyang' (재배 시 탄산시비가 딸기 '매향' 의 품질과 저장성에 미치는 영향)

  • This study was conducted to find out the effects of $CO_2$ fertilization (1,000ppm) on the quality and storability of 'Maehyang' strawberry fruits. Qualities such as firmness, soluble solid, and acidity of strawberry fruits showed higher numbers in those treated with $CO_2$ fertilization compared to those after harvest. Strawberry fruits were stored at $8^{\circ}C$; MA condition using $20,000cc{\cdot}m^{-2}{\cdot}day^{-1}{\cdot}atm^{-1}$ OTR (oxygen transmission rate) films and conventional condition using unsealed PE box stored for 20 and 10 days, respectively. Fresh weight loss rate was less than 1.0% in MA storage regardless of $CO_2$ fertilization treatment. Concentrations of oxygen, carbon dioxide, and ethylene in OTR films did not show any significant difference between $CO_2$ fertilization treatment and control (nontreatment) during storage. $CO_2$ fertilization treatments maintained higher firmness after storage regardless of storage methods, but soluble solid, acidity, and surface color did not differ among the treatments. Visual quality and off-flavor based on sensory evaluation was the highest in $CO_2$ fertilization treated strawberry and stored at a MA condition, and was the lowest in $CO_2$ fertilization treated strawberry and those stored in a conventional condition, respectively. The fungal incidence rate of strawberry fruits showed less in $CO_2$ fertilization treatment than control during both MA and conventional storage. These results suggest that $CO_2$ fertilization can improve firmness, increase visual quality after harvest and storage at $8^{\circ}C$, and the MA storage method enhances the shelf-life of 'Maehyang' strawberry fruits.

  • Grading of the urushi lacquer is quite difficult because of large variations depending on origin, growing conditions, time acquisition, storage conditions. It is urgently required to establish a grading system of urushi lacquer by combining traditional method with scientific methods. Traditional grading of urushi lacquer was done by visual inspection of 10 experts who are working on urushi lacquer industry. Common aspects of the experts were color, odor, viscosity, spread properties on Hanji, color and drying properties on glass based the traditional grading system. In addition, rubbing on Hannj and heating with alcohol lamp was also used to grade urushi lacquer. The grading results of 10 experts showed that chinese urushi lacquer (E) of 7.03, japanese urushi lacquer (C) of 6.84, domestic urushi lacquer of (A) of 6.41 and another chinese urushi lacquer (D) of 5.27, and domestic urushi lacquer (B) of 2.50 in descending order. The degree of spread on Hanji was not consistent among 10 experts. These results indicated that the traditional grading system was pretty much personal and required of developing more objective method based on scientific background.

The Effect of Light Stimulation on the Reduction of Unilateral Neglect of Stroke Patients: Single Subject Research Design (뇌졸중 환자에게 불빛 자극이 편측무시 감소에 미치는 영향: 단일 사례 연구)

  • Objective : The purpose of this study was to investigate the effect of left visual stimuli for reducing the severity of unilateral neglect. Method : This research used a reversal design(ABAB) of single case study. The subject was a 60-year-old male patient who had left unilateral neglect. Remedial approach is more valuable method than compensatory approach in term of reducing the unilateral neglect. One of the method of remedial approach is controlled sensory stimulation. Result : I performed the line bisection test before and after the stimulation, and while doing that, put blinking lights to the patient from the left side. The average errors in the line bisection test decreased in B B', the intervention period, compared with A A'. The MVPT left response behavior score increased from 13 to 18. Conclusion : This shows that the light stimulation is effective in the reduction of unilateral neglect.

Effect of Postharvest Treatments on Storage Quality of Buckwheat Sprouts (메밀 새싹채소의 저장품질에 대한 수확 후 처리공정 효과)

  • The storage quality of fresh buckwheat sprouts, as influenced by pretreatment and packaging within processing steps, was investigated to establish appropriate postharvest handling treatment for the commodity. After harvest, the sprouts were dipped in chlorine water (100 ppm), rinsed twice with clean water, pre-cooled with iced water, de-watered, and packed in plastic trays. Sprout samples taken from each processing step were stored at $5^{\circ}C$ for 6 days to measure quality attributes. Viable cell counts of mesophilic aerobes and coliform bacteria were lower by about 1 log scale in the postharvest treated samples compared to an untreated control, although the initial microbial reduction due to the postharvest treatments was offset by cell growth during storage. All sprout samples showed a decrease of fresh weight by approximately 4% after 6 days of storage. However, moisture and soluble solid contents were maintained at the initial levels of the sprouts. No significant difference in surface color was observed among sample treatments. For sensory properties including discoloration, wilting, decay, and visual quality, there were no significant differences among sample treatments. The present results suggest that proper postharvest processing treatments can exert positive effects on extending the shelf-life of fresh buckwheat sprout.

Post-operative Analgesic Effect of Epidural Midazolam Administered with Morphine (경막외강에 Morphine과 동반 투여한 Midazolam의 진통효과)

  • Background: Opioids and local anesthetics have been administered epidurally for the purpose of the postoperative analgesia. However opioids have a serious risk of respiratory depression and local anesthetics have the risks of hypotension, sensory block, or motor one. In recent years, reports of spinal administration of midazolam for acute postoperative pain control have appeared in the literature. This study was performed to observe the effect of epidural midazolam in patient-controlled analgesia (PCA) device. Methods: Forty-five patients scheduled for the elective total hysterectomy were randomly selected; epidurally take morphine only (group I, n=15), morphine plus 0.1% bupivacaine (group II, n=15), or morphine plus midazolam (group III, n=15). The visual analogue scale (VAS) at rest and with movement, the sedation score, the degree of the satisfaction, the total amounts of a morphine usage, and the incidence of the side effects were observed. Rusults: The VAS at rest of group II and III were decreased significantly than that of group I. The VAS with movement of group III was significantly decreased than that of group I and II. The sadation score and the cumulative dose of a morphine were statistically insignificant within groups. Conclusion: Epidural morphine plus midazolam was proven to be clinically effective in the post-operative pain control especially for the pain with movement, compared with epidural morphine only and morphine plus 0.1% bupivacaine.

Muscle Activities in the Lower Limbs for the Different Movement Patterns on an Unstable Platform

  • We performed experimental studies on the muscle activities in the lower limbs for the different movement patterns on an unstable platform. A training system for postural control using an unstable platform that we previously developed was applied for the experiments. This unstable platform provides 360 degrees of movement allowing for training of posture in various directions and provides simultaneous excitations to visual sensory, somatic sensation and vestibular organs. Compare with the stable platform, keeping body balance on the unstable platform requests more effective sensation from vision, vestibular sense and somatic sense. Especially, the somatosensory inputs from the muscle proprioceptors and muscle force are crucial. To study the muscle activities for the different movement patterns and find the best training method for improving the ability of postural control through training and improving the lower extremity muscular strength, fifteen young healthy participants went through trainings and experiments. The participants were instructed to move the center of pressure following the appointed movement pattern while standing on the unstable platform. The electromyographies of the muscles in the lower limbs were recorded and analyzed in the time and the frequency domain. Our experimental results showed the significant differences in muscle activities for the different movement patterns. Especially, the spectral energy of electromyography signals in muscle for the movement pattern in anterior-posterior direction was significantly higher than those occurred in the other patterns. The muscles in the lower leg, especially tibialis anterior and gastrocnemius were more activated compared to the others for controlling the balance of body on the unstable platform. The experimental results suggest that, through the choice of different movement pattern, the training for lower extremity strength could be performed on specific muscles in different intensity. And, the ability of postural control could be improved by the training for lower extremity strength.

The Efficacy of Repeated Radiofrequency Medial Branch Neurotomy for Lumbar Facet Syndrome

  • Objective : Radiofrequency (RF) medial branch neurotomy is an effective management of lumbar facet syndrome. However, pain may recur after period of time. When pain recurs, it can be repeated, but the successful outcome and duration of relief from repeated procedures are not clearly known. The objective of this study was to determine the success rate and duration of pain relief from repeated radiofrequency medial branch neurotomy for lumbar facet syndrome. Methods : A retrospective review of medical records was done on 60 consecutive patients, from March of 2006 to February of 2009, who had an initial successful RF neurotomy but subsequently underwent repeated procedures due to recurrence of pain. All procedures were done in carefully selected patients after at least two responsive medial branch nerve blocks. C-arm fluoroscopic guide, impedance, sensory and motor threshold monitoring tools were used for the precise placement of electrodes. Responses of repeated procedures were compared with initial radiofrequency neurotomy for success rates and duration of pain relief. Results : There were 48 females and 12 males. Mean age was 52.4 years (range, 26-83). RF medial branch neurotomy was done on one side in 38 and both sides in 22 patients, each covering at least three segments. Average visual analog scale at last procedure was 6.8. Twelve patients had previous lumbar operations, including 4 patients with instrumentations. Fifty-five patients had two procedures and five patients had three procedures. Mean duration of successful pain relief (> 50% of previous pain for at least 3 months period) after initial radiofrequency neurotomy was 10.9 months (range, 3-28) in 51 (85%) patients. From repeated procedures, successful pain relief was seen in 50 (91%) patients with average duration of 10.2 months (range, 3-24). Five patients had third procedure, which was successful in 4 (80%) patients with mean duration of 9.8 months (range, 5-16). This was not statistically different from initial results. There were no permanent neurological complications from the procedures. Conclusion : Results of this study indicate that the frequency of success and durations of relief from repeated RF medial branch neurotomy for lumbar facet syndrome are similar to initial results that provided relatively prolonged period of pain relief without major side effects Each procedure seems to provide successful pain relief for about 10 months in more than 85% of carefully selected patients when properly done.

Effects of Myofascial Release on Nerve Conduction Studies and Pain Scale in Middle-Aged Women (중년여성에서 근막이완요법이 신경전도와 통증에 미치는 영향)

  • The purpose of this study was to examine the effect of myofascical release (MR) on the degree of pain and nerve conduction velocity (NCV) in middle-aged women. Participants were 28 middle-aged women and MR carried out three times (1, 3, 5 day) at intervals of two times. We did survey about changes of pain before the MR and how they changed after the MR. Also measured pressure pain threshold (PPT) and visual analogue scale (VAS) by using the algometer at trapezius muscle. In median nerve, we did motor nerve conduction velocity (MNCV) test and sensory nerve conduction velocity (SNCV) test for measuring incubation period, amplitude and nerve conduction. The most painful time was 18~21 and the most painful part was shoulder. The pain scale, PPT and VAS after the MR had significantly decreased than before the MR. The latency was significantly decreased and the amplitude was significantly increased in the MNCV and the latency was significantly decreased in the SNCV after the MR. Also it was effective in ameliorating pain scale and latency of NCV. Consequently, the MR can be effective in prevent pain scale caused by fatigue in middle-aged women as replacement therapy.

  • We have long kept our Cartesian tradition in which Mind privileges over Body; the invention of printing technology has accelerated the tradition in which the intelligence and legibility of media were considered as a higher human value. However, the development of media and technology are supporting the multisensory mechanisms of the body as it was in preliterate era when we communicated with our whole bodily experiences. The development and spread of mass-media and new technologies have broaden the boundary of human sensory that are largely dominated by visual information and expanded it to auditory, olfactory and even gustatory sensation. Since 1960's, some philosophers and artists have recognized the human body as a subjective matter, starting the movement in which the body plays a role as an essential factor in study of human perception and cognition, aesthetics in art, and sociology, and the changed perspectives are practiced vigorously in the field of new media art; the theory of Maurice Merleau-Ponty who clarified the body as a general means for the body's possession of world, so that recognize the importance of cognition of one's body and approved the embodiment could be applied and practiced here. Therefore, we discuss how Merleau-Ponty's philosophical theory can be practiced and how McLuhan's perspective could be applied on the notion of body's extension in media by analysing some new media art cases.
