A Study on the Side Effect of the Splint Therapy for the Patient with Craniomandlbular Disorders (교합상을 이용한 치료가 두개하악장애 환자에 미치는 부작용에 관한 연구)

  • 저자는 두개하악 장애로 교합상치료를 받고있는 42명의 환자를 대상으로 치료전, 치료후 1개월, 3개월, 6개월째에 주관적 평가, 임상적 검사, 근전도 검사, 컴퓨터 교합 분석을 시행하여 다음과 같은 결론을 얻었다. 1. 주관적 평가인 visual analogue scale과 Helkimo's anamnestic dysfunction index는 유의하게 감소하였다. 임상적 검사시 MM(mandibular movement), TM(TMJ capsule palpation), EM(extraoral muscle palpation), Di(Helkimo's clinical dysfuction index)는 유의하게 감소하였고 무통성 개구량은 유의하게 감소하였다. 2. 42명의 환자중 10명에서 교합변화가 관찰되었으며, 5명에서 치주질환, 2명에서 치아 과민감, 1명에서 구토경향이 관찰되었다. 3. 치주칠환을 보이는 10명의 환자중 3명은 교합상치료 이전부터 치주질환에 이환된 상태였으며 3명에서는 교합변화가 함께 관찰되었다. 4. 교합변화를 보인 10명의 환자중 교합상의 장착시간을 줄였을 때, 3명이 원래의 교합상태로 회복되었고 4명은 부분적으로 회복되었으며 3명에서는 변화를 관찰할 수 없었다. 5. 악관절 내장 제 5기 환자에서 교합변화가 더 빈번히 나타났으며 통계적으로 유의한 차이를 보였다. 6. Habitual clenching 시와 Maximal clenching 시 사이의 치아접촉점 수, maximal clenching 시의 치아 접촉점의 수와 접촉 치아수 사이에 매우 유의한 상관관계를 보였고, habitual clenching 시의 치아접촉점의 수와 접촉 치아수, Habitual clenching 시와 maximal clenching시 사이의 치아 접촉시간에서도 유의한 상관관계를 보였다.

Challenges in neuro-machine interaction based active robotic rehabilitation of stroke patients

  • Study results in the last decades show that amount and quality of physical exercises, then the active participation, and now the cognitive involvement of patient in rehabilitation training are known of crux to enhance recovery outcome of motor dysfunction patients after stroke. Rehabilitation robots mainly have been developing along this direction to satisfy requirements of recovery therapy, or focusing on one or more of the above three points. Therefore, neuro-machine interaction based active rehabilitation robot has been proposed for assisting paralyzed limb performing designed tasks, which utilizes motor related EEG, UCSDI (Ultrasound Current Source Density Imaging), EMG for rehabilitation robot control and feeds back the multi-sensory interaction information such as visual, auditory, force, haptic sensation to the patient simultaneously. This neuro-controlled and perceptual rehabilitation robot will bring great benefits to post-stroke patients. In order to develop such kind of robot, some key technologies such as noninvasive precise detection of neural signal and realistic sensation feedback need to be solved. There are still some grand challenges in solving the fundamental questions to develop and optimize such kind of neuro-machine interaction based active rehabilitation robot.

Correlation Analysis of Organic Acid Comprehensive Profile Markers with Chemotherapy Induced Peripheral Neuropathy in Cancer Patients (항암제 유발 말초신경병증환자와 유기산검사 마커와의 상관성 연구)

  • Objectives: The purpose of this study is to evaluate the urinary organic acid comprehensive profile for chemotherapy induced peripheral neuropathy (CIPN). Methods: Participants are 66 patients with CIPN who had symptom (Visual analog scale ${\geq}30mm$, Eastern Cooperative Oncology Group ${\leq}2$). Participants were tested with organic acid comprehensive profile markers. Results: Positive Correlation was observed in the neurotransmitter metabolism markers, N-methyl-D-aspartate (NMDA) modulators markers, detoxification markers, energy production markers, amino acid metabolism markers, and intestinal dysbiosis markers. Especially, all the neurotransmitter metabolism markers were showed positive rate of 44%. In addition, neuro-endo-immune was associated with energy metabolism (mitochondrial dysfunction) in CIPN of cancer patient. especially detoxification, intestinal bacterial hyperplasia, vitamin deficiency (folate, complex B group, vitamin C). Conclusions: Significant urinary organic acid comprehensive profile results were obtained in cancer patients who induced peripheral neuropathy by chemotherapy.

The Biomechanical Correlation Analysis of Upper Body according to Forward Head Posture (머리전방자세에 따른 상체의 생체역학적 상관분석)

  • Background: The purpose of this study is to analysis of correlation upper body according to forward head posture. Methods: The subjects of this study were 40 female university students who were equally and randomly allocated to a forward head posture group, normal group. Using general posture system, electromyograph, visual analogue scale, tape measurement, neck disability index were evaluated. Results: There was positive correlation between posture analysis and Sternocleidomastoid, neck flexion (p<.05). There was positive correlation between Craniovertebral angle (CVA) and trapezius upper, VAS (p<.05). There was negative correlation between posture analysis and CVA (p<.05). There was negative correlation between Cranial rotation angle and CVA (p<.05). Conclusion: Increased forward head posture lead to increase of pain, muscles activity, so it suggests to be necessary on the prevention of dysfunction and limited activities daily living.

  • The purposes of this study were to assess the effect of therapeutic exercise and to offer an approach to the physical therapy and rehabilitation procedure of the temporomandibular joint (TMJ) following surgery. In this research, 42 patients with TMJ surgery were assigned one of two groups. The experimental group included 21 patients who performed therapeutic exercise, and the control group included 21 patients who did not perform therapeutic exercise. Conservative therapy such as an ice pack, a hot pack, and pulsed ultrasound was applied to both groups. Treatment was applied twice a day during the admission period and, after discharge, everyday for six weeks. Visual analogue scale (VAS), incisal biting force, and joint ROM were measured before surgery and at 30 days after surgery. The results were as follows: VAS (p<0.05), mouth opening (p<0.01), lateral excursion to unaffected side (p<0.05), and protrusion (p<0.05) between experimental group and control group showed statistically significant differences. Incisal biting force and lateral excursion to affected side between experimental group and control group showed no statistically significant difference.

A Survey Study on Characteristics Associated with Fractures in Elderly People (노인골절 환자의 골절 관련 특성에 대한 연구)

  • Purpose: The purpose of this study was to identify characteristics associated with fractures in elderly people in order to provide basic data for fracture preventive programs for the elderly people. Method: The participants were 84 patients over age of 65, who were admitted to the orthopedic department in a hospital in Chungnam province. Data were collected from Sep. 1, 2002 to Aug, 30, 2003 through personal interviews using a structured questionnaire. The data were analyzed using SPSSPC program. Result: Physical characteristics before the fracture included weakness or paralysis in the extremities (29.8%), need of assistance or appliances (13.1%), difficulty on balance (28.6%), visual disturbances (26.2%), hearing impairment (17.9%), speech disturbances (2.4%), urinary dysfunction (21.4%), and sleep disturbances (54.8%). The fractures occurred most frequently in winter (32.1%), between 1 pm and 6 pm (48.8%), on weekends (41.6%), in the road (58.3%) while wearing snickers (27.4%) or shoes (27.4%). The region of fractures occurred most frequently was lower extremities (57.1%), and the causes of fractures were loss of balance (31.0%) and car accident (25.0 %). A significance difference was found for type of accident, footwear at the time of the accident, place of the accident according to gender and age. Also a significance difference was found for type of accident and place of accident according to season(p<.05). Conclusion: Therefore, these results should be considered when a fracture preventive program for elderly people is designed.

  • Objectives The purpose of this study is to investigate that causes of motion sickness in oriental medicine. Methods The internal and external studies about motion sickness were searched Results and Conclusions The motion sickness is induced by conflict of balance system including vestibular, visual and proprioceptive system. The motion sickness is more common in female and in children between 2 and 12 years old. In western medicine, antihistamine and anticholinergic has been used for treatment of motion sickness, but these anti-motion sickness drug turn out to be not a perfect solution and have several side effects. On the other hands, In oriental medical terminology, there is no words equivalent to the "motion sickness", but we consider the motion sickness as state with dizziness, nausea and vomiting. The motion sickness can be induced by either internal or external causes and the internal causes can thought to be a constitutional factor of an individuals, and the general transportation can be an external cause. The important internal cause is a dysfunction of the spleen, stomch(脾胃不調), retention of phlegm and fluids(痰飮), and deficiency of the kidney jing(腎精不足). The wind(風) and fire(火) in the upper part of the body, especially in the head, also can be an important cause of the motion sickness.

The Effect of Rectus Abdominis Functional Massage on Forward head posture and Pain in Patients with Chronic Neck Pain (복직근 기능적 마사지가 만성 목통증 환자의 머리전방자세와 통증에 미치는 영향)

  • Background: The purpose of this study was to determine the effects of cervical deep muscle flexion exercise (CCFE) on craniovertebral angle, pain, and neck disability for patients with chronic neck pain Methods: The subjects of this study were randomly divided into three groups of 30 patients with chronic neck pain: rectus abodominis functional massage (n=10), cervical deep muscle flexion exercises group (n=10), and the control group(n=10). To assess visual analog scale (VAS) was used to test the neck pain, To assess neck posture was used to craniovertebral angle, VAS was used to test the neck pain, neck disability index (NDI) was used to test the neck dysfunction. All measurements were performed before and after each intervention was applied 3 times a week for 4 weeks. Results: In the results of all measurements, 2 groups except for the control group showed a significant change in the recovery of posture, neck pain, neck disability index (p<.05). Conclusions: Our results of this study showed that applying cervical deep muscle flexion exercise and rectus abodominis functional massage to patients with chronic neck pain improved cervical posture, neck pain, neck disability.

Comparison of Random and Blocked Practice during Performance of the Stop Signal Task

  • Purpose: We investigated the changes in the stop-signal reaction time (SSRT) and the no-signal reaction time (NSRT) following motor sequential learning in the stop-signal task (SST). This study also determined which of the reduction0s of spatial processing time was better between blocked- and random-SST. Methods: Thirty right-handed healthy subjects without a history of neurological dysfunction were recruited. In all subjects, both the SSRT and the NSRT were measured for the SST. Tasks were classified into two categories based on the stop-signal patterns, the blocked-SST practice group and random-SST practice group. All subjects gave written informed consent. Results: In the blocked-SST group, both the SSRT and the NSRT was significantly decreased (p<0.05) but not significantly changed in the random-SST group. In the SSRT and the NSRT, the blocked-SST group was faster than the random-SST group (p<0.05). In the post-test SST after practice of each group, the SSRT was significantly decreased in the random-SST group (p<0.05), but the NSRT showed no significant changes in either group. Conclusion: These findings demonstrate that random-SST practice resulted in a decrease in internal processing times needed for a rapid stop to visual signals, indicating motor skill learning is acquired through improved response selection and inhibition.

A Case Study of a Female Patient with Patellofemoral Pain Syndrome for Effect of Hip Joint Traction and Hip Posterolateral Muscles Strengthening on Knee Pain, Range of Motion, and Lower Extremity Function Scale (슬개대퇴통증 증후군 환자에게 적용한 고관절 견인과 근력 강화 운동이 무릎 통증, 관절가동범위, 하지기능에 미치는 영향-사례 연구)

  • Background: The case study examined the effect of a hip joint traction and hip posterolateral muscles strengthening on knee pain, range of motion, and lower extremity function scale of patients with patellofemoral pain syndrome (PFPS). Although PFPS has previously been attributed to quadriceps dysfunction, more recent research has linked this condition to impairment of the hip musculature and kinematic. Methods: Subject is a 27-years-old female with PFPS. Performed hip joint traction with belt and posterolateral muscles(hip abductors, external rotators) strengthening for 4 weeks, 3 times a week, once a day. Before and after the therapy, measurements were made on the visual analog scale (VAS) and of the ROM, and a lower extremity functional scale (LEFS) was conducted. Results: The results showed positive changes in VAS and range of motion and lower extremity functional scale. First VAS of knee changed from 6 to 2. Second hip joint range of motion showed that internal rotation recored from $53^{\circ}$ to $58^{\circ}$ and external rotation recorded from $32^{\circ}$ to $37^{\circ}$. Third The lower extremity functional scale showed before therapy of 44; after therapy, 63. Conclusion: The hip joint traction and hip posterolateral muscles strengthening was effective in alleviating knee pain, increasing ROM and Lower extremity functional scale of the PFPS patients.
