The Effect of Korean Medical Treatment on 4 Patients with Acute Traumatic Meniscal Tear (급성 외상성 반월연골 파열에 대한 한의학적 치료 증례보고 4례)

  • Objectives : The purpose of this study is to report the effect of Korean medical treatment on 4 patients with acute traumatic meniscal tear, presenting severe knee pain and limitation of range of motion of the knee joint. Methods : 4 patients with acute traumatic meniscal tear had been treated with acupuncture, herbal treatment, moxibustion, and wet-cupping therapy. From administration to discharge, we recorded subjective pain by using a visual analogue scale and by measuring the range of motion of the knee joint once per week. Results : The visual analogue scale of knee pain of all 4 cases reduced to 0 after treatment. The range of motion of the knee joint also increased considerably after treatment. Conclusions : In this study, Korean medical treatment may have been effective in reducing knee pain and increasing the range of motion of the knee joint.

Relationship of neck disability index, shoulder pain and disability index, and visual analogue scale in individuals with chronic neck pain

  • Objective: Several muscles surrounding neck are vital not only for neck motion, but for upper extremities motions as well. Neck pain would affect neck and shoulder disability. The Neck Disability Index (NDI), Shoulder Pain and Disability Index (SPADI), and Visual Analogue Scale (VAS) are increasingly used to evaluate treatment effectiveness after chronic neck pain. The purpose of this study was to analyze the correlation of neck pain, shoulder pain, and quality of life in patients with chronic neck pain. Design: Cross-sectional study. Methods: Forty-three patients with neck pain participated in this study. Participants were instructed to place a mark on each item of three clinical measures that best represented their experience of his/her neck and shoulder problem over the last week. The outcome measures of the study were NDI, SPADI, and VAS for neck and shoulder pain and disability. Results: The NDI was significantly correlated with the VAS-Neck, SPADI-Pain, SPADI-Disability, SPADI-Total, and VAS-Shoulder (p<0.05). The VAS-Neck was significantly correlated with the VAS-Neck, SPADI-Pain, SPADI-Disability, SPADI-Total, and VAS-Shoulder (p<0.05). The SPADI-Total was significantly correlated with the NDI, VAS-Neck, SPADI-Pain, SPADI-Disability, and VAS-Shoulder (p<0.05). The SPADI-Pain was significantly correlated with the NDI, VAS-Neck, SPADI-Disability, SPADI-Total, and VAS-Shoulder (p<0.05). The SPADI-Disability was significantly correlated with the NDI, VAS-Neck, SPADI-Total, SPADI-Pain, and VAS-Shoulder (p<0.05). The VAS-Shoulder was significantly correlated with the NDI, VAS-Neck, SPADI-Pain, SPADI-Disability, and SPADI-Total (p<0.05). Conclusions: Neck disability in patients with chronic neck pain should be considered with disability and pain of the shoulder.

Do Severity Score and Skin Temperature Asymmetry Correlate with the Subjective Pain Score in the Patients with Complex Regional Pain Syndrome?

  • Background: The diagnostic criteria of complex regional pain syndrome (CRPS) have mainly focused on dichotomous (yes/no) categorization, which makes it difficult to compare the inter-patient's condition and to evaluate the intra-patient's subtle severity over the course of time. To overcome this limitation, many efforts have been made to create laboratory methods or scoring systems to reflect the severity of CRPS; measurement of the skin temperature asymmetry is one of the former, and the CRPS severity score (CSS) is one of the latter. However, there has been no study on the correlations among the CSS, temperature asymmetry and subjective pain score. The purpose of this study was to evaluate whether there is any correlation between the CSS, skin temperature asymmetry and subjective pain score. Methods: Patients affected with CRPS in a unilateral limb were included in this study. After making a diagnosis of CRPS according to the Budapest criteria, the CSS and skin temperature difference between the affected and unaffected limb (${\Delta}T$) was measured in each patient. Finally, we conducted a correlation analysis among the CSS, ${\Delta}T$ and visual analogue scale (VAS) score of the patients. Results: A total of 42 patients were included in this study. There was no significant correlation between the ${\Delta}T$ and VAS score (Spearman's rho = 0.066, P = 0.677). Also, the CSS and VAS score showed no significant correlation (Spearman's rho = 0.163, P = 0.303). Conclusions: The ${\Delta}T$ and CSS do not seem to reflect the degree of subjective pain in CRPS patients.

  • Background: This study was to investigate the effect of 8-weeks medical exercise therapy on ankle pain, range of motion, stress symptom after traumatic injury, and depression, in a 51 years old stroke patient with right ankle joint inflammation. Method: The 8-weeks medical exercise therapy program was applied to 4 grades of Dosage 1 (1-3 weeks), Dosage 2 (4-5 weeks), Dosage 3 (6-7 weeks), and Dosage 4 (8 weeks) on right ankle joint inflammation in a female with right hemiplegia admitted to D hospital located in Gyeonggi-do. Result: The findings showed that visual analogue scale (VAS) scores improved from 8 to 0-1 scores, passive range of motion (ROM) increased to 5 degrees more than before, Korean-version impact of event scale-revised (IES-R-K) scores increased from 61 to 31 scores, and Korean-version beck depression inventory II (BDI-II-K) scores decreased from 51 to 17 scores. As such, the 8-week medical exercise therapy program may decrease the pain, increase ROM, improve stress after traumatic injury, and improve depression symptom. Conclusion: The presented evidence suggests that exercise and physical activity have beneficial effects on depression symptoms. It is possible to apply the medical exercise therapy for modulating pain experience and treating pain. Also, it may be effective methods to treat the psychological aspects of pain.

Effect of Cervical HVLA Technique and Mulligan MWM Techniques on Pain, Function in Patients with Lateral Epicondylitis (경추 도수교정과 멀리건기법이 주관절 외측상과염의 통증과 기능에 미치는 영향)

  • Background: This study was to investigate the effect of cervical HVLA technique and mulligan MWM technique on patients with lateral epicondylitis. Methods: 36 participants were randomly allocated to the two groups; group I involved high velocity low amplitude (HVLA) cervical mobilization technique, mulligan mobilizations with movement (MWM) technique (n=18). group II mulligan MWM (n=18). Patient attended for three times a week for six weeks. Results: The change of visual analogue scale (VAS) was statistically and significantly decreased in both of the group I and the group II. The change of patients rated tennis elbow evaluation (PRTEE) was statistically and significantly decreased in both of the group I and the group II. The change of pressure pain threshold (PPT) was statistically and significantly increased in both of the group I and the group II. Nevertheless, There is no statistical differences between group I and group II. Conclusion: After experiment, pressure pain threshold and visual analogue scale, patients rated tennis elbow evaluation were improved in both group. But Cervical HVLA group does not showed the change better than mulligan MWM group.

Effects of Acupuncture on Upper Back Myofascial Pain and Pain Pressure Threshold (경견부 근막통증증후군에 대한 침치료 효과와 압통역치의 변화)

  • Objective : This study was performed to confirm the effects of acupuncture on myofascial pain syndrome(MPS) through the change of visual analogue scale(VAS) and pain pressure threshold(PPT) and the usefulness of pressure algometer on the evaluation of pain. Methods : We perfomed this study with 20 outpatients complaining of upper back pain. Before acupuncture therapy(AT), immediately after AT and 2-3 days after AT, we respectively checked visual analogue scale(VAS) and pain pressure threshold(PPT) through pressure algometer, with patients seated and relaxed. The PPT was checked at major trigger point of upper trapezius, levator scapulae, supraspinatus, infraspinatus, rhomboideus minor. and the patients were needled at the same points and maintained for 15 minutes. Results : VAS of immediately after AT was mild higher than that of before AT, but not significantly different. and VAS of 2-3 days after AT was significantly lower than before AT and immediately after AT. PPT of immediately after AT was lower than before PT, but not significantly different. PPT of 2-3 days after AT was significantly higher than that of before AT and immediately after AT. Also PPT was significantly correlated with VAS. Conclusion : PPT of omen was signicantly lower than that of men. and there was no significant difference by age. PPT was increased according to pain duration. Effectiveness of acupuncture on myofascial pain syndrome through PPT and VAS is showed at 2-3 days after AT rather than immediatly after AT. and pressure algometer is useful for the evaluation of Acupuncture therapy on myofascial pain syndrome.

The Effects of Spiral Taping Theraphy-A Randomisation Controlled Trial

  • Objectives : This study was designed to estimate the effects of Spiral Taping theraphy on low back pain or neck pain patients by using pressure pain threshold, visual analogue scale(VAS), and range of motion(ROM). Methods : The patients were allocated into two groups by randomisation. Spiral Taping theraphy group consisted of 11 patients and acupuncture theraphy group consisted of 10 patients. The degree of improvement of neck pain was evaluated by pressure pain threshold, VAS, and ROM before treatment and after treatment. The patients were commonly treated with acupuncture theraphy. After acupuncture Spiral Taping theraphy was peformed in Spiral Taping theraphy group. Results : Pressure pain threshold and VAS were significantly different between two groups. In Spiral Taping theraphy group the pressure pain threshold of the all points was significantly increased and VAS was significantly decreased than control group. But there was no significant difference between the two groups in ROM. Conclusion : The effectiveness of Spiral Taping theraphy on low back pain and neck pain patients was shown through pressure algometer and VAS. These imply that Spiral Taping theraphy may be useful for on low back pain or neck pain. Further study is needed about Spiral Taping theraphy.

Effect of Kinesio Taping on Isokinetic Muscular Function and Pain of Knee Joint in Elderly Women (키네시오 테이핑이 고령자의 슬관절 등속성 근 기능 및 통증에 미치는 영향)

  • The aim of this study were to evaluate the effect of kinesio taping on isokinetic muscular function and pain of knee joint. The subjects for this study were twelve elderly women($65{\sim}70$years) suffered by knee joint pain over more than three years. The taping methods performed on 3 times a week for 8 weeks for improvement of isokinetic muscular function. Also, the change of visual analogue scale(VSA) and varval rating scale(VRA) has been measured before and post taping 8 weeks. Paired t-test was performed for data analysis at the 0.05 level of significance. After 8 weeks, the results of the study are as follow; First, peak torque, peak torque % BW, total work and ipsilateral balance ratio were significantly improved. And, work fatigue, visual analogue scale and verval rating scale were decreased significantly after 8 weeks taping. The results suggest that applying kinesio taping on knee joint may improve isokinetic muscular function of elderly women. Therefore kinesio taping method was effective for pain relief and prevention of knee joint pain in the elderly women.

A Comparative Efficacy of Propacetamol and Ketorolac in Postoperative Patient Controlled Analgesia

  • Background: Ketorolac has been used as a postoperative analgesia in combination with opioids. However, the use of ketorolac may produce serious side effects in vulnerable patients. Propacetamol is known to induce fewer side effects than ketorolac because it mainly affects the central nervous system. We compared the analgesic effects and patient satisfaction levels of each drug when combined with fentanyl patient-controlled analgesia (PCA). Methods: The patients were divided into two groups, each with n = 46. The patients in each group were given 60 mg of ketorolac or 2 g of propacetamol (mixed with fentanyl) for 10 minutes. The patients were then given 180 mg of ketorolac or 8 g of propacetamol (mixed with fentanyl and ramosetron) through PCA. We assessed the visual analogue pain scale (VAS) at the time point immediately before administration (baseline) and at 15, 30, and 60 minutes, and 24 hours after administration. Also, the side effects of each regimen and each patient's degree of satisfaction were assessed. Results: There was a significant decline in the VAS score in both groups (P < 0.05). However, there were no significant differences in the VAS scores between the groups at each time point. Satisfaction scores between the groups showed no significant difference. Conclusions: The efficacy of propacetamol is comparable to that of ketorolac in postoperative PCA with fentanyl.

Effects of the BUDDEUMI Therapy on the Relief of Shoulder Pain among Middle Aged Women (중년여성의 견갑통 통증완화를 위한 부항과 뜸 겸용요법의 적용효과)

  • This study is to determine the effects of BUDDEUMI(moxibustion and cupping a boli) therapy on the relief of shoulder pain among middle aged women. methods : A total of 39 middle aged women with shoulder pain are selected : 20 in the experimental group and 19 in the control group. The data was collected by using questionnaires. The BUDDEUMI(moxibustion and cupping a boli) therapy on the should and back was administered to the experimental group, and no treatment was given to the control group. All of the subjects were examined on a subjective symptom scale and on the grade of discomfort and pain in everyday life as developed by Japan's Industrial Fatigue Research Committee of the Industrial Hygienics Society(1988), Sohn, Mong Ho(1999), Visual Analogue Scale as developed by Cline (1992). SPSS/win 12.0 was used for Date analysis, General features demonstrated by frequency number and percentage. The effects of the BUDDEUMI(moxibustion and cupping a boli) therapy was analyzed by a t-test. The homogeneity testing revealed that all study variables are equally distributed between experimental and control groups. The hypothesis, 'the experimental group who received BUDDEUMI(moxibustion and cupping a boli) therapy will decrease in the grade of discomfort of shoulder', is supported. The hypothesis, 'the experimental group who received BUDDEUMI(moxibustion and cupping a boli) therapy will decrease in the Visual Analogue Scale is supported. Conclusion : As a result of this study, BUDDEUMI(moxibustion and cupping a boli) therapy will be able to be used as the self care therapy to improve the symptoms of middle aged women who has the shoulder pain.
