삼차신경통에 대한 봉약침을 병행한 한방치료 치험 1예 (A Case of Trigeminal Neuralgia Treated by Korean Medical Treatment Including Bee Venom Pharmacopuncture)

  • Objectives : The purpose of this report is to show the clinical efficiency of korean medical treatments with Bee Venom pharmacopuncture therapy on patient of Trigeminal Neuralgia. Methods : The patient was treated with Bee Venom pharmacopuncture, acupuncture, electroacupuncture, cupping venesection treatment, and herbal medicine. Visual Analog Scale was used to evaluate the improvement of pain after all treatments. Results : Visual Analog Scale was improved from 10 to 2 after korean medical treatment including Bee Venom pharmacopuncture. Conclusions : Korean medical treatments with Bee Venom pharmacopuncture can be effective for improving symptoms of Trigeminal Neuralgia.

건식수치료가 요통환자의 통증과 요부장애에 미치는 영향 (The Effects of Dry-Hydrotherapy on The Low Back Disability and Pain for Patients with Low Back Pain)

  • Purpose: The purpose of this study was to evaluate the effects of dry-hydrotherapy to release pain in patient with low back pain. Methods: The subjects of the study were fifty patient with low back pain in orthopedic clinic. Fifty patients who had low back pain were randomly divided into 2 groups, 1) dry-hydrotherapy group 2) control group. Visual analog scale(VAS) and Oswestry low back pain disability questionnaire were measured before and after treatment. The treatment plan Was as follows; 15 min. exercise per day and 5 days per week for 2 weeks. Results: 1. Before dry-hydrotherapy, there was no significant difference in VAS score and Oswestry low back pain disability questionnaire score between two groups. 2. The VAS score was significantly decreased in both groups after 2-week dry-hydrotherapy and the decrease in the AS score in dry-hydrotherapy group was significantly lower than in control group. 3. The Oswestry low back pain disability questionnaire score was significantly decreased in both groups after 2-week dry-hydrotherapy and the decrease in Oswestry low back pain disability questionnaire score dry-hydrotherapy group was significantly lower than in control group. Conclusion: In conclusion, the dry-hydrotherapy was effective to alleviate pain in patients with low back pain.

오십견 환자의 치료에 있어서 저주파자극기의 효과 (Efficacy of Low Frequency Stimulator in Patients with Frozen Shoulder)

  • Background: Frozen shoulder is not an uncommon disease, which is associated with chronic pain and joint movement limitation. However, there are numerous devices to assist in the treatment of shoulder pain, but their efficacy has not been proven and their use remains immensely controversial. Therefore, a randomized clinical study was conducted to determine the effectiveness of a low-frequency stimulator for the treatment of frozen shoulder. Methods: A randomized clinical trial was carried out on 40 patients with frozen shoulder, with 40 patients assigned to two groups; a control treatment group (group C, n = 20) and a low frequency stimulator application group (group T, n = 20). Both groups were given a routine treatment modality, such as trigger point injection, intramuscular stimulation or suprascapular nerve block etc. The level of the shoulder pain was evaluated using a 100mm VAS (visual analog scale) at each visit, with the limitation in the range of motion simultaneously evaluated. Results: All the subjects improved after treatment, with the VAS scores after termination of treatment showed a statistically significant reduction (P < 0.05). However, there was no significant difference between the two groups. One month after termination of 5 cycles of treatment, group T maintained their improved state, whereas the pain in some of those in group C reemerged, which also showed a statistically significant difference (P < 0.05). The limitation in the range of motion improved, with most subjects able to resume daily activity. Conclusions: Although the low frequency stimulation provided no more pain relief than routine treatment, the effect was significantly prolonged. From this result, low frequency stimulation can be considered to aide the therapeutic effect of classical frozen shoulder therapy.

팔체질침(八體質鍼)으로 치료한 복합부위통증증후군 1형 환자 1례에 대한 증례보고 (A Clinical Case Study of a Patient with Complex Regional Pain Syndrome (CRPS) Type I Treated by 8 Constitution Acupuncture)

  • Objectives : The purpose of this study is to report the clinical effects of 8 constitution acupuncture on a patient with complex regional pain syndrome (CRPS) type I. Methods : In this study, a patient with CRPS was treated by 8 constitution acupuncture therapy. He was diagnosed in the Cholecystonia of 8 constitution diagnosis. The progress of symptoms were evaluated by visual analog scale (VAS) and sleeping time. VAS and sleeping time were evaluated at 7:00 am daily. Results : VAS decreased from 10 to 4, while sleeping time increased from 30 minutes to 3 hours. Conclusion : The results suggest that 8 constitution acupuncture is effective for treating symptoms of CRPS type I.

4주간 복합 운동치료가 유방암 림프부종 중년여성의 통증, 견관절 가동범위에 미치는 영향 (The Influence of 4 wks Complex Therapeutic Exercises on Visual Analog Scale of Pain and Range of Motion for Middle-Aged Women with Breast Cancer-Related Lymphedema)

  • PURPOSE: This study aimed to investigate the effect of complex theraputic exercise on visual analog scale of pain, shoulder' range of motion for middle-aged women with breast cancer related lymphedema. METHODS: The 14 middle-aged women involved voluntary in this study and then they were divided into two groups(n =7 per group). The complex exercise program was implemented over 4 weeks, 60 minutes per day, with 3 types of exercise for stimulation whereas the control group was performed a classical decongestive physiotherapy in a same day. For data analysis, the mean and standard deviation were estimated; 2 way repeated measures ANOVA was carried out. RESULTS: First, The level of VAS was significantly reduced on time, interaction effect in the group. Second, most factors of ROM were significantly increased on time, interaction effect whereas extension was not significantly increased. CONCLUSION: In conclusion, Our results showed that complex therapeutic exercise could improve or maintain VAS and ROM of shoulder joint for middle-aged women with breast cancer related lymphedema.

요부안정화운동이 요부불안정성을 가진 산업체 만성요통환자의 통증관련 기능에 미치는 효과 (The Effects of Lumbar Stabilization Exercise on Pain-related Functions of Industrial Workers with Chronic Low Back Pain and Lumbar Instability)

  • 본 연구의 목적은 요부안정화운동이 요부 불안정성을 가진 산업체 만성요통환자의 통증관련 기능에 미치는 효과를 알아보고자 하였다. 산업체 만성요통환자 20명을 대상으로 대조군과 실험군으로 10명씩 할당하여 신체기능은 배근력, 유연성, 균형능력을 측정하였고, 통증은 visual analog scale(VAS)를 이용하였다. 연구결과 두군 모두 운동 후 배근력과 균형능력은 유의하게 향상되었고, 통증은 유의하게 감소되었다. 군간 비교에서는 실험군이 대조군에 비해 균형능력이 유의하게 향상되었고, 통증은 유의하게 감소되었다. 결론적으로 요부안정화운동은 요부 불안정성을 가진 산업체 만성요통환자의 균형능력을 향상시키고 통증 감소에 긍정적인 영향을 미친다.

Evaluation of efficacy of Valsalva maneuver for attenuating propofol injection pain: a prospective, randomized, single blind, placebo controlled study

  • Background: Pain on injection is a limitation with propofol use. The effect of the Valsalva maneuver on pain during propofol injection has not been studied. This maneuver reduces pain through the sinoaortic baroreceptor reflex arc and by distraction. We aimed to assess the efficacy of the Valsalva maneuver in reducing pain during propofol injection. Methods: Eighty American Society of Anesthesiologists class I adult patients undergoing general anesthesia were enrolled and divided into two groups of 40 each. Group I (Valsalva) patients blew into a sphygmomanometer tube raising the mercury column up to 30 mmHg for 20 seconds, while Group II (Control) patients did not. Anesthesia was induced with 1% propofol immediately afterwards. Pain was assessed on a 10-point visual analog scale (VAS), where 0 represented no pain, and 10, the worst imaginable pain, and a 4-point withdrawal response score, where 0 represented no pain, and 3, the worst imaginable pain. Scores were presented as median (interquartile range). Results: We analyzed the data of 70 patients. The incidence of pain was significantly lower in the Valsalva than in the control group (53% vs. 78%, P = 0.029). The withdrawal response score was significantly lower in the Valsalva group (1.00 [0.00-1.00] vs. 2.00 [2.00-3.00], P < 0.001). The VAS score was significantly lower in the Valsalva group (1.00 [0.00-4.00] vs. 7.00 [6.25-8.00], P < 0.001). Conclusions: A prior Valsalva maneuver is effective in attenuating injection pain due to propofol; it is advantageous in being a non-pharmacological, safe, easy, and time-effective technique.

한.양약의 병행투여가 요추간판탈출증의 보전적치료에 미치는 영향 (Efficency of Simultaneous Administration of Herb Medicine and NSAIDs in Herniated Lumbar Intervertebral Disc)

  • Objectives : Herniated lumbar intervertebral disc (HIVD) is one of the cause of low back pain and radiculopathy. To evaluate the efficiency of simultaneous administration of herb medicine and non-steroid-anti-inflamatory drugs (NSAIDs), we investigated 48 HIVD patients confirmed by CT or MRI and treated with acupuncture, bed rest, herb medicine, physical therapy and NSAIDs. Methods : The patients were divided into four groups as follows : The A group were 11 cases treated with only herb medicine A. The A+W group were 14 cases treated with herb medicine A and NSAIDs. The B group were 11 cases treated with herb medicine B. The B+W group were 12 cases treated with herb medicine Band NSAIDs. We evaluated the pain degree by visual analog scale (VAS). Results : The mean VAS was reduced to $4.89{\pm}2.62$ after 2 weeks therapy and pain decrease was the most prominent in the group having within a week duration ($3.25{\pm}2.49$). Pain scale was decreased to $4.57{\pm}2.95$ in A+W group, $4.91{\pm}1.97$ in A group, $4.72{\pm}2.68$ in B group, and $5.41{\pm}2.67$ in B+W group after 2 weeks therapy. Conclusions : These results indicate that oriental medical theraphy is useful enough to treat the HIVD patients and herb medicine decrease the pain of the HIVD effectively. Therefore, the more research about herb medicine for pain killing drugs should be done.

급성기(急性期) 요통(腰痛)의 치료(治療)에 자락요법(刺絡療法)의 병행(竝行)이 미치는 영향(影響) (The Effect of the Treatment with Venesection on Treating Low Back Pain of Acute Stage)

  • Objective : This study is designed in order to evaluate the therapeutic effect of venesection on acute low back pain. Methods : From Nov. 1st 2003 to Apr. 30th 2004, 40 cases of low back pain patients at acute stage were divided into 2 groups; one group(control group) took only acupuncture therapy, and the other group(test group) took acupuncture and venesection therapy. Pain rating score(PRS) was used to assess the pain for its intensity, frequency, duration, and aggravating factors. And visual analog scale(VAS) was used, too. PRS and VAS was checked twice, before the treatment and after the treatment. Treatment was done twice a day. Results : Both acupuncture therapy and acupuncture with venesection therapy showed good effect on low back pain of acute stage. And the latter group showed better effect on decreasing pain than former. It was proved by the difference between PRS and VAS checked before treatment and what checked after treatment. But it was not proved statistically. Conclusion : Only acupuncture and acupuncture with venesection can be recommended as a useful therapy to treat low back pain of acute stage.

  • Objective : This study is designed in order to evaluate the therapeutic effect of Scalp Acupuncture and Do-ma Acupuncture related with Governer Vessel(Dok) and Bladder Meridian(Chok-taeyang) on acute low back pain Methods : We investigated 10 patients suffering from acute low back pain were treated from Mach. 1st 2005 to Jun. 30th 2005. 10 patients, were diagnosed only straightened curvature on lateral view of lumbar spine X-ray and get Scalp acupunture related with Governer Vessel(Dok) and Bladder Meridian(Chok-taeyang) and Do-ma acupuncture. Pain rating score(PRS) was used to assess the pain for its intensity, frequency, duration, and aggravating factors. And visual analog scale(VAS) was used, too. Results : Both Scalp acupunture related with Governer Vessel(Dok) and Bladder Meridian(Chok-taeyang) and Do-ma acupuncture therapy showed good effect on acute low back pain. It was proved by the difference between PRS and VAS checked before treatment and what checked after treatment. In the evaluation of treatment effect, Exellent was 2(20%), Good was 5(50%), Moderate was 3(30%), Poor was 0(0%). Conclusion : The therapeutic effect of Scalp Acupuncture and Do-ma Acupuncture on acute low back pain can be recommended as a useful therapy to treat acute low back pain.
