Purpose: This study was conducted in order to present basic policy data for introduction of a home-based physical therapy (HBPT) policy. Methods: The study surveyed physical therapists and patients for six weeks, in order to identify satisfaction with physical therapy. The statistical significance of the management plan with regard to costs and cost estimation and payment methods was determined and managed according to the operating principals of HBPT, eligibility criteria, number of visits, treatment time, and team approach. Results: Through the survey all groups of patients and physical therapists recognized the need for HBPT. They felt that the most desirable methods for activation of physical therapy visits involve precise diagnosis and evaluation through a team approach. While making regular visits to the patient three times a week, an expert visiting physical therapist with 3 to 5 years of experience in the clinical field could provide central nerve developmental treatment in less than 60 minutes; this service could be provided at a reasonable cost, minimizing unnecessary hospital visits, reducing time, medical expenses and facilitating a quick recovery due to psychological satisfaction resulting from a decrease in the psychological anxiety patients often experience in medical institutions. Conclusion: Based on the study results provided above, if HBPT is to be introduced, appropriate pricing and programs should be based on the results of pilot projects.

Background: The PyeongChang 2018 Winter Paralympic Games (WPG) being one of the most successful Paralympic Games (PG) in modern athletic world history, hosted the largest number of elite athletes representing 49 National Paralympic Committees (NPCs). Objects: The present investigation highlighted the demographic and clinical characteristics of injured athletes and non-athletes and the physiotherapy services provided during the PyeongChang 2018 WPG. Methods: Prospective descriptive epidemiology study, in which the study group comprised of 201 participants (51 athletes and 150 non-athletes) who were admitted to and utilized the polyclinic physiotherapy service of 2018 PyeongChang WPG in Physiotherapy Department of Paralympic Village from March 1, 2018 to March 20, 2018. Results: Qualitative frequency analysis of injury type demonstrated highest number of chronic injuries (51%, n = 100) in athletes and non-athletes. Anatomical injury site analysis revealed that the spine and shoulder areas were affected with equal frequency for athletes (54.9%, n = 14), whereas for non-athletes, the frequencies of spine and shoulder area injuries were 36.7% (n = 55) and 26% (n = 39), respectively. The Pyeongchang WPG showed a high rate of athletes visiting the physiotherapy service during the pre-competition period (33.3%, n = 50), which may have led to smaller incidence rate of traumatic injury. The physiotherapy treatment service analysis demonstrated that manual therapy (35.4%, n = 230) was most commonly utilized, followed by transcutaneous electrical nerve stimulation/interference current therapy (TENS/ICT), therapeutic massage and therapeutic exercise. Conclusion: We established the importance of prophylactic and preventive physiotherapy services to reduce the risk of sports injuries during WPG.
Purpose: This study was conducted to identify health problems and support received from the health and welfare service using MDS-HC(Minimun Data Set for Home Care) in the aged living at home. Method: Eighty-one elderly persons were selected from those listed in community-welfare service centers in Seoul and Kyunggi Province between December 2002 and January 2003. Result: Eleven health problems per elderly person on average were identified, and the frequent care needs were in order: preventive health measure, health promotion, visual function, depression & anxiety, communication disorders, social function, pain, environmental assessment, oral health, cognition and falls. The number of health problems by the level of ADL was ‘ADL 1(Independence)’ 9.87, ‘ADL 2(Partial independence)’ 12.78, ‘ADL 3(Dependence)’ 13.73. Utilization of formal health & welfare services among the elderly was ‘meals on wheels’ 40.7%, ‘home helper’ 38.2%, ‘visiting of social welfare worker’ 21.0%, ‘physical therapy’ 19.6%, ‘day care center’ 12.3%, ‘volunteer's service’ 9.9%, ‘home visiting care’ 3.7%, ‘occupational therapy’ 3.7%, and ‘speech therapy’ 2.5%. To closely examine the effects of the massage treatment as one method of curing hypertension by exploring the effects that one-time circulation-centered massage has on a hypertensive's blood pressure, this study selected as the subject ten among fifty-year-old essential hypertensives visiting N clinic, and measured and compared their blood pressures before and after I gave them massage. After stabilizing them, I measured their blood pressures. In their prone and supine postures, I gave them rubbing massage for twenty minutes from positions proximal to their hearts to ones distal from their hearts to help their artery circulation and circulation-centered stroking massage for ten minutes from positions distal from their hearts to ones proximal to their hearts to help their vein circulation. After the massage, I had them rest for five minutes and measured their blood pressures. Regarding each item analyzed, I calculated the mean and standard deviation before and after the massage. The difference before and after the massage was tested by corresponding t-test and the level of significance was made p<0.05. The findings are as follows. First, systolic pressure was $157.70{\pm}7.48$ before the massage and $143.00{\pm}9.39$, which is less than before the massage, after the massage, showing statistically significant difference (p<.05). Secondly, diastolic pressure was $97.80{\pm}6.56$before the massage and $95.20{\pm}5.18$, which is less than before the massage, after the massage, not showing statistically significant difference (p<.05).
This study aims to investigate the awareness and needs of patients as consumers and therapists as providers on the introduction of home physical therapy in the long-term care insurance. The survey participants were 96 patients and 132 physical therapists to a questionnaires, we analyzed for frequency of each questionnaire about awareness and necessity. As a result of the test, they considered that physical activity support services provided by nurse and nurses aid ware not specialized enough(pt 60,4%, PT 75,4%) and quality therapy should be available through a home physical therapy(pt 47.9%, PT 59.2%). Also, patients responded that the priorities to improve long-term care insurance were given to lowering expenses(35,4%) and expanding coverage of subjects(32.3%) while the physical therapists responded that professionals should be expanded(73.8%) and diverse rehabilitation services(20.2%) and a rehabilitation team should be introduced. They responded that the reasons of home physical therapy were needed that it was hard for patients to visit medical centers(PT 30.0%) themselves and it would be improvable for them to get physical function in elderly(pt 47.1%) through the service. In conclusion, as most patients as well as physical therapists responded that home visit therapy services should be introduced and their awareness of it was very high, it is suggested that the service should be quickly introduced as soon as possible.
