신생아집중치료실 미숙아 부모의 자율면회 방문 현황과 부모 스트레스와의 상관관계 (Correlations between a Flexible Parental Visiting Environment and Parental Stress in Neonatal Intensive Care Units)

  • Purpose: The purpose of this study was to examine the correlation between a flexible parental visiting environment and parental stress in neonatal intensive care units (NICUs). Methods: The study participants included 60 parents of premature infants in NICUs. Structured questionnaires and interviews, as well as observations by researchers using a caregiving behavior checklist, were used to measure the flexibility of the parental visiting environment and parents' stress levels. Quantitative and qualitative data were collected concurrently and were initially analyzed as separate data sets. Data collection extended from March 11, 2018 to June 30. 2018 and the data were analyzed using descriptive statistics, the independent t-test, one-way analysis of variance, and Pearson correlation coefficients. Results: There was a negative correlation (r=-.30, p=.021) between parental stress and the total number of visits in 7 days. We also found that the average duration of each visit and the number of caregiving behaviors performed by parents were positively correlated (r=.73, p<.001). Conclusion: When designing a flexible visiting environment for parents, parents should be encouraged to visit their babies. By doing so, stress can be reduced both for babies and for parents. Therefore, it is suggested that the related polices and regulations in South Korea should be changed to provide more a flexible visiting environment to promote better parent-child attachment and family adjustment.

도시 저소득층 만성질환노인을 위한 보건소 방문간호서비스의 경제성 분석 (Economic Evaluation of Visitng Nurse Services for the Low.;.income Elderly with Long-term Care Needs)

  • Purpose: This study aimed to evaluate economic viability of public health center visiting nurse services for the low-income elderly with long-term care needs. Method: The sample consisted of 252 community dwelling elderly who enrolled in public health center visiting nurse services for three months or more. Data was collected on physical (ADL and IADL) and cognitive impairments of the elderly, contents and frequency of visiting nurse services, cost per visit, and costs of alternative services for long-term care. Result: The mean score of ADL and IADL levels of the elderly was 2.80.4904, which indicated these patients were mostly independent. Eighty four percent of the elderly subjects were cognitively intact. Among visiting nurse services supplied, providing assessment was 34%, followed by education and counseling 26%, medication 22%, and referral. The mean cost per visit was 17,824.1 won, which transformed into a total cost per person per year of 161,130.2 won. Comparing the cost of a visiting nurse service with those of other long-term care alternatives, the visiting nurse service was the least costly alternative, followed by an outpatient clinic, hospital based home care, and nursing home. Conclusion: Overall, the results of the study provide evidence of the economic viability of visiting nurse services for the low-income elderly among long-term care alternatives.

방문간호사의 직무스트레스, 조직몰입 및 대처방법과 이직의도 (Job Stress, Organizational Commitment, Way of Coping and Turnover Intention among Korean Visiting Nurses)

  • Purpose: The purpose of the study was to identify factors related to turnover intention among Korean visiting nurses. Methods: The data from 192 of 208 nurses working in southern part of Korea were collected for analysis during in-service education in May 19~20, 2011. Descriptive statistics, chi-square tests, ANOVA, and logistic regression analysis were performed using SPSS 19.0 program. Results: Among the general characteristic factors, young, married, university graduation, lower satisfaction with income and longer work experience as a nurse were associated with higher odds of turnover intention. Organizational commitment was associated with low turnover intention. Way of coping was not statistically significantly associated with turnover intension. Conclusion: Stress from the organizational system was found to be the most important variable that explains the turnover intention in this study. Use of sensible communication methods and introduction of effective conflict resolution system is suggested to reduce turnover intention. Further research is recommended to identify the job demands and organizational systems of visiting nurses.

보건 및 복지영역 서비스 연계에 대한 방문간호사의 인식 (A Study on Visiting Nurses' Perception of the Service Referral between Health and Welfare)

  • This study was carried out to grasp visiting nurses' perception of the service referral between health and welfare with a view to providing the basic data for the visiting nursing activities. A questionnaire survey was conducted on public health nurses in 25 health centers in Seoul from Feb. 12, 2001 to Mar. 15, 2001. A total of 151 questionnaires were collected and they were analysed by use of SPSS/WIN 7.5. The results of the survey are as follows. 1. In general, visiting nurses were burdened with heavy workloads. On average, a visiting nurse covered 5 ‘dong's(the smallest administrative unit), 564 households, and 1223 persons. They spent almost a quarter of their working hours moving from home to home and recording the charts after home visiting. They took 30-60 minutes to provide their services when visiting homes. As for the frequency of home visiting, they were following the instructions recommended by the government. However, their services were still wasteful, not skill-oriented, in that they spent more time assessing ‘subjects’ rather than providing their ‘services’ for them. 2. As for the degree of service performance, visiting nurses scored average 2.94 and 2.28 on the four-point scale in the area of health and welfare respectively. The Pearson coefficient between the two variables was high(.56). According as the health services increased, the welfare services increased as well, which showed that the service referral between the two areas should be essential. 3. ‘The necessity of cooperation with social welfare staff’ scored average 4.49, and ‘the degree of cooperation with social welfare staff’ scored average 3.16 on the five-point scale; There was a statistically significant difference (average 1.33) between the two variables. Such a big difference between perception and practice results from the lack of political support that connects the two service areas comprehensively. Therefore it is recommendable to establish a so-called ‘Visiting Nursing Center’ in the ‘dong’ office in order to provide integrated services of health and welfare at once in cooperation with social welfare staff. That's the way to meet the public needs directly and it's more efficient as well in terms of cost-saving.

보건소 방문간호사의 타부서로의 이동 의도 -부산광역시 보건소를 중심으로- (Turnover Intention of Home Visiting Nurses of Public Health Centers in Busan)

  • This study aimed to investigate predictors of turnover intention of home visiting nurses at 16 public health centers in Busan. There are two groups of independent factors: non-work related(i. e., age, educational level, working duration as nurses in hospitals, and certificate), and work related factors(i. e., working duration as nurses in public health centers, working duration as home visiting nurse, position, number of households visited per week, workload, cooperation among staff, support by supervisors, supply of vehicles, and supply of materials). Data were collected with self-administrated questionnaires which consisted of 4 items of non-work related factors, 9 items of work related factors, and 1 item of turnover intention to other department. Data were analyzed using an univariate logistic regression and multiple logistic regression. According to the results, heavy workload (Yes vs No, OR=4.31, $95\%$ CI=1.16-16.04) was the most powerful predictor on the turnover intention. In conclusion. this result was similar to those of other studies on the predictors of turnover intention of clinical nurses at hospitals. To decrease the turnover intention among home visiting nurses at public health centers in Busan, alleviating the workload is needed.

비정규직 방문간호사의 직무불안정성이 직무만족, 조직몰입에 미치는 영향 (Effects of Non-Regular Visiting Nurses' Job Insecurity on Job Satisfaction and Organizational Commitment)

  • Purpose: The aim of this study was to identify the job insecurity, job satisfaction, and organizational commitment of non-regular visiting nurses working at public health centers, and evaluate the effects of job insecurity on the job satisfaction and organizational commitment. Methods: This study was a descriptive survey study. One hundred forty-three visiting nurses, who were non-regular hired by 16 public health centers in B city, were selected using a convenient sampling method. Results: The results were analyzed using SPSS/Win 21.0. A feeling of helplessness (among job insecurity factors) and retention intention significantly affected the job satisfaction of non-regular visiting nurses, and their explanation power was 20%. Retention intention, a feeling of helplessness, and age significantly influenced the organizational commitment, and their explanation power was 26%. Conclusion: Therefore, it would be necessary to stabilize the employment type, provide appropriate promotion and compensation for nurses according to their work performance, and reduce the number of non-regular employees to increase the job satisfaction and organizational commitment of non-regular visiting nurses.

대도시 저소득층 만성질환 노인을 위한 가정.방문간호 원가산정 - 환자 중증도 및 활동기준원가계산법(ABC) 적용 - (Estimation of Home-visiting Care Costs for Low-income Elderly with Chronic Disease in a Metropolitan City Using the Severity Classification and ABC(active-based costing))

  • Purpose: The purpose of this study was to estimate of home-visiting nursing costs for low-income elderly with chronic disease in a metropolitan city using the severity classification and ABC(active-based costing). Methods: First, the HHC activity pool was established. The performance time of each nursing activity were estimated. Second, nursing resources(labor costs, operating costs, and traffic expenses) were analyzed and nursing cost per minute was calculated. And then the cost of each activity was estimated. Third, 202 visiting cases were classified into three group by their severity. And then nursing cost per visit according to their severity was estimated. Results: 59 nursing activities were included in HHC activity pool. The average working time of 59 nursing activity was 6.7minutes and nursing cost per minute was 489 won. According severity, nursing cost per visit were in class I, 54,296 (won), class II 83,124(won), and class III 93,455(won).

충남지역 보건소 방문간호사의 직무만족도와 직무 스트레스 (A Study on Occupational Satisfaction and Stress of Visiting Nurses at District Health Centers in Chungnam Province)

  • Purpose: The purpose of this study is to identify visiting nurses' occupational satisfaction and their stress who are working at district health centers in Chungnam province. Methods: The subjects of the study were 109 visiting nurses of 16 district health centers in Chungnam province. The data were collected by self report questionnaires from March 1 to 31, 2010. The data were analyzed by Cronbach's ${\alpha}$, t-test, ANOVA (Sheffe's test), and correlation coefficient using the SPSS program. Results: The results of this study were as follows. The occupational stress level of the subjects was 46.7 and the level of subjects' occupational satisfaction was 2.46. There was a significant negative correlation between occupational satisfaction and occupational stress (r=-.627). Conclusion: The occupational stress level of the subjects, and stress levels of 'work environment', 'job insecurity' and 'organizational structure' were higher than those of the Korean women workers' standard. The level of subjects' occupational satisfaction was lower than that of the nurse practitioners. There was a significant correlation among occupational stress and 'burden of workload' and 'visiting nurse career'. The study showed that occupational stress was correlated with occupational satisfaction. The results suggest that further follow-up study on visiting nurse is necessary to relieve their occupational stress and increase occupational satisfaction.

  • Purpose: This study was attempted to provide quality improvement and revitalization of visiting nurse services by Importance-Performance Analysis of persons being provided with the services. Methods: The subjects were 350 people being provided with visiting nurse services from a long term home care institution in the kangwon Province during the period of data collection between Aug. 16 and Sep. 15, 2011. Data analysis was conducted through t-test, ANOVA, and IPA by using the SPSS/WIN 12.0 program. Results: As a result of the Importance-Performance Analysis of visiting nurse services, it turned out that the strength on reliability, assurance, and empathy should be maintained with good work and the aspects of responsiveness appeared to be intensively improved, and the tangibility proved to be subject to improvement. Conclusion: For the activation of visiting nurse services, methods for improving the performance in the intensive care area and management strategy establishment which highlights the advantages of strength maintenance area are needed.

방문간호를 통한 암환자의 위기중재 효과에 관한 연구 (The Effect of Crisis Intervention by the Visiting Nurse with Patients with Cancer)

  • This study was undertaken at Yonsei University Medical Center to identify the crisis responses and nursing problems of patients who had been diagnosed with cancer, and changing patterns of grieving over time periods, and to analyse the effectiveness of follow up care through home visiting nursing. This study was carried out in three stages. The 1st study data were collected from a total of 205 patients who had been diagnosed with cancer from Sept.1 to Dec. 31, 1987 using a cross-sectional method. The 2nd study data were collected three times from 30 patients with cancer at 4 weeks intervals from March 1 to June 31, 1988 using a longitudinal method. The 3rd study data were collected from two different groups from March 1 to June 31, 1988. One was an experimental group who was visited by nurses and the other one was a control group not visited by nurses. The subjects of the 3rd study consisted of 60 patients with cancer and a Quasi-experimental research design was used. The results were as follows ; 1. The patients did not experience one stage at a time among the five stages of grieing, denial anger, bargaining, depression and acceptance, as identified by Kubler Ross. They experienced a combination of stages, especially of the bargaining and the depression stages. This stages did not change with the passing of time. 2. The patients expressed more physical and socioecounomical problems than emotional problems. And they used more problem coping methods than emotional coping methods. 3. Follow up care through home visiting nursing positively influenced the patient's quality of life, especially their physical well-being and symptom control The patients responded positively to the home visiting nursing, stating that it was helpful In them. It was concluded that the development of a home visiting nursing program is needed for the effective home care of patients with cancer.
