  • Objectives: We compared the associations of 3 computed tomography (CT)-based abdominal adiposity indexes with non-alcoholic fatty liver disease (NAFLD) among middle-aged Korean men and women. Methods: The participants were 1366 men and 2480 women community-dwellers aged 30-64 years. Three abdominal adiposity indexes-visceral fat area (VFA), subcutaneous fat area (SFA), and visceral-to-subcutaneous fat ratio (VSR)-were calculated from abdominal CT scans. NAFLD was determined by calculating the Liver Fat Score from comorbidities and blood tests. An NAFLD prediction model that included waist circumference (WC) as a measure of abdominal adiposity was designated as the base model, to which VFA, SFA, and VSR were added in turn. The area under the receiver operating characteristic curve (AUC), integrated discrimination improvement (IDI), and net reclassification improvement (NRI) were calculated to quantify the additional predictive value of VFA, SFA, and VSR relative to WC. Results: VFA and VSR were positively associated with NAFLD in both genders. SFA was not significantly associated with NAFLD in men, but it was negatively associated in women. When VFA, SFA, and VSR were added to the WC-based NAFLD prediction model, the AUC improved by 0.013 (p<0.001), 0.001 (p=0.434), and 0.009 (p=0.007) in men and by 0.044 (p<0.001), 0.017 (p<0.001), and 0.046 (p<0.001) in women, respectively. The IDI and NRI were increased the most by VFA in men and VSR in women. Conclusions: Using CT-based abdominal adiposity indexes in addition to WC may improve the detection of NAFLD. The best predictive indicators were VFA in men and VSR in women.

  • Objectives : Our purpose of this study was to evaluate the association between abdominal adipose tissue and oriental obesity pattern identification. Methods : This study was performed in 78 healthy obese(BMI(body mass index)${\geq}25kg/m^2$) women in Seoul. Subjects underwent abdominal CT(computed tomography) scanning and were asked to complete the oriental obesity pattern identification questionnaire. Subjects were given written consent and this study was performed under permission of institutional review board of Kyung-hee East-west Neo medical center. Results : 1. VFA(visceral fat area) and VSR(visceral/subcutaneous adipose tissue ratio) were significantly correlated with stagnation of the liver qi(肝鬱, gan-yu)(p<0.05). But, other patterns were not significantly correlated with oriental obesity pattern identification(p>0.05). 2. We evaluated a difference of oriental obesity pattern identification score in visceral obesity group(n=34) versus non-visceral obesity group(n=44). Scores of all patterns except food accumulation(食積, shi-ji) were significantly higher in the visceral obesity group than in the non-visceral obesity group(p<0.05). Especially, there was a further significant difference in stagnation of the liver qi(肝鬱, gan-yu)(p<0.01). Conclusions : Generally, the stagnation of the liver qi(肝鬱, gan-yu) has a close relation with stress and depression. This study suggests that stress and depression might be correlated with visceral fat, and the use of oriental obesity pattern identification would be helpful for planning a treatment schedule of visceral obesity in the clinic.

  • Object : Waist circumference(WC), waist-hip ratio(WHR), waist-stature ratio(WSR), and body mass index(BMI) are commonly used for evaluating obesity. This Research were done to determine what is more sensitive obesity indexes(WC, WHR, WSR, BMI) Correlated with body composition such as body fat mass, body fat(%), visceral fat area, and fat free mass. And what is more sensitively correlated obesity indexes with % changes of body composition during weight reduction treatment. Methods : This clinical retrospective research were carried out 127 cases of female obese outpatients with weight reduction treatment during 1 month. Bioelectrical impedence analysis(for body composition) and body size(for anthropometric obesity indexes) were estimated in pre-treatment and post-treatment to evaluate the obesity indexes. Pearson correlation coefficients were used to select useful obesity index. Result & Conclusion : BMI is useful index for diagnosis and evaluation of obesity. WSR is sensitively correlated with visceral fat area and body fat(%). So, WSR is useful index for evaluating abdominal obesity and risk factors of metabolic syndrome. WC is correlated with both body fat mass and fat free mass. WHR is not optimal for diagnosis and evaluation of obesity.

  • Chitosan, hydroxycitrate and L-carnitine have been known to be antiobesity components. The purpose of this study was to evaluate the combined effects of chitosan, hydroxycitrate and L-carnitine mixture as a potential antiobesity supplement in overweight women. Pre-menopausal healthy females who were overweight (percent ideal body weight > 110) were included in this study. Forty-nine subjects randomly received a placebo (n = 25) or antiobesity-supplement (n = 24), which was a mixture of chitosan, hydroxycitrate, and L-carnitine. Before and after the eight-week experimental period, anthropometric parameters, blood components and computerized tomography were measured. At baseline, the two groups were well matched in terms of age, body mass index and lipid profile. After the eight weeks of potential antiobesity supplementation, the subjects' body fat percent had decreased significantly (p < 0.001) by 5.6% (39.1 $\pm$ 1 vs 36.9 $\pm$ 1%) while lean body mass increased (p < 0.01). Vsceral fat area at the L4 vertebra decreased significantly (p < 0.01) by 8.6% in the supplemented group and the total fat area at the L4 vertebra showed a tendency to decrease (p = 0.051) by 2.4%. Also, in the group given the antiobesity-supplement rather than the placebo, the fasting triglyceride level decreased significantly (p < 0.05) by 10.0%. In addition, serum total cholesterol levels in the antiobesity-supplement group showed a tendency to decrease (p=0.159) by 2.7% (194 $\pm$ 6 vs 189 $\pm$ 6 mg/dl). No side effects were found in either group during the intervention. In conclusion, the present study demonstrated that taking a mixture of chitosan, hydroxycitrate, and L-carnitine as a potential antiobesity supplement for eight weeks produced advantageous changes in the weight and visceral fat accumulation of overweight women without any side effects. • This study was performed to examine the combined effects of L-carnitine and isoflavone supplementation on weight reduction and body fat distribution in overweight women. Overweight/obese women (body mass index > $23kg/m^2$) who were not diagnosed any type of diseases were included in this study and sixty subjects ($41.1{\pm}1.5$ years, $25.9{\pm}0.3kg/m^2)$ were randomly assigned to a placebo (n=30) or a supplement group (n=30, L-carnitine 300 mg+isoflavone 40 mg/day). We measured anthropometric parameters, abdominal fat distribution by computerizd tomography and blood components before and after the 12 week intervention period. After the 12 weeks of supplementation, subjects in L-carnitine and isoflavone supplement group showed a significant reduction of body weight (p < 0.001), body fat % (p < 0.05), and waist to hip ratio (p < 0.01) whereas placebo group did not show any changes. In a CT-scanned results, total fat area at L4 level was significantly reduced by 8.1% (p < 0.01) with the reduction of visceral fat area (-11.1%, p < 0.001) and subcutaneous fat area (-7.0%, p < 0.05) in the supplement group. The supplementation of L-carnitine and isoflavone showed the significant improvement of HDL-C (p < 0.01) and apoB (p < 0.05) concentrations, however, change values in those markers were not significant compared with those of the placebo group. In addition, a significant increase of adiponectin level (p<0.001) was observed in the supplement group after the intervention. The result of present study demonstrated that supplementation of 300 mg L-carnitine and 40 mg isoflavone per day fur 12 weeks can give beneficial effects on weight reduction and visceral fat accumulation. These potential antiobesity supplement can produce more favorable effects when combined with lifestyle modification.

  • Objectives : This study was performed to evaluate the effect of the electroacupuncture therapy on abdominal fat deposit without diet, exercise and herbal medication. It was also designed to check the changes of free fatty acid in blood and catecholamine in urine to observe the mechanism of the electroacupuncture efficacy. Methods : 10 volunteers over 85 cm(male, 90 cm) in waist circumferences were recruited, and 10 electroacupuncture treatments were applied on their abdominal subcutaneous fat area for 4-5 weeks. We measured anthropometric factors, abdominal fat area with CT scanning, and the changes of free fatty acid in blood and catecholamine in urine before treatment and after the last treatment. Results : In this study, significant changes were shown in weight, waist circumference, waist/hip ratio, body mass index and body fat percent after the electroacupuncture therapy. There were also significant decreases of visceral fat area, visceral/subcutaneous fat area ratio in CT scanning. The blood level of free fatty acid and the urine level of catecholamine were increased after treatments, but the changes were not statistically significant. Conclusions : This study showed the efficacy of the electroacupuncture therapy on the abdominal fat deposit. But significant changes couldn't be found out in free fatty acid and catecholamine. Further studies that compensate for the limitations of this study are required to confirm the mechanism of the electroacupuncture efficacy.

  • In middle-aged men, abdominal obesity has been an important risk factor of coronary artery disease (CAD) as well as a predictor of hypertension, dyslipidemia, insulin resistance and glucose intolerance. Particularly, risks from abdominal obesity increase when adipose tissue accumulates in visceral compartment. Many studies showed that weight reduction by caloric restriction improves abdominal obesity and reduces lots of cardiovascular risk factors. Testosterone treatment also results in a significant decrease in visceral fat area and normalizes endocrine metabolism. However there is no study that compare the effect of caloric restriction with that of testosterone treatment. The purpose of this study is to investigate the effect of caloric restriction and that of testosterone treatment on body fat distribution, serum lipids and glucose metabolism in male patients with CAD. Forty five middle-aged overweight-obese men with CAD participated in 12 weeks' program. They were matched with age, body weight, body mass index (BMI) and divided into three groups : control group (n = 15) , caloric restriction group (-300 kcal/day, n = 15) and testosterone treatment group (testosterone undecanoate tablets, n = 15) . After 12 weeks, control group did not have any changes in anthropometries, lipid profile, body fat distribution, glucose metabolism and hormonal status. Expectedly, caloric restriction group showed decreases in body weight, BMI, waist to hip ratio, % body fat. Ten percentage of total cholesterol and 23% of triglyceride in serum were also decreased. In body fat distribution, total fat areas at both L1 and L4 levels were significantly reduced in this group without reduction in muscle of thigh and calf. However, testosterone treatment group did not have any significant changes in body weight, % body fat, serum lipid profile and abdominal fat distribution. In conclusion, weight reduction by caloric restriction is more beneficial in body fat distribution and serum lipid level than testosterone treatment in overweight male patients with CAD. This result suggests that modest weight reduction is possible to help decrease risk factors of CAD.

  • This thesis estimates relationships between area and volume of visceral adipose tissue about health risk. Visceral adipose tissue quantity showed the quantitative relationship, where degree of area was similar to the volume.Threrfore, it is more useful to use the area which is simpler, than the volume when using visceral adipose tissue quantity for estimating the degree of danger factors related with fatness.

  • Anthropometry, computed tomography(CT) at the umbilical level, nutrient intake, blood pressure, serum levels of lipids and lipoproteins and response of glucose, c-peptide, insulin, and free fatty acid(FFA) during oral glucose tolerance test(OGTT) were estimated on 11 normal-weight controls and 35 overweight and obese middle-agd men. The areas of total abdominal, subcutaneous and visceral were determined by CT scanning technique. Total abdominal fat area correlated the most significantly with the levels of serum lipids, lipoproteins and insulin among several obese indices. Compared with normal-weight controls, overweight and obese men with abdominal fat lower than 29000$\textrm{mm}^2$ showed an increase in waist-hip ratio, areas of total abdominal(35%), visceral and subcutaneous fat and C-peptide response area during OGTT, though age, percent ideal body weight, body mass index, % body fat, and all biochemical indices except C-peptide response area were not different between two groups. Overweight and obese men with abdominal fat greater than 29000$\textrm{mm}^2$ showed a higher values in total abdominal fat(85%), serum levels of triglyceride, total-and LDL-cholesterol, the ratio of LDL-to HDL-cholesterol, and response areas of FFA, insulin and C-peptide during OGTT than normal-weight controls. Overweight and obese men with great abdominal fat showed an increase in alcohol ingestion and percent calorie intake per total energy expenditure, compared with normal-weight controls. Our results indicate that obesity and a certain level of total abdominal fat accumulation is required to observe abnormal levels of serum lipids, lipoproteins and insulin in Korean middle-aged men. In addition, increased alcohol and calorie intake and decreased physical activity could partly explain total abdominal fat accumulation in men.
