Initial Clinical Experience with Cytoreductive Surgery and Hyperthermic Intraperitoneal Chemotherapy in Signet-Ring Cell Gastric Cancer with Peritoneal Metastase

  • Purpose: Cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) have been shown to improve survival in select patients with gastric cancer and peritoneal metastases. It remains unclear, however, whether this multimodal treatment protocol is also beneficial for signet-ring cell gastric cancer (SRC) patients with peritoneal metastases. Materials and Methods: Clinical data of patients scheduled for upfront systemic chemotherapy consisting of 5-FU (2,600 $mg/m^2$), folinic acid (200 $mg/m^2$), docetaxel (50 $mg/m^2$), and oxaliplatin (85 $mg/m^2$) followed by CRS and HIPEC using cisplatin (50 $mg/m^2$) at the Comprehensive Cancer Center, University Hospital T$\ddot{u}$bingen, Germany were retrospectively analyzed. Results: Eighteen consecutive patients for whom irresectability has been ruled out by a computed tomography scan were enrolled. However, complete cytoreduction could only be achieved in 72% of patients. When categorizing patients with respect to the completeness of cytoreduction, we found no difference between both groups considering tumor- or patient-related factors. The overall complication rate following complete cytoreduction and HIPEC was 46%. Within a median follow-up of 6.6 (0.5~31) months, the median survival for CRS and HIPEC patients was 8.9 months as opposed to 1.1 months for patients where complete cytoreduction could not be achieved. Following complete cytoreduction and HIPEC, progression-free survival was 6.2 months. Conclusions: In SRC with peritoneal metastases, the prognosis appears to remain poor irrespective of complete CRS and HIPEC. Moreover, complete cytoreduction could not be achieved in a considerable percentage of patients. In SRC, CRS and HIPEC should be restricted to highly selective patients in order to avoid exploratory laparotomy.

Relation of Physical Activity and Visceral Adipose Tissue Accumulation in Korean Obese and Overweight Women (한국 비만 및 과체중 여성에서 내장 지방과 신체 활동 사이의 연관성)

  • Objectives Physical activity is known to be beneficial to prevent weight gain and to be physically healthy. In this study, we tried to find out the relationship between body composition with physical activity. Methods International Physical Activity Questionnaire(IPAQ)-short form was given to premenopausal women aged 20~55 and BMI${\geq}23\;kg/m^2$. Subcutaneous and visceral adipose tissue of the subjects by CT scanning was obtainedin a single slice. They were given written consent and this study is performed under the permission of institutional review board of Kyung Hee East-west Neo Medical Center. Results The mean of physical activity of total subjects was 1935.18 MET-min/week(n=63). When three groups of physical activity pattern(inactive, minimally active, and HEPA(Health-enhancing physical activity) active) was analyzed, all of them were heavily relied on the activity intensities of walking. The VSR(visceral/Subcutaneous adipose tissue Ratio) was correlated with total physical activity in vigorous activity group. Conclusions The physical activity of Korean obese and overweight women was on the average, but the vigorous activity is thought to be necessary. The vigorous physical activity was related with visceral adipose tissue.

Correlation between Abdominal Fat Distribution and Abdominal Temperature in Korean Premenopausal Obese Women

  • Objectives: Considering that homeothermy is a major component of metabolic rate, body temperature might play a role in the pathophysiology of obesity. This study aimed to determine the relationship between abdominal fat distribution and abdominal temperature in Korean, premenopausal, obese women. Methods: Weight and height were measured in 26 premenopausal, obese women to calculate body mass index (BMI). Obesity was defined as a $BMI{\geq}25kg/m^2$. Waist circumference (WC) was also measured as well as abdominal fat by computed tomography (CT) and abdominal temperature by digital infrared thermographic imaging (DITI). Results: Visceral abdominal fat area was found to have a significant negative correlation with the temperature of Guanyuan (CV4, lower abdomen acupoint). We also found the visceral-subcutaneous fat ratio had a significant negative correlation with the temperature of CV4 and Right Tianshu (RST25, lateral navel acupoint). Only visceral fat and its ratio to subcutaneous fat had a significant correlation with abdominal temperature. Subcutaneous fat area and total fat area were not correlated with abdominal temperature. Conclusions: This study suggests that abdominal visceral fat has a significant negative correlation with abdominal temperature. Further study is needed to uncover the relationship between abdominal fat distribution and temperature regulation in obese individuals and to define the role of body temperature in the pathogenesis of obesity.

The Effect of A-3826G Polymorphism of Uncoupling Protein-Ion Visceral Fat Area in Overweight Korean Women

  • Uncoupling protein-1 (UCP-1) plays a major role in thermogenesis, and has been implicated in the pathogenesis of obesity and metabolic disorders. The aim of this study was to estimate the effects of A-3826G polymorphism of UCP-1 gene on body fat distribution. Two hundred forty eight Korean female overweight subjects with BMI more than 25 kgfm2 participated in this study. The areas of abdominal subcutaneous and visceral fat of all subjects were measured from computed tomography cross sectional pictures of the umbilical region. Subcutaneous fat areas of upper and lower thigh were also measured. Body composition was measured by bio-impedance analysis, and serum concentrations of biochemical parameters, such as glucose, triglyceride, cholesterol etc, were also measured. Genotype of UCP-1 was analyzed by polymerase chain reaction (PCR) restriction fragment length polymorphism (RFLP) method. The frequencies of UCP-1 genotypes were AA type; $27.8\%,\;AG\;type;\;51.2\%\;and\;GG\;type;\;21.0\%,$ and the frequency of G allele was 0.47. Body weight, BMI, WHR, SBP, DBP and body compositions were not significantly different by UCP-1 genotype. Abdominal visceral fat area was significantly higher in AG and GG type compared with AA type (p=0.009), but subcutaneous fat areas were not significantly different by UCP-1 genotype. Among biochemical parameters, LDL cholesterol level was significantly higher in GG type compared with AA and AG types (p=0.033). Among all subjects, 121 subjects finished 1 month weight loss program containing hypocaloric diet and exercise. The reduction of body weight and BMI were lower in GG type compared with AA/AG type even though statistical significances were not found (p > 0.05). These results suggest that UCP-1 genotype has a significant effect on visceral fat accumulation among Korean female overweight subjects with BMI more than $25\;kg/m^2$.

  • In order to research the interrelationship of symptoms of 5 visceral diseases through Pearson's correlative coefficient and to understand the diagnostic importances of each symptom and sexual differences in the main symptoms of each visceral diseases using Cronbach alpha. For this 500 female and 120 male cases of questionnaire were collected and analyzed. In the main symptoms of 5 visceral diseases, there were a little differences between this results and existing symptom-complex system. And in the comparison of most frequently manifesting symptoms to male and female subject, Heart and Pancreas and Kidney disease were almost same, but Liver disease was different and Lung disease was slightly different. Conclusively, the diagnostic reliability of the abstracted symptom-complex of 5 viscera was graded from Kidney(Cronbach alpha 0.8771555), Pancreas(0.865978), Liver(0.815013), Heart(0.78653) to Lung disease(0.6294695) sequentially. And the each Cronbach alpha was valuable to be permitted as diagnostic criteria of 5 visceral diseases.

A Study on the Exploration of Treatment Area of Visceral Chuna Manual Therapy Using Ultrasound Image Data (초음파 영상 데이터를 활용한 내장기 추나요법 시술 부위 탐색 연구)

  • Objectives This study was conducted to confirm anatomical information near the treatment areas of visceral chuna manual therapy and to secure stability and effectiveness during the treatment. Methods For 50 healthy adult men and women, a total of 13 ultrasound images were taken of the 7 treatment areas which are the representative treatment areas of visceral Chuna manual therapy. Results The treatment area of the bottom of the liver can be palpated around the right side ST19 and CV14. The treatment area of the gallbladder and the common bile duct can be palpated around the right side SP16 and ST20. The treatment area of the cardia and the pylorus can be palpated around the left side KI21, right side ST20, ST21, KI19, KI20, CV12, and CV13. The origin point of the mesentery root can be moved to the left and lower left from CV12 and can be palpated. The treatment area of the ileocecal valve and the cecum can be palpated around the right side SP14. The treatment area for the colic flexure can be palpated around the both side LR13. The treatment support point for the kidneys can be palpated around both side BL51. Conclusions It is thought that if the ultrasound image data from the meridians around the treatment site is used as an auxiliary, it will be more effective in terms of safety and effectiveness during the treatment of visceral Chuna manual therapy.

Prognostic Analysis of Stage I Non-Small Cell Lung Cancer Abutting Adjacent Structures on Preoperative Computed Tomography

  • Background: Early non-small cell lung cancer (NSCLC) that abuts adjacent structures requires careful evaluation due to its potential impact on postoperative outcomes and prognosis. We examined stage I NSCLC with invasion into adjacent structures, focusing on the prognostic implications after curative surgical resection. Methods: We retrospectively analyzed the records of 796 patients who underwent curative surgical resection for pathologic stage IA/IB NSCLC (i.e., visceral pleural invasion only) at a single center from 2008 to 2017. Patients were classified based on tumor abutment and then reclassified by the presence of visceral pleural invasion. Clinical characteristics, pathological features, and survival rates were compared. Results: The study included 181 patients with abutting NSCLC (22.7% of all participants) and 615 with non-abutting tumors (77.3%). Those with tumor abutment exhibited higher rates of non-adenocarcinoma (26.5% vs. 9.9%, p<0.01) and visceral/lymphatic/vascular invasion (30.4%/33.1%/12.7% vs. 8.5%/22.4%/5.7%, respectively; p<0.01) compared to those without abutment. Multivariable analysis identified lymphatic invasion and male sex as risk factors for overall survival (OS) and disease-free survival (DFS) in stage I NSCLC measuring 3 cm or smaller. Age, smoking history, vascular invasion, and recurrence emerged as risk factors for OS, whereas the presence of non-pure ground-glass opacity was a risk factor for DFS. Conclusion: NSCLC lesions 3 cm or smaller that abut adjacent structures present higher rates of various risk factors than non-abutting lesions, necessitating evaluation of tumor invasion into adjacent structures and lymph node metastasis. In isolation, however, the presence of tumor abutment without visceral pleural invasion does not constitute a risk factor.

A Study on the Reliability and Validity of the Five-Visceral Weak Children Questionnaire for Children with Anorexia (식욕부진 소아 대상 오장허약아 설문지의 신뢰도 및 타당도 연구)

  • Objectives This study aimed to assess the reliability and validity of the Five-Visceral Weak Children Questionnaire (FWCQ) in childhood anorexia and to propose optimal cutoff values. Methods We conducted two surveys, spaced one month apart, targeting the parents of 366 children aged 2-9 years who had been diagnosed with anorexia for at least one month. To evaluate the reliability of the FWCQ, we calculated Cronbach's alpha coefficient and employed the test-retest method. Additionally, correlation analysis was performed between the FWCQ and each visceral question, and discriminant validity was assessed by comparing responses from the anorexic and normal groups. Furthermore, we determined the cutoff value of the spleen weak score, which is closely associated with appetite, for the anorexic group. Results The first survey yielded responses from 336 participants, followed by 171 responses in the second survey conducted one month later. The FWCQ demonstrated high internal consistency (Cronbach's alpha = 0.776), and test-retest results were consistent. The correlation coefficients between the FWCQ and each visceral question were all statistically significant. Moreover, significant discriminant validity was observed between the anorexic and normal groups in the FWCQ score and the heart, spleen, and kidney weak scores. The optimal cutoff value for the spleen weak score in the anorexic group was found to be 5.5 points (with a sensitivity of 73.44% and specificity of 66.47%). Conclusions The FWCQ exhibited strong reliability and validity in children with anorexia.

The Effect of Nutrition Education on Visceral Fat Reduction and Diet Quality in Postmenopausal Women (폐경 여성의 내장지방 및 식사의 질에 미치는 영양 교육의 효과)

  • This study investigated the effects of the nutrition education on body weight, visceral fat and diet quality in the postmenopausal women. The subjects (n = 101) were randomly divided into two groups: Nutrition education + Exercise (NEE) group (n = 51) and Exercise only (EO) group (n = 50). Nutrition education was consisted of counseling in portion control, food selection for low carbohydrate, high fiber food items and for the improvement in micronutrient intakes and diet quality. After 6 months, the reduction in the body weight and visceral fat area was significantly greater in the NEE than in the EO group. The NEE subjects were further divided into two groups according to the amount of visceral fat area reduction; high visceral fat area loss (HVL) group with a visceral fat area reduction 2.35% or greater and low visceral fat area loss (LVL) group with a reduction less than 2.35%. In the HVL group, the reduction in body weight, BMI, percent body fat, waist to hip ratio and visceral fat area was significantly greater than that in the LVL group. We observed a significant increase in the serum HDL-cholesterol level and a decrease in systolic blood pressure, fasting blood sucrose, total and LDL-cholesterol levels in the HVL group compared to the LVL group. The energyadjusted protein, fiber, calcium, vitamin $B_6$, vitamin C, vitamin E intakes were significantly increased in the HVL compared to LVL group. The index of nutritional quality (INQ) and mean adequacy ratio (MAR) were also increased in the HVL group compared to the LVL group. These results show that our nutrition education program was an effective intervention measure for the reduction of body weight and visceral fat, blood pressure, glucose and lipid levels in the blood and also for the improvement of nutrient intake and diet quality in postmenopausal women who are overweight.

  • Background : To assess the utility of HRCT in the evaluation of visceral pleural invasion and to determine whether visceral pleural invasion and bronchovascular bundle thickening on the same lobe could be related to the recurrence and survival in non-small cell lung cancer (NSCLC) Method: Eighty one patients, which were fulfilled long-term follow-up at least 18 months (maximum 103 months) among which 434 patients had underwent curative surgical resection for NSCLC from 1986 to 1995, were studied. They were analyzed to evaluate whether the prognostic factors such as the recurrence and survival depend on visceral pleural invasion and bronchovascular bundle thickening to the same lobe. Thirty two patients adjacent to a chest wall or a fissure were evaluated for visceral pleural invasion by HRCT. CT criteria included abutting pleura along the chest wall, abutting and/or compressing fissure, croosing fissure, and pleural tail. Results: The positive predictive value and the negative predictive value of crossing fissure were 100% and 100%, respectively. Two patients showing spiculated interface between a mass and abutting fissure were confirmed to have visceral pleural invasion at surgery. Visceral pleural invasion confirmed at surgery was significant to local recurrence and survival (p<.05. p<.05, respectively). Brochovascular bundle thickening to the same lobe on CT scan was significant to survival (p<.05) but was not significant to local and distant recurrence (p>.05). Conclusion : Visceral pleural invasion and bronchovascular bundle thickening to the same lobe have a role in predicting prognosis such as recurrence and survival in NSCLC. Therefore, the analysis of visceral pleural invasion on CT scan and the pathological analysis of bronchovascular bundle thickening to the same lobe may be necessary to predict the prognosis in NSCLC.
