정신분열병 환자의 인지적/행동적 특성평가를 위한 가상현실시스템 구현 (A Virtual Reality System for the Cognitive and Behavioral Assessment of Schizophrenia)

  • 정신분열병은 망상이나 환각과 같은 양성증상과 감정적 둔마와 같은 음성증상이 대표적인 사고장애로서 외부입력 정보를 통합하거나 체계적으로 처리하는 능력이 매우 부족하다. 즉, 정신분열병 환자는 시각, 청각, 촉각 등의 자극을 종합하고 통합하여 인지하지 못한다. 본 연구에서는 뇌 인지 통합 모델(Brain Multimodal Integration Model)에 기반하여 정신분열병 환자의 인지 능력을 측정하기 위한 가상현실시스템을 제안한다. 정신분열병 환자의 지각, 인지, 운동능력을 측정하기 위한 가상현실시스템은 환자에게 시각과 청각의 멀티모달 자극을 제시하여, 환자로 하여금 일정시간 동안 자극을 기억하고 처리하여 주어진 과제를 수행하도록 하였다. 수행 결과를 통해 환자의 멀티모달 자극 통합능력 및 작업기억 통합능력, 네비게이션 능력을 평가한다. 임상연구를 통해 개발된 가상현실시스템을 WCST과 같은 기존 검사방법들과 비교하여 검증하였는데, 가상현실로 측정한 파라미터와 WCST의 파라미터 및 SPM 점수 사이에 매우 유의미한 상관관계를 보여 가상현실시스템의 유용성을 확인할 수 있었다.

  • Virtual Reality(VR) is a new technology which makes humans communicate with computer. It allows the user to see, hear, feel and interact in a three-dimensional virtual world created graphically. In this paper, we introduced VR into psychotherapy area and developed VR system for the exposure therapy of acrophobia. Acrophobia is an abnormal fear of heights. Medications or cognitive-behavior methods have been mainly used as a treatment. Lately the virtual reality technology has been applied to that kind of anxiety disorders. A virtual environment provides patient with stimuli which arouses phobia, and exposing to that environment makes him having ability to over come the fear. In this study, the elevator stimulator that composed with a position sensor, head mount display, and audio system, is suggested. To illustrate the physiological difference between a person who has a feel of phobia and without phobia, heart rate was measured during experiment. And also measured a person's HR after the virtual reality training. In this study, we demonstrated the subjective effectiveness of virtual reality psychotherapy through the clinical experiment.

Effect Analysis of Virtual-reality Vestibular Rehabilitation based on Eye-tracking

  • Vertigo is one of the most common complaints encountered by physicians and the patients are steadily increasing. These patients are exposed to the risk of secondary accidents such as falls due to vertigo. There are two ways to improve this symptom: medication and rehabilitation. Although temporary symptomatic improvement may be expected in patients treated with medication, vertigo may recur and medication can delay central compensation. In contrast vestibular rehabilitation exploits central mechanisms of neuroplasticity to increase postural stability and enhance visual-vestibular interactions in situations that generate conflicting sensory information. However, vestibular rehabilitation may be compromised by incorrect performance of exercises, and there is a need for active effort and interest from the patient during rehabilitation. To solve these problems, we decided to apply FOVE HMD for eye-tracking and Unity3D to create virtual reality. The proposed eye-tracking based algorithm calculates the concentration of users with eye tracking data and calculates the motion width of the patient with nystagmus, thus the severity of the patient according to the score can be determined. According to our experimental test against healty group and patients group, this result showed the meaningful data to use define the contents result.

  • This study addresses the effectiveness of a VR model that is designed to relieve distress and improve communication during pediatric dental treatment for 3-9 year-old young patients. This is due to the challenging nature of the dental treatment for both young patients and pediatric dentists. The proposed model is designed to alleviate the unpleasant experience a young patient might go through during the dental treatment through providing a VR content to be displayed in on a head-mounted device during the treatment session. In addition, the model is designed to help decrease the frequent movement of the young patient during the dental treatment session by directing the patient's head to the immersive VR content. Furthermore, the model could solve the communication difficulties that might occur between pediatric dentists and young patients during the treatment session by showing live instructions related to adjusting body postures

  • 의료 산업의 발전으로 환자 친화적 의료 환경 구축과 공중보건 의료 서비스의 개선이 이루어지고 있는데 이러한 의료 환경 변화 속에서 다양한 의료 서비스를 받는 아동의 통증에 대한 관심이 필요하다. 아동 환자의 통증은 기존의 통증 예방 및 관리 방법인 비약물적 관리방법과 약물적 관리방법을 통해 조절되고 있다. 국·내외적으로 비약물적 관리방법 중에 주의전환 요법의 일종으로 가상현실(Virtual Reality; VR)의 적용이 대두되고 있는 실정이다. 본 연구는 국·내외 VR 콘텐츠의 개발 사례와 관련 연구들을 고찰하고, 분석하여 아동을 위한 VR의 임상적용 시에 고려할 특성으로 '존재', '쌍방향 참가', '사회적 상호작용', '주문제작', '구체화'를 도출하였다. 이를 바탕으로 아동의 통증 감소를 위한 6개의 VR 콘텐츠 활성화 방안을 제안하였다.

The Virtual Waiting Time of the M/G/1 Queue with Customers of n Types of Impatience

  • We consider M/G/1 queue in which the customers are classified into n+1 classes by their impatience time. First, we analyze the model of two types of customers; one is the customer with constant impatience duration k and the other is patient customer. The expected busy period of the server and the limiting distribution of the virtual waiting time process are obtained. Then, the model is generalized to the one in which there are classes of customers according to their impatience duration.

Computational analysis of the electromechanical performance of mitral valve cerclage annuloplasty using a patient-specific ventricular model

  • We aimed to propose a novel computational approach to predict the electromechanical performance of pre- and post-mitral valve cerclage annuloplasty (MVCA). Furthermore, we tested a virtual estimation method to optimize the left ventricular basement tightening scheme using a pre-MVCA computer model. The present model combines the three-dimensional (3D) electromechanics of the ventricles with the vascular hemodynamics implemented in a lumped parameter model. 3D models of pre- and post-MVCA were reconstructed from the computed tomography (CT) images of two patients and simulated by solving the electromechanical-governing equations with the finite element method. Computed results indicate that reduction of the dilated heart chambers volume (reverse remodeling) appears to be dependent on ventricular stress distribution. Reduced ventricular stresses in the basement after MVCA treatment were observed in the patients who showed reverse remodeling of heart during follow up over 6 months. In the case who failed to show reverse remodeling after MVCA, more virtual tightening of the ventricular basement diameter than the actual model can induce stress unloading, aiding in heart recovery. The simulation result that virtual tightening of the ventricular basement resulted in a marked increase of myocardial stress unloading provides in silico evidence for a functional impact of MVCA treatment on cardiac mechanics and post-operative heart recovery. This technique contributes to establishing a pre-operative virtual rehearsal procedure before MVCA treatment by using patient-specific cardiac electromechanical modeling of pre-MVCA.

가상현실을 이용한 수술 전 안내 및 수술실 환경 체험이 수술 전 정보만족도와 불안 감소에 미치는 효과 (The Effect of Preoperative Guidance and Operating Room Environment Experience Using Virtual Reality on Satisfaction with Preoperative Information and Anxiety Reduction)

  • Purpose: The purpose of this study was to determine the effect of preoperative guidance and operating room environment experience using virtual reality on increasing satisfaction with information and reducing anxiety in preoperative patients undergoing general anesthesia and local anesthesia. Methods: A non-equivalent control group quasi-experimental design was employed. The participants were 80 surgical patients from 4 wards (40 experimental group and 40 control group) of the general hospital located in Gyeonggi-do. Data collection was conducted from June to November 2023 after completing the control group survey in January 2023. Data were analyzed using Chi-square, t-test, and Mann-Whitney U test using SPSS 23.0 program. Results: Satisfaction with preoperative information was higher in the experimental group than that of the control group. Additionally, anxiety related to surgery in the experimental group was significantly lower than that of the control group. The preoperative state anxiety score in the experimental group was not significantly lower than that of the control group. Conclusions: These results suggest that providing patient education and information using virtual reality technology can not only alleviate patients' anxiety related to surgery, but also have the potential to be used as an effective intervention to improve positive patient experiences.

심한 교모와 구치부 상실을 보이는 환자의 전악 수복: Jaw motion tracking과 digital workflow를 활용한 증례 보고 (Full-mouth rehabilitation of severely attrited dentition with missing posterior teeth: a case report using digital workflow with jaw motion tracking)

  • 최근 다양한 증례 보고에서 소개되고 있는 jaw motion tracking은 환자의 안궁 이전 및 개별화된 하악 운동 경로를 기록한 후, 이를 computer-aided-design/computer-aided-manufacturing (CAD-CAM) 소프트웨어의 가상공간상에 재현하는 방법이다. 본 증례의 환자는 오랜 기간 구치부의 상실로 인해 교합평면의 붕괴가 관찰되었기에, 수직 교합 고경의 증가를 동반한 완전 구강 회복술을 계획하였다. 우선 jaw motion tracking을 진행하여 새로운 중심위 상에서 환자의 하악 운동을 기록한 후, 이 정보를 환자의 초진 구내 데이터 및 3차원 안면 스캔 데이터와 조합하여 가상 환자를 생성하였다. 가상 환자 상에서 진행한 디지털 왁스업과 임플란트 식립 계획을 바탕으로 임시 보철물을 제작하였다. 새롭게 설정된 수직 교합 고경 상에서 적절한 견치 유도를 보이는 임시 보철물의 검증을 통해, 최종 보철물로 이행하였다. 이처럼 디지털 치의학 기술의 장점을 활용하여 환자는 저작능과 심미성의 개선에 만족하였기에 본 증례를 보고하고자 한다.
