The Influence of Pituitary Adenoma Size on Vision and Visual Outcomes after Trans-Sphenoidal Adenectomy : A Report of 78 Cases

  • Objective : The aims of this study were to investigate the quantitative relationship between pituitary macroadenoma size and degree of visual impairment, and assess visual improvement after surgical resection of the tumor. Methods : The medical records of patients with pituitary adenoma, who had undergone trans-sphenoidal adenectomy between January 2009 and January 2011, were reviewed. Patients underwent an ocular examination and brain MRI before and after surgery. The visual impairment score (VIS) was derived by combining the scores of best-corrected visual acuity and visual field. The relationship between VIS and tumor size/tumor type/position of the optic chiasm was assessed. Results : Seventy-eight patients were included (41 male, 37 female). Thirty-two (41%) patients experienced blurred vision or visual field defect as an initial symptom. Receiver operating characteristic curve analysis showed that tumors <2.2 cm tended to cause minimal or no visual impairment. Statistical analysis showed that 1) poor preoperative vision is related to tumor size, displacement of the optic chiasm in the sagittal view on MRI and optic atrophy, and 2) poorer visual prognosis is associated with greater preoperative VIS. In multivariate analysis the only factor significantly related to VIS improvement was increasing pituitary adenoma size, which predicted decreased improvement. Conclusion : Results from this study show that pituitary adenomas larger than 2 cm cause defects in vision while adenomas 2 cm or smaller do not cause significant visual impairment. Patients with a large macroadenoma or giant adenoma should undergo surgical resection as soon as possible to prevent permanent visual loss.

Understanding the Social Dilemma of Hahoe Folk Village Management (민속 마을 관리상 발생하는 사회적 갈등의 이해 -하회마을을 중심으로-)

  • This study has been conducted in order to better understand a concrete and evidential disagreement that resulted from a conflict caused by the exploitation of folk village for tourism purposes. The purpose of this study was to analyze more realistically the phenomenon of the disagreement that originated from sightseeing in the hahoe folk villages by focusing on the problem between the ive groups as a potential and continual impediment to tourism. Two groups, residents and tourists, were compared in order to determine the underlying factors of the disagreement regarding exploitation of folk villages for tourism The two groups were clearly split: inhabitants of the village considered such exploitation negative, while tourists accepted it as positive. The two groups showed different understandings of the topic residents were in favor of the preservation of the physical resources, but against exposure of their private lives to the public, while tourists strongly respected the preservation of the resources of the village. A comparison of the common phenomena of disagreement between the two groups was performed, Variables that showed different responses between the two groups were loss of privacy of individual houses, verbal aggression and physical conflicts between the groups, and lack of administration. In most of the cases of disagreement, residents considered the disagreement phenomena to be more serious than tourists did except in one case: tourists placed a greater importance on the ugly view of the street market. Finally, in order to analyze the factors that impacted the level of the disagreement, a multiple regression analysis was conducted. Disagreement factors affecting tourists were verbal aggression between the groups, complaints about lodging and dining facilities, complaints about tourist facilities, and negligence of the administration in responding to requests. Among these, the most influential factor was verbal aggression between the groups. In fact, individual feelings between tourists and residents proved to be the biggest influence on the level of disagreement.

The Sense of Place of Manghae-temple and Mt. Jinbong through Viewing Context (조망경관의 맥락으로 본 망해사와 진봉산의 장소성)

  • This study focused on the one and only temple of on the coastline of the West Sea, Kimje's Manghae Temple and Mt. Jinbong. The purpose of this study was to research the unique sense of place of the area by researching the location and view that clearly shapes the identity of the landscape. The following are the results. 1. The cultural landscape of Manghae Temple and the natural landscape of Mt. Jinbong have characteristics which take in the coastline and skyline, respectively. Specifically, from the existential vertical-horizontal images of "sky - land - Mt. Jinbong" and "moon - cloud - sea", an ascending reflection of the landscape surroundings can be found. 2. The "Sea - Manghae temple - Mt. Jinbong - sky" is the representation of the moderate 'inside-space' which belongs to the particular landscape area and which also shows the topophilia to Manghae Temple. Through this kind of interpretation, the sense of place of Manghae Temple and Mt. Jinbong reveals an intra-structure of an Imaging Landscape implying harmony and moderation, which is a unified organization of the surrounding phenomena(Temple and Mountain) and the essence (the doctrine of Buddhism) that matches Jin-muk's asceticism and lifestyle. 3. While the cultural landscape of Manghae Temple has a strong religious reference, the natural landscape Mt. Jinbong emphasizes the geography of the landscape. In other words, the motivating factor of Manghae Temple is a metaphorical sense of place such as through the "prospect of the sea" or "the Western Sea Paradise" and Mt. Jinbong, the landmark of the Kimje-Mankyung Plains and the focal point of the West Sea sunset, is highlighted as a simile for this sense of place. 4. Keeping this sense of place and territory respectively and showing the. bond with the sense of place which develops rhythmically and continuously, Manghae Temple and Mt. Jinbong are sublimated into a unified intra-structure, which reflects the Imaging landscape characteristics of "Mt. Jinbong, a focal point of the Mankyung Plains facing the West Sea" and "Manghae Temple, looking out to sea".

Fracture Mechanics Analysis of the Weldment in Pulley for Belt Conveyor (컨베이어용 풀리의 용접부위에 관한 파괴역학 설계기술 개발)

  • The drive pulley, which is employed for loading and unloading raw materials in a steel mill, is usually manufactured by use of various welding processes. In this study the weldment in the pulley, in which TIG and $CO_2$ welding processes are used, has been analyzed from view point of fracture mechanics. Fracture toughness tests have been performed according to ASTM E813. A servo-hydraulic testing machine (10kN) has been employed. Also the crack propagation tests (Mode I) have been performed with compact tension specimen in compliance with ASTM E647. To predict the critical crack size in the weldment, finite element stress analysis for the drive pulley under real operating conditions have been performed. In addition, the residual stresses at the weldment and in heat-affected zone have been obtained by hole drilling method. The planar critical crack size have been predicted for the drive pulley by considering the stress analysis results and the residual stresses due to welding process. For the drive pulley considered in this study, it has been concluded that the most important factor in determining the critical crack size is the welding residual stress in the transverse direction. Also the effect of stress concentration at the root of the weldment have been noticeable. For the planar crack, the fatigue crack growth life from an initial crack size of 2mm to the critical crack size obtained as in the above have been predicted. The predicted lives were between 55, 900 and 72, 000 cycles depending on the shape of the elliptical crack. The predicted lives were in fairly good agreement for the drive pulley considered in this study.

  • This study analyzed the Entrepreneurial Competencies of Science and Technology Personnel from the point of view of the industry's demanders. It is necessary to change institutional and policy support from quantitative support to improvement of qualitative level. Specifically, this study used the Borich's Needs Assessment Model to determine what capabilities are currently lacking and which should be supplemented. As a result of analysis, it showed that the current level was lower than expected level. Especially, expectation level was high in order of team working, problem solving ability, communication, abstract thinking, job knowledge, Autonomy, leadership, risk-taking, and commercialization are all lower than the overall average. In the analysis of educational need, the most important factor of entrepreneurial competence appeared in order of problem solving ability, teamworking, job related knowledge, abstract thinking, and communication. This means that it is necessary to logically solve the problems that are encountered in various situations in the organization by using cooperation between the teams and their own job knowledge. In addition, education should be conducted to effectively communicate the results to others.

  • This study examined contents of interaction that played an important role in learners' accomplishment of educational goals at web-based educational system. The purpose of the study was to find out design strategies of web-based education website that could give learners more satisfactory interaction. The study firstly investigated concept and values of web-based education as well as interaction by literature study to analyze web-based educational design principles at actual web-based education websites from point of view of interaction and to examine what factors should be used and reinforced at existing teaching. The findings were as follow: Web-based education websites made use of interaction with 'each design factor', such as colors, texts and images, etc that were based on communication designs to give 'information' by systematic interaction more than by social interaction.

  • A rapidly increasing demand of voice and data through mobile telecommunication service and the development of technologies related with the service are sufficiently said to make the service become an indispensible communicating means, not just selective one. However, in the other side of the generalizing of mobile telecommunication service, there is said to be a worsening situation of unbalanced benefits due to the problem of information difference between the non. The more advanced mobile telecommunication service proceeds, the more deepens the problem, and it is generally known that the problem cannot be solved by the ability of free market itself as seen in the wired telecommunication market. Therefore, we need to set up a systematic policy of mobile telecommunication service for solving this problem, and also think over whether to include the service within the range of universal service. In this study, we examine the required factors for including the service within the range of universal service, and analyze the validity of the including in the view point of institutional aspects.

Simulation Analysis for a Partial Appointment System of Outpatients in a Private Clinic (개인 병원의 부분 예약제 도입을 위한 시뮬레이션 분석 사례)

  • The waiting time of patients is an important factor related with service quality of a hospital system. An appointment system of patients is not popular, in special, in a private hospital. An appointment system is expected to diversify patients in time and to reduce patient's waiting time. Three appointment rules are suggested and analyzed to compare the waiting time of appointment and walk-in patients using simulation analysis. The results of two way ANOVA show that the time slot of 30 minutes for allocating appointment patients is better than the time slot of 60 minites in view of patient's waiting time. The results of the appointment level 1 also show that the constant pattern rule and the zigzag pattern rule give shorter waiting time for walk-in patient and for appointment patient, respectively.

Notes on the Thermal Stresses for Aluminum Superstructures (알미늄 선루선(船樓선)의 열응력분석(熱應力分布))

  • The effect of thermal stress on a ship's hull is not considered to be serious by most naval architects. Frequently, however, cracking of hulls has been reported which occurred at sea while there were no external forces except the heat from the sun. Detailed investigations have been made of these reports and it has been reliably determined that the damage was initiated by solar heating. The author is not interested in all steel ship or in the applicability and validity of the formular itself, as it has already been proven by the experiments such as S.S. Boulder Victory. The author therefore proceeds directly to calculate the stress distribution on he hull and superstructure of the prototype model ship. These calculations are based on the experimental nonsymetrical temperature gradient data taken earlier on the Boulder Victory. The calculations were made principally to determine the extent of stresses which occurred on an all-steel ship in one case and secondly, those that occurred on a ship with a steel hull and an aluminum superstructure. From the calculations, the author expected the stress distribution of the two case would show distinctly different aspects, but the acquired results were very similar. Generally, at the point of junction of the steel hull and aluminum superstructure sharp peak stresses appeared. At the juncture of the superstructure and the main deck the ship with the aluminum superstructure registered almost 1000 psi more stress than did the ship with the all-steel construction. In the view of these findings, the author recommends to ship designers that pay particular attention to the point of junction of steel and aluminum plate. The author has proven that it is extremely important that a greater safety factor be used at the aluminum-steel junction point than at any other point. Although thermal effects cause high juncture-point stresses in all-steel ships, they are not nearly as critical as in ship constructed of two or more metals.
