Changes of Tracheal Anastomosis Site in Growing Rats (흰쥐에서 기관절제 단단 문합부의 성장에 따른 변화)

  • Growth of suture line and anastomosis is required for long-term success after the tracheal and bronchial surgery in infant and pediatric patient. We used various suture materials in these cases, but the results were differ. To select the adequate suture material in tracheal surgery, we tried next. Tracheal anastomoses were performed in 150 Sprague Dawley rats, aged 4 to 8 [mean 5.8] weeks and weight 62 to 106[mean 83.6] gram, to compare polydioxanone[PDS] 7-0, polyglactin 910[Vicryl]7-0, and polypropylene [prolene] 8-0 suture materials. In 150 rats, only 29[20%] were lived over 300 days, and the weight was 250 to 320[mean 289.5]gram. Cross sectional area of the anastomoses and two or three tracheal rings below anastomosis site were measured under microscope, and calculated and compared as Hsieh`s equition. Cross-sectional area,anastomosis site/normal site 100, were 89.4 $\pm$ 5.34% in PDS group[n=9], 75.7 $\pm$ 6.06% in Vicrylgroup [n = 10], and 80.8$\pm$ 4.06% in Prolene group[n = 10]. Histopathologic studies were done for all autopsies or put in death around 300 days postoperatively. PDS absorblion was not seen 16 weeks after suture but disappeared over 24 weeks slide. Vicryl absorbtion was noted postoperative 8 to 16 weeks, with marked tissue reaction. Prolene showed least tissue reaction, but the suture material was persisted with regional fibrotic capsule.Causes of death were respiratory failure in 76 cases, tracheal rupture in 22 cases, hemorrhage, biting, starvation and etc. in 23 cases. With the brief review of literatures, we report the results.

Surgical Treatment of Achilles Tendon Rupture with Absorbable Suture Materials Only (흡수성 봉합사만를 이용한 아킬레스건 파열의 수술적 치료)

  • Purpose: To report the clinical results from using absorbable suture materials instead of nonabsorbable materials which have been used more commonly to repair Achilles tendon. Materials and Methods: We retrospectively reviewed 21 cases of acute Achilles tendon rupture, treated surgically from 2004 to 2011. Mean follow-up period is 6 months. We repaired Achilles tendon using size 1 Vicryl (Polyglactin 910, Ethicon) for core suture and size 3-0 Vicryl for epitendinous suture. At three months after surgery, we evaluated clinical results with single heel raise height by centimeters, differences of calf circumference and passive range of motion of ankle joint, compared to contralateral side. Also we recorded clinical results with subjective satisfaction grades. Results: At three months after surgery, 20 of 21 patients were able to perform single heel raise over 5 cm in height. Calf circumference differences were less than 1 cm in 12 cases, between 1 cm to 3 cm in 5 cases, more than 3 cm in 4 cases. There was no difference in range of passive motion in 19 cases. All patients satisfied with daily activity except 2 cases with mild discomfort. There was no complication such as rerupture, elongation or infection. Conclusion: We experienced excellent clinical results from repairing Achilles tendon with using absorbable suture materials in terms of functional outcomes and patient's satisfaction without any complication. So we may consider using absorbable suture materials instead of nonabsorbable materials to repair Achilles tendon.

A Case Report of Traumatic Tracheoesophageal Fistula (외상성 기관식도루 -수술체험 1례-)

  • Acquired, traumatic tracheoesophageal fistula [TEF] is rare and difficult problem to manage. This 55 years old man met with a roller accident of a tractor. During accident, he received a penetrating injury on the left upper sternal border. At local clinic, he received closed thoracotomy drainage [CTD]for relief of pneumothorax[left]. Three days after CTD, he complained abdominal pain and hematemesis. The endoscopy revealed large ulcer at the stomach, so he received subtotal gastrectomy. On 10th day post subtotal gastrectomy, he developed aspiration and coughing from a TEF. The esophagogram showed large TEF at the mid-trachea level. So he transfered to our hospital for operation. This patient was operated on for late TEF three weeks after injury. We have used absorble 4-0 Vicryl sutures to repair trachea. We repair all esophageal injuries with two layers of nonabsorbable silk suture. Where suture line on the esophagus, the strap muscle was interposed for reinforcement. And for feeding, the feeding jejunostomy was performed. Postoperatively the osteomyelitis of the manubrium site was developed, so on the 30th postoperative day, an ostectomy of manubrium, both clavicle and fight 1st, 2nd ribs, and the pectoralis major musculo-cutaneous flap coverage were performed.

Complete Transsection of Right Main Bronchus by Blunt Trauma -A Case Report- (둔상에 의한 우측 주기관지 완전 절단 치험 1례)

  • Main bronchial injury after blunt trauma is very rare in all bronchial injuries and the pathogenesis is variable and not well known in everycases. We report a case of complete transsection of right main bronchus by blunt trauma. This 24-year old patient was transferred from a local hospital with a chest tube. Because of the severe subcutaneous emphysema and tension pneumothorax, we inserted one more chest tube resulting no obvious interval change. With the impression of bronchial injury, we performed an exploratory thoracotomy. We couldn't proceed bronchoscopy in the operation room because of his unstable vital sign. After opening of the chest wall, we could identify completely transsected right main bronchus. We anastomosed the bronchus with 4~0 Vicryl interruptedly. After operation, the patient was recovered without any complication.

Subcision Using a Spinal Needle Cannula and a Thread for Prominent Nasolabial Fold Correction

  • Deepening of the nasolabial crease is an esthetically unpleasing aging phenomenon occurring in the midface. Various treatment modalities have been introduced to improve the appearance of prominent nasolabial folds, all of which have pros and cons. Currently, a minimally invasive technique using synthetic dermal fillers is most commonly used. A simple and easy subcision procedure using a wire scalpel has also been used and reported to be effective for prominent nasolabial fold correction, with minimal complications. As an alternative to the wire scalpel, we used a 20-gauge metal type spinal needle cannula (Hakko Co.) and 4-0 Vicryl suture (Ethicon Inc.) for subcision of nasolabial folds. This technique is less expensive than the use of a wire scalpel and easily available when needed. Therefore, on the basis of favorable results, our modified subcision technique may be considered effective for prominent nasolabial fold correction.

Plastic Reconstruction of Tuberculous Bronchostenosis with Rib Cartilage (결핵성 기관지협착 환자에 있어서 늑연골을 이용한 기관지성형술 치험 1례 보고-)

  • We experienced plastic reconstruction of tuberculous bronchostenosis with patient`s rib cartilage. He suffered from coughing and sputum for 2months, and was treated for pulmonary tuberculosis 10 years ago. In preoperative bronchoscopy, left main bronchus was fibrotic obstructive and LUL bronchus was severely destructed. After thoracotomy, we harvested the rib cartilage at the 6th rib, and designed semicircular and tubular graft. And then onlayed the graft over the longitudinal bronchotomy site by simple interrupted sutures with 4-0 Vicryl Postoperative course was good, coughing and sputum disappeared. In postoperative bronchoscopy, the patch graft was good in that position, and the internal diameter was sufficient, but the granuloma was found in the stoma of LUL bronchus. He was discharged without any other event.

  • Tracheobronchial rupture due to blunt chest trauma is an uncommon injury although the incidence is increasing. Early diagnosis and primary repair of tracheobronchial rupture not only restore a normal lung function but also avoid the difficulties and complications associated with delayed diagnosis and repair We present one case of the tracheal transsection caused by traffic accident. Patient suffered from progressive dyspnea, subcutaneous emphysema on the neck and anterior chest wall and tension pneumothorax at both sides were noted. Although both closed thoracostomy were done, massive air leakage through the chest tube continued and subcutaneous emphysema spread to the anterior abdominal wall and scrotum and the degree of dyspnea aggravated. With the impression of tracheobronchial injury, we performed the emergency operation. Preoperative bronchoscopy at the operation room was proceeded, which revealed the trachea was near totally transsected in transverse direction. Operation was performed through collar incisiion on the anterior neck, and the trachea was anastomosed with 4-0 $Vicryl^{(R)}$ interruptedly. Postoperative course were uneventful and patient discharged without any complications.

REMOVAL OF A PIERCING BAR IN THE MOUTH FLOOR BY FLUOROSCOPY: REPORT OF A CASE (형광 투시법을 이용한 구강저에서의 피어싱 바의 제거: 증례보고)

  • The piercing is increasing in western culture, and has also become gradually popular among Korean teenagers recently However, the accident hap-pens sometimes such as loss of a bar into the tongue or mouth floor during the procedure. Because of the rare cases, few treatment of choices can be made when a bar is lost inside the mouth floor. In this case, the bar was removed under fluoroscope, without giving significant damage to the adjacent structure. 18 years old female patient visited the emergency room in our hospital. She attempted to pierce her tongue herself and could not find the bar which was lost in the mouth floor. The radiographs revealed that the needle was somewhere inside the mouth floor, but if incision were to be made it would be too deep to find the bar. The bar was removed through fluoroscope easily. Primary closure was done by 5-0 Vicryl and stitch-off was done 1 week later. The patient didn't have any speech problems and complications.

Pyogenic granuloma of the hard palate leading to alveolar cleft: a case report

  • This case report describes a rare occurrence of pyogenic granuloma (PG) in the hard palate deviating from its typical gingival location that led to the formation of an alveolar cleft. The aggressive growth pattern of the lesion, with atypical progression from a pedunculated nodule to an alveolar cleft, raised concern. The diagnosis was based on magnetic resonance imaging and computed tomography findings, which revealed a tadpole-shaped lesion originating from the midline hard palate. The differential diagnosis included a minor salivary gland tumor. Surgical excision was performed under general anesthesia and resulted in a mucosal defect without nasolabial fistula formation or bone exposure. The palatal defect was packed with oxidized regenerated cellulose and closed with Vicryl Rapide sutures, both of which contributed to the patient's successful outcomes. Our comprehensive approach, extending across the stages of surgical planning, execution, and postoperative care, demonstrated the advantages of a multidisciplinary strategy for the accurate diagnosis and effective treatment of palatal PGs. This report makes a meaningful contribution to the existing literature on common oral lesions by emphasizing the importance of a broad differential diagnosis and a systematic approach to oral pathologies. It also raises clinical awareness of PGs with atypical presentations and the diagnostic challenge that they pose.

HISTOLOGIC STUDY OF THE AUTOGENOUS AURICULAR CARTILAGE GRAFTS IN THE RABBITS FEMER (가토 대퇴골에 이식한 자가 이연골에 관한 조직학적 연구)

  • The transplantation of cartilage, especially auricular cartilage, has assumed a role of importance in the field of plastic and reconstructive surgery. From long years ago, many reports have appeared in the literature describing the experimental and clinical results of the use of cartilage. At present, the evidence for survival of autograft of cartilage is admitted, But, the results for interrelationship between the bone and cartilage grafts with or without perichondrium is not so conclusive. The purpose of this study were observed as to whether autogenous cartilage grafts were fixed by means of tie with 4-0 vicryl and fibrin adhesive on the femur or microscopic findings of union state in 16 rabbits. We sacrified the experimental animals after 1, 2, 4, 6 weeks postoperatively and made the specimens as a routine laboratory procedures and stained with Hematoxylin-Eosin stain, Verhoeff-van Gieson elastic fiber stain, and alcian blue periodic acid-Schiff(AB-PAS) for mucopolysaccharide. Histologic evaluation was performed under microscope. The obtaind results were as follows : 1. Fibrous union was formed between the grafting cartilage and the femur, nor any findings of calcification and formation of new bone. 2. Partial fibrous adhesion was observed in fibrin adhesive groups on 6 weeks postoperatively. 3. Appositional growth has performed more in fibrin adhesive groups than tie groups. 4. There are little difference in both for new copillary proliferation and fibroblast activations. 5. Degenerative changes have apperared more in tie groups than adhesive groups, but not related to the healing periods.
