Immediate Effects of Vibration Stimulation on the Range of Motion and Proprioception in Patients with Chronic Ankle Instability: Randomized Crossover Study (만성발목불안정성 환자의 진동자극이 가동범위 및 고유수용성감각에 미치는 즉각적인 효과: 무작위 교차 연구)

  • PURPOSE: This study examined the effect of vibration stimulation of a vibration foam roller on the change in the range of motion of the ankle joint and proprioception in patients with chronic ankle instability. An additional aim was to provide basic data for rehabilitation programs for chronic ankle instability patients. METHODS: This study was a randomized crossover design of 22 patients with chronic ankle instability. All subjects were divided into a vibrating group, a non-vibrating group, and a control group. The vibration and non-vibration groups performed the interventions, but the control group did not. For the measurement, the range of motion and proprioception of the ankle joint was measured using an electronic protractor (Electrogoniometer, BPM Pathway, UK). RESULTS: The vibration group showed significant differences in the dorsiflexion angle, dorsiflexion proprioception, and plantar flexion proprioception (p < .05). The non- vibration group showed significant differences in the dorsiflexion angle and dorsiflexion proprioceptive sensation (p < .05). The vibration group and the control group showed significant differences in dorsiflexion proprioception and plantar flexion proprioception (p < .05). CONCLUSION: The range of motion and proprioception of the ankle joint were improved in the group that received vibration stimulation after the intervention than before the intervention. Future research will be needed on patients with various diseases.

The Effect of Convergence Intervention of Focal Vibration Stimulation and Bilateral Upper Extremity Training on Recovery of Upper Limb Function in Stroke Patients (국소 진동 자극과 양쪽 팔 훈련의 융합 중재가 뇌졸중 환자의 팔 기능 회복에 미치는 효과)

  • The purpose of this study was to investigate the convergence effect of focal vibration stimulation and bilateral upper limb training on the recovery of upper limb function when applied to stroke patients. For 20 stroke patients, divided into an experimental group that performed convergence intervention with focal vibration stimulation and bilateral upper limb training, and a control group who performed only bilateral upper limb training. It was conducted 20 times for 4 weeks, 30 minutes per session. Vibration stimulation was applied to the affected side of the experimental group for 30 minutes during training. Results were measured for the degree of recovery of the affected upper limb function, amount of use on the affected and unaffected sides, the quality and satisfaction in performance on use of both upper limbs. Comparisons were made within groups using a paired-sample t-test and between groups using covariance analysis. As a result of the study, the experimental group showed a significant difference in dexterity and the amount of use on the affected than the control group. The effect size was more than the small effect size in all evaluation items. Through this study, it is thought that the convergence intervention of focal vibration stimulation and bilateral upper limb trainingcan be used clinically as an effective intervention for the recovery of arm function in stroke patients.

The Effect of Resistance Exercise with Vibration Stimulation on Balance and Gait of Experienced Back Pain Adults (진동자극 저항운동이 허리통증 경험자의 균형과 보행에 미치는 영향)

  • The purpose of this study was to investigate the effect of resistance exercise with vibration stimulation methods on static balance and gait parameters in experienced back pain adults. This study was Three group pretest-posttest design. A total of 30 experienced back pain adults voluntarily participated in the study. Subjects were randomly assigned to the resistance exercise with 8 Hz vibration stimulation(n=10), resistance exercise with 30 Hz vibration stimulation(n=10), and lumbar stabilization exercise groups(n=10). The static balance and the gait parameters, as such stance phase, swing phase, stride length and cadence, were measured using balance measuring equipment and gait analysis treadmill at before and after 6 week. Intervention of each group were performed, three times a week for 6 weeks, and 30 minutes a day. The effect of intervention on static balance, stance phase, swing phase, stride length and cadence were significantly differences after 6 weeks in each group(p<.05). In the comparison of the effects between the groups, static balance and stance phase were significantly difference after 6 weeks(p<.05), but in the swing phase, stride length and cadence, there were no significant differences. As a result, it is considered that resistance exercise with vibration stimulation improved leg muscle strength by a mechanism causing muscle contraction, and the strengthened leg muscle enhanced had a positive effect on balance ability. And improved balance ability was considered a more positive effect on walking ability by allowing the body to stably control posture while moving.

Narrative Review on the Mechanism of Whidam's Vibrator Sugi Therapy (휘담식 진동기 수기요법의 기전에 대한 서술적 고찰)

  • Objective : This paper provides a narrative review of the research literature on the neurophysiological and neurochemical mechanisms of local vibration while studying the treatment principles and mechanisms of Whidam's vibrator Sugi therapy. Methods : Searches related to vibration therapy research were conducted in PUBMED using "Vibration", "Whole Body Vibration", "Localized Vibration", and "Focal Vibration". The Conditions were limited to review and systematic review. Results : Roberto Casale's paper was selected as an inquiry task and reviewed critically and narratively by referring to other papers. The stimulation process of local vibration (LV) was broadly classified into receptor transmission (pain reception phase), ascending sensory pathway to the spinal cord (segmental phase), and action of the cortex and subcortical structures (systemic control phase) according to the pain pathway. In addition, the role of C-tactile mechanoreceptors, changes in neurotransmitters and neurohormones, LV stimulation below perception threshold (lower threshold), pain control and kinesiologic illusions were specially addressed. In addition, the expression and function of Piezo Channels were added to supplement the human pain and tactile sensing mechanism. Conclusions : LV exerts pain control mechanisms through different interactions that can interfere with pain transmission and pain perception. Since LV provides sufficient neurophysiological reasons for clinical application, it is necessary to expand the use of Whidam's vibrator Sugi therapy to a wider range of clinical applications.

Development of a Somatosensory Stimulation System for the Improvement of Postural Stability (자세 안정성 개선을 위한 체성감각 자극 시스템 개발)

  • This paper proposes a somatosensory stimulation system for the improvement of postural stability using vibration as somatosensory stimulation. This system consists of vibratory stimulation and postural response measurement. To evaluate this system, the center of pressure(COP) was closely observed in turn with simultaneous or separate mechanical vibratory stimulations to flexor ankle muscles (tibialis anterior, triceps surae) and two plantar zones on both feet while standing on a stable and an unstable support. The simultaneous vibratory stimulations cleared influenced postural stability and the effects of vibrations were higher with the unstable support. In separate vibratory stimulations, the extent of the COP sway reduced when the direction of the vibratory stimulations and that of the inclination of body coincided for flexor ankle muscle stimulations. In the contrary, the extent of the COP sway increased when the direction of the stimulations and that of body inclination coincided for plantar zone stimulations. These results can be useful for the development of rehabilitation systems that utilizes somatosensory inputs for postural balance.

The Effects of IASTM Using Vibration Stimulation on Shoulder Muscle Activity, Flexibility and Pain of Chronic Shoulder Pain Patients (진동을 이용한 IASTM이 만성 어깨통증 환자의 어깨 근활성도, 유연성, 통증에 미치는 영향)

  • Purpose : Instrument-assisted soft tissue mobilization (IASTM) has been widely used to improve pain and range of motion. However, research on IASTM using vibration stimulation is lacking. Therefore, this study investigates how IASTM using vibration stimulation affects chronic pain on muscle activity and flexibility of the shoulders. Methods : A total of 20 patients with chronic shoulder pain were divided into the experimental group and the control group. The experimental group (n=10) used vibration stimulus for IASTM, while the control group (n=10) used general IASTM. For six weeks, this training lasted for 30 min a day, twice a week. Muscle activity was evaluated using the surface electromyograph. The back and reach test was used to assess flexibility, the visual analogue scale to measure pain, and the paired t-test to compare the groups before and after the experiment. An independent ANCOVA was conducted to assess differences in the degree of transition between the two groups before and after the experiment. Results : Significant differences in muscle activity, flexibility, and pain in both groups before and after the experiment (p<0.05) were observed, as well as in the muscle activity of the pectoralis major and associated pain (p<0.05). however, muscle activity and flexibility of the upper trapezius and infraspinatus were not significantly different between groups (p>0.05). Conclusion : This study confirms the importance of IASTM using vibration. Design and manufacture of IASTM using vibration based on our results would be useful in the management of shoulder pain, Further clinical evidence are needed.

The Analysis of GRF and joint angles of young and older adult by Vibration Stimulation on the Ankle-Joint in stair-descent activity (족관절에 인가한 진동자극이 계단 하강 동작에서 청년과 노인의 관절각도와 지면반발력에 미치는 영향)

  • The purpose of this study was to investigate changes in the center of pressure (COP), ground reaction force (GRF) and joint angles of elderly people and young people while stair-descent. The participants in this experiment were 5 elderly people and 5 young people, each of which was asked to descend stairs of three different heights (8 cm, 16 cm, and 32 cm). As they climbed down the stairs, they received vibration stimulation on the lower limb. The change of COP, GRF and joint angles were analyzed during the standing phase. COP decreased as the Achilles tendon and tibialis anterior tendon were vibrated. Vertical GRF increased as the Achilles tendon was vibrated, and the joint angle differed according to vibration stimulation conditions. These results mean that ankle joint, knee joint and hip joint were influenced by the vibrations on the lower limb as the participants descended the stairs. It was concluded that the vibration stimulation on the lower limb allowed the participants to efficiently climb down the stairs.

  • This paper deals with an experimental study on the reaction of muscles by functional electrical stimulation(FES) with the design and fabrication of an ankle reaction apparatus. The ankle reaction apparatus consists of a circuit part and a kinematic part, and it monitors reaction time for a particular angle of joint rotation according to FES. The experimental results showed that the change of the ankle rotation angle was linearly proportional to the change of the magnitude of FES. It also showed that the muscle's reaction time was constant no matter how large the magnitude of the stimulus. The results of this paper can be applied to develop an active-type walking-assistance robot.

Effects of Bridging Exercise using Vibration Stimulation and ADIMs on the Lengthening and Thickness of Transversus Abdominis in Healthy Adults

  • Purpose: This study was conducted in an effort to determine the effects of various abdominal drawing-in maneuver (ADIM) on the thickness and length of the transversus abdominis (TrA) when using lumbar stabilization exercises on healthy adults. Methods: 72 healthy adults were divided into four groups of 18 subjects each, to which different ADIM methods were applied. 1) a simple ADIM exercise, 2) an ADIM with pressure bio-feedback units, 3) an ADIM exercise with sling, and 4) an ADIM exercise with sling and vibration. Changes in the thickness and sliding length of TrA were measured when ADIM was conducted in the supine position prior to exercise and again when beginning the exercises. Following exercise, changes in the thickness and sliding length of TrA were measured using the same methods. Differences in group measurements prior to and following exercise were compared using a one-way analysis of variance. A paired t-test was applied to compare the before and after differences within each group. Results: Differences in TrA thickness change revealed that the ADIM exercise with sling and vibration group showed a significant difference in measurements taken prior to and following exercise. Differences in TrA length change revealed that the ADIM exercise with sling and vibration group showed a significant difference in measurements taken prior to and following exercise. Conclusion: ADIM exercise with vibration stimulation conducted in the bridge posture while in a prone position using a sling can be recommended as an effective exercise to improve the function of lumbar TrA.

Measurement of Internal Temperature Distribution for the Evaluation of Focused Ultrasound (FUS) Stimulation Devices (집속초음파 자극기의 성능평가를 위한 팬텀 내부온도 측정)

  • This research is to measure real-time temperature distribution inside a tissue-mimicking phantom for the safety and effectiveness evaluations of focused ultrasound (FUS) device capable of linear scanning stimulation. Since the focusing area of the FUS stimulation device is smaller than diameter of conventional thermal probe and keeps moving, it is impossible to monitor temperature distribution inside the phantom. By using the phantom with a thin film temperature sensor array inserted, real-time temperature change caused by the FUS device was measured. The translation of the measured temperature peak was also tracked successfully. The present phantom had been experimentally proven to be applicable to validate the performance and safety of the therapeutic ultrasound devices.
