Identification and antimicrobial susceptibility of bacteria from the uterus of bitches with pyometra (개 자궁축농증에서의 원인균 분리 및 항생제 감수성 검사)

  • This study was performed to isolate and identify bacteria from uterus with pyometra and examine their susceptibility to antimicrobial agents. Uterus of 16 bitches with pyometra were surgically removed by ovariohysteroctomy and then bacteria were isolated and identified. Also, susceptibility test to 15 antimicrobial agents was performed. Out of 16 bitches, 11 strains of Escherichia coli, 2 strains of Serratia marcescens, and 1 strain of Staphylococcus aureus and Salmonella spp. were identified. In antimicrobial susceptibility test, the majority of isolates were susceptible to enrofloxacin, norfloxacin, chloramphenicol, nalidixic acid, gentamicin, trimethprim-sulfamethazole, tetracycline, and moderately susceptible to carbenicillin, amikacin, ampicillin, neomycin, but resistant to vancomycin, streptomycin, bacitracin and colistin. In conclusion, E coli was the most common bacteria isolated from bitches with pyometra and those susceptible antimicrobial agents could be recommended to medical therapy of pyometra.

Trigeminocardiac Reflex Induced by Electrohemostasis during Total Ear Canal Ablation in a Dog (개의 전이도적출술 중 전기지혈에 의해 발생한 삼차신경심장반사 1례)

  • A 14 kg, 9-year-old, spayed female, Cocker Spaniel was presented to the Veterinary Medical Teaching Hospital of Seoul National University with a history of head tilt and circling. Otitis externa and media were diagnosed by computerized tomography, and total ear canal ablation was performed. In preanesthetic evaluation, systemic hypertension and second-degree atrioventricular block were observed, but there was no regurgitation through the heart valves. Systemic hypertension was managed with amlodipine (0.1 mg/kg, PO, BID) for the anesthesia. The dog was premedicated with cefazolin (22 mg/kg, IV) and midazolam (0.2 mg/kg, IV). Anesthesia was induced with alfaxalone (2 mg/kg, IV) and maintained with isoflurane and 100% oxygen following intubation. During surgery, vital signs (heart rate, respiratory rate, blood pressure, end tidal carbon dioxide partial pressure and body temperature) were maintained within normal ranges, but bradycardia was observed and corrected with glycopyrrolate (5 ug/kg IV, twice). During subcuticular suture, electrohemostasis was applied at the incision line, which was close to the trigeminal nerve. In no time at all, heart rate dramatically decreased from 110 to 60 beats per minute. No additional treatment was done because mean blood pressure was maintained above 70 mmHg. The heart rate recovered according to the decrease of end tidal isoflurane concentration and there were no complications associated with the anesthesia and surgery. Sudden bradycardia after electrical stimulation around the trigeminal nerve was considered as trigeminocardiac reflex (TCR). It is recommended to be careful of bradycardia from TCR when electrocautery is used in the craniofacial area during surgery.

Application of epoxy putty external skeletal fixator for stabilization of tarsocrural arthrodesis in small dogs and cats

  • Two small-breed dogs and two cats, with an average body weight of 4.88 kg (range: 4.3-5.5 kg), suffered hindlimb lameness due to luxation with or without fractures of the tarsocrural joint. These patients underwent tarsocrural arthrodesis with epoxy putty external skeletal fixator. The animals' skins were incised minimally, and the articular cartilage of the tarsocrural joint was removed, followed by autogenous cancellous bone grafting. Epoxy putty and positive Centerface®, pins with diameters 1.2 mm and 2.0 mm, were used for connecting bar and as a full pin fixation, respectively. All the patients regained the ability to bear weight on the affected limb within 3-7 days and resumed a normal gait within 9-15 weeks. The external skeletal fixator frame was removed within 13-17 weeks without major complications. Tarsocrural arthrodesis using epoxy putty external skeletal fixator resulted in excellent outcomes without severe postoperative complications in this study. Epoxy putty external skeletal fixator can be a valuable surgical option for tarsocrural arthrodesis in patients weighing less than 5.5 kg.

Isolation of Bacteria from Clinical Specimens in Veterinary Medical Teaching Hospital and Trend of Antimicrobial Susceptibility (대학 동물병원 임상 검체로부터 분리된 호기성 세균과 항생제 감수성 양상)

  • Bacteria that are resistant to several different groups of antibiotics have increased during the past few years. The importance of surveillance of antimicrobial resistance is now widely recognized. Unfortunately, this development has not been documented continuously in veterinary medicine in Korea. Therefore, the clinical prevalence and trend of antimicrobial susceptibility of aerobic isolates were investigated in this study. Total 121 isolates of aerobic bacteria were isolated from clinical specimens of dogs and cats at Veterinary Medical Teaching Hospital of Seoul National University from May 2001 to October 2002. Among them, the most common isolated species was Staphylococcus spp. (48 isolates), followed by E.coli (26 isolates), Enterococcus spp. (21 isolates), Klebsiella pneumoniae (9 isolates), Streptococcus spp. (6 isolates), Enterobacter cloacae (3 isolates), Pseudomonas aeruginosa (3 isolates), Corynebacterium xerosis (2 isolates), Chryseomonas spp. (2 isolates), and Providencia stuartii (1 isolate). The susceptibility of isolates to antibiotics was determined by the disk diffusion method. Gram-positive bacterial isolates were showed high susceptibilities to amikacin, amoxacillin/clavulanate, ceftazidime, and oxacillin, while Gram-negative bacterial isolates were showed high susceptibilities to amikacin and ceftazidime. Staphylococcus spp. were showed high susceptibilities to amikacin, amoxicillin/clavulanate, ceftazidime, cephalothin, and oxacillin. Streptococcus spp. and E.coli were showed high susceptibilities to amikacin and ceftazidime. Of the 48 staphylococci, seven Methicillin Resistant staphylococci were observed (14.6%), distributed among S. auricularis (1), S. hemolyticus (2), S. sciuri (1), S. saprophyticus (1), S. warneri (2) isolates. One strain of E.coli and one strain of Corynebacterium xerosis were resistant to all antibiotics tested. And, resistance trends between the you 2000 (from July 1999 to September 2000) and 2002 (from May 2001 to October 2002) were compared. Resistance to antibiotics was increased in both Gram-positive and Gram-negative bacterial isolates (p< 0.05). The resistance rates of Staphylococcus spp., E.coli and Klebsiella pneumoniae to all antibiotics tested were also increased (p<0.05). This study investigated increasing resistance between the year 2000 and 2002 in Veterinary Medical Teaching Hospital of Seoul National University. Surveillance resistance is helpful to alert to veterinarian and select of appropriate therapy. Antimicrobial susceptibility surveillance of isolates should urgently be continued in veterinary medicine.

  • A 6-year-old, spayed female Cocker Spaniel was presented with generalized seborrhea sicca. Initially gallbladder mucocele (GM) was suggested by high liver enzyme activities and a stellate pattern in ultrasonography. There were no responses during 16 months of medical treatments for GM. Hormonal assay and histopathological findings were consistent with a diagnosis of hypothyroidism. Levothyroxine therapy led to rapid resolution of dermatologic, serum chemical and ultrasonographic abnormalities. This report describes serial changes of skin lesions, serum chemistry and ultrasonography in a dog with concurrent GM and hypothyroidism following treatment of hypothyroidism.

Evaluation of the clinical accuracy of six portable blood glucose meters in dogs

  • Portable blood glucose meters (PBGMs) are widely used because of their practicality. However, the accuracy of PBGMs has frequently been questioned. The objectives of this study were to evaluate factors that might interfere with measurements made using PBGMs, and to assess the clinical utility of 6 PBGMs. The glucose concentrations measured using the PBGMs were compared with those obtained using a reference method. The agreement between the measured values was assessed using Spearman correlation analysis, Passing-Bablok regression analysis, Bland-Altman plots, and consensus error grid analysis. Mann-Whitney and Kruskal-Wallis tests were performed to identify the parameters affecting glucose measurement. The results indicated that all of the PBGMs tested perform adequately for use in veterinary practice. In most cases, measurements made using PBGM corresponded well with the blood glucose values obtained using the reference method. Error grid analysis revealed that most of the PBGM values fell within zones A and B. However, some measurements of blood glucose concentrations < 80 mg/dL fell into zone C. PCV, and triglyceride and total protein concentration, significantly affected the output of some of the PBGMs. Therefore, clinicians should be aware of the characteristics of the PBGM that they use.

Evaluation of Diseases Affecting Hindlimb Lameness in Domestic Small Breed Dogs

  • Lameness is a variation of normal gait in an animal, and it means that one or more limbs cannot be used correctly to allow the animal to walk. In the usual context, the incidence of hindlimb lameness in dogs is most likely the result of trauma, joint diseases, and/or congenital diseases. Generally speaking, the factors influencing hindlimb lameness include the animal's specific breed, size, weight, and whether it engages in frequent or strenuous activities. Many studies have investigated the incidence of lameness of large breed dogs, as compared to small breed dogs. Considering that many domestic dogs are small breeds, the lameness of small breed dogs with a high population in Korea was evaluated. The major causes of hindlimb lameness were found to be joint, musculoskeletal, and neurological abnormalities and the most were identified as joint diseases. Among the joint diseases, it was noted that a patellar luxation was the most common, of which the grade 3 medial patellar luxation was the highest rated type of joint disease noted.

Monte Carlo Algorithm-Based Dosimetric Comparison between Commissioning Beam Data across Two Elekta Linear Accelerators with AgilityTM MLC System

  • Purpose: Elekta synergy® was commissioned in the Seoul National University Veterinary Medical Teaching Hospital. Recently, Chung-Ang University Gwang Myeong Hospital commissioned Elekta Versa HDTM. The beam characteristics of both machines are similar because of the same AgilityTM MLC Model. We compared measured beam data calculated using the Elekta treatment planning system, Monaco®, for each institute. Methods: Beam of the commissioning Elekta linear accelerator were measured in two independent institutes. After installing the beam model based on the measured beam data into the Monaco®, Monte Carlo (MC) simulation data were generated, mimicking the beam data in a virtual water phantom. Measured beam data were compared with the calculated data, and their similarity was quantitatively evaluated by the gamma analysis. Results: We compared the percent depth dose (PDD) and off-axis profiles of 6 MV photon and 6 MeV electron beams with MC calculation. With a 3%/3 mm gamma criterion, the photon PDD and profiles showed 100% gamma passing rates except for one inplane profile at 10 cm depth from VMTH. Gamma analysis of the measured photon beam off-axis profiles between the two institutes showed 100% agreement. The electron beams also indicated 100% agreement in PDD distributions. However, the gamma passing rates of the off-axis profiles were 91%-100% with a 3%/3 mm gamma criterion. Conclusions: The beam and their comparison with MC calculation for each institute showed good performance. Although the measuring tools were orthogonal, no significant difference was found.

Tibiotarsal and Ulnar Fracture Repair in a Great Horned Owl (Bubo virginianus)

  • A 1.4 kg adult great homed owl was presented to the University of Missouri-Columbia Veterinary Teaching Hospital after being found by the side of the highway. Physical examination revealed soft tissue injuries to the left wing and leg, and good body condition (body score 4/5). The radiographs revealed comminuted fracture of the diaphysis of the left tibiotarsus (severe) and ulna (mild). Closed reduction of the fracture was performed to the left tibiotarsus and ulna. System combining an intramedually (IM) Kirschner pin, IM Kirschner pin for external skeletal fixation, and polymethylmethacrylate was used for fracture repair. At 13 weeks, radiographs revealed that bridging callus was well formed over cortices of the fracture area. No physical, behavioral, or other assessable impairment was found during the rehabilitation period.
