Nutritional Situation in Korea (한국인영양(韓國人營養)의 현황(現況))

  • According to the dietary surveys and national food balance sheets, the dietary state of person living in Korea is ingesting a high level of grain and vegetable diet and very low animal sources. The daily calory intake is about 2,300-2,600 Cal. per head in which a total protein intake is 69-86 gm respectively. These data indicate no shortage of calory and total protein intake. However, the intake of animal protein is only 4-11 gm and the fat is 8-18 gm, both of which are far below the requirment. The low fat intake brings about a low level of fat soluble vitamins, and that V-A intake is as short as the half amount of the allowance. Riboflavine is also about a half of dietary requirment. Both thiamin and ascorbic acid intake are sufficient. The calcium intake is 0.26-0.5 gm per day which is inadequate. In fact, biochemical findings through field survey show; serum protein 6.7 gm, hemoglobin 13.1 gm and hematocrite 41.5% all of which are lower than the Kwon's report. The serum V-A and uunrinary riboflavin value are also lower than the Williams'es report. Clinically, inflamation of the eyes, cheilosis, pathological signs of tangue, xerosis of the hand and arm, or the like are frequently seen. On the other hand, there is no suitable diet available for the weaning infant and delayed weaning is a common practice. A school lunch program is still in an early stage of development. Another example can be cited that the Korean infant grows faster than the Japanese until one year of age, and no difference is noted between the Korean and the American infants up to six monthes of age, thereafter the former lags in the growth rate. This might be mainly due to the nutritional reasons as mentioned above, and in part due to the high inidence of intestinal parasitism and others.

LOW GRADE OSTEOSARCOMA ON THE MANDIBLE;A CASE REPORT (하악골에 발생된 Low grade 골육종;증례보고)

  • Osteosarcoma is a primary malignant tumor arising most often in the long bone, but rare in jaw bone. Especially osteogenic sarcoma of the mandible is a rare tumor, comprising less than 0.5% of all head and neck tumors. Osteosarcoma occurs chiefly in young persons, and presenting very survival rates. Histopathologically this tumor can be classified three types, osteoblastic, fibroblastic, chondroblastic and classified from low grade to high grade by anaplasia or mitosis of the tumor cells. Sometimes, the exact diagnosis of osteosarcoma is difficult especially in low grade osteosarcoma, even if clinical or radiographical findings suggest to osteosarcoma. So suspcious to malignant bone tumor in clinical or radiological findings, biopsy should be taked from deep portion and multiple area, and sharing the patient history and radiographs with pathologist will assist in the development of the diagnosis. We report a case of low grade osteosarcoma on the mandible initially difficulty in accurate diagnosis. The patient visited our hospital for routine dental treatment but radiographic findings displayed ill-defined radiolucency with osteoid formation on the mandible. Final diagnosis was difficulty in initial biopsy but subsequent biopsy taked from deep portion presented infiltrative growth and mitosis of the tumor cell in some area with small osteoid bone formation, so we could reach final diagnosis as low grade sarcoma. We treated this patient with bloc resection of the mandible and immediate reconstruction with iliac corticocancellous block.

Newborn heating screening (신생아 청력장애의 선별검사와 의의)

  • Hearing loss in newborns is the most frequently occurring birth defect. If hearing impaired children are not identified and managed early, it is difficult for many of them to acquire the fundamental language, social and cognitive skills that provide the foundation for later schooling and success in society. All newborns, both high and low risk, should be screened for hearing loss in the birth hospital prior discharge (Universal Newborn Heaing Screening, UNHS). Objective physiologic measures must be used to detect newborns and very young infants with hearing loss. Recent technological developments have produced screening methods and both evoked otoacoustic emission (EOAE) and auditory brainstem response (ABR) have been successfully implemented for UNHS. Audiologic evaluation should be carried out before 3 months of age and infants with confirmed hearing loss should receive intervention before 6 months of age. All infants who pass newborn hearing screening but who have risk indicators for other auditory disorders and/or speech and language delay receive ongoing audiologic surveillance and monitoring for communication development. Infants with sensorineural hearing loss are managed with hearing aids and receive auditory and speech-language rehabilitation therapies. Cochlear implants can be an outstanding option for certain children aged 12 months and older with severe to profound hearing loss who show limited benefit from conventional amplifications.

The absorbed doses from each exposure program of the Orthopos$\textregistered$ panoramic machine (Orthopos$\textregistered$ 파노라마방사선촬영기의 각종 프로그램에 따른 흡수선량)

  • Purpose: The objective of this study was to estimate the radiation absorbed doses in certain critical organs in the head and neck region with 16 imaging programs available on the Orthopos/sup (R)/ panoramic machine. Materials and Methods: A Rando phantom and LiF TLD chips were used for dosimetry. The absorbed doses were measured at the thyroid gland, the submandibular gland, the parotid gland, the mouth floor. the maxillary sinus, the brain, the mandibular body, the mandibular ramus. the 2nd cervical spine and the skin over TMJ area. Results : The overall absorbed doses with imaging programs available on the Orthopos/sup (R)/ panoramic machine were much less than that of standard program (program 1) except program 8, 11, and 16. Generally, the absorbed doses to the bone marrow of the mandibular ramus and the parotid gland were high, but the absorbed doses to the bone marrow in the mandibular body, brain, maxillary sinus, and, especially, the thyroid gland were very low. Conclusion : The modified imaging programs available on the Orthopos/sup (R)/ panoramic machine can be effectively used in aspect of radiation protection.

Nurse′s Perception of Technological Development, Caring Attributes and Professional Self-Concept in YanBian (연변지역 간호사의 의료기술발달과 돌봄의 속성에 대한 지각 및 전문직 자아개념)

  • Purpose: The purpose of this study was to describe technological development, caring attributes and professional self-concept as perceived by nurses in YanBian. Method: Data were collected using an instrument containing 137 Likert items was administered to 477 RN's working in general hospitals in YanBian. The instrument contained sections which examined technological influences questionnaire(TIQ), caring attributes questionnaire(CAQ), and professional self-concept nursing inventory(PSCNI). Result: Descriptive and inferential statistics revealed by marital status and position. Married, working special ward nurses reported a higher TIQ score than that of unmarried and working general ward and OPD. PSCNI and CAQ score of head or supervisor nurses were higher than that of staff nurses. Subjects revealed very low score of CAQ, while PSCNI score was higher than that of other Asian countries such as Korea, Beijing China, HongKong China and Japan as proved in former study. Conclusion: Useful information for educators and nurse administrators is provided from this results. Further study needs to be done to discuss in the light of cultural and environmental differences between YanBian(Korean-Chinese) and Korean nurses.

Difference in Radiation Absorbed Dose According to the Panoramic Radiographic Machines (파노라마방사선촬영기종에 따른 흡수선량 차이)

  • Purpose: The primary objective of this study was to estimate the radiation absorbed doses in certain critical organs in the head and neck region with an Orthopos plus, a Panelipse, and a Panex-E machines. The second objective was to compare the absorbed doses between 5 inch by 12 inch and 6 inch by 12 inch image field for the Orthopos plus. Materials and Methods: Rando phantom and LiF TLD chips were used for dosimetry. The absorbed doses were measured at the thyroid gland, the submandibular gland, the parotid gland, the mouth floor, the maxillary sinus, the brain, the mandibular body, the mandibular ramus, the 2nd cervical spine and the skin over TMJ area. Results: The highest absorbed dose value was recorded at the mandibular ramus for the Orthopos plus with narrow image field. Higher absorbed dose values were recorded at the parotid gland, the mouth floor, the submandibular gland, and the 2nd cervical spine. The doses in the parotid gland were 597 μGy and 529 μGy with Orthopos plus, 638 μGy with Panelipse, and 1094μGy with Panex-E. Corresponding figures for the mandibular ramus were 2363 Gy and 1220 μGy, 248 μGy, and 118 μGy. The absorbed doses to the thyroid gland, the maxillary sinus, the brain, and the skin over TMJ were very low. Conclusion: Higher exposure values were recorded for the Orthopos plus than Panelipse and Panex-E. There was no significant differences of the absorbed doses according to the image field size.

  • Solid freeform fabrication (SFF) technology plays a major role in industry and represents a reasonable percentage of industrial rapid prototyping/tooling/manufacturing (RP/RT/RM) development applications. However, SFF technology still has long way to progress to achieve satisfactory process speed, surface finish and overall quality improvement of its application. Today, three dimensional printing (3DP) technique that is one of SFF technology is receiving many interests, and is applied by various fields. It can fabricate three dimensional objects of solid freeform with high speed and low cost using ink jet printing technology. However, need long curing time after manufacture completion. And it must do post-processing process necessarily to heighten strength of objects because strength of fabricated objects is very weak. Therefore, in this study, we proposed an improved 3DP process that can solve problems of conventional 3DP process. The general 3DP process is method to spout binder simply through printer head on powder, but proposed process is method to cure jetted UV resin by UV lamp after jet UV resin using printhead on powder. The hardening of resin is achieved strongly at early time by UV lamp in proposed method. So, the proposed process can fabricate three dimensional objects with high speed without any post-processing.

A Study on the Self-Disclosure of Nurses After Human Relations Training (인간관계훈련 전후의 자기노출인식변화에 관한 연구)

  • This study attempted to measure self-disclosure changes of nurses a result of human relations training. The study population consisted of 49 nurses in Seoul. The instrument used in this study was the Self-disclosure questionnaire developed by Jourard in 1958, was designed to measure verbalized aspects of self-disclosure defined as a voluntary act of revealing personal data about oneself including beliefs, values, feelings and perceptions to another person. The SDG instrument is devided into six areas: attitude, taste and interest, personality, money, work and body. The main findings were summarized as follows: 1. After training, the subjects disclosed more than before. 2. Subjects tended to very the amount of self-disclosure with respect to the category of information to which an item about the self belonged. Two clusters of aspects emerged, a high disclosure cluster including Attitude, Tastes and Interests, Personality and Work, and a low disclosure cluster comprised of Money and Body. 3. Before and after training, there was significant difference in the self-disclosure to the peer nurses. Subjects showed the highest self-disclosure to friend, with lesser amounts to patient. 4. Charge and head nurses showed the highest differences in self-disclosure resulting from human relations training.

A Study on high Quality of Antiwashout Underwater Concrete (수중불분리성콘크리트의 고품질화 연구)

  • In case of constructing concrete structure under water, generally concrete mixed with antiwashout admixture, high range water reducer, or AE-water reducing agent etc has been manufactured to ensure the quality of antiwashout underwater concrete because of being difficulty in ascertaining construction situation by the naked eye. The properties of high quality antiwashout underwater concrete that were aimed at affluent fluidity, workability and the compressive strength of 450 kgf/$\textrm{cm}^2$ at 28 ages using two types of blended cements are following as;(1) Setting time of antiwashout underwater concretes using blended cements was more greatly delayed than that of control concrete, however, was satisfied with criteria value of "Quality standard specification of antiwashout admixture for concrete".(2) As a test results of slump flow, efflux time and box elevation of head, it was found that workability of high quality antiwashout underwater concrete was improved. (3) Heat evolution amount of OPC was 1.5 times as high as that of two types of bended cements in 72 hours. (4) Suspended solids of antiwashout underwater concrete using blended cements was more than that of control concrete, also compressive strength of high quality antiwashout underwater concrete was very low in early age, but was better than that of control concrete as to increasing ages.

Gamma Camera Based FDG PET in Oncology

  • Positron Emission Tomography(PET) was introduced as a research tool in the 1970s and it took about 20 years before PET became an useful clinical imaging modality. In the USA, insurance coverage for PET procedures in the 1990s was the turning point, I believe, for this progress. Initially PET was used in neurology but recently more than 80% of PET procedures are in oncological applications. I firmly believe, in the 21st century, one can not manage cancer patients properly without PET and PET is very important medical imaging modality in basic and clinical sciences. PET is grouped into 2 categories : conventional(c) and gamma camera $based_{(CB)}$ PET. $_{CB}PET$ is more readily available utilizing dual-head gamma cameras and commercially available FDG to many medical centers at low cost to patients. In fact there are more $_{CB}PET$ in operation than cPET in the USA. $_{CB}PET$ is inferior to cPET in its performance but clinical studies in oncology is feasible without expensive infrastructures such as staffing, rooms and equipments. At Ajou university Hospital, CBPET was installed in late 1997 for the first time in Korea as well as in Asia and the system has been used successfully and effectively in oncological applications. Ours was the fourth PET operation in Korea and I believe this may have been instrumental for other institutions got interested in clinical PET. The fellowing is a brief description of our clinical experience of FDG CBPET in oncology.
