Discomfort on Wearing Thigh-Length Compression Stockings in Orthopedic Surgical Patients (정형외과 수술환자의 대퇴길이 압박스타킹 착용 불편감)

  • Purpose: This study aimed to investigate the degree of discomfort and to identify factors related with discomfort in patients wearing thigh-length compression stockings after orthopedic surgery. Methods: The participants were 105 patients wearing thigh-length compression stockings after orthopedic surgery at two hospitals in B city. Data were collected by the scale of discomfort in wearing compression stockings from July 5 to October 20, 2011. Results: Mean score of discomfort on wearing compression stockings was significantly higher at post-op. day 3 than day 1 (t=-2.30, p=.004). Discomfort scores for 'itching skin' and 'tightened leg' items were higher than others. There was significant difference in discomfort at post-op. day 1 according to surgical regions. The total score of discomfort at post-op. day 3 was positively related with the body mass index (r=.20, p=.041). Conclusion: Wearing thigh-length compression stockings to prevent deep vein thrombosis may cause physical and psychological discomfort for orthopedic surgical patients. To reduce discomfort in wearing compression stockings, nurses need to intervene discomfort and problems related to the stockings post-operatively.

  • Inflammatory bowel disease (IBD) is a well-recognized risk factor for thrombotic events in adults but data on children are scarce. In the great majority of adult patients, thrombotic events are usually deep vein thrombosis and pulmonary embolism. Other sites such as jugular veins are extremely rare. We present a case of Lemierre syndrome in an adolescent girl with active ulcerative colitis and discuss possible risk factors. This is the first reported case of severe Lemierre syndrome with thrombus extension to cranial veins in a patient with ulcerative colitis. Early recognition of Lemierre syndrome in patients who present with rapidly worsening symptoms of neck pain, fever and signs of pharyngitis is imperative because it increases a chance of favorable prognosis. It is important for pediatricians treating IBD patients not to underestimate possible thrombotic events in children with IBD. Recognition of additional risk factors is crucial for prompt diagnosis and adequate treatment.

  • Hypercoagulability disorders are commonly encountered in clinical situations in patients with a variety of cancers. However, several hypercoagulability disorders presenting as first symptoms or signs in cancer patients have rarely been reported. We herein described a case of a woman with adenocarcinoma of the lung presenting with deep vein thrombosis, nonbacterial thrombotic endocarditis, recurrent cerebral embolic infarction, and heart failure.

  • The incidence of pulmonary thromboembolism increases with age. The risk factors of pulmonary thromboembolism include surgery, malignancy, obesity, lupus anticoagulants, and vascular conditions such as deep vein thrombosis. Thromboembolism in younger patients or in unusual locations, the possibility of primary thrombophilic conditions should be considered. Primary thrombophilic states include myeloproliferative disorders (MPD). JAK2 V617F mutation is found commonly in patients diagnosed with MPD, in 90~95% of polycythemia vera (PV) and in 50~60% of essential thrombocytosis (ET) patients. Sometimes the JAK2 V617F mutation is found in cases without MPD. The relationship between JAK2 V617F mutation and thrombosis has not been defined. Recently, clinical evidence suggests that this mutation may be variably associated with thrombosis. We present one case of pulmonary thromboembolism in a young patient, who was positive for the JAK2 V617F mutation and did not have MPD.

Experimental Studies on the Effect of Qingbaozhuyutang(?胞逐瘀湯), Bee Veenom on the Blood Stasis Pattern (청포축어탕 및 봉독(蜂毒)이 어혈병태모형(瘀血病態模型)에 미치는 영향(影響))

  • In order to investigate the effect of Qingbaozhuyutang(淸胞逐瘀湯), Bee Veenom on 'Blood Stasis Pattern', here We regard Thrombosis and Elevated Blood viscosity as Blood Stasis Pattern. rats were injected Endotoxin into the caudal vein to make experimental thrombosis model(control group), and at sample group, rats were orally administrated solid extract of Qingbaozhuyutang(淸胞逐瘀湯) 0.5g/200g or were injected Apitoxin 1mg/Kg into the abdominal region(sample group), and then we observed platelet, fibrinogen, prothrombin time and FDP level. Another group were injected HA into the muscular rump to make experimental elevated blood viscosity instead of Endotoxin. Thereafter we measured whole blood viscosity and plasma viscosity. The results were summarized as follows. 1. Platelets were increased significantly in sample I compared with the control group. 2. Fibrinogen levels were increased significantly in sample I compared with the control group. 3. Prothrombin times were shortened significantly in sample II and sample III compared with the control group. 4. FDP levels were decreased significantly in sample II and sample III compared with the control group. 5. Whole blood viscosity and plasma viscosity were decreased significantly in all shear rates compared with the control group. According to the above results, it is considered that Qingbaozhuyutang(淸胞逐瘀湯) has significant effects on platelets, fibrinogens, prothrombin times, FDP levels, whole blood viscosity and plasma viscosities, therefore it seems to be applicable to disease related to 'Blood Stasis Pattern' through the thrombosis and elevated blood viscosity. But, Veenom has a little effect. the mechanism concerned for the effects is to be investigated in future.

  • Varicella zoster virus (VZV) causes two diseases: Varicella, a generalized, primary infection, and herpes zoster (zoster), a secondary infection caused by latent VZV reactivation. Zoster can also be caused by latent VZV reactivation after a varicella vaccination. The complications associated with varicella include cutaneous infections, which are the most common, as well as pulmonary and neurological involvement. However, a deep venous thrombosis (DVT) has been rarely described as a varicella-associated complication. Here, we describe the case of a child with varicella zoster who developed a DVT that completely resolved after intravenous acyclovir and subcutaneous low-molecular-weight heparin treatment.

Study on the effect of Boyanghwanotang on Thrombosis and Elevated Blood Viscosity (혈전증(血栓症) 및 고점도혈증(高粘度血症)에 관(關)한 보양환오탕(補陽還五湯)의 실험적(實驗的) 연구(硏究))

  • In order investigate the effect of Boyanghwanotang (BT) on thrombosis, Sprague-Dawley rats injected endotoxin into the caudal vein were oral administrated solid extract of BT-182. 4mg/200g (Sample A) and 364.8mg/200g (Sample B), and then observed Platelet, fibrinogen, prothrombin time and FDP. Another group injected hydrocortisone acetate into the muscular rump were adopted to observe the effect of BT on elevated blood Viscosity, administrated Sample A and Sample B, thereafter measured body weight, body temperature, whole blood viscosity, plasma viscosity, hematocrit, RBC, WBC, hemoglobin, glucose, albumin, total protein, triglyceride, cholesterol and prothrombin time. 1. Platelet was increased significantly in Sample B compared with the control group. 2. Fibrinogen of the trial groups compared with the control group was increased, and revealed a significance in Sample B. 3. Prothrombin time was decreased significantly in the trial groups compared with the control group. 4. FDP was decreased in the trial groups compared with the control group. 5. Body weight was increased significantly in Sample B after 7 days, body temperature revealed a significance in the trial groups after 5 days. 6. Whole blood viscosity and plasma viscosity of lower shear rates, both were highly decreased in the trial groupn than the control group. 7. The increase of WBC and decrease of hematocrit, RBC and hemoglobin was showed in the trial groups, and Sample B revealed a significance on the increase of WBC and decrease of RBC. 8. The increase of prothrombin time and decrease of glucose, albumin, total protein, triglyceride and cholesterol was showed in the trial group, and Sample B revealed a Significance on the increase of prothrombin time and decrease of albumin, total protein and triglyceride. 9. As compare with Sample A and Sample B, generally Sample B was highly Significant. According to the above results, it was considered that Boyanghwanotang may be the curative effects for thrombosis and elevated blood viscosity.

Clinical Analysis of the Arterial Bypass Surgery for Chronic Ischemia of the Lower Extremities (하지 만성 허혈에 대한 동맥 우회술의 임상적 고찰)

  • Arterial bypass for the chronic ischemia of the lower extremities underlying atherosclerotic obliterans has been performed with a number of alternative conduits from 1941 by Kunlin. It is indicated for the limb salvage of patients with threatened limb loss despite of several controversies in surgical treatment of atherosclerotic obliterans. From March 1991 to January 1995, 26 arterial bypasses were performed in 23 patients with the chronic ischemia of the lower extremities in our hospital. Their mean follow up period is 18.9 months ranging from 4 months to 44 months. Mean age is 60.9 years ranging 47 years to 76 years and the most prevalent incidence is the 6th decade. 21 patients are male and 2 patients are female. 19 of 23 patients are smokers. Clinical classifications by Fontaine are class II[21.7% , class III[34.8% and class IV[43.5% .Diabetes mellitus[47.8% , hypertension[43.5% , hyperlipid-emia[26% , tuberculosis[21.7% , cerebrovascular accident[13.0% and cardiac diseases[8.7% are associated. Aorto-single femoral bypass in 4 cases, aorto-bifemoral bypass in 5 cases, aortofemoral & femoropopliteal bypass in 2 cases, femoropopliteal bypass in 10 cases, popliteotibial bypass in 3 cases, femoropedal bypass[composite graft bypass in 2 cases were surgically approached. There are complicated early thrombosis in 4 cases those are required immediately reoperation, wound infection in 3 cases, hematoma in 3 cases, and so on. Postoperative complication rate is 53.8%.Postoperative patency rates are 84.6% at 6 months, 75.0% at 1 year, 70.0% at 2 years and 66.7% at 3 years. We usually used 6 mm & 8 mm graft for bypass, and the rate of thrombosis formation is 28.6%[2/7 in 6 mm graft and 12.5%[2/16 in 8 mm separately. In according to the graft materials, the rate of thrombosis formation is higher in the group using artificial graft than in that using autologous saphenous vein[16.6% vs 12.5% . Limb salvage rate is 76.9%. Postoperative mortality rate is zero %.

Three-dimensional conformal radiotherapy for portal vein tumor thrombosis alone in advanced hepatocellular carcinoma

  • Purpose: We sought to evaluate the clinical outcomes of 3-dimensional conformal radiation therapy (3D-CRT) for portal vein tumor thrombosis (PVTT) alone in patients with advanced hepatocellular carcinoma. Materials and Methods: We retrospectively analyzed data on 46 patients who received 3D-CRT for PVTT alone between June 2002 and December 2011. Response was evaluated following the Response Evaluation Criteria in Solid Tumors. Prognostic factors and 1-year survival rates were compared between responders and non-responders. Results: Thirty-seven patients (80.4%) had category B Child-Pugh scores. The Eastern Cooperative Oncology Group performance status score was 2 in 20 patients. Thirty patients (65.2%) had main or bilateral PVTT. The median irradiation dose was 50 Gy (range, 35 to 60 Gy) and the daily median dose was 2 Gy (range, 2.0 to 2.5 Gy). PVTT response was classified as complete response in 3 patients (6.5%), partial response in 12 (26.1%), stable disease in 19 (41.3%), and progressive disease in 12 (26.1%). There were 2 cases of grade 3 toxicities during or 3 months after radiotherapy. Twelve patients in the responder group (15 patients) received at least 50 Gy irradiation, but about 84% of patients in the non-responder group received less than 50 Gy. The 1-year survival rate was 66.8% in responders and 27.4% in non-responders constituting a statistically significant difference (p = 0.008). Conclusion: Conformal radiotherapy for PVTT alone could be chosen as a palliative treatment modality in patients with unfavorable conditions (liver, patient, or tumor factors). However, more than 50 Gy of radiation may be required.

Portal Vein Thrombosis in a Dog with Dirofilariasis (심장사상충에 감염된 개에서 발생한 문정맥혈전증)

  • A ten-year-old female mongrel dog was presented to Veterinary Medical Center, Chungbuk National University with the signs of anorexia, weakness, and hemoglobinuria. Patient had been diagnosed as dirofilariasis based on heartworm antigen test and treated with adulticide (melarsomine) at local hospital one day before admission. On laboratory examinations, there were hypochromic and microcytic regenerative anemia, thrombocytopenia, moderate neutrophilia, and increase ALT, AST, and ALP. Radiographic exam showed main pulmonary artery bulging, pulmonary infiltration and hypervascularity, reduced abdominal serosal detail and mild hepatomegaly. Abdominal ultrasonographic exam showed mild peritoneal effusion and large hyperechoic thrombi at trifurcation of the porta hepatica and the splenic vein. In addition, intraluminal low density area and intravascular filling defect were confirmed on contrast enhanced CT scanning at the same anatomic locations. Patient was treated with anticoagulant and thrombolytic therapy. On day 42 after treatment, complete resolution of thrombi was confirmed via ultrasonography and improvement of clinical signs was observed.
