  • Purpose : The purpose of this study was to investigate the effect of cross bag weight and carrying bag position type on vastus medialis oblique and vastus lateralis muscle activity ratio during step up. Method : Twenty healthy subjects participated in this study. subjects randomized performed step up with bag weight less than 15%, more than 15% of totalbody weight and without bag and carrying bag position ipsilateral and contralateral. muscle activity was measured with surface electromyography. One-way analysis of variance with repeated measures was used with a significance level of 0.05 Result : The results of this study were as follows: 1. The muscle activity ratio decreased significantly ipsilateral bag position more than 15% of body weight(p<.05). Also, the muscle activity ratio increased significantly contra lateral bag position more than 15% of body weight(p<.05). Conclusion : In conclusion, it was found that applied carrying bag position with weight on VMO,VL muscle activity ratio during step up down. Therefore carrying bag position can be used to selective activate vastus medialis oblique muscle in subjects with patellofemoral pain syndrome.

  • Purpose: The purpose of this study was to investigate electromyographic(EMG) activity of vastus medialis oblique(VMO) and vastus lateralis(VL) following the application of patellar taping during stair stepping. Methods: Both VMO-VL onset timing and VMO/VL ratio of 15 participants with patellofemoral pain syndrome (PFPS) were measured using an surface EMG uint(Myosystem 1400A, Noraxon Inc., USA). The measurements were taken under three conditions in random order of patellar taping, placebo taping, and no-taping. Data were analysed using $1{\times}3$ repeated measures ANOVA. Results: There was a significant difference with patellar taping on VMO/VL amplitude compared with the placebo taping and no-taping conditions during stair stepping, but no significant difference on VMO-VL onset timing. Conclusion: These result indicate that patellar taping on the vastus medialis oblique has effect on the VMO/VL ratio. Increased values of VMO/VL ratio may contribute to patellar realignment and explain the mechanism of pain reduction following patellar taping in participants with PFPS.

Comparison of Muscle Activity of Vastus Lateralis and Medialis Oblique among Knee Extension Angles at 90°, 135°, 180° in Sitting Position

  • Purpose: This study compared the muscle activities of the Vastus medialis oblique (VMO) and the Vastus lateralis (VL) at three different knee extension angles: 90°, 135°, and 180° in the sitting position. Methods: Twenty subjects between 20 and 30 years of age participated in the study. A mobile phone application called the Clinometer was used to measure the knee joint angle. Electromyography (EMG) was performed to measure the muscle activities of the VMO and VL muscles during knee isometric extension exercises. The pulling sensor was used to maintain 70% of the maximum strength of the knee extensor continuously in the sitting position. After attaching the EMG sensor, the subjects were asked to perform isometric knee extension exercises randomly among three knee extension angles (90°, 135°, or 180°) in the sitting position. One-way repeated measures analysis of the variance and a Bonferroni post hoc test was used to identify the VMO and VL muscle activity during knee extension angles among 90°, 135°, and 180°. Results: The VMO and VL muscle activities increased with increasing knee extension angle in the sitting position (p<0.01). Conclusions: Knee extension exercise at a 180° angle in the sitting position can be recommended to increase the muscle activity of the VMO and VL muscle activities efficiently.

  • Background: The purpose of this study is to identify effects of neuro-muscular control training on vastus medialis oblique (VMO) after menisectomy of the knee. Methods: The subjects of this study are women aged 42 and 39 each who did menisectomy. Case 1 was applied quadriceps setting exercise and neuro-muscular contrlol training and case 2 was applied quadriecps setting. Intervention was done 5 times a week for 2 weeks. Measurement of muscle activity on VMO and vastus lateralis (VL) was standardized signals of each muscle to %RVC using surface EMG. Results: On comparison of exercise before and after on VMO and VL, VL activation of case 2 was increased more than case 1. Conclusion: Quadriecps-setting exercise and selective neuro-muscular control training of VMO is effective intervention on VMO activity and muscle activity ratio of VMO to VL.

Effect of the Abdominal Bracing Maneuver on Muscle Activity of the Trunk and Legs during Walking in Healthy Adults

  • Objective: This study aimed to investigate the effect of the application of abdominal brace techniques on muscle activity of the trunk and lower extremities when walking. Design: Cross-sectional study Methods: This study was conducted on 26 healthy adults in their 20s, and the subjects performed two conditions in random order: walking with the abdominal bracing technique and walking in an abdominal relaxation state (normal gait). Muscle activity was measured on the dominant side of all subjects using surface electromyography, and the attachment sites were the erector spinae, external oblique, internal oblique, vastus lateralis, and vastus medialis oblique muscles. Each condition was measured three times to calculate and analyze the average value. Results: When walking using the abdominal brace technique, the muscle activity of the erector spinae, external oblique, internal oblique, and vastus lateralis increased significantly (p<0.05), and the muscle activity of the vastus medialis increased as well but was not significant. Conclusions: The results of this study indicate that it is possible to be used as an effective guide to increasing the muscle activity and stability of the trunk and lower extremities through the application of the abdominal bracing technique during walking.

신발굽 높이와 Wedge 위치 변화가 하지 근활성도에 미치는 영향 (Effect on the Lower Extremity Muscle Activity According to the Heel Height and Wedge Position)

  • 본 연구는 30명의 여성에게 내 외측 wedge 삽입과 함께 flated heel 또는 5cm heel을 착용하도록 한 후, 넙다리네갈래근 중 안쪽빗넓은근(VMO)과 가쪽넓은근(VL)의 근활성도를 측정하여 비교하였다. 연구 결과 내외측 wedge 모두에서 flated heel 군에서는 가쪽넓은근이, 5 cm heel 군에서는 안쪽빗넓은근의 근활성도가 높게 나타났으며 가쪽넓은근에 대한 안쪽빗넓은근의 근활성도비(% VMO/VL) 또한 5cm heel 군에서 유의하게 높게 나타났다. 이는 안쪽빗넓은근과 가쪽넓은근의 근활성도에 영향을 미치는 외적 변수가 wedge의 내외측 위치보다는 힐의 높이에 의함을 알 수 있었다. 차후 무릎통증이나 무릎 불안정성으로 인한 하지 재활훈련 또는 하지보조기 제작 시에 하지 근육의 선택적 활성화를 유도하기 위한 변수로써 heel 높이를 고려할 수 있을 것이다.

Comparison of the Electromyographic Changes in the Vastus Medialis Oblique and Vastus Lateralis Muscles According to the Knee Joint Angle During Squat Exercise Using a Gym Ball

  • Background: Squatting is one of the best functional exercises to strengthen the quadriceps muscle in independent gait and activities of daily living. Although the use of a gym ball during squat exercise is the most common way of strengthening the vastus medialis oblique (VMO) muscle, published data on this subject are limited. Objects: The purpose of this study was to compare the sequential muscle activation of the VMO and vastus lateralis (VL) muscles during squat exercise using a gym ball at different knee flexion angles. Methods: Forty healthy adults were randomly divided into experimental (squat exercise using a gym ball) and control (squat exercise alone) groups, in which squats were performed at $45^{\circ}$ and $90^{\circ}$ knee flexion. Electromyographic (EMG) activity data were collected over 10 seconds under the 2 angles of knee flexion ($45^{\circ}$ and $90^{\circ}$). Results: There was significant group and time interaction effect for VMO and VL muscle activation at $45^{\circ}$ knee flexion. This was similarly demonstrated at $90^{\circ}$ knee flexion. No significant group main effect and time main effect for VMO and VL muscle activation were noted at $45^{\circ}$ knee flexion, respectively. In contrast, there was significant group main effect and time main effect for VMO and VL muscle activation at $90^{\circ}$ knee flexion. These significant differences were demonstrated through two-way analysis of variance over repeated measurements, suggesting that the EMG activity of the VMO muscle during squatting with a gym ball showed remarkable improvement compared to that of the VL muscle. Conclusion: This research suggests that squat exercise using a gym ball may be more beneficial in improving the activity of VMO than of the VL muscle at both $45^{\circ}$ and $90^{\circ}$ of knee flexion, respectively. We highly recommend squat exercises with a gym ball for selective strengthening of the VMO muscle in knee rehabilitation.

건강한 성인의 경골 회전 테이핑 방법에 따른 내측사광근과 외측광근의 근활성도 비교 (A Comparison of Vastus Medialis Oblique and Vastus Lateralis Electromyography Activities According to Different Tibial Rotation Taping Methods in Healthy People)

  • PURPOSE: Femur and tibia alignment in the knee joint is important to the biomechanics of lower limb movement. The purpose of this study was to compare vastus medialis oblique (VMO) and vastus lateralis electromyographic muscle activities according to tibial rotation taping methods. METHODS: Twenty-nine healthy subjects (13 males and 16 females) in the 20s, without knee joint-related diseases or disorders, participated in our study. After identifying each subject's dominant foot, the maximal voluntary isometric contraction (MVIC) was determined using a manual muscle tester. The activity of each target muscle was measured at 50% MVIC in isometric muscle contraction and at a $30^{\circ}$ knee flexion position before and after applying internal and external rotation taping by the Mulligan concept and in the neutral position. Non-elastic tape was used to stabilize the tibia rotation position. RESULTS: In the males, VMO muscle activity was significantly increased in the tibia internal rotation position ($47.2{\pm}14.6$, $mean{\pm}SD$) than in the neutral position ($39.3{\pm}14.9$) (p<.05). CONCLUSION: The results of this study indicate that when applying tibia internal rotation taping in healthy males, VMO muscle activity significantly increases during isometric extension of the knee. Therefore, this study provides a basis for selecting the appropriate taping method, in consideration of the available treatments in clinical practice for patients with knee problems.

동적 반 웅크리기 운동 시 공을 사용한 엉덩이관절 모음이 안쪽빗넓은근과 가쪽넓은근의 근활성도에 미치는 영향 (The Effect of Hip Adduction on the Activation of the Vastus Medialis Oblique and the Vastus Lateralis)

  • Purpose : The purpose of this study was to determine the effects of hip adduction using a ball on the activation of the vastus medialis oblique (VMO) and the vastus lateralis (VL) during dynamic semisquat exercises. Methods : Twenty seven participants performed three repetitions of a double-leg semisquat and squeeze semisquat (semisquat with hip adduction using a ball) at $60^{\circ}$ knee flexion. The activation of the VMO and the VL was recorded at dominant leg during both semisquat exercises using surface electromyography (MP 100). EMG data were normalized to the maximal voluntary isometric contraction (MVIC) of the quadriceps produced during seated, isometric knee extension. Results : Results of paired t-test analyses revealed that there were no differences between activity of the VMO and the VL in both double-leg semisquat and squeeze semisquat. The activity of the VMO was significantly increased in squeeze semisquat whereas there was no difference in activity of the VL between double-leg semisquat and squeeze semisquat. Conclusion : Combining hip adduction using a ball with semisquat at $60^{\circ}$ knee flexion preferentially increases the activity of the VMO. We can conclude that semisquat at $60^{\circ}$ knee flexion with hip adduction using a ball selectively recruits the VMO. Rehabilitation for quadricep imbalance should consider these findings when selecting exercises which could preferentially activate the VMO.

PNF 다리 굽힘 패턴 시 복부 드로잉-인 기법 동시적용이 동측 몸통과 다리의 근활성도에 미치는 효과 (Effect of PNF Leg Flexion Pattern on Muscle Activity of Ipsilateral Trunk and Leg with and without Abdominal Drawing-in Maneuver)

  • Purpose: The purpose of this study was to investigate the effect of the simultaneous abdominal drawing-in maneuver (ADIM) on the muscle activity of the ipsilateral trunk and leg during proprioceptive neuromuscular facilitation (PNF) leg flexion, adduction, and external rotation with knee flexion (D1) patterns. Methods: The participants were 20 healthy adult males and females (18 males and 2 females). The maneuvers were performed by a physical therapist who fully understands the PNF leg patterns (D1) and their application in clinical practice. The participants were trained and allowed to practice for 15 minutes prior to applying ADIM, to ensure adequate learning as evidenced by the pressure biofeedback unit. In this study, we measured the muscle activity of the trunk and leg when the PNF leg pattern (D1) was performed by the physical therapist either sustaining or releasing the ADIM. Muscle activity was measured on the right transverse abdominis muscle (TRA), the external abdominal oblique muscle (EO), the internal abdominal oblique muscle (IO), the erector spinae muscle (ES), the vastus medialis oblique muscle (VMO), the vastus lateralis oblique muscle (VLO), and the tibialis anterior muscle (TA) and compared using the mean values from averaging three repeated measurements. Results: The muscle activity of the transversus abdominis, the external abdominal oblique, the internal abdominal oblique, the vastus medialis oblique, and the vastus lateralis oblique was significantly greater (p < 0.05), and the muscle activity of the erector spinae was significantly less (p < 0.05) during PNF leg pattern (D1) when the ADIM contraction was sustained compared to when it was not. Conclusion: These results suggest that sustaining ADIM during PNF leg pattern (D1) training increases the trunk and leg muscle activity, resulting in more effective training.
