Effect of Prostaglandin $E_1$ on Cutaneous Microcirculation of Flap or Replantation

  • Recently prostaglandin $E_1(PGE_1)$ has been shown to ensure flap survival by producing vasodilation of the peripheral vessels and platelet disaggreation. However, direct observation and detailed quantitative studies of the effects of $PGE_1$ on the cutaneous microcirculation have not been reported. In the present study, we investigated cutaneous microcirculatory changes in the rabbit ear chamber(REC) with an intravital microscope following intravenous administration of $PGE_1$. The results obtained in this study indicate that $PGE_1$ administered intravenously at a rate of 200ng/kg/min might act directly on the vessels and cause dilatation of metarterioles and capillaries without affecting vasomotion and systemic blood pressure. Clinically in order to evaluate the effect of an intravenous administration of $PGE_1$ on the cutaneous microcirculation, cutaneous blood flow, skin temperature and transcutaneous $Po_2$ in the pedicle or free flap of operated patients were evaluated by the combination of several measurements following the administration of $PGE_1$. The present study suggests that improvement of cutaneous microcirculation by $PGE_1$ may enhance the survival rate of flap or replantation. Both vessel arterial ischemia and venous congestion are main factors of tissue necrosis in the flap surgery. Vasodilatory or antithrombotic agents have been used in salvage of flap necrosis. However, the therapeutic effects of those drugs are still not well elucidated. Recently prostaglandin $E_1(PGE_1)$ has been shown to ensure flap survival by producing vasodilatation of the peripheral vessels and platelet disaggregation[1-3]. Emerson and sykes[4] have obtained significant improvement in the flap survival in the rat using $PGI_2$. Suzuki et al.[5] have reported prolonged flap survival length by using $PGE_1$ in the rabbit and concluded that $PGE_1$ improved the microcircuration in the flap. However, direct observation and detailed quantitative studies of the effects of $PGE_1$ on the cutaneous microcirculation have not been reported. In the present study, we investigated microcirculatory changes in the rabbit ear chamber[6,7] with an intravital microscope following intravenous administration of $PGE_1$.

한국산 생약제들의 혈압강하작용에 대한 연구(III) -박달수피의 혈압에 대한 작용- (Effect of Betula schmidtii Regel on Blood Pressure)

  • This study was undertaken to search for a new antihypertensive or hypotensive agents in natural crude products in Korea. Betula schmidtii Regel, a tree belonging to the Betulaceae family, is cultivated for the most part of our country, and their cortecies or tree milk has been used as folk remedy, which exerted good antiinflammatory effect on rheumatic fever, arthritis or rheumatism. The rats were treated with several extracts from Betula schmidtii Regel, that is, extracts of water, methanol from inner cortecies, extracts of ether, methanol, hexane, chloroform or butanol from outer and total cortecies. In this experiment, we measured the blood pressure and heart rate in anesthetized rats, and atropine, diphenhydramine, phentolamine, propranolol, epinephrine, hexamethonium and hydralazine were pretreated prior to Betula extracts to clarify the mechanism of the hypotensive action. The results of the experiment were as follows; 1. BOIE(Betula outer cortex water-insoluble extract) and BICE(Betula inner cortex chloroform extract) elevated the blood pressure and heart rate. 2. BIME(Betula inner cortex methanol extract), BIBE(Betula inner cortex butanol extract) and BIWE (Betula inner cortex water extract) significantly lowered the blood pressure and heart rate. 3. With the pretreatment of hydralazine, the hypotensive effect of Betula schmidtii Regel was significantly blocked. From the above results, it is suggested that the hypotensive effect of Betula schmidtii Regel is due to the direct vasodilation of blood vessel.

5종의 한약 추출물이 함유된 헤어 에센스제품(HHRHG0202-80)이 in vitro 및 in vivo에서 육모 및 탈모 방지에 미치는 효과 (The Effect of Hair Essence (HHRHG0202-80) Containing Five Herbal Extracts on Hair Growth and the Prevention of Alopecia in vitro & vivo)

  • Backgrounds : Androgenetic alopecia is a relatively common disorder, but its precise mechanism is not elucidated. There are two commercial drugs approved by FDA. One(finasteride) has an inhibition activity of 5$\alpha$-reductase(type 2) and the other(minoxidil) has a vasodilation activity. Objectives : A verified herbal remedy for baldness is needed for medicinal treatment or preventing alopecia, which could be demonstrated by alopecia-related in vitro & in vivo tests Methods : On the basis of oriental pharmacognosy, we classified many herbal medicines into four groups (malnutrition, aging, alopecia and gray hair) according to its effect. The mitosis induction of hairy dermal papillae cell and the metabolic inhibition for type 2 $5{\alpha}-reductase$ were tested with five herbal extracts. Also, five herbal extracts were added to the normal essence formulation (HHRHG0202-80) in ranges of 0.1~0.3%, which was applied two-mouse models to validate each hair growing activity in vivo. Results : Stimulation of follicular papillae cell proliferation was observed in treatment of three herbal extracts (Glycyrrhizae radix:159.7%, Corni fructus : 144.7%, and Coicis semen : 136.6%) at a dose of $10\mu\textrm{g}/ml$. Three herbal extracts (Biotae semen. Glycyrrhizae radix and Coicis semen) showed inhibitory activity for $5{\alpha}-reductase$(type 2) at 93.18%, 73.36% and 47.6%, respectively at the same dose. We observed the enhancement of hair growth activity in C57b1/6 mouse and the inhibition of alopecia in AGA mouse after topical administration of the hair essence. Conclusions : Hair essence product, which contains five medicinal plants, would be used for the remedy for male pattern baldness (MPB) and the other alopecia diseases.

인삼열매 추출물의 혈행개선과 피부톤 개선에 미치는 영향 (Effect of Ginseng-Berry Extract on the Improvement of Blood Microcirculation and Skin Brightness)

  • Several studies have demonstrated that ginseng-berry extract has several beneficial properties, including anti-inflammatory, antioxidant, and vasodilation properties. Ginseng-berry extract has also been shown to have the great potential against skin aging. Its beneficial mechanism against skin aging, however, has not been examined in detail. Also, the effects of ginseng-berry extract on microcirculation and skin cellular responses have not been investigated. Inhibition of skin microcirculation is the primary cause of many adverse biological effects, which is responsible for the skin aging and darkening. We investigated the beneficial effects of ginseng-berry extract on blood circulation, transcutaneous oxygen pressure in vivo model and also on skin microcirculation, cellular response and skin brightening effect in clinical trial. We found that oral administration of ginseng-berry extract markedly increased blood flow rate and transcutaneous $O_2$ pressure, but decreased transcutaneous $CO_2$ pressure. Also, it improved skin tone on cheeks, as is skin brighteness. These results suggest that ginseng-berry extract is a potent candidate for the treatment of skin aging and brightening by improving skin microcirculation.

  • Nitric oxide(NO) is synthesized via the oxidation of L-arginine by a family of nitric oxide synthases(NOS), which are either constitutive(cNOS) or inducible(iNOS). The induction of iNOS in tissues can lead to the sustained production of high concentrations of NO which may exert pro-inflammatory effects including vasodilation. edema, cyototoxicity, and its activity can be mediated by various pro-inflammatory cytokine, including interferon ${\gamma}(INF-{\gamma})$. tumor necrosis factor, IL- 1 and IL-6. The enzyme, iNOS, became a new target for pharmacologcal research with the aim to find new substances for the treatment of chronic inflammatory disorders. Murine macrophages produce large amounts of NO when activated with $TFN-{\gamma}$ plus LPS. The murine macrophage-like cell line, RAW 264.7, is a suitable cell model on which to perform vitro studies regarding the iNOS system. Semen jugrandis is a fatty walnut seed found in Korea. The walnut have been used in foik medicine to improve virility, to relieved asthma, and to relieve constipation. Sesquiterpenelactones were isolated from this plant. In the course of screening for NO inhibitory activity from medicnial plants, the aqueous extract of this plant was found to have a significant activity. The result are summarized as followings. 1. The viability of cells incubated in the presence of semen jugrandis increased mare than non incubated cells. 2. Semen jugrandis suppressed the production of NO in tissues dependent on density. 3. Semen jugrandis suppressed the induction of iNOS in tissues dependent on density can lead to reduced production of NO. 4. Semen jugrandis suppressed the production of superoxide in tissue depend on density. According to the above mentioned results, semen jugrandis could be applied production of NO and superoxide can lead to reduction of chronic inflammatary. And as a depence matter come into a virus of microbe and tumor cells.

A Curcuminoid and Two Sesquiterpenoids from Curcuma zedoaria as Inhibitors of Nitric Oxide Synthesis in Activated Macrophages

  • The overproduction of nitric oxide (NO) by inducible nitric oxide synthase (iNOS) is known to be responsible for vasodilation and hypotension observed in septic shock and inflammation. Inhibitors of iNOS, thus, may be useful candidates for the treatment of inflammatory diseases accompanied by overproduction of NO. In the course of screening oriental anti-inflammatory herbs for the inhibitory activity of NO synthesis, a crude methanolic extract of Curcuma zedoaria exhibited significant activity. The activity-guided fractionation and repetitive chromatographic procedures with the EtOAc soluble fraction allowed us to isolate three active compounds. They were identified as 1,7-bis (4-hydroxyphenyl)-1,4,6-heptatrien-3-one (1), procurcumenol (2) and epiprocurcumenol (3) by spectral data analyses. Their concentrations for the 50% inhibition of NO production $(IC_{50})$ in lipopolysaccharide (LPS)-activated macrophages were 8, 75, 77 ${\mu}M$, respectively. Compound 1 showed the most potent inhibitory activity for NO production in LPS-activated macrophages, while the epimeric isomers, compound 2 and 3 showed weak and similar potency. Inhibition of NO synthesis by compound 1 was very weak when activated macrophages were treated with 1 after iNOS induction. In the immunoblot analysis, compound 1 suppressed the expression of iNOS in a dose-dependent manner. In summary, 1,7-bis (4-hydroxyphenyl)-1,4,6-heptatrien-3-one from Curcuma zedoaria inhibited NO production in LPS-activated macrophages through suppression of iNOS expression. These results imply that the traditional use of C. zedoaria rhizome as anti-inflammatory drug may be explained at least in part, by inhibition of NO production.

Monocrotaline에 의해 유발된 폐고혈압 흰쥐에 있어 Enalapril 및 Ginkgo biloba Extract(EGb 761)의 병용 투여시 억제효과 (Inhibitory Effect of Enalapril in Combination with Ginkgo biloba Extract (EGb 761) on the Monocrotaline-induced Pulmonary Hypertension Rats)

  • Effects of Ginkgo biloba extract (EGb 761) on the anti-pulmonary hypertensive action of enalapril were evaluated in rats. Pulmonary hypertension was induced by monocrotaline treatment (60mg/kg, i.p.) in normotensive rats. In the systolic pulmonary artery pressure, the control group was 33$\pm$2 mmHg, comparing to the normal group of 19$\pm$1 mmHg. That of enalapril group(20mg/kg/day, p.o.) was 26$\pm$2 mmHg. In the isolated lung preparation, acetylcholine, which was endothelium dependent vasodilator, induced the decrease of pulmonary artery perfusion pressure(-2.0$\pm$0.7 mmHg) in normal group, but the increase of that of 3.4$\pm$0.6 and 3.0$\pm$0.9 mmHg in control and enalapril group, respectively. And that of the combined group was -0.5$\pm$0.2 mmHg. In the isolated pulmonary artery, acetylcholine(10-5M) induced the relaxation of 65$\pm$6% in normal group, but 15 and 8% in control and enalapril group, respectively. And that of the combined group was resulted 55$\pm$2%. These results suggested that co-administration of Ginkgo biloba extract(EGb 761) potentiated the anti-pulmonary hypertensive effects of enalapril through the increase of pulmonary vasodilation due to the protection of endothelial cell by antioxidant action of Ginkgo biloba extract (EGb 761).

Transcranial Doppler를 이용(利用)한 태음인 중풍환자의 혈류속도(血流速度)와 혈관(血管) 반응성(反應性)에 대한 임상적(臨床的) 고찰(考察) (Clinical study of blood flow and vascular reaction in Taeumin CVA patients using Transcranial Doppler)

  • Purpose Brain vessles have autoregulation function, so even when perfusion pressure drops, cerebral blood flow remain stable by vasodilation. Latest research on this reserve of cerebral vessels is being done using TCD, which measures the reserve of the vessels. We did a research comparing cerebral vessel and peripheral vessel reserve between Taeumin, who are more likely to suffer CVA, and the normal. We observed blood flow of Internal carotid artery siphon and radial indicis artery of the two group with TCD. Method We picked 20 people out of patients diagnosed as cerebral infarction at Cheon-An Oriental hospital of Daejeon University. They were diagnosed as Taeumin with QSCCII questionnaire and constitutional differentiation. Using TCD, we measured highest blood flow rate, mean blood flow and asymmetric counting blood flow of Internal carotid artery siphon and radial indicis artery at rest. And then we measured again after stimulating cerebral vessels, by triggering hypercapnia by self apnea and peripheral vessels by palm heating. Result At rest, mean blood flow rate of Internal carotid artery siphon showed significant decrease compared to control group. Blood flow rate of Internal carotid artery siphon after hypercapnia showed significant decline in highest blood flow rate and mean blood flow compared to control group. Cerebral vessel reaction after the hypercapnia induction showed great change in experiment group than the control group. Peripheral vessel reaction after palm heating showed significant decline in experiment group compared to control group. Conclusion In conclusion, measuring the alteration of blood flow used in diagnosing cerebral infarction, is more sensitive when vessel stimulation is done. Non-invasive TCD is effective especially in case of Taeumin who are more likely to suffer vascular disorder than others.

Taurine relaxes human radial artery through potassium channel opening action

  • The vascular actions and mechanisms of taurine were investigated in the isolated human radial artery (RA). RA rings were suspended in isolated organ baths and tension was recorded isometrically. First, a precontraction was achieved by adding potassium chloride (KCl, 45 mM) or serotonin (5-hydroxytryptamine, 5-HT, $30{\mu}M$) to organ baths. When the precontractions were stable, taurine (20, 40, 80 mM) was added cumulatively. Antagonistic effect of taurine on calcium chloride ($10{\mu}M$ to 10 mM) -induced contractions was investigated. Taurine-induced relaxations were also tested in the presence of the $K^+$ channel inhibitors tetraethylammonium (1 mM), glibenclamide ($10{\mu}M$) and 4-aminopyridine (1 mM). Taurine did not affect the basal tone but inhibited the contraction induced by 5-HT and KCl. Calcium chloride-induced contractions were significantly inhibited in the presence of taurine (20, 40, 80 mM) (p<0.05). The relaxation to taurine was inhibited by tetraethylammonium (p<0.05). However, glibenclamide and 4-aminopyridine did not affect taurine -induced relaxations. Present experiments show that taurine inhibits 5-HT and KCl -induced contractions in RA, and suggest that large conductance $Ca^{2+}$-activated $K^+$ channels may be involved in taurine -induced relaxation of RA.

삼음교(三陰交) 침자(鍼刺)가 부교감신경(副交感神經)의 방광분지(膀胱分枝), 경골신경(脛骨神經) 및 혈압(血壓)에 미치는 영향(影響) (Neurologic Study of SP6(Sanyinjiao, 三陰交) Acupuncture on Bladder parasympathetic Nerve, Tibial Nerve and Blood Pressure in Rats)

  • Objective : The effects of SP6(Sanyinjiao, 三陰交) acupuncture stimulation on bladder parasympathetic nerve activity (PNA), tibial nerve activity (TNA) as well as blood pressure were examined in anesthetized rats and the results are as follows. Methods : The parasympathetic nerve leading to the bladder was exposed retroperitoneally, and the tibial nerve in the hindlimb was exposed through inguinal area. PNA and TNA were recorded using a bipolar platinum-iridium wire electrode and it was connected to preamplifier. Blood pressure was measured using cannula that was inserted into femoral artery and was recorded with blood pressure sensor that is connected with transducer amplifier. The mean value of all parameters measured for 30 seconds before the stimulation was expressed as 100%. An acupuncture needle was inserted into the SP6 region to a depth of 4 mm. Results : Under intact state (IS), acupuncture stimulation for 120 seconds induced significant increase of PNA which was accompanied with decrease of blood pressure. At the same time, TNA showed temporary increase only with acupuncture stimulation. In 4 mg/kg of naloxone-administrated state (NAS), significant decrease of PNA was seen and it was accompanied with increase of blood pressure. TNA also showed temporary increase only with acupuncture stimulation under the NAS just like the IS. Conclusion : consider that SP6(Sanyinjiao, 三陰交) acupuncture stimulation activated parasympathetic nerve and caused vasodilation to lead into the diuresis, but naloxone reversed the effect of acupuncture for antidiuresis.
