Arterial Spin Labeling MRI for Quantitative Assessment of Cerebral Perfusion Before and After Cerebral Revascularization in Children with Moyamoya Disease

  • Objective: To determine the correlation between cerebral blood flow (CBF) on arterial spin labeling (ASL) MRI and the degree of postoperative revascularization assessed on digital subtraction angiography in children with moyamoya disease (MMD). Materials and Methods: Twenty-one children (9 boys and 12 girls; mean age, 8.4 ± 3.6 years; age range, 3-16 years) with MMD who underwent both pseudocontinuous ASL MRI at 1.5T and catheter angiography before and after superficial temporal artery encephaloduroarteriosynangiosis were included in this retrospective study. The degree of revascularization in the middle cerebral artery (MCA) territory was evaluated on external carotid angiography and was graded on a 3-point scale. On ASL CBF maps, regions of interest were manually drawn over the MCA territory of the operated side at the level of the centrum semi-ovale and over the cerebellum. The normalized CBF (nCBF) was calculated by dividing the CBF of the MCA territory by the CBF of the cerebellum. Changes in nCBFs were calculated by subtracting the preoperative nCBF values from the postoperative nCBF values. The correlation between nCBF changes measured with ASL and the revascularization grade from direct angiography was evaluated. Results: The nCBF value on the operated side increased after the operation (p = 0.001). The higher the degree of revascularization, the greater the nCBF change was: poor revascularization (grade 1), -0.043 ± 0.212; fair revascularization (grade 2), 0.345 ± 0.176; good revascularization (grade 3), 0.453 ± 0.182 (p = 0.005, Jockheere-Terpstra test). The interobserver agreement was excellent for the measured CBF values of the three readers (0.91-0.97). Conclusion: The nCBF values of the MCA territory obtained from ASL MRI increased after the revascularization procedure in children with MMD, and the degree of nCBF change showed a significant correlation with the degree of collateral formation evaluated via catheter angiography.

Monitoring Cerebral Perfusion Changes Using Arterial Spin-Labeling Perfusion MRI after Indirect Revascularization in Children with Moyamoya Disease

  • Objective: To assess the role of arterial spin-labeling (ASL) perfusion MRI in identifying cerebral perfusion changes after indirect revascularization in children with moyamoya disease. Materials and Methods: We included pre- and postoperative perfusion MRI data of 30 children with moyamoya disease (13 boys and 17 girls; mean age ± standard deviation, 6.3± 3.0 years) who underwent indirect revascularization between June 2016 and August 2017. Relative cerebral blood flow (rCBF) and qualitative perfusion scores for arterial transit time (ATT) effects were evaluated in the middle cerebral artery (MCA) territory on ASL perfusion MRI. The rCBF and relative time-to-peak (rTTP) values were also measured using dynamic susceptibility contrast (DSC) perfusion MRI. Each perfusion change on ASL and DSC perfusion MRI was analyzed using the paired t test. We analyzed the correlation between perfusion changes on ASL and DSC images using Spearman's correlation coefficient. Results: The ASL rCBF values improved at both the ganglionic and supraganglionic levels of the MCA territory after surgery (p = 0.040 and p = 0.003, respectively). The ATT perfusion scores also improved at both levels (p < 0.001 and p < 0.001, respectively). The rCBF and rTTP values on DSC MRI showed significant improvement at both levels of the MCA territory of the operated side (all p < 0.05). There was no significant correlation between the improvements in rCBF values on the two perfusion images (r = 0.195, p = 0.303); however, there was a correlation between the change in perfusion scores on ASL and rTTP on DSC MRI (r = 0.701, p < 0.001). Conclusion: Recognizing the effects of ATT on ASL perfusion MRI may help monitor cerebral perfusion changes and complement quantitative rCBF assessment using ASL perfusion MRI in patients with moyamoya disease after indirect revascularization.

Future Challenges for Risk-Based Management of 135 Environmental Hazardous Substances in Children's Products (위해성에 근거한 어린이용품 함유가능 환경유해인자 135종의 관리를 위한 향후 과제)

  • As concerns have been raised about health effects of children's products containing hazardous chemicals on children, the Korea Ministry of Environment announced 135 environmental hazardous substances in children's products (EHSCP) which were subject to risk assessment. Therefore, it is necessary to identify the current status of EHSCP for management due to their various exposure routes and types. In this study, we investigated the exposure assessment of EHSCP as well as relevant policies implemented in domestic and foreign countries. Domestic and foreign cases of detection of EHSCP were reviewed in the literature and websites of related agencies. Due to the lack of reference values for child-specific toxicity of EHSCP, those for adults and regulatory status of EHSCP were also investigated and reviewed with literature. The related agencies of foreign countries provided 13 child-specific reference doses (RfDs), 76 RfDs, and 42 reference concentrations as toxicity reference values of the EHSCP. Among substances investigated in the studies to assess exposure of hazardous substances in children's products by the Korea Ministry of Environment, 23 substances were included in 135 EHSCP. The same studies identified 9 substances which were excluded from the 135 EHSCP. For regulatory status for hazardous substances in children's products, 44 substances (32.6 %) and 84 substances (62.2 %) of 135 EHSCP were under regulation in Korea and foreign countries, respectively. In the present study, we found that a list of 135 EHSCP should be revised to ensure children's safety.

The Effect of Muscle Relaxation Therapy on Nausea, Vomiting and Anxiety of Children with Chemotherapy (근육이완요법이 항암화학요법을 받는 아동의 오심, 구토 및 불안에 미치는 효과)

  • The purpose of this study is to assess the effect of progressive muscle relaxation therapy on nausea, vomiting and anxiety experienced by children who were histologically diagnosed as malignancy and have received cancer chemotherapy. Twenty subjects with 10 in experimental group and 10 in control group, between the age of 7 to 12 years participated in this study. All subjects were admitted for inpatients chemotherapy at the S hospital between April to August 1998. The methods used for interventional muscle relaxation were both the 16 items of the progressive muscle relaxation therapy developed by Jacobson and the 4 items of the muscle relaxation therapy by Wolpe. The method for assessing nausea, vomiting was the Index of Nausea and Vomiting by Rhodes et al and that for anxiety was State-Trait Anxiety Inventory for Children by Spielberger. The data was analysed by repeated measures ANOVA and Scheffe test using SAS program. The results were as follows : 1. On measurement of nausea, vomiting, in the experimental group, the mean values of the second and the third measurements showed tendency to slightly decrease compared with the first measurement, but there was no statistically significance. And in the control group, there were no significant differences among first measurement, second measurement and third measurement. Also there was no significant difference in the mean values assessing nausea, vomiting between experimental and control group (F=1.33, p=0.2645). 2. The results assessing state anxiety showed, in the experimental group, progressive decrease in mean values at each measurement, while the control group showed no significant differences between the mean values of each measurement. Also there was statistically significant difference in state anxiety measurements between experimental and control group(F=4.36, p=0.05). In conclusion, muscle relaxation therapy for school-aged children with chemotherapy could not be effective to decrease nausea, vomiting, but it could be effective to decrease anxiety.

Gender specific effect of major dietary patterns on the metabolic syndrome risk in Korean pre-pubertal children

  • There is a lack of data on metabolic risk factors during pre-puberty, which is important for identifying the subgroups of youth, at whom early interventions should be targeted. In this study, we evaluated the prevalence of metabolic risk factors and its subsequent relations with dietary patterns in Korean pre-pubertal children through a cross-sectional sample (n = 1,008; boys = 513) of pre-pubertal children (aged 8-9 years) from a sub-study of the Korea Metabolic Syndrome Research Initiatives (KMSRI) in Seoul, Korea. Measures of anthropometry and blood pressure as well as fasting blood samples were used in the analysis. A three-day food records were collected. The metabolic syndrome was defined according to the age-adjusted National Cholesterol Education Program Adult Treatment Panel III guidelines. An added metabolic risk score was calculated for each subject by summing the quintile values of the five individual risk factors. Among the 5 risk components of metabolic syndrome, high waist circumference (WC) was the major factor (P < 0.001). A significant increasing trend of the added metabolic syndrome risk score was observed with the increase of WC (P (trend) < 0.001) among both genders. The cutoff point for high WC for pre-pubertal children was 61.3 cm for boys and 59.9 cm for girls. The prevalence of high triglyceride (TG) values was significantly higher in girls than it was in boys (P < 0.01). Girls in the highest quintile of balanced dietary pattern scores had lower TG values (P (trend) = 0.032) than did those in the lowest quintile. Moreover, girls in the highest quintile of western dietary pattern scores showed increasing trend for the added metabolic risk score (P (trend) = 0.026) compared with those in the lowest quintile. Adverse associations exist between western dietary patterns and the accumulation of metabolic risks among girls, not in boys, even during pre-puberty.

Measurements on Radiographs of the Foot in Normal Children (족부측방촬영을 통한 정상 아동의 족근골 및 중족골간 각도 계측 결과)

  • Radiographs were made of the feet of one hundred and twenty four normal children who were ranged in age from 1 to 13 years. On lateral radiographs that were made with the feet in standing, the talo-first metatarsal, calcaneal pitch and calcaneo-fifth metatarsal angles and arch ratio were measured. Most of the children in this study revealed flattened feet in earlier ages and the mean values were changed with age up to normal range later in the early school ages. These data might provide one of the standard for assessment of the flatfoot in children. However, the diagnosis and treatment of the flatfoot should be based on clinical assessment and radiographic values should not determine clinical management even if the values are beyond the normal range.

  • Purpose: The cardiopulmonary exercise test (CPET) is an important clinical tool for evaluating exercise capacity and is frequently used to evaluate chronic conditions including congenital heart disease. However, data on the normal CPET values for Korean children and adolescents are lacking. The aim of this study was to provide reference data for CPET variables in children and adolescents. Methods: From August 2006 to April 2009, 76 healthy children and adolescents underwent the CPET performed using the modified Bruce protocol. Here, we performed a medical record review to obtain data regarding patient' demographics, medical history, and clinical status. Results: The peak oxygen uptake ($VO_{2Peak}$) and metabolic equivalent ($MET_{Max}$) were higher in boys than girls. The respiratory minute volume $(V_E)/CO_2$ production ($VCO_2$) slope did not significantly differ between boys and girls. The cardiopulmonary exercise test data did not significantly differ between the boys and girls in younger age group (age, 10 to 14 years). However, in older age group (age, 15 to 19 years), the boys had higher $VO_{2Peak}$ and $MET_{Max}$ values and lower $V_E/VCO_2$ values than the girls. Conclusion: This study provides reference data for CPET variables in case of children and adolescents and will make it easier to use the CPET for clinical decision-making.

  • This study attempts to identify the professional competency of local children's centers' workers, and examine their impact on case management performance. To achieve the purpose of this study, 189 children's centers' workers with 2 or more years of experiences in Busan and Gyeongnam areas participated in the research. The results showed that the professional skills and capabilities of workers have substantial impact on case management performance. Moreover, the study also proved that social work values have a moderating effect in the practice. This study suggested that for the purpose of improving the competency and professional practice in local children's centers, it is necessary to equip workers with specified education programs and effective law manuals.

  • The childhood reading and physical environments are an important factor for shaping personality and values established. Therefore, We have to provide a quality environment for correct reading habits to children. the purpose of this study is to provide a basic interior data through the analysis of case study. for this purpose this study has analyzed of 5 children's reading room of public libraries built in Seoul since 2000. especially the research which it analyzed these reading room of forms of space, furniture, color, finish material, lighting. the results of this study are follows : Chairs and desks were not considered of children's scale or uniformly made. compositions of convenient facilities has insufficient component. finish material and lighting plan is similar to adults reading room plan. To give correct reading habits and interest to children, considered space design is necessary.

Content Analysis of Food Advertising Directed at Korean Children

  • Children have become the target of intense and specialized food advertising efforts. This study examines the content of TV food advertising and discussed possible concerns from the perspective of children. A total of 102 ads were collected from two cable networks aimed at children. Most of the food ads used emotional appeals and did not provide information clues such as nutrition values. Ads with high sales pressure and celebrity endorsements appeared frequently and the ads for snack foods and high-carbohydrate bakery products were dominant. Understanding food ads can be a significant indicator to establish the orientation of consumer ad education and build educational programs appropriate for children. The issue that appears conspicuous in child targeted advertising and marketing is the childhood obesity and food advertising relation; subsequently, there is a need to evolve advertising that targets children into a positive educational information source.
