Objectives: The current study reviews the implementation and evaluation of the Chronic Disease Self-Management Program (CDSMP) in the United States (U.S.) to illustrate the program's potential contribution to improving health among Korean adults with chronic conditions while saving healthcare costs. Methods: This study examines existing literature on the history, theoretical background, essential elements, and delivery outcomes of CDSMP with special focus on the successes and challenges to be faced in the implementation of CDSMP to Koreans with chronic conditions. Results: CDSMP is designed to empower people with chronic conditions to develop skills necessary for medical, social role, and emotional management of chronic conditions. Recent studies show the utility of CDSMP in achieving the Triple Aim health reform goals (i.e., better care, better health, better value). Lessons learned from the U.S. experience emphasize the importance of establishing evidence-based studies, collaborating with community partners, and diversifying funding sources to make CDSMP more sustainable. Conclusion: The current study demonstrates the replicability of CDSMP and potential for expansion in Korea. More concerted efforts among academia, government, and communities are needed to deliver CDSMP to Korean adults and identify its effectiveness within the Korean context in terms of meeting the Triple Aim goals of better care, better health, and better value.
The aim of this article is to propose a framework for standardization of knowledge based on a case study of clinical operation to guide initial hospital knowledge management projects. Both quantitative and qualitative data for the project from 2009 through 2012 were collected for analysis. In a study of knowledge management project of a lab in S Hospital's Eye Center, this research examines the standardization of operational knowledge for competence enhancement of the lab. The lab defined and standardized the knowledge concern of the lab operation and the in-group from 2008. Organizations are often unaware of the potential value of tacit knowledge in both clinical and administrative area. The main finding from the case study is that analysis of such tacit knowledge of a lab in a hospital can be transformed into explicit knowledge for organizational capabilities such as improved quality, responsiveness and competitive advantage. Concepts from Nonaka's theory are used to investigate processes of knowledge standardization, examining the strategic value of tacit knowledge and the need to rely primarily on internal documentation processes. Since there has been limited research on hospital knowledge management. it may also be useful to researchers in generating propositions for further study.
Because arrhythmia occurs irregularly, it should be examined for at least 24 hours for accurate diagnosis. For this reason, this paper developed firmware software for arrhythmia detection and prevented consumption of temporal and human resources and enabled continuous management and early diagnosis. Prior to the experiment, the interval between the R peaks of the QRS Complex was calculated using the Pan-Tompkins algorithm. The developed firmware software designed and implemented an algorithm to detect arrhythmia such as tachycardia, bradycardia, ventricular tachycardia, persistent tachycardia, and non-persistent tachycardia, and a data transmission format to monitor the collected data based on BLE. As a result of the experiment, arrhythmia was found in real time according to the change in BPM as designed in this paper. And the data quality for BLE communication was verified by comparing the sensor's serial communication value with the Android application reception value. In the future, wearable devices for real-time arrhythmia detection will be lightweight and developed firmware software will be applied.
This study attempts to present specific technical measures and case models to meet the needs of caregivers and maximize the effectiveness of senior care facilities by utilizing Wellness IT Service. IT services for supporting elderly care need to go beyond productivity and efficiency and now require the new choice and standards to satisfy the Wellness concept of well-being and happiness. Therefore, this study explores to develop the existing human-based service into the Wellness IT Service by utilizing IT based Wellness in the elderly care facility and thereby to improve the quality of elderly care service. Therefore, this study analyzes the relevant research background and the various feasibility of technical applications of Wellness IT service, and review the new value creation in the elderly care facilities. Lastly this study provides a new foundation for Wellness architecture and a service model for using Wellness IT infrastructure of the elderly care facilities.
Personal sound amplifiers(PSAPs) provide accessible and affordable healthcare to individuals with hearing disability. Many studies are in progress for affordable PSAP development, but still people do not have a best fitting profile for the PSAP depending on their hearing test. As a result, they do not have a personalized and profiled music and sound, which are very helpful for those who has hearing problems. In this paper, we propose a device and mobile system to provide music with an equalizer value according to the hearing condition of an individual to prevent the hearing loss. In order to overcome the limit of frequency band of the equalizer in a smart phone, we developed bluetooth controlled equalizer based on the fitting profiles.
Purpose: This study was done to examine the threshold value of estimated height loss at which the risk of osteoporosis increases and to verify its discriminative ability in the detection of osteoporosis. Methods: It was conducted based on epidemiological descriptive methods on 732 Korean women at a public healthcare center in Seoul between July and November 2010. ANOVA, Pearson correlation, logistic regression analysis and receiver operating characteristics (ROC) curve were used for data analysis. Results: There was an age-related correlation between bone mineral density (lumbar spine: F=37.88, p<.001; femur: F=54.27, p<.001) and estimated height loss (F=27.68, p<.001). Estimated height loss increased significantly with decreasing bone mineral density (lumbar spine: r=-.23, p<.001; femur: r=-.34, p<.001). The odds ratio for the point at which the estimated height loss affects the occurrence of osteoporosis was found to increase at a cut-off value of 2 cm and the area under ROC curve was .71 and .82 in lumbar spine and femur, respectively. Conclusion: The optimal cut-off value of the estimated height loss for detection of osteoporosis was 2 cm. Height loss is therefore a useful indicator for the self-assessment and prognosis of osteoporosis.
Purpose: The aim of this paper is to reduce the stress and the disturbance occur in selective action of patient. This article will also help us to control spreading and reduce the order and bacteria produced by leprosy disease among the lepers. This will be achieved with the help of several variables and these variables help us on health benefits is 5S (Sort, Straighten, Shine, Standardize, Sustain) i.e. ratio of sectional morphology, lessor 90 degree angles, day light, universal design building and maximum ventilation. The replies from the questionnaire were collected based on varying levels of satisfaction and gloominess on the scale of 1-10. Methods: The multi-layer methodological framework for maximising the healing environment obtained from the observation of schemes and parameters of ANOVA: (Analysis Of Variance between five deciding factors) are. Firstly applied for the calculation of the patient's satisfaction for U.V light from sun on ECOTECT simulation, secondly the number of $90^{\circ}$ angle, along the corridors. Thirdly understanding the ways to represent people's perceptual structures and way finding with Space Syntax software. Fourthly the ratio of depth to height of the building typology and finally interviewing the subjects describing their special experiences based on scale value. The focus of this testing of human subjects was to receive data for the existence of image scheme in way-finding and to identify the mechanisms by which sun light impacts human (lepers) health. Results: AMPVA studies concluded that there is similarity between I and O plan as well as L and H plan whereas U plan was difference among the five selected architectural shapes. 최근 들어 국내외 대기업에서는 웨어러블 디바이스 사업에 전폭적으로 투자하고 있으며, 지난해 대비 26% 가량 많은 사용자들이 웨어러블 디바이스 기반의 다양한 서비스를 제공받고 있다. 기존의 헬스케어, 스마트워크, 스마트 홈 환경에서 폭넓게 사용되고 지고 있으며 융합서비스 환경에서 접목할 수 있도록 도입되어지고 있다. 그러나 G사의 제품이 상용화 되면서 개인정보보호 이슈가 사회적으로 큰 파장을 불러일으키고 있으며, 통신의 데이터 관리 및 보안 쪽에서 위험성이 증대되고 있다. 또한 기존의 무선 환경에서 사용되었던 암호체계를 사용하고 있어 신규 및 변종 보안 위협에 대한 취약성이 발생하고 있다. 본 논문에서는 웨어러블 디바이스 기반에서 안전한 통신을 수행할 수 있는 프로토콜에 대해서 연구를 한다. 등록 및 인증과정에서 코드 값을 기반으로 서명값을 생성한다. 서명 값을 기반으로 안전한 통신을 수행할 수 있도록 통신기법을 설계하도록 한다. 웨어러블 디바이스환경에서 발생하는 공격기법에 대해서 안전성을 분석하여, 기존의 암호시스템과 제안시스템의 성능평가를 수행하여 대략 14%의 효율성을 확인할 수 있었다.
DMIDR (General Electric Healthcare, USA)은 GE 사(社)의 최신 장비로써 PSF (Point Spread Function reconstruction), TOF(Time of Flight)와 Q.Clear의 적용이 가능하다. 특히, Q.Clear는 보정 알고리즘으로써 복셀(voxel)단위 신호 잡음 제거로 기존 OSEM (Ordered Subset Expectation Maximization)의 한계를 넘어설 수 있다. 따라서 이러한 재구성 및 보정 알고리즘의 성능 평가를 통해 정확한 SUV를 구현하며, 병변 검출 능력에 도움이 되는 알고리즘의 조합을 확인하고자 하였다. H/B(Hot & Background) Ratio 2:1, 4:1, 8:1의 비율로 NEMA/IEC 2008 PET phantom을 제작하였다. DMIDR의 NEMA test protocol을 이용하여 영상 획득을 하였다. 재구성 조합은 (1) VPFX(VUE point FX(TOF)), (2) VPHD-S(VUE point HD+PSF), (3) VPFX-S(TOF+PSF), (4) QCHD-S-400(VUE point HD+Q.Clear(${\beta}-strength$ 400)+PSF), (5) QCFX-S-400(TOF+Q.Clear(${\beta}-strength$ 400)+PSF), (6) QCHD-S-50(VUE point HD+Q.Clear(${\beta}-strength$ 50)+PSF), (7) QCFX-S-50(TOF+Q.Clear(${\beta}-strength$ 50) + PSF)의 7 가지로 구성하였다. H/B Ratio 및 재구성 알고리즘 별로 측정된 결과를 이용하여 CR (Contrast Recovery)와 BV (Background Variability)을 구하였다. 또한, 각 조합의 count를 측정하여 SNR (Signal to Noise Ratio)과 RC(Recovery Coefficient)를 구하고 SUV (Standardized Uptake Value)를 측정하였다. 구의 크기가 가장 작은 10 mm와 13 mm에서는 VPFX-S, 17 mm 이상에서는 QCFX-S-50에서 가장 높은 CR 결과를 보였다. BV와 SNR의 비교에서는 QCFX-S-400과 QCHD-S-400에서 좋은 값을 보였다. SUV 측정 결과는 H/B ratio와 비례하여 증감하는 양상을 보였다. SUV에 대한 RC의 경우 H/B ratio와 반비례하는 양상을 보였으며, 재구성 알고리즘 중에서는 QCFX-S-50이 가장 높은 값을 보였다. 또한, Q.Clear에 ${\beta}-strength$ 400이 적용된 재구성 알고리즘들이 낮은 값 분포를 보였다. Q.Clear가 적용된 재구성 조합은 ${\beta}-strength$를 높이면 신호잡음이 억제되어 영상 품질면에서 우수한 결과를 보였고 ${\beta}-strength$를 낮추면 선예도가 증가하며, partial volume effect가 감소하여 기존의 재구성 조건에 비하여 높은 RC에 근거한 SUV 측정이 가능하였다. 이러한 진보된 알고리즘의 사용으로 보다 정확한 정량화와 미세병변 검출능력을 향상 시킬 수 있으나 상관 관계를 고려하여 목적에 맞는 최적화 과정이 필요할 것으로 사료된다.
Objectives: The effectiveness of Blood ${\beta}$-ketone testing by using self-monitoring strip on diabetic patients. Methods: Clinical effectiveness of blood ${\beta}$-ketone testing was assessed through correlation with reference test that measured blood ${\beta}$-ketone value through gas chromatography or enzyme method, diagnostic accuracy, time taken for the test and time taken for confirmative diagnosis of diabetic ketosis by selecting literatures on researches that conducted this test on ketosis(suspected) patients or diabetic ketosis(suspected) patients. Each of the stages from literature search to application of selection standards and extraction of data were carried out independently by the Subcommittee along with 2 researchers. Results: 7 reports were selected. ${\beta}$-ketone testing displayed high level of correlation in the range of r=0.92~0.99 with test using enzyme method as the reference standard. Regarding the diagnostic accuracy, sensitivity of 0.82, specificity of 0.74. The time taken for the test was 30seconds for the index test, which is shorter than reference standard test. Conclusion: Blood ${\beta}$-ketone testing was assessed to be a safe and effective test to monitor ketosis and assess the level of risk of ketosis by measuring the blood ${\beta}$-ketone on ketosis patients and diabetic ketosis patients since it has high level of correlation with reference test and short period of testing.
