Development of Case Management System and Analysis of Economic Feasibility under the Fee-For-Service Reimbursement (행위별 수가 지불제도 하에서의 사례관리시스템 개발 및 경제성 분석)

  • Background : Recently, we have experienced various changes in the healthcare environment. Healthcare organizations are facing a financial crisis due to more competitive relationships among themselves as well with low health insurance fees. The purposes of the current study were: (1) to develop a data warehouse-based system for evaluating and monitoring the case management activities, and (2) to measure and analyze its effects. Methods : In order to collect the data for the study, the database on discharged patients was utilized at a university hospital located in Seoul from June 1, 2002 through December 31, 2002. Initially, a data warehouse was built for the case management system. The case management activities were analyzed using structured methodology to establish the case management system. Results : The findings of this study were as follows: (1) A case management system was developed to make it possible to monitor of healthcare quality and resource utilization. The Case management System included monitoring functions regarding utilization reviews, critical pathways, and clinical indicators. (2) Utilizing the case management system, unplanned readmissions were documented among total discharged patients during two months from November 1, 2002 through December 31, 2002. The unplanned readmission rate was 2.3%(276 patients) in total of 11,960 discharged patients. Among them 81 patients(0.7% of total discharges, 29.3% of unplanned readmission) were readmitted to the same physician in charge under the same diagnosis. No significant differences were found in the demographic variables such as gender and age among the patients. (3) After implementing the case management system, 2.9% of average length of stay reduced. Applying cost-benefit analysis, the 2.9% reduction of length of stay represents net profit of ${\backslash}$ 279,592,000 in the year of 2004. In addition, applying value acceleration analysis, cumulative net benefit of ${\backslash}$ 1,481,000,000 was expected by the year of 2007. Also we were able to expect ${\backslash}$ 247,800,000 of cumulative benefit for the prospective 5 years in value linkage analysis. It represents average ${\backslash}$ 787,700,000 of pure net benefit a year. Conclusion : The value of present study would be not only implementing the knowledge management system into the existing case management activities, but also evaluating its effects and estimating its financial benefits. This study suggested that the case management system would be a supportive tool for monitoring and improving the quality of healthcare, and a cost-effective tool for increment of healthcare organization's financial benefit.

  • As medical healthcare industry is growing up rapidly these days, providing various new healthcare service is considered carefully. Health information is considered to be more important than financial information; therefore, protecting health information becomes a very significant task. Ransomware is now targeting industry groups that have high information value. Especially, ransomware has grown in various ways since entering maturity in 2017. Healthcare industry is highly vulnerable to ransomeware since most healthcare organizations are configured in closed network with lack of malware protection. Only meeting the security criteria is not the solution. In the case of a successful attack, restoration process must be prepared to minimize damages as soon as possible. Ransomware is growing rapidly and becoming more complex that protection must be improved much faster. Based on ISO 27799 and 27002 standard, we extract and present security measures against advanced ransomware to maintain and manage healthcare system more effectively.

  • Healthcare organizations are overwhelmingly embracing smart value-based care strategies, which focuses on providing superior treatment at a significantly lower cost and quality of service (QoS). For these purposes, fifth generation (5G) of mobile service provides an innumerable improvement that clearly outperforms previous generations e.g., 3G and 4G. However, as with most advancements, 5G is projected to introduce new challenges, prompting the community to think about what comes next. This research was conducted to examine the most recent smart 5G technology applications and the solutions they provide to the healthcare industry. Finally, the paper discusses how the upcoming 6G technology has the potential to transform the future of healthcare sector even beyond the current 5G systems.

Personal Identification Based on Radio Signal Strength for Ubiquitous Healthcare Systems

  • Personal identification is essential for the automatic measurement of biosignal information in home healthcare systems. Personal identification is usually achieved with passive radio frequency identification (RFID), which does little more than store a unique identification number. However, passive RFID is not ideal for automatic identification. We present a user identification system based on radio signal strength indication (RSSI) using ZigBee for active RFID tags. Personal identification is achieved by finding the largest RSSI value from aggregated beacon messages that are periodically transmitted by active RFID tags carried by users. Obtaining reliable person!'.! identification without restricting the orientation requires a certain distance between the closest active RFID tag from the ZED and the second closest tag. The results show that the closest active RFID tag from the ZED and the second closest tag must be at least 70 cm apart to achieve reliable personal identification.

Data-Linking Infrastructure for the Health Technology Assessment (의료기술평가 기반으로서의 데이터 연계)

  • With the recent change of healthcare environment including rapid technological development, evidences are more and more important and necessary to support relevant policies in health technology assessment to provide safe and effective health services, utilizing medical resources efficiently. Despite of the emphasis on the importance of real world data and real world evidence in health care research, current infrastructure supporting clinical research is considerably weak due to absence of legal and institutional basis. However, in accordance with the Article 26 of the Health and Medical Technology Promotion Act, there is a limited legal apparatus that can be used only in public data with other dataset for the purpose of healthcare technology assessment at the National Evidence-based Collaborating Agency. Although the use of linked data from various sources was often required in the field of clinical research, it was not yet working well due to insufficient environmental conditions. In order to support the decision-making of medical practice and health care policies, data-linking platform for clinical research is needed. If the legal system that can link up to the data of the private institutions without violating the significant value such as the protection of private informations is established, it will be a decisive foundation reinforcing the researches and policy making processes for the improvement of the national health care system.

  • Recently, as the non-face-to-face environment has developed due to COVID-19 and environmental pollution, the importance of online digital healthcare is increasing, and venture start-ups and activities such as health care, telemedicine, and digital treatments are also actively underway. This study conducted the impact on the acceptability of digital healthcare smartwatches with an integrated approach of the expanded integrated technology acceptance model (UTAUT2) and the behavioral inference model (BRT). The most advanced integrated technology acceptance model for innovative technology acceptance research was used to identify major factors such as utility expectations, social effects, convenience, price barriers, lack of alternatives, and behavioral intentions. For the study, about 410 responses from ordinary people in their teens to 60s across the country were collected, and based on this, the hypothesis was verified using structural equations after testing reliability and validity of the data. SPSS 23 and AMOS 23 were used for research analysis. Studies have shown that personal innovation has a significant impact on the reasons for acceptance (use value, social impact, convenience of use), attitude, and non-use (price barriers, lack of alternatives, and barriers to use). These results are the same as the results of previous studies that confirmed the influence of the main value of innovative ICT on user acceptance intention. In addition, the reason for acceptance had a significant effect on attitude, but the effect of the reason for non-acceptance was not significant. It can be analyzed that consumers are interested in new ICT products and new services, but purchase them more carefully and selectively. This study has evolved from the acceptance analysis of general-purpose consumer innovation technology to the acceptance analysis of consumer value in smartwatch digital healthcare, which is a new and important area in the future. Industrially, it can contribute to the product's purchase and marketing. It is hoped that this study will contribute to increasing research in the digital healthcare sector, which will play an important role in our lives in the future, and that it will develop into in-depth factors that are more suitable for consumer value through integrated approach models and integrated analysis of consumer acceptance and non-acceptance.

Relationship between Medical Screening and Unmet Healthcare Needs: Using KNHANES(Korea National Health and Nutrition Examination Survey) (국민건강영양조사를 이용한 건강검진과 필요의료서비스 미충족 간 관련성 분석)

  • Purposes: The purpose of this study, was to identify similar factors between reasons for unscreening and unmet health care needs through prior research, and based on this, we wanted to figure out the relevance between the medical screening and unmet health care needs. Methodology: The analysis was conducted using data from 9,640 adults aged 19 or older who don't have a missing value from 16,277 participants in the 7th, 1st&2nd Year (2016&2017) of the National Health and Nutrition Examination Survey (KNHANES). Unmet health care needs were investigated as a self-reported questionnaire of whether medical service was required but not received. And the analysis was performed through the Chi-Square Test and Multi-logistic Regression analysis. Findings: As a result of the analysis, unmet healthcare needs were higher who received only one type of screening comparative to who screened both. and were highest who screened neither. Practical Implications: Unmet health care needs are the center of a vicious cycle, such as morbidity and mortality, which is detrimental to the quality of life, and continues to increase. Therefore, it is necessary to find ways of realizing health care that guarantees the health rights of all citizens by policy guarantee and support for the subjects to recognize the importance of thorough education of screenings rather than only health screening or cancer screening.

  • As medical service industry is transformed into the market centered on consumers, medical service quality patients recognize is emphasized as the powerful means to ensure competitive advantage among hospitals in the fierce medical markets. Many researches have been done on the definition of medical service quality, developing a scale to measure it, patient satisfaction, hospital repurchase and oral transmission intention, but integrated studies have not been done sufficiently on the patient' cognitive emotional aspects. For these research purposes, based on service relation, service quality, physical surroundings, human services, corporate reputation, service value, emotional response, customer satisfaction and repurchase intention, this paper suggests a theoretical modeling composed of hypotheses on the relations of each theoretical variable. In addition, the moderating effect of service relationships is investigated based on the structural equation model.

  • Purpose The purpose of this study is to analyze the causal relationship between user satisfaction, expected satisfaction, quality of service, perceived value, and expertise that should be emphasized in personalized medical information services for the development of personalized medical information services based on big data analysis and the spread of their demand. Design/methodology/approach This study established research models and hypotheses on the basis of the theory of reuse intent, and applied the PLS methodology for analysis, the factors that make it applicable to personalized medical services in the theory of service quality and satisfaction. Findings According to the empirical analysis result, this study confirmed that it can be seen that the expertise, perceived value, and quality of medical services did not directly affect the user's intention to reuse, but formed a direct causal relationship through variables such as whether they met expectations.

  • Current U-Healthcare has become specialized in Wellness center in the prevention of future health care concept of a health center, treatment. Wellness is the development of which has been emerging field of fitness system to evolve along with the key areas of U-Healthcare. Traditional Fitness system, one between the trainer and the user: 1. Consultation was only possible, the number of trainers are limited, this problem can be reduced individual coaching time existed as more number of users who can receive coaching at certain times of the day. In this paper, we design a mechanism that allows management by grouping field value of a DB table in the user's BMI(Body Mass Index) index and fitness trainer system to efficiently coaching a plurality of users similar BMI index.
