Management of Cannula Wound Problems with Vacuum-Assisted Therapy for a Child Who Received Berlin Heart EXCOR Implantation

  • A child being supported with an extracorporeal ventricular assist device, such as the Berlin Heart EXCOR (Berlin Heart GmbH, Berlin, Germany), must have at least 2 large cannulae for a long period. Management of cannula wounds is crucial since a cannula forms a track of prosthetic material passing the mediastinum to the heart. Deep wound complications, if they occur, can be troublesome and difficult to control with conventional methods. We applied vacuum-assisted closure to a patient who had Berlin-Heart EXCOR and a gap at the cannulation site. Herein, we describe the technical aspects of management in detail.

3DCA제작을 통한 Vac-Lock 사용시 효율성향상에 대한 연구 (The evaluation of effectiveness of 3DCA using vacuum bag(Vac-Lock) for patient immobilization device)

  • 목 적 : 본원에서는 Whole body용 Vac-Lock을 이용하여 환자의 전신체형을 뜨고 있다. 그러나 몇 가지 애로사항이 발생하여 본 연구에서는 이러한 문제점을 찾아 보고, 애로사항을 해결하고자 자체 제작한 3DCA에 대해 알아보고자 한다. 대상 및 방법 : 본원에서 사용중인 Whole body용 Vac-Lock의 크기는 2종류가 있는데, 그에 따라 제작한 3DCA를 제작하는데 재질은 목재를 이용하였는데, 그것은 비용절감과 직접 제작을 하기 위해서는 쉽게 재질을 다룰 수 있고, 모양을 내기 위해서는 목재가 적정하였다고 사료되었다. 결 과 : Vac-Lock을 사용하면서 애로사항을 4가지로 분석하였고, 이러한 문제점을 해결하기 위해 3DCA를 제작 사용하게 되었다. 첫 번째 문제는 많은 제작 인원으로, 3DCA를 사용하여 전에는 환자의 전신체형을 뜨는데 5명 내지 6명이 동원 되었는데, 이제는 한사람이 할 수 있게 되었다. 두 번째는 제작 시간으로 3DCA를 사용하기 전에는 많은 인원이 동원되어도 1시간 이상이 소요되었으나, 이제는 한사람이 10분 이내로 단축할 수 있었다. 세 번째는 Vac-Lock이 큰 관계로 체형을 뜬 후 치료기기와 충돌하는 경우가 있었으나, 지금은 그러한 현상을 방지할 수 있었다. 네 번째로 3DCA를 사용하기 전에는 Vac-Lock의 외형이 깔끔하지 못했는데, 이제는 매끄럽게 체형을 뜰 수가 있었다. 결 론 : 환자가 치료받은 자세를 치료시 마다 동일하게 유지하기 위해 Vac-Lock을 사용하였는데, Vac-Lock 제품의 특성상 사용시 조금한 불편함이 근무자에게 애로사항으로 다가왔다. 이번 연구도 그러한 불편함을 조금이나마 줄여 보고자 하였고, 실제 문제점을 찾아 보고, 그 문제점을 해결하고자 3DCA를 제작 사용하였는데 많은 효과를 보고 있다.

First Case of Esophagectomy Using a Robotic Single-Port System for Laryngo-Esophagectomy

  • A 58-year-old female patient was diagnosed with hypopharyngeal cancer with extension to the invasion of the upper esophagus. After 2 cycles of durvalumab as neoadjuvant therapy, total laryngo-esophagectomy using a single-port (SP) system via a subcostal incision was done. The operation was completed within 41 minutes, and the patient recovered without esophagectomy-related complications. The patient received total laryngectomy and esophagectomy using a robotic SP system via a 3-cm-long subcostal incision and gastric pull-up under laparotomy. During the postoperative period, the patient suffered from anastomotic leakage, but recovered with vacuum therapy. Here, we report the first successful human case of esophagectomy using an SP system.

직장암 환자에서 방사선치료시 Air-vacuum Cushion의 유용성 (The Role of Air-Vacuum Cushion Device in Patients with Rectal Cancer in Radiation Therapy)

  • 목적 : 본 연구에서는 골반부위 방사선치료 환자중 직장암 환자에서 고정기구로 사용한 air-vacuum cushion의 조사면의 변화에 따른 고정기구의 유용성을 후향적으로 분석하였다. 대상 및 방법 : 충남대학교병원 치료방사선과에서 1998년 8월부터 1999년 8월까지 직장암으로 방사선치료를 받은 20명의 환자를 대상으로 하였다. 환자들은 모두 복와위 자세로 치료를 받았고 대조군 10명과 실험군 10명으로 각각 나누어서 연구를 실행하였다. 전체 치료과정중 EPID를 이용하여 후면조사면은 x축(right-left), y축(cranio-caudal)에 대한 영상을 얻었고, 측면조사면은 z축(anterior-posterior), y축(cranio-caudal)에 대한 영상을 얻었다. 전체 치료과정 중 얻은 영상들을 모의치료시 촬영한 기준필름과 matching 기법을 이용하여 오차를 측정하였다. 통계적 유의성 확인은 t-검정법을 사용하였다. 결과 : 대조군에서 10명 환자의 후면 조사영역의 경우 x축, y축에 대한 평균이동값은 각각 0.02 mm, 0.78 mm이었고 계통오차과 우연오차의 표준편차값은 각각 2.13 mm, 2.40 mm와 1.46 mm, 1.51 mm이었다. 측면조사면에서 z축, y축에 대한 평균이동값은 각각 2.96 mm, 0.47 mm이었고 계통오차와 우연오차의 표준편차는 각각 3.69 mm, 1.96 mm와 2.79 mm, 1.48 mm이었다. 실험군 10명 환자의 후면 조사면에서 x축, y축에 대한 평균이동값은 각각 -0.33 mm, 0.81 mm이었고 계통오차과 우연오차에 대한 표준편차는 각각 1.71 mm, 3.08 mm와 1.40 mm, 1.88 mm이었다. 측면조사면에서 z축, y축에 대한 평균이동값은 각각 2.98 mm, 0.74 mm이었고 계통오차과 우연오차에 대한 표준편차는 각각 4.75 mm, 2.65 mm와 2.69 mm, 1.86 mm이었다. 본 연구에서는 대조군과 실험군간의 조사면에 대한 평균 이동값과 우연오차는 거의 일치하였으나 계통오차는 실험군에서 다소 큰 경향을 보였다. 후면조사면과 측면조사면에서 air-vacuum cushion에 의한 조사면 변화의 통계적 유의성이 없었다. 결론 : 직장암 환자의 방사선 치료시 고정기구로 사용한 air-vacuum cushion의 사용이 방사선치료의 정확도에 이익을 주지 않는 것으로 생각된다.

An Overview of Techniques in Enzyme Immobilization

  • Immobilized enzymes have become the subject of considerable interest due to their excellent functional properties such as reusability, cost-effectiveness, and optimality during the past decades. Enzyme immobilization technology is not only used in industrial processes, but also a component technology of products for medical diagnostics, therapy, food industry, bio energy, and biomaterial detection. In this review, new methods for enzyme immobilization are introduced, and the advantages and disadvantages of a variety of techniques in enzyme immobilization will be also discussed.

만성 창상의 치료에서 이동식 음압요법의 신고안 (The Management of Chronic Wound with Modified Portable Negative Pressure Therapy)

  • Purpose: V.A.C. is a new concept which is closed dressing with negative pressure to promote wound healing. It has been widely used as the treatment of chronic and acute wounds such as pressure sores, burns, stasis ulcers, and other complicated wounds. However It has disadvantages such as high cost and the need of specific equipment. In this article, we described new method to overcome these disadvantages. Methods: We made newly innovated equipment with 50 cc syringe and spring to create negative pressure. From May 2006 to May 2007, we applied it to two patients with chronic wound. Results: The treatment period was 5 weeks for one case and 3 weeks for the other case. Both patients were healed completely without admission and wound healing was accelerated. During follow-ups, there were no complications. The mean cost for single dressing was 9,590 won. Conclusion: Modified portable negative therapy using newly innovated equipment could accelerate wound healing better than conventional dressing. It lowers the number of dressings, saves cost of treatment, and enables treatment as outpatient basis.

뇌출혈 동물 모델에서 부항이 뇌 인지기능 회복에 미치는 효과 (Effects of Cupping Therapy on Memory Impairment after Hemorrhage in Rats)

  • Hemorrhage is a devastating type of stroke, accounts for 15-20% of all strokes. This disease can cause cognitive dysfunction with a very high mortality rate. Cupping therapy of Traditional Korean medicine has frequently been used to relieve a variety of diseases or clinical conditions, although not in the memory loss after hemorrhage. This study was designed to evaluate the effects of cupping therapy on learning and memory with Y-maze test, as well as its effects on different molecular changes in hippocampus following the induction of hemorrhage in rats. Cupping, using vacuum cupping machine, was applied at target area for 5 min daily for 7 consecutive days, commencing 1 day after brain impairment. As a result, induction of hemorrhage enhanced memory deficit, suppressed brain-derived neurotrophin factor (BDNF) in the hippocampus. Cupping treatment effectively reversed collagenase-induced cognitive impairment in SD rats which was represented by improvement of spontaneous alterations in Y-maze test. In addition, BDNF expression was enhanced after cupping therapy. The present results suggest that the therapeutic effects of cupping treatment after hemorrhage is involved in expression of BDNF.

두겹 압박방식 $3M^{TM}$ $Coban^{TM}$을 이용한 울혈성 정맥궤양의 치료: 증례보고 (Treatment of Venous Stasis Ulcer with $3M^{TM}$ $Coban^{TM}$ 2 Layer Compression System: A Case Report)

  • Purpose: Venous stasis ulcer is the most severe form of chronic venous insufficiency and this commonly appears in the lower limb. Pharmacological therapy, reconstruction of the venous system, surgical management, cellular therapy and compression therapy are known as the treatments of venous stasis ulcer, but relapses are common, which make it a typical chronic wound. We report here on a case of recurrent venous stasis ulcer that healed with compression therapy without any other treatment. Methods: A 35-year-old man with a 13 years history of venous stasis had developed an ulcer on the distal third portion of the lower left limb which was developed 12-year before enrollment in this study. He had been treated with vacuum assist closure, 2 times of cell therapy and 3 times of skin graft for 8 years, but the lesion recurred. From November, 2008 compression therapy was done with the 3M $Coban^{TM}$ 2 Layer Compression System (3M, St. Paul, USA). The ulcer at that time was oval shaped and $3{\times}4$ cm in size. A comfort layer bandage was applied from the proximal phalanx of the great toe to the knee. A compression layer bandage was applied on the previous layer with it being overlapped one half the width of the comfort layer bandage. The dressing was changed every 4 days and the change was recorded with photography. Results: A total of 12 $Coban^{TM}$ 2 Layer Compression Systems were used. The size of the ulcer decreased to $2.5{\times}2.5$ cm in one month, to $2{\times}2$ cm in 2 months, it was $1{\times}1.8$ cm in size at 3 months and it completely healed in 4 months. Conclusion: The venous stasis ulcer was completely healed using the 3M $Coban^{TM}$ 2 Layer Compression System. This method was easy to apply, made the patient comfortable and it provided an excellent compression effect. As in the previous studies, this compression therapy has been proven to play an important role for the treatment and prevention of venous stasis ulcer.

  • 조직공학재생의학(조재학)은 생명과학과공학의 기본개념을 응용하여 생체조직을 만들고, 복원시키고, 변형시키기 위하여 새로운 디바이스나 생체조직 대용품을 만드는 학문분야이다. 조재학은 다학제간 연구개발하는 분야로써 매우 혁신적인 건강관리 및 치료 학문분야이다. 조재학의 큰 특징은 발전 속도가 빠른 첨단 과학 기술을 바탕으로 하고 있기 때문에 발전 속도가 빠르다는 점과 여러 분야의 지식과 기술을 함께 이용하기 때문에 다른 학문보다 관련 분야에 대하여 더욱 더 폭 넓은 이해를 필요로 하게 되므로 서로 다른 분야의 지식을 가진 과학자들의 협동 연구가 요구된다. 본 총설에서는 현재의 연구개발 동향 및 결과와 미래 가능성의 이해를 돕기위해 고찰하였다.

Role of Non-Thermal DBD Plasma on Cell Migration and Cell Proliferation in Wound Healing

  • Plasma technology isbeing developed for a range of medical applications including wound healing. However, the effect of plasma on many cells and tissues is unclear. Cell migration and cell proliferation are very important biological processes which are affected by plasma exposure and might be a potential target for plasma therapy during wound healing treatment. In this study, we confirmed the plasma exposure time and incubation time after plasma treatment in skin fibroblast (L-929 cells) to evaluate the optimal conditions forplasma exposure to the cell in-vitro. In addition, we used a scratch method to generate artificial wound for evaluating the cell migration by plasma treatment. Where, the cells were treated with plasma and migration rate was observed by live-cell imaging device. To find the cell proliferation, cell viability assay was executed. The results of this study indicate the increased cell proliferation and migration on mild plasma treatment. The mechanisms for cell migration and cell proliferation after plasma treatment for future studies will be discussed.
