Prevalence and Associated Factors of Abnormal Cervical Cytology and High-Risk HPV DNA among Bangkok Metropolitan Women

  • Background: Many strategies are required for cervical cancer reduction e.g. provision of education cautious sexual behavior, HPV vaccination, and early detection of pre-invasive cervical lesions and invasive cancer. Basic health data for cervical cytology/ HPV DNA and associated factors are important to make an appropriate policy to fight against cervical cancer. Aims: To assess the prevalence of abnormal cervical cytology and/or HPV DNA and associated factors, including sexual behavior, among Bangkok Metropolitan women. Materials and Methods: Thai women, aged 25-to-65 years old, had lived in Bangkok for ${\geq}5$ years were invited into the study. Liquid-based cervical cytology and HPV DNA tests were performed. Personal data were collected. Main Outcomes Measures: Rates of abnormal cytology and/ or high-risk HPV (HR-HPV) and factors associated with abnormal test (s) were studied. Results: Abnormal cytology and positive HR-HPV were found in 6.3% (279/4442 women) and 6.7% (295/4428), respectively. The most common abnormal cytology was ASC-US (3.5%) while the most common HR-HPV genotype was HPV 16 (1.4%) followed by HPV 52 (1.0%), HPV 58 (0.9%), and HPV 18 and HPV 51 at equal frequency (0.7%). Both tests were abnormal in 1.6% (71/4428 women). Rates of HR-HPV detection were directly associated with severity of abnormal cytology: 5.4% among normal cytology and 13.0%, 30.8%, 40.0%, 39.5%, 56.3% and 100.0% among ASC-US, ASC-H, AGC-NOS, LSIL, HSIL, and SCC, respectively. Some 5% of women who had no HR-HPV had abnormal cytology, in which 0.3% had ${\geq}$ HSIL. Factors associated with abnormal cytology or HR-HPV were: age ${\leq}40$ years, education lower than (for cytology) or higher than bachelor for HR-HPV), history of sexual intercourse, and sexual partners ${\geq}2$. Conclusions: Rates for abnormal cytology and HR-HPV detection were 6.3% and 6.7% HR-HPV detection was directly associated with severity of abnormal cytology. Significant associated factors were age ${\leq}40$ years, lower education, history of sexual intercourse, and sexual partners ${\geq}2$.

미국 내에 거주하는 한인 어머니들의 건강정보 필요성과 탐색 행동연구 (Health Information Needs and Seeking Behaviors among Korean Mothers of Young Children in the United States)

  • 선행연구들은 어머니들의 적극적인 건강정보 탐색자로서의 역할, 가족들의 건강을 돌보는 매니저로서의 역할에 주목해왔다. 그러나 미국 내에 거주하는 한인 이민자 어머니들의 건강정보 필요성이나 탐색 행동에 관한 연구는 미비하다. 본 연구는 커뮤니티를 기반으로 한 설문 연구로서, 한인 이민자 어머니들의 건강정보 필요성과 탐색에 대한 행동을 알아보고자 하였다. 만0세부터 10세의 한인 어머니들 사이에서 가장 중요하다고 여겨지는 건강 주제들은 예방접종, 치아건강, 육아, 영양/식단, 그리고 운동 등으로 밝혀졌다. 또한 특정 주제들에 한하여 아이들의 성별과 나이에 따라 어머니들의 건강 정보 필요성이 다르다는 것을 알 수 있었다. 마지막으로, 미국뿐만 아니라 한국의 정보 전문가들이 건강 정보 서비스를 개발하는데 있어서 염두에 둬야 할 점들을 제시하고 있다.

국내 로타바이러스 감염의 역학 (Clinical Epidemiologic Profile of Rotavirus Infections in Korea)

  • Primary rotavirus gastroenteritis infection usually occurs in infants under 2 years of age worldwide regardless of level of hygiene, quality of water, food or sanitation or type of behavior. In Korea, the disease mostly occurred in infants under 2 year old, and usually in late fall to early winter with the highest incidence shown in November until early 1990s. However, since then, relatively the age incidence of this infection has increased in children over 2 years old, and it has mainly occurred in late winter to early spring with the highest incidence shown between February and March. And rotavirus gastroenteritis serotypes varied year to year as well as varied by year within the same region in our country. The comparison of severity scores between age groups in Korea showed that severity score was lowest in infants less than 3 months and highest in infants more than 6 months and less than 12 months, and hospitalization period between age groups was longest in infants older than 3 months and less than 3 months and highest in infants more than 6 months and less than 12 months and less than 6 months. In this aspect, rotavirus vaccines should be given to infants less than 2 years of age in whom the incidence of rotavirus gastroenteritis is the highest, and vaccination should be ideally started before 3 months considering hospitalization period and severity of rotavirus gastroenteritis. However, It should be careful for the introduction of new rotavirus vaccine, and the effectiveness of rotavirus vaccines should be assessed by long-term post marketing surveillance. And we guess an multivalent vaccine may be an ideal rotavirus vaccine to prevent primary infection caused by variable rotavirus strains in Korea. In addition, clinical epidemiology studies on rotavirus gastroenteritis which evaluate the age and serotypes related severity should be continuously conducted in various regions.

Knowledge and Attitudes Not Strongly Related to COVID-19 Prevention, Behaviors, and Vaccination Among Filipino College Students

  • The effects of the COVID-19 pandemic across various sectors are far-reaching, and the education sector is not an exemption. Approved and recognized vaccines are clinically proven to provide protection and have been considered one of the primary mitigations against the virus. Knowledge, attitude, and related practices (henceforth KAP) toward the virus is widely considered to be an essential cognitive key in public health for health intervention, prevention, and promotion. Previous studies revealed a positive correlation between knowledge and attitude. This study investigated the KAP and their relationship among the 857 college students in one state college in the Philippines using an online survey. Using descriptive statistics, the findings showed that the majority of the respondents have been fully vaccinated against COVID-19. Respondents were shown to be knowledgeable about COVID-19, specifically about its transmission, clinical symptoms, treatment, risk groups, isolation, prevention, and control. Positive attitudes toward individual and governmental mitigation and control of the virus were shown. Their scores on practices revealed that they always exhibit protective and preventive behavioral practices. Pearson correlation revealed a weak significant relationship between knowledge and practice, and a fair significant relationship between attitude and practice. No significant relationship is found between knowledge and attitude. This study hopes to find its place in the literature worthy of consideration as empirical support toward understanding a specific group's perceptive risk and preventive behavior toward the virus, policy making, and health intervention and promotion.

Associations Between Conventional Healthy Behaviors and Social Distancing During the COVID-19 Pandemic: Evidence From the 2020 Community Health Survey in Korea

  • Objectives: Many studies have shown that social distancing, as a non-pharmaceutical intervention (NPI) that is one of the various measures against coronavirus disease 2019 (COVID-19), is an effective preventive measure to suppress the spread of infectious diseases. This study explored the relationships between traditional health-related behaviors in Korea and social distancing practices during the COVID-19 pandemic. Methods: Data were obtained from the 2020 Community Health Survey conducted by the Korea Disease Control and Prevention Agency (n=98 149). The dependent variable was the degree of social distancing practice to cope with the COVID-19 epidemic. Independent variables included health-risk behaviors and health-promoting behaviors. The moderators were vaccination and unmet medical needs. Predictors affecting the practice of social distancing were identified through hierarchical multiple logistic regression analysis. Results: Smokers (adjusted odds ratio [aOR], 0.924) and frequent drinkers (aOR, 0.933) were more likely not to practice social distancing. A greater degree of physical activity was associated with a higher likelihood of practicing social distancing (aOR, 1.029). People who were vaccinated against influenza were more likely to practice social distancing than those who were not (aOR, 1.150). However, people with unmet medical needs were less likely to practice social distancing than those who did not experience unmet medical needs (aOR, 0.757). Conclusions: Social distancing practices were related to traditional health behaviors such as smoking, drinking, and physical activity. Their patterns showed a clustering effect of health inequality. Therefore, when establishing a strategy to strengthen social distancing, a strategy to protect the vulnerable should be considered concomitantly.

Thermal imaging and computer vision technologies for the enhancement of pig husbandry: a review

  • Pig farming, a vital industry, necessitates proactive measures for early disease detection and crush symptom monitoring to ensure optimum pig health and safety. This review explores advanced thermal sensing technologies and computer vision-based thermal imaging techniques employed for pig disease and piglet crush symptom monitoring on pig farms. Infrared thermography (IRT) is a non-invasive and efficient technology for measuring pig body temperature, providing advantages such as non-destructive, long-distance, and high-sensitivity measurements. Unlike traditional methods, IRT offers a quick and labor-saving approach to acquiring physiological data impacted by environmental temperature, crucial for understanding pig body physiology and metabolism. IRT aids in early disease detection, respiratory health monitoring, and evaluating vaccination effectiveness. Challenges include body surface emissivity variations affecting measurement accuracy. Thermal imaging and deep learning algorithms are used for pig behavior recognition, with the dorsal plane effective for stress detection. Remote health monitoring through thermal imaging, deep learning, and wearable devices facilitates non-invasive assessment of pig health, minimizing medication use. Integration of advanced sensors, thermal imaging, and deep learning shows potential for disease detection and improvement in pig farming, but challenges and ethical considerations must be addressed for successful implementation. This review summarizes the state-of-the-art technologies used in the pig farming industry, including computer vision algorithms such as object detection, image segmentation, and deep learning techniques. It also discusses the benefits and limitations of IRT technology, providing an overview of the current research field. This study provides valuable insights for researchers and farmers regarding IRT application in pig production, highlighting notable approaches and the latest research findings in this field.

Progressive Pulmonary Fibrosis: Where Are We Now?

  • Interstitial lung diseases (ILDs) are a diverse collection of lung disorders sharing similar features, such as inflammation and fibrosis. The diagnosis and management of ILD require a multidisciplinary approach using clinical, radiological, and pathological evaluation. Progressive pulmonary fibrosis (PPF) is a distinct form of progressive and fibrotic disease, occurring in ILD cases other than in idiopathic pulmonary fibrosis (IPF). It is defined based on clinical symptoms, lung function, and chest imaging, regardless of the underlying condition. The progression to PPF must be monitored through a combination of pulmonary function tests (forced vital capacity [FVC] and diffusing capacity of the lung for carbon monoxide), an assessment of symptoms, and computed tomography scans, with regular follow-up. Although the precise mechanisms of PPF remain unclear, there is evidence of shared pathogenetic mechanisms with IPF, contributing to similar disease behavior and worse prognosis compared to non-PPF ILD. Pharmacological treatment of PPF includes immunomodulatory agents to reduce inflammation and the use of antifibrotics to target progressive fibrosis. Nintedanib, a known antifibrotic agent, was found to be effective in slowing IPF progression and reducing the annual rate of decline in FVC among patients with PPF compared to placebos. Nonpharmacological treatment, including pulmonary rehabilitation, supplemental oxygen therapy, and vaccination, also play important roles in the management of PPF, leading to comprehensive care for patients with ILD. Although there is currently no cure for PPF, there are treatments that can help slow the progression of the disease and improve quality of life.

농촌노인과 도시노인의 건강행위 방해요인에 대한 비교 연구 (A Comparative Study on Barrier Factors in Health Behaviors of Urban vs Rural Elderly)

  • The purpose of this study was to identify the barrier factors of health behaviors of urban and rural elderly and to compare the health behaviors and level of barriers between two groups, and finally to get the basic informations about the adequate nursing strategies to promote the health state of urban and rural elderly. The subjects of this study were 177 over the age of 65, 81 elderly lived in Seoul and 96 elderly lived in rural areas. The instruments for this study were the health behavior scale(14 items) and the barrier scale (118 items) developed by Gu et al(2003). For the data analysis, SPSS PC program was utilized for descriptive statistics, ${\chi}^2$- test, t-test, Pearson correlation. The results of this study were ; 1. The mean score of health behaviors (range 1-4) was 2.69 in urban elderly and 2.33 in rural elderly ; there was significant difference(t=5.03, P=.00). 2. There were significant differences in levels of barriers(range 1-3) between the two groups, such as calcium intake(t=-3.16, P=.00), regular exercise(t=-3.80, P=.00), exercise time(t=-5.54, P=.00), use of stress reduction method(t=-3.45, P=.00), regular check up(t=-3.89, P=.00), vaccination(t=-3.83, P=.00). Higher levels of barriers were found in rural elderly than in urban elderly. 3. Lack of habituation, lack of will power and lack of knowledge in calcium intake; lack of time, lack of habituatuion, lack of family support, lack of will power and lack of environment in exercise; lack of perceived benefit, lack of time, lack of will power and lack of knowledge in use of stress reduction method; lack of time, lack of interest, lack of habituation and lack of will power in disease prevention were significantly higher in rural elderly than in urban elderly. In the conclusion, nursing interventions should be planned based on the social environment of elderly. To promote the health state of elderly, interventions to decrease the barrier levels and to reduce the barrier factors to health behaviors should be implemented.

손실과 이득 메시지 프레이밍 광고상황에서 COVID-19에 대한 관여와 사회적 거리두기 실천의도와의 관계에 영향을 미치는 광고태도의 매개효과 (Mediating Effect of Attitude toward Advertisement on the Relationship between COVID-19 Involvement and Behavioral Intention of Social Distancing in Loss versus Gain Advertising Context)

  • COVID-19 감염예방을 위한 손실과 이득 메시지 프레이밍 광고상황에서 COVID-19에 대한 관여와 사회적 거리두기 실천의도와의 관계를 확인하고 이들 간 관계에서 광고태도의 매개효과를 확인하였다. 대한민국에 거주하는 19세 이상 성인 379명을 대상으로 하였다. 2개 유형의 메시지 프레이밍 중 1개 광고를 무작위 할당하여 보도록 한 후 COVID-19에 대한 관여, 광고태도 및 사회적 거리두기 실천의도를 측정하였다. COVID-19에 대한 관여가 사회적 거리두기 실천의도에 영향을 미쳤는데 COVID-19에 관여함으로써 COVID-19에 대한 위험이나 심각성을 인식하여 위험감소행위를 시도하기 때문으로 생각되어 COVID-19에 대한 관여를 높이는 전략이 매우 필요하다. 또한 광고태도가 COVID-19에 대한 관여와 사회적 거리두기 실천의도와의 관계에서 매개작용을 하였다. 따라서 병원이나 지역사회 종사 간호사가 현장에서 대중과 소통하고 정책을 보급할 때 긍정적 광고태도를 유도할 수 있는 효과적인 메시지를 제시함으로써 건강관리 리더로서 역할을 충실히 해 나가야 할 것이다.

포르말린으로 유도된 통증 유발 쥐에서 무침주입기를 이용한 봉독약침의 진통효과 (Comparative Study of Therapeutic Effect of Needle-free Bee Venom Aqua-acupuncture (BVA) into Zusanli (ST36) in the Rat Formalin Test)

  • Bee Venom aqua-acupuncture (BVA) simultaneously exerts pharmacological effects of biologically active compounds, existed in the whole bee venom, and medicinal effect of the stimulation of acupuncture points. BVA has been considered as a promising therapeutic method for treating various chronic diseases, mainly accompanying severe pain and inflammation. As a painless injection device, jet injectors have been commercially marketed for various clinical applications including insulin injection and vaccination. Among them, a pressure-driven jet injector system could be used for intradermal delivery of a variety of drugs. The aim of this study was to investigate the analgesic effects of the BVA using a needle-free injector (Biojector $2000^{\circledR}$, Bioject Inc., OR, USA), compared to the conventional BV aqua-acupuncture using a typical syringe. Adult Sprague-Dawley rats were injected with bee venom $(0.08mg/kg,\;50{\mu}l)$ using Biojector $2000^{\circledR}$ (BVA-B) or a syringe (BVA-5) into the Zusanli (ST36) acupoint, 30 minutes before plantar injection of 2% formalin. It was found that BVA-B-, or BVA-5-treated rats, compared to controls, exhibited significantly less licking behavior during the late phase in the rat formalin test, when compared to controls. During early phase, however, those effects were not significant but substantial. The analgesic effect of BVA-B was also compatible with that of the conventional BVA-5. In the immunohistochemical studies, BVA-B significantly suppressed the expression of formalin-evoked c-fos, a biomarker of neuronal activity, in the lumbar dorsal horn of the spinal cord. These results indicated that BVA-B waseffective in the modulation of pain in the rat formalin test, compared to BVA-5. Taken together, the needle-free jet injector system could be substituted for the conventional aqua- acupuncture with the advantage of little pain.
