남.녀 노인의 인플루엔자 백신 접종 실태와 영향요인 (The Correlates of Influenza Vaccination among Korean Elderly Men and Women)

  • Purpose: This study aimed to investigate the rate of and factors associated with influenza vaccination in Korean elderly people. Methods: For this retrospective cross-sectional study, we used data from the Korean National Health and Nutrition Examination Survey (KNHANES) 2008. A total of 1,516 men and women aged 65 or above were included. The data were analyzed using descriptive statistics and logistic regression. Results: The overall rate of influenza vaccination in 2008 was 73.6%. The vaccination rate was lower in women (73.2%) than in men (74.2%; p<.001). It was lower in the elderly aged 80 and above, those living in metropolitan areas, smokers, those who did not receive regular health screening, those who had no history of hospitalization within one year, and those who had liver diseases than in the other groups. The rate of vaccination was higher in those with cancer. Logistic regression analysis revealed that for both men and women, the significant correlates associated with vaccination were demographic factors, chronic disease and health behavior. Conclusion: The influenza vaccination coverage needs to be improved. Efforts should be made to reach the elderly population with relatively low influenza vaccination rates and to increase awareness about the importance of vaccination at a personal and community level.

인유두종바이러스 백신 미접종 간호대생의 접종의도 영향 요인 (Factors Influencing Human Papillomavirus Vaccination Intention among Unvaccinated Nursing Students in Korea)

  • Purpose: This study was performed to identify factors associated with human papillomavirus vaccination intention among unvaccinated nursing students. Methods: Two hundred-and-five female nursing students from three universities completed self-administered questionnaires including participants' characteristics, human papillomavirus-related knowledge, attitude toward human papillomavirus vaccination, and human papillomavirus-related health beliefs. Multivariate logistic regression analysis was used to determine significant independent predictors of human papillomavirus vaccination intention. Results: Of 205 participants, 134 (65.4%) reported an intention to obtain a vaccination against human papillomavirus. As a result of the analysis of the bivariate relationships, family history of cervix cancer, perceived needs, importance of prevention, perceived susceptibility, perceived benefit, and perceived barrier were significantly related to vaccination intention. A multivariate logistic regression model identified factors of human papillomavirus vaccination intention: higher importance of prevention (Adjusted Odds Ratio [AOR]: 4.20, 95% Confidence Interval [CI]: 1.73~10.19), higher perceived benefit (AOR: 6.94, 95% CI: 2.01~23.98), lower perceived barrier (AOR: 0.39, 95% CI: 0.20~0.73). Conclusion: The results of this study indicated significant factors influencing the intention to obtain human papillomavirus vaccination in unvaccinated nursing students. Also, the importance of prevention, perceived susceptibility, perceived benefit, and perceived barrier in obtaining human papillomavirus vaccination should be taken into account when developing educational programs.

한우에서 수송열의 예방을 위한 백신접종과 예방치료의 효과 (Effect of Vaccination and Mass Medication for the Prevention of Shipping Fever in Korean Native Cow)

  • The objective of this study was to compare occurrence of the shipping fever associated with vaccination and mass medication in Korean Native Cow. Lack of vaccination (Pasteurella haemolytica, infectious bovine rhinotracheitis, bovine viral diarrhea, parainfluenza 3, and bovine respiratory syncytial virus) and mass medication before and after transportation, 67% of gram-negative cocci were detected. However, with vaccination and mass medication, only 33% of Gram-negative cocci were detected. When antimicrobial susceptibility for the detected bacterium was tested, apramycin, ampicillin, amilacin, clindamycin, gentamycin, kanamycin, penicillin, and streptomycin were resistant, whereas tylocin, amoxicillin, linocospetin, nalidixic acid, norfloxacin, oxalinic acid, and cefotaxime were susceptible. Morbidity and average therapeutic effect were 33% and 1.0time with vaccination and mass medication and were 78% and 5.3 times without vaccination and mass medication, respectively. Therefore, the results suggest that shipping fever would be considerably decreased with vaccination including mass medication before and after transportation.

한국 노인의 폐렴구균 백신 접종률 및 관련 요인 (Pneumococcal Vaccination Rate and Related Factors in Older Adults in Republic of Korea)

  • 65세 이상 노인의 폐렴구균 국가무료 백신 접종은 2종류 중 한 개에 대해서만, 그것도 보건소에서만 접종이 가능하여 다른 국가예방접종 감염병과 달리 전산등록 자료에 의한 백신 접종률 파악이 불가능한 실정이다. 따라서 이 연구는 65세 이상 노인에게 권고되고 있는 2종의 폐렴구균 백신 모두에 대하여 백신 접종률을 파악함과 동시에 백신 접종 이유나 미접종 이유, 백신 접종 관련 요인을 파악하고자 하였다. 본 연구의 모집단은 65세 이상 노인이 되며, 최종 분석대상자수는 총 1,150명이었으며 컴퓨터 전화조사 방법을 사용하였다. 조사내용은 폐렴구균 백신 접종현황(접종여부-2회, 접종일자, 백신종류, 접종기관종류, 접종비용 지불여부, 자료원), 백신 접종을 한 이유, 백신 접종에 대해서 알게 된 정보원, 백신 접종을 하지 않은 이유, 백신 접종 관련 요인이었다. 65세 이상 노인의 폐렴구균 백신 접종률은 56.2%로 미국(2017년) 69.0%, 영국(2019년) 69.2% 보다 낮은 수준이었다. 폐렴구균 백신 접종 관련 요인으로는 여자가, 연령이 낮을수록, 시부 거주자가, 직업이 없을 때, 고학력자가, 의료보장이 있을 때, 기혼자가, 동거가족이 있을 때에 백신 접종률이 높게 나타났다. 또한 국가 권고 성인 예방접종을 인지하고 있을 때, 의사에게 성인 예방접종의 필요성을 설명 받은 경험이 있을 때, 본인의 성인 예방접종 기록을 보관하고 있을 때, 성인 예방접종이 감염병 예방에 도움이 된다고 높게 인지하고 있을 때, 국가나 지방자치단체의 성인 예방접종 홍보 경험이 있을 때 백신 접종률이 높게 나타났다. 앞으로 국가지원 대상이 아닌 성인 예방접종에 대해서도 정확한 접종기록 관리를 위한 대안의 개발이 필요하며, 국가의 감염병관리 정책 근거 자료로 활용할 수 있도록 정기적인 성인 예방접종률 산출이 이루어져야 할 필요가 있다.

당뇨병 환자의 인플루엔자 예방접종률과 관련 요인 (The Estimated Proportion for Influenza Vaccination and Related Factors in Korean Diabetics)

  • Background: The purpose of this study was to investigate the estimated proportion for influenza vaccination and to identify factors associated with influenza vaccination in Korean diabetic patients. Methods: Data from the fourth, fifth, and sixth (except for 2013) Korea National Health and Nutrition Examination Survey (n=3,726) was used. A chi-square test was performed to investigate the estimated proportion for influenza vaccination, and a multiple logistic regression analysis was used to identify the factors associated with self-reported influenza vaccination. Results: In men, 28.8% of diabetes patients 30-64 years of age, and 76.1% of elderly (over 65 years of age) diabetes patients received influenza vaccination. In women, 37.7% of diabetes patients 30-64 years of age, and 78.4% of elderly diabetes patients received influenza vaccination (p<0.0001). The determinants of influenza vaccination were marriage, hypertension (odds ratio [OR], 1.55; 95% confidence interval [CI], 1.07-2.24), residence within a mega city, unemployment (OR, 3.94; 95% CI, 1.24-12.54), and exercise via (weekly) walking for diabetic men; and hypertension (OR, 1.71; 95% CI, 1.16-2.52), chronic disease (OR, 1.81; 95% CI, 1.08-3.02), and exercise via walking (OR, 2.65; 95% CI, 1.49-4.73) for diabetic women. Conclusion: Influenza vaccination remains relatively low in young diabetic patients. It is necessary to recommend vaccination to young diabetic patients, and to devise other strategies to improve vaccination.

Results: In 1980s, there are many infants and children management object, but now there are rapidly decreasing infants and children management subject of a Primary Health Care Post because of rural exodus phenomenon of 1990's and a low birthrate of 2000's. Currently, the infants and children that registed, and managed to a Primary Health Care Post is most multi-cultural families. A multi-cultural family assistance program is provided these infants and children to the objects. Conclusions: Infant vaccination execution in Primary Health Care Posts showed to high vaccination rate because the prevention of vaccination delay or omission of the interior child who raised geographical accessibility to object sons nowadays it difficult to management of transportation of vaccine and storage management for a little shroud objects limited and management of inoculation business in quality. A main problem is as follows. There are a little vaccine transportation, trouble of proper temperature maintenance of a storage process, and having a lot of vaccine abolition as a little objects, educational training shortage of health care practitioner regarding to execute an inoculation. Therefore, vaccination business needs measures for management in quality in Primary Health Care Posts.

성인의 대상포진 예방접종 시행의 영향 요인 (Factors Affecting Herpes Zoster Vaccination in Adults)

  • Objectives : This study aimed investigate general characteristics, vaccination status and health beliefs related to vaccination for herpes zoster and to identify variables. Methods : The structured questionnaire was conducted from March 31, 2016 to April 20, 2016 with 312 outpatients who visited a university hospital. The data were analyzed by frequency analysis, t-test, $x^2-test$, correlation analysis and logistic regression analysis via SPSS statistics 22.0. Results : The vaccination rate of the herpes zoster was 16.0% and the vaccination cost was the most common reason for rejecting inoculation. More highly educated people with more than a university degree(OR=4.28, p=.002), those with higher susceptibility to herpes zoster(OR=1.56, p=.000), those more aware of the benefits of vaccination(OR=1.51, p=.009) and those with lower disability for vaccination(OR=0.74, p=.003) were more likely to be vaccinated. Conclusions : Therefore, to increase the vaccination rate of herpes zoster in adults over 50 years old, continuous publicity and education are needed to emphasize the susceptibility of herpes zoster and the benefit of vaccination. In addition, it is necessary to develop policies at the national level for the expensive vaccination cost that obstructs vaccination.

영아를 위한 어머니의 로타바이러스 예방접종 이행과 관련 요인: 건강신념모형을 중심으로 (Factors Associated with Rotavirus Vaccination Behavior among Mothers of Infants: using the Health Belief Model)

  • Purpose: This study was conducted to identify the sociodemographic characteristics and health belief related to rotavirus vaccination behavior among mothers of infants. Methods: Data collection was performed through an on-line survey between 13 and 16 October, 2016, and the subjects targeted were 240 mothers with infants between the ages of 8 months and 12 months. The collected data were analyzed using $X^2$ test, Fisher's Exact test, ANOVA, and Logistic Regression with SPSS 23.0 program. Results: The complete rotavirus vaccination rate was 76.7%, the incomplete vaccination rate was 9.2%, and the non-vaccination rate was 14.2%. Factors associated with complete rotavirus vaccination were the mother's age (OR=3.306, 95% CI=1.087 to 10.054), the perceived benefits (OR=6.500, 95% CI=2.163 to 19.528) and the self-efficacy (OR=11.568, 95% CI=2.921 to 45.818), and approximately 40.8% of explanation power was observed. Conclusion: To increase the rotavirus vaccination rate, counseling and education by nurses should be performed to increase the self-efficacy and public awareness of the benefits of rotavirus vaccination for mothers with infants.

중·고등학교 보건교사의 자궁경부암 예방백신에 대한 지식과 암 정보추구행위 및 교육의지 (The Relationships among Knowledge, Information Seeking Behavior, and Willingness for Education about Human Papillomavirus Vaccination in the Middle or High School Teachers)

  • Purpose: The purpose of this study was to identify the relationships among knowledge, cancer information seeking behavior, and Willingness for education about Human Papillomavirus (HPV) vaccination among school teachers. Methods: The subjects were 135 teachers who had were working in the middle or high school in Korea. Data were collected by a self-report questionnaire, 2011. Data were analyzed by using the SPSS WIN 18.0 program. Results: There was a positive relationship between information seeking behavior and Willingness for education about HPV vaccination. There were differences in Willingness for education according to necessity of HPV vaccination for premarital women above 16-year old and marital women below 45-year old, and necessity of education for students. The Willingness for education were predicted by information seeking behavior, groups who HPV vaccination is necessary to premarital women above 16-year old, and HPV vaccination education for student is necessary. These variables explained 25.0% of the variance of the Willingness for education about HPV vaccination. Conclusion: The Willingness for education about HPV vaccination among middle or high school teachers could be improved by the emphasis of the preception that HPV vaccination is necessary to middle or high school students and is related to the prevention of cervical cancer.

Knowledge of Human Papillomavirus Infection, Cervical Cancer and Willingness to pay for Cervical Cancer Vaccination among Ethnically Diverse Medical Students in Malaysia

  • The primary objective of this study was to assess the knowledge of medical students and determine variation between different cultural groups. A secondary aim was to find out the willingness to pay for cervical cancer vaccination and the relationships between knowledge and attitudes towards Human Papillomavirus vaccination. A cross-sectional survey was conducted in a private medical university between June 2014 and November 2014 using a convenient sampling method. A total of 305 respondents were recruited and interviewed with standard questionnaires for assessment of knowledge, attitudes and practice towards human papilloma virus and their willingness to pay for HPV vaccination. Knowledge regarding human papilloma virus, human papilloma virus vaccination, cervical cancer screening and cervical cancer risk factors was good. Across the sample, a majority (90%) of the pupils demonstrated a high degree of knowledge about cervical cancer and its vaccination. There were no significant differences between ethnicity and the participants' overall knowledge of HPV infection, Pap smear and cervical cancer vaccination. Some 88% of participants answered that HPV vaccine can prevent cervical cancer, while 81.5% of medical students said they would recommend HPV vaccination to the public although fewer expressed an intention to receive vaccination for themselves.
