Free-Breathing Motion-Corrected Single-Shot Phase-Sensitive Inversion Recovery Late-Gadolinium-Enhancement Imaging: A Prospective Study of Image Quality in Patients with Hypertrophic Cardiomyopathy

  • Objective: Motion-corrected averaging with a single-shot technique was introduced for faster acquisition of late-gadolinium-enhancement (LGE) cardiovascular magnetic resonance (CMR) imaging while free-breathing. We aimed to evaluate the image quality (IQ) of free-breathing motion-corrected single-shot LGE (moco-ss-LGE) in patients with hypertrophic cardiomyopathy (HCM). Materials and Methods: Between April and December 2019, 30 patients (23 men; median age, 48.5; interquartile range [IQR], 36.5-61.3) with HCM were prospectively enrolled. Breath-held single-shot LGE (bh-ss-LGE) and free-breathing moco-ss-LGE images were acquired in random order on a 3T MR system. Semi-quantitative IQ scores, contrast-to-noise ratios (CNRs), and quantitative size of myocardial scar were assessed on pairs of bh-ss-LGE and moco-ss-LGE. The mean ± standard deviation of the parameters was obtained. The results were compared using the Wilcoxon signed-rank test. Results: The moco-ss-LGE images had better IQ scores than the bh-ss-LGE images (4.55 ± 0.55 vs. 3.68 ± 0.45, p < 0.001). The CNR of the scar to the remote myocardium (34.46 ± 11.85 vs. 26.13 ± 10.04, p < 0.001), scar to left ventricle (LV) cavity (13.09 ± 7.95 vs. 9.84 ± 6.65, p = 0.030), and LV cavity to remote myocardium (33.12 ± 15.53 vs. 22.69 ± 11.27, p < 0.001) were consistently greater for moco-ss-LGE images than for bh-ss-LGE images. Measurements of scar size did not differ significantly between LGE pairs using the following three different quantification methods: 1) full width at half-maximum method; 23.84 ± 12.88% vs. 24.05 ± 12.81% (p = 0.820), 2) 6-standard deviation method, 15.14 ± 10.78% vs. 15.99 ± 10.99% (p = 0.186), and 3) 3-standard deviation method; 36.51 ± 17.60% vs. 37.50 ± 17.90% (p = 0.785). Conclusion: Motion-corrected averaging may allow for superior IQ and CNRs with free-breathing in single-shot LGE imaging, with a herald of free-breathing moco-ss-LGE as the scar imaging technique of choice for clinical practice.

중등도의 허혈성 승모판막 폐쇄부전 환자의 관상동맥 우회로 조성술 시 승모판막 수술의 유무에 따른 원상 결과 (Clinical Results of Mitral Valvular Surgery in Patients with Moderate Ischemic Mitral Regurgitation Undergoing Coronary Artery Bypass Grafting)

  • 배경 : 관상동맥협착증에 동반되는 중등도의 만성 허혈성 승모판막 폐쇄부전의 경우 승모판막에 대한 수술적 치료가 필요한지에 대하여 많은 논란이 있다. 저자는 이러한 환자들을 대상으로 임상 결과를 알보고자 하였다. 대상 및 방법: 1992년 1월부터 2005년 2월까지 관상동맥 우회로 조성술을 시행 받은 환자 중 중등도의 허혈성 승모판막 폐쇄부전이 있던 44명의 환자를 대상으로 후향적 연구를 시행하였다. 승모판막 수술을 시행 받은 환자는 20명이었고 (group 1) 승모판막 수술을 시행 받지 않은 환자는 24명이었다(group 2). Group 1에서 체외심폐순환 시간이 유의하게 걸었으나 (p<0.01) 나이, 성별, 수술 전 심박출 계수, 문합한 혈관의 수 등 다른 인자는 두 group 간에 차이가 없었다. 수술 후 심초음파 시행 시기는 평균 $21.2{\pm}28.0$개월이었고 평균 추적관찰기간은 $30.1{\pm}29.6$개월이었다. 결과: 두 group 간에 수술 사망률은 유의한 차이가 없었다(group 1 vs group 2, 15.0% vs 8.3%, p=0.493). Group 1에서 group 2보다 승모판막 폐쇄부전도 ${(0.81{\pm}0.91\;vs\;1.50{\pm}0.05,\;p=0.046)}$, 폐쇄부전 정도의 경감 ${(1.75{\pm}0.93\;vs\;0.70{\pm}1.26,\;p=0.009)}$에서 차이를 보였다. 수술 후 섬박출 계수 ${(34.1{\pm}11.4%\;vs\;41.6{\pm}12.9%)}$, 좌심실 수축기 용적 ${(118.2{\pm}63.9\;ml%\;vs\;85.6{\pm}281\;ml)}$, NYHA functional class ${(2.1{\pm}0.2\;vs\;2.4{\pm}1.2)}$는 차이가 없었고, 5년에서의 생존율도 유의한 차이를 보이지 않았다${(85{\pm}8%\;vs\;82{\pm}8%)}$. 수술 사망에 대한 유의한 위험 인자는 없었으며 만기 사망에 대한 위험인자는 수술 전 심방 세동이 있던 경우(p=0.042)였고 승모판막 수술의 유무는 만기 사망과 유의한 관계를 보이지 않았다. 결론: 중등도의 승모판막 폐쇄부전이 있는 경우 승모판막을 수술하는 것이 수술 후 폐쇄부전의 정도가 의미 있게 감소하였으나 생존율 및 심장 기능에 있어서는 별 차이가 없었다. 이러한 환자에서 승모판막 수술의 필요성 및 심실 기능의 향상을 위한 좀 더 전향적인 연구가 필요하리라 생각된다.

Percoll에 의한 미니돼지 정액내 세균 제거가 정자 성상과 수정란 분할에 미치는 영향 (Effect of Bacteria Eliminated Sperm by Percoll Method on Sperm Quality and Embryo Cleavage in Miniature Pig)

  • The objectives of this study was to evaluate the efficiency of the bacteria eliminated sperm by percoll gradient method on sperm quality and embryo cleavage in vitro in pig. The semen of miniature pig collected by gloved-hand method pre-warmed ($37^{\circ}C$) in thermos bottle, and separated by 65% percoll. Analysis of sperm ability was estimated by examining viability, capacitation and acrosome reaction using chlortetracycline (CTC) and the abnormality. Also, fertility of sperm was monitored with cleavage rate of embryo after IVF using separated and un-separated sperm by percoll. The result, viability of separated sperm was significantly(p<0.05) higher($83.6{\pm}$2.0 vs $59.0{\pm}4.4%$) than un-separated sperm. The results of CTC analysis showed the percentage of F- and B-patterned separated sperm was higher in separated that than un-separated sperm. On the contrary, the percentage of AR-patterned form unseparaed sperm was significantly(p<0.05) higher($13.6{\pm}0.8$ vs $8.1{\pm}0.6%$) than separated sperm. Also, abnormality of un-separated sperm was significantly(p<0.05) higher($2.2{\pm}0.4$ vs $16.8{\pm}2.8%$) than separated sperm. However, the cleavage rates of embryo using separated sperm by percoll and un-separated sperm had not significantly difference on 2 cell stage(9.25 vs 11.88%), 4 cell stage(26.76 vs 24.51%) and >4 cell stage(63.99 vs 63.61%) at 48h of IVF. Therefore, the sperm separated by percoll method showed improvement in sperm quality than un-separated sperm in miniature pig.

학습방법, 학습계획, 과제 난이도가 소프트웨어 학습에 미치는 영향 (The effects of learning method, learning schedule, and task difficulty on the learning of computer software)

  • 다양한 전자제품의 조작법을 빠르고 정확하게 학습하는 것은 일상적이고 중요한 과제가 되었다. 특히 소프트웨어는 여러 제품들의 통제 및 조작에서 핵심적인 지위를 차지하고 있다. 본 연구는 기존 학습연구에서 중요한 변인으로 연구되어온 학습방법, 학습계획, 과제난이도가 소프트웨어 학습에 미치는 영향을 검증하였다. 실험1에서는 2 (학습방법: 경험적 vs. 언어적) ${\times}$ 2 (학습계획: 간격 vs. 덩이진) ${\times}$ 2 (난이도: 쉬움 vs. 어려운)의 피험자간 요인설계를 사용하여 각 조건에서 참가자들이 윈도우 무비메이커를 사용하여 파일을 조작하는 방법을 학습하는 실험을 실시하였다. 그 결과 학습계획에 따른 수행의 차이는 발견할 수 없었으나, 언어적 학습보다 경험적 학습에서 참가자들은 더 빠르게 평가과제를 완료하였다. 특히 과제난이도가 높아질 경우 참가들은 언어적 조건에서 경험적 조건보다 두드러진 수행저하를 보였는데, 이는 과제가 어려워질수록 경험적 학습이 효과적인 학습방법이 라는 것을 시사한다. 즉 소프트웨어 학습에서 간단한 조작의 경우에는 매뉴얼 혹은 지시문의 형태로 구성된 언어적 학습으로 충분하지만 어려운 과제의 경우에는 체험 프로그램이나 투토리얼 모드를 통해 학습하는 것이 효과적일 것이다. 추가실험에서 난이도 증가에 따른 언어적 학습의 선형적 이득을 확인하기 위해 난이도를 3단계로 세분화하여 검증하였으며 (실험 2) 학습계획의 효과를 확인하기 위해 학습시행간 간격을 증가시켰으나 (실험3) 유의미한 결과를 발견하지는 못했다.

Feasibility of Free-Breathing, Non-ECG-Gated, Black-Blood Cine Magnetic Resonance Images With Multitasking in Measuring Left Ventricular Function Indices

  • Objective: To clinically validate the feasibility and accuracy of cine images acquired through the multitasking method, with no electrocardiogram gating and free-breathing, in measuring left ventricular (LV) function indices by comparing them with those acquired through the balanced steady-state free precession (bSSFP) method, with multiple breath-holds and electrocardiogram gating. Materials and Methods: Forty-three healthy volunteers (female:male, 30:13; mean age, 23.1 ± 2.3 years) and 36 patients requiring an assessment of LV function for various clinical indications (female:male, 22:14; 57.8 ± 11.3 years) were enrolled in this prospective study. Each participant underwent cardiac magnetic resonance imaging (MRI) using the multiple breath-hold bSSFP method and free-breathing multitasking method. LV function parameters were measured for both MRI methods. Image quality was assessed through subjective image quality scores (1 to 5) and calculation of the contrast-to-noise ratio (CNR) between the myocardium and blood pool. Differences between the two MRI methods were analyzed using the Bland-Altman plot, paired t-test, or Wilcoxon signed-rank test, as appropriate. Results: LV ejection fraction (LVEF) was not significantly different between the two MRI methods (P = 0.222 in healthy volunteers and P = 0.343 in patients). LV end-diastolic mass was slightly overestimated with multitasking in both healthy volunteers (multitasking vs. bSSFP, 60.5 ± 10.7 g vs. 58.0 ± 10.4 g, respectively; P < 0.001) and patients (69.4 ± 18.1 g vs. 66.8 ± 18.0 g, respectively; P = 0.003). Acceptable and comparable image quality was achieved for both MRI methods (multitasking vs. bSSFP, 4.5 ± 0.7 vs. 4.6 ± 0.6, respectively; P = 0.203). The CNR between the myocardium and blood pool showed no significant differences between the two MRI methods (18.89 ± 6.65 vs. 18.19 ± 5.83, respectively; P = 0.480). Conclusion: Multitasking-derived cine images obtained without electrocardiogram gating and breath-holding achieved similar image quality and accurate quantification of LVEF in healthy volunteers and patients.

Early Outcomes of Endoscopic Vein Harvesting during the Initial Learning Period

  • Background: The endoscopic vein harvesting (EVH) method has been used in coronary artery bypass surgery in many countries. We started using the EVH method recently, and investigated the results during the early learning period. Methods: Between March 2012 and June 2014, 75 patients (31 patients in the EVH method group, and 44 patients in the open method group) who underwent isolated first-time coronary artery bypass grafting using vein grafts were retrospectively analyzed with respect to the early outcomes including graft patency and risk factors for leg wound complications. For assessing the patency of vein graft, we performed coronary computed tomography angiography during the immediate postoperative period and 6 months later. Results: Mean harvesting time of endoscopic method was about 15 minutes. Patency rate during the immediate operative period and the 6-month patency rate were similar between the two groups (postoperative period: EVH 100% vs. open method 94.4%, p=0.493; at 6 months: EVH 93.3% vs. open method 90.9%, p=0.791). Leg wound complications occurred more frequently in the open method group (EVH 3.2% vs. open method 13.6%, p=0.127). According to the analysis, age was an independent risk factor for leg wound complications. Conclusion: EVH is a feasible method even for beginners and can be performed satisfactorily during their learning period.

삼첨판막 대치술의 장기간 임상성적 (Long-Term Clinical Results of Tricuspid Valve Replacement)

  • 삼첨판막대치술의 장기 결과에 대한 연구결과는 보고된 바가 많지 않다. 이에 저자들은 삼첨판막대치술의 위험인자를 분석해 보고, 삼첨판막대치술의 장기 결과를 알고자 연구를 시행하였다. 대상 및 방법: 대상 환자들은 1978년 10월부터 1996년 12월까지 삼첨판막대치술을 시행 받은 환자 70명을 대상으로 후향적인 연구를 진행하였는데, 7명의 환자들은 2차례의 삼첨판막대치술을 시행 받아 총 77예의 삼첨판막대치슬을 시행하였다. 환자들의 평균 나이는 38.8$\pm$15.9세였으며, 26예에서는 조직판막을, 51예에서는 기계판막을 이식하였다. 결과: 수술 사망률은 15.6%였고, 만기 사망률은 12.3%였다. 5년, 10년 그리고 13년에서의 생존율은 조직판막과 기계판막이 각각 81.3% vs. 100%, 66.1% vs. 100%, 60.6% vs. 100%였다(p=0.0175).판막과 관련된 재수술이 없을 확률은 5년, 10년 그리고 13년에서 조직판막과 기계판막이 각각 100% vs. 93.9, 100% vs. 93.9% 그리고 58.3% vs. 93.9%였다(p=0,3274). 판막과 관련된 재수술을 시행할 확률은 조직판막이 2.27%/환자-년이었고, 기계판막이 1.10%/환자-년이었다. 수술 사망과 관련된 위험인자 분석상, 수술 전 복수, 간비대, NYHA class가 나쁠수록, 그리고 삼첨판막대치술을 여러 번 받는 경우가 유의한 위험인자로 분석되었고, 조직판막의 사용과 심장수술을 여러 번 받는 경우가 만기 사망과 관련된 유의한 위험인자로 분석되었다. 걸론: 기계판막을 이용하여 삼첨판막대치술을 시행한 환자들의 장기간 생존율이 조직판막을 이용한 환자들보다 우수하였다. 따라서 수술 후에 적절한 추적관찰이 가능하다면, 기계판막을 이용하여 삼첨판막대치술을 시행하는 것이 필요하리라 생각한다.

Post Milking Teat Dip Effect on Somatic Cell Count, Milk Production and Composition in Cows and Buffaloes

  • The effect of post milking teat dipping on somatic cell count (SCC) of milk was determined in 20 Crossbred cows and 20 Murrah buffaloes selected from institute's herd. The animals were divided into two groups of 10 each. Animals of Group I (control) were teat washed with water before the milking while Group II animals were applied teat dipping solution after the completion of milking. The cows were milked 3 times a day while buffaloes were milked twice a day. The milk samples were collected from control and treated animals on day 0, 5, 10, 15, respectively. The milk samples were analyzed for milk constituents like fat, protein, lactose, chloride, IgG, NEFA, pH and EC and total and differential somatic cell counts. The changes in milk composition and somatic cell counts were significantly different (p<0.01) between the animals and between the breeds. However SCC, chloride content (p<0.05) and epithelial cells (p<0.01) varied during different days of study. The alterations in SCC, epithelial cells, TLC, lymphocyte, neutrophil, IgG, and protein content were significantly different (p<0.01) between control and treated groups. The pH, EC, protein, SCC, epithelial cells, lymphocyte and neutrophil cells of milk declined significantly (p<0.05) after the application of teat dipping, the respective values were 6.5 vs 6.40, 2.28 vs 2.37 mhos, 3.33 vs 4.04%, 1.00 vs $0.87{\times}10^5cells/ml$, 0.39 vs 0$0.34{\times}10^5cells/ml$, 0.36 vs $0.31{\times}1,000cells/ml$ and 0.17 vs $0.14{\times}1,000cells/ml$ in cows. However in buffaloes, epithelial cells, lymphocytes, neutrophils, EC and SCC declined (p<0.05) after application of teat dipping, the values being 0.37 vs $0.29{\times}10^5cells/ml$, 0.37 vs $0.25{\times}1,000cells/ml$, 0.14 vs $0.11{\times}1,000cells/ml$, 2.56 vs 2.37 mhos and 0.94 vs $0.73{\times}10^5cells/ml$, respectively. The study indicated that post milking teat dipping could be used as an effective method for the lowering of SCC in milk of crossbred cows and buffaloes.

Polarograph 에 依한 Monazite 鑛石속의 Uranium 의 定量에 關한 硏究 (Determination of Uranium in Monazite Sand by Polarographic Method)

  • A polarographic method for rapid determination of Uranium in the presence of foreign ions was proposed. The method is based on the measurement of polarogram in the sulfuric acid as supporting electrolyte. In this medium Uranyl ions give well defined reduction waves, and half-wave potentials are -0.19V vs. S.C.E. as first wave, and uncertain volt. vs. S.C.E. as second wave in $2.4 N-H_2SO_4.$ The first wave has a linear relationship between the concentration of Uranyl ion and wave height. The author also studied a method for rapid determination of Uranium in Korean monazite sand without eliminating the foreign ions. The Korean monazite sands were analyzed by this method and satisfactory result were obtained.

EULER METHOD VS. GESS METHOD FOR DYNAMICAL SYSTEMS

  • In this paper we introduce GESS method and show that dynamics of the system y'=A(s,t,y) y is more faithfully approxi-mated by GESS method that by Euler method. Numerical experiments are given for the comparison of GESS method with Euler method.
