  • Introduction: Facial fractures (FFs) occur after high- and low-energy trauma; differences in associated injuries and outcomes have not been well articulated. Objective: To compare the epidemiology, management, and outcomes of patients suffering FFs from high-energy and low-energy mechanisms. Methods: We conducted a 6-year retrospective local trauma registry analysis of adults aged 18-55 years old that suffered a FF treated at the Santa Barbara Cottage Hospital. Fracture patterns, concomitant injuries, procedures, and outcomes were compared between patients that suffered a high-energy mechanism (HEM: motor vehicle crash, bicycle crash, auto versus pedestrian, falls from height > 20 feet) and those that suffered a low-energy mechanism (LEM: assault, ground-level falls) of injury. Results: FFs occurred in 123 patients, 25 from an HEM and 98 from an LEM. Rates of Le Fort (HEM 12% vs. LEM 3%, P = 0.10), mandible (HEM 20% vs. LEM 38%, P = 0.11), midface (HEM 84% vs. LEM 67%, P = 0.14), and upper face (HEM 24% vs. LEM 13%, P = 0.217) fractures did not significantly differ between the HEM and LEM groups, nor did facial operative rates (HEM 28% vs. LEM 40%, P = 0.36). FFs after an HEM event were associated with increased Injury Severity Scores (HEM 16.8 vs. LEM 7.5, P <0.001), ICU admittance (HEM 60% vs. LEM 13.3%, P <0.001), intracranial hemorrhage (ICH) (HEM 52% vs. LEM 15%, P <0.001), cervical spine fractures (HEM 12% vs. LEM 0%, P = 0.008), truncal/lower extremity injuries (HEM 60% vs. LEM 6%, P <0.001), neurosurgical procedures for the management of ICH (HEM 54% vs. LEM 36%, P = 0.003), and decreased Glasgow Coma Score on arrival (HEM 11.7 vs. LEM 14.2, P <0.001). Conclusion: FFs after HEM events were associated with severe and multifocal injuries. FFs after LEM events were associated with ICH, concussions, and cervical spine fractures. Mechanism-based screening strategies will allow for the appropriate detection and management of injuries that occur concomitant to FFs. Type of study: Retrospective cohort study. Level of evidence: Level III.

  • Although it is agreed that the gamma-ray bursts (GRBs) invoke highly relativistic jets with bulk Lorentz factors of a few hundreds, the exact physical mechanism producing such powerful gamma-rays still remains debated. Three outstanding and important questions in the field concern (1) the composition of GRB jets (i.e., matter-dominated vs Poynting-flux-dominated), (2) the involved radiative process responsible for the observed gamma-rays (i.e., synchrotron mechanism vs photospheric radiation), and (3) the distance of the emitting region from the central engine where the prompt gamma-rays are released (i.e., ~10^12 cm vs 10^14 cm vs 10^16 cm). I will present recent important breakthroughs that we have made, which answer these three questions.

  • TCP, which was developed on the basis of wired links, supposes that packet losses are caused by network congestion. In a wireless network, however, packet losses due to data corruption occur frequently. Since TCP does not distinguish loss types, it applies its congestion control mechanism to non-congestion losses as well as congestion losses. As a result, the throughput of TCP is degraded. To solve this problem of TCP over wireless links, previous researches, such as split-connection and end-to-end schemes, tried to distinguish the loss types and applied the congestion control to only congestion losses; yet they do nothing for non-congestion losses. We propose a novel transport protocol for wireless networks. The protocol called VS-TCP (Variable Segment size Transmission Control Protocol) has a reaction mechanism for a non-congestion loss. VS-TCP varies a segment size according to a non-congestion loss rate, and therefore enhances the performance. If packet losses due to data corruption occur frequently, VS-TCP decreases a segment size in order to reduce both the retransmission overhead and packet corruption probability. If packets are rarely lost, it increases the size so as to lower the header overhead. Via simulations, we compared VS-TCP and other schemes. Our results show that the segment-size variation mechanism of VS-TCP achieves a substantial performance enhancement.

  • Objectives: Driven by a growth of single-person households and individualized lifestyles, solo dining in restaurants is an increasingly recognizable trend. However, a research gap exists in the comparison of solo and group diners' menu-decision making processes. Based on the self-control dilemma and the temporal construal theory as a theoretical framework, this study compared the ordering intentions of solo vs. group diners with healthy vs. indulgent (less healthy) entrées. The mediating role of consumption orientation and the moderating role of amount of menu nutrition information were further explored to understand the mechanism and a boundary condition. Methods: A scenario-based online survey was developed using a 2 (dining social context: solo vs. with others) × 3 (amount of menu nutrition information: no nutrition information vs. calories vs. calories/fat/sodium), between-subjects, experimental design. Consumers' level of nutrition involvement was controlled. A nationwide survey data (n = 224) were collected from a crowdsourcing platform in the U.S. Data were analyzed using multivariate analysis of covariance, independent t-test, univariate analysis of covariance, and moderated mediation analyses. Results: Findings reveal that solo (vs. group) diners have less (vs. more) intentions to order indulgent menu items due to a more utilitarian (vs. more hedonic) consumption orientation in restaurant dining. Findings also show that solo (vs. group) diners have more (vs. less) intentions to order healthy menu items when the restaurant menu presented nutrition information including calories, fat, and sodium. Conclusions: The findings contribute to the literature of foodservice management, healthy eating, and consumer behavior by revealing a mechanism and an external stimuli of solo vs. group diners' healthy menu-decision making process in restaurants. Furthermore, the findings provide restauranteurs and health professionals with insights into the positive and negative impacts of menu nutrition labelling on consumers' menu-decisions.

  • Polygala tenuifolia (PT) is one of the most well-known traditional herbal medicines in Korea which is commonly used for the treatment of cardiovascular symptoms. The anti-ischemic effects of PT in isolated rat heart was investigated by analyzing changes in blood pressure, aortic flow, coronary flow, and cardiac output. And, its underlying mechanism was examined by quantitating intracellular calcium content in rat neonatal cardiomyocytes. Rats were divided into two groups: an ischemia-induced group without any treatment, and an ischemia-induced group treated with PT. Ischemia of isolated heart was induced by stopping the supply of oxygen and buffer for 10 min. The isolated heart was exposed to PT for the first 5 min of 10 min ischemia. PT treatment significantly prevented the decreases of perfusion pressure, aortic flow, coronary flow, and cardiac output under ischemic conditions. In addition, hemodynamics (except heart rate) of the PT-treated group was significantly recovered 60 min after reperfusion compared to the control group (systolic aortic pressure: 83.3% vs. 64.9%, aortic flow volume: 69.5% vs. 48.7%, coronary flow volume: 77.7% vs. 58.4%, and cardiac output: 71.6% vs. 51.2%, p < 0.01). As for the underlying mechanism, PT significantly prevented intracellular calcium increase which was induced by isoproterenol (p < 0.01), suggesting that the anti-ischemic effect of PT is mediated by inhibition of intracellular calcium increase.

  • A kinetic study is reported for nucleophilic substitution reactions of benzyl 4-pyridyl carbonate 6 with a series of alicyclic secondary amines in MeCN. The plot of pseudo-first-order rate constant ($k_{obsd}$) vs. [amine] curves upward, which is typical for reactions reported previously to proceed through a stepwise mechanism with two intermediates (i.e., a zwitterionic tetrahedral intermediate $T^{\pm}$ and its deprotonated form $T^-$). Dissection of $k_{obsd}$ into the second- and third-order rate constants (i.e., $Kk_2$ and $Kk_3$, respectively) reveals that $Kk_3$ is significantly larger than $Kk_2$, indicating that the reactions proceed mainly through the deprotonation pathway (i.e., the $k_3$ process) in a high [amine] region. This contrasts to the recent report that the corresponding aminolysis of benzyl 2-pyridyl carbonate 5 proceeds through a forced concerted mechanism. An intramolecular H-bonding interaction was suggested to force the reactions of 5 to proceed through a concerted mechanism, since it could accelerate the rate of leaving-group expulsion (i.e., an increase in $k_2$). However, such H-bonding interaction, which could increase $k_2$, is structurally impossible for the reactions of 6. Thus, presence or absence of an intramolecular H-bonding interaction has been suggested to be responsible for the contrasting reaction mechanisms (i.e., a forced concerted mechanism for the reaction of 5 vs. a stepwise mechanism with $T^{\pm}$ and $T^-$ as intermediates for that of 6).

  • Kinetic studies of the reactions of O,O-dimethyl Z-S-aryl phosphorothioates with X-benzylamines have been carried out in dimethyl sulfoxide at $85.0^{\circ}C$. The Hammett (log $k_2$ vs ${\sigma}_X$) and Br$\ddot{o}$nsted [log $k_2$ vs $pK_a$(X)] plots for substituent X variations in the nucleophiles are discrete with a break region between 4-Me and H, while the Hammett plots (log $k_2$ vs ${\sigma}_Z$) for substituent Z variations in the leaving groups are linear. The sign of the cross-interaction constant (${\rho}_{XZ}$) is positive for both the strongly and weakly basic nucleophiles. Greater magnitude of ${\rho}_{XZ}$ (= 2.54) value is observed with the weakly basic nucleophiles compared to with the strongly basic nucleophiles (${\rho}_{XZ}$ = 0.17). The deuterium kinetic isotope effects ($k_H/k_D$) involving deuterated benzylamines [$XC_6H_4CH_2ND_2$] are primary normal ($k_H/k_D$ > 1). The proposed mechanism is a stepwise with a rate-limiting leaving group expulsion from the intermediate involving a frontside nucleophilic attack with a hydrogen bonded, four-center-type transition state for both the strongly and weakly basic nucleophiles.
