Dose estimation of cone-beam computed tomography in children using personal computer-based Monte Carlo software (PCXMC 소프트웨어를 이용한 소아에서의 CBCT 환자선량 평가)

  • Objective: The purpose of the study was to calculate the effective and absorbed organ doses of cone-beam computed tomography (CBCT) in pediatric patient using personal computer-based Monte Carlo (PCXMC) software and to compare them with those measured using thermoluminescent dosimeters (TLDs) and anthropomorphic phantom. Materials and Methods: Alphard VEGA CBCT scanner was used for this study. A large field of view (FOV) (20.0 cm × 17.9 cm) was selected because it is a commonly used FOV for orthodontic analyses in pediatric patients. Ionization chamber of dose-area product (DAP) meter was located at the tube side of CBCT scanner. With the clinical exposure settings for a 10-year-old patient, DAP value was measured at the scout and main projection of CBCT. Effective and absorbed organ doses of CBCT at scout and main projection were calculated using PCXMC and PCXMCRotation software respectively. Effective dose and absorbed organ doses were compared with those obtained by TLDs and a 10-year-old child anthropomorphic phantom at the same exposure settings. Results: The effective dose of CBCT calculated by PCXMC software was 292.6 μSv, and that measured using TLD and anthropomorphic phantom was 292.5 μSv. The absorbed doses at the organs largely contributing to effective dose showed the small differences between two methods within the range from -18% to 20%. Conclusion: PCXMC software might be used as an alternative to the TLD measurement method for the effective and absorbed organ dose estimation in CBCT of large FOV in pediatric patients.

  • Recently, the bandwidth available to an end user has been dramatically increasing with the advancing of network technologies. This high-speed network naturally requires faster and/or stable data transmission techniques. The UDT(UDP based Data Transfer protocol) is a UDP based transport protocol, and shows more efficient throughput than TCP in the long RTT environment, with benefit of rate control for a SYN time. With a NAK event, however, it is difficult to expect an optimum performance due to the increase of fixed sendInterval and the flow control based on the previous RTT. This paper proposes a rate control method on following a NAK, by adjusting the sendInterval according to some degree of RTT period which calculated from a set of experimental results. In addition, it suggests an improved flow control method based on the TCP vegas, in order to predict the network congestion afterward. An experimental results show that the revised flow control method improves UDT's throughput about 20Mbps. With combining the rate control and flow control proposed, the UDT throughput can be improved up to 26Mbps in average.

  • Objectives : The 7-zone-diagnostic system is a diagnostic device to predetermine bodily locations by measuring the energy of body. This study was to investigate the relation between the different patterns of Zone 1, 2, 3 of Factor AA in CP-6000A(VEGA, Germany), 7-zone-diagnostic system and clinical parameters. The purpose of this study was relation Korean traditional medicine and western medicine with the data from 7-zone-diagnostic system and the clinical parameters. Methods : This study was carried out with the data from some clinical parameters. We made three groups according to the Factor AA patterns of CP-6000A. The Factor AA pattern of Group A is that the red bar graph of zone 1, 2, 3 were higher than the normal range and the others were the normal range. The Factor AA pattern of Group B was that the red bar graph of zone 1, 2, 3 was the normal range and the others were the normal range. The Factor AA pattern of Group C was that the red bar graph of zone 1, 2, 3 was lower than the normal range and the others were the normal range. After the data from clinical parameters to correspond with conditions of each group were selected, the data from clinical parameters among each groups analyzed statistically. Results : The values of GOT, GPT, r-GPT, Triglyceride, BUN, Uric acid of group A was higher than group C. Gastroscope of group A and B was higher than group C. Conclusions : It is thought that the red bar graph of zone 1, 2, 3 is higher, the group has the higher energy and the energy has a character of fire(熱). Those patterns have a high risk of hyperlipermia and liver, stomach disease.

The Relationships of Plasma Leptin, Backfat Thickness and TDN Intake across Finishing Stage of Holstein Steers

  • Six 16 months old Holstein steers were offered ad libitum feed for 7 months, to determine the (1) relationships of backfat thickness (BFT) to plasma leptin, and insulin; and (2) associations of TDN intake/kg body weight (BW) to plasma leptin, BFT and insulin. Feed intake, body weight and BFT were measured on selected monthly ages from day 1 to 8, day 1 and 8, and day 8, respectively. Blood was sampled on day 8 and the plasma was analyzed for leptin, insulin, glucose, NEFA, total cholesterol and triglyceride. Body weight and BFT increased, while TDN intake per kg BW decreased from 16 to 23 months old. Plasma leptin increased and mimicked the level of insulin, resulting to significant correlation (r=0.54; p<0.002). TDN intake was negatively related to plasma leptin (r=0.49; p<0.004), insulin (r=0.41; p<0.02) and BFT at 12 to 13th rib (r=0.48; p<0.005). Backfat thickness at 12 to 13th rib was positively related to plasma leptin (r=0.45; p<0.01). Negative associations of TDN intake with plasma leptin and BFT during finishing period suggest long-term involvement of adipose tissues in the feed intake regulation of steers fed high concentrate diet.

Clinical experience of open heart surgery -70 cases- (개심술 70예의 임상경험)

  • Seventy cases of open heart surgery were performed in the department of Thoracic and Cardiovascular Surgery, Pusan Paik Hospital, Inje College, from Oct. 1985 to Oct. 1986. And the results were summarized as follows. 1. Among the 70 cases, there were 48 cases of congenital heart anomalies and 22 cases of acquired rheumatic valvular heart diseases. Age range of the congenital patients was 7 months to 31 years with the mean age of 10 years, and the acquired patients was 18 to 62 years with the mean age of 40 years. 2. The heart-lung machine used for cardiopulmonary bypass was Sarns 7000, 5-head roller pump, and the number and type of oxygenators were 5 of membrane type and 65 of bubble type. For all cases GIK [glucose-insulin-potassium] solution was used as cardioplegic solution for myocardial protection during operation. 3. Among the 48 congenital anomalies, there were 12 cases of ASD group, 29 of VSD group, 3 of ECD, 3 of TOF and one of PDA + MR, and to all of which the appropriate radical operations were applied. 4. Among the 22 acquired valvular diseases, there were 11 cases of mitral valve diseases [MS; 4, MSr; 3, MRs; 4], 3 cases of aortic valve diseases [AR:1, ARs;1, ASr;1], 4 cases of double valve diseases [MRs+TR; 3, MRs+ARs; 1] and 4 cases of triple valve diseases [MSr+ASr+TR; 3, MSr+Ar+TR; 1]. To all the diseased mitral and aortic valves, artificial valve replacement was applied except one [As], in which valve plication was applied. And to all the diseased tricuspid valve, DeVega annuloplasty was applied. 5. The number of replaced artificial valves were 29 in 25 patients [congenital; 3, acquire; 22]. In MVR, 6 of mechanical valves [St. Jude Medical valve; 6] and 15 of tissue valves [Carpentier-Edward valve; 11, lonescu-Shiley valve; 4] were used. In AVR, 6 of mechanical valves [St. Jude Medical valve; 6] and 2 of tissue valves [Carpentier-Edward valve; 2] were used. 6. Postoperative complications were occurred in 12 cases. Among them 11 cases were recovered with intensive cares, but one patient [VSD + Fistula of Valsalva sinus] was expired with low cardiac out put syndrome.

Molecular Behavior of $SF_6+H_2$ Structure II Hydrates (sII $SF_6+H_2$ 하이드레이트의 분자 거동)

  • Sulfur hexafluoride ($SF_6$), one of the most potent greenhouse gases, is known as a hydrate former and has been studied at the high pressure up to 1.3 GPa with gas mixtures and with aqueous surfactant. Since we regard $SF_6$ as a potential promoter molecule that can stabilize hydrate structure more effectively compare to the other promoters, further investigation is required to verify the stabilizing ability of $SF_6$ in the hydrate structure. However, the insoluble nature of $SF_6$ in water or gases hinders fine scale analyses. This work discusses the data obtained by using molecular dynamics simulations of structure II (sII) clathrate hydrates containing $SF_6$ and $H_2$. The simulations were performed using the TIP4P/Ice model for water molecule and a previously reported $SF_6$ molecular model (optimized at the pure $SF_6$ single phase system (Olivet and Vega, 2007)), and a $H_2$ molecular model (adapted from the THF+$H_2$ hydrate system (Alavi et al., 2006)). The simulations are performed to observe the stability of $SF_6$ and $H_2$ in the sII clathrate hydrate system with varying temperature and pressure conditions and occupancies of $SF_6$ and $H_2$, which cannot be easily tuned experimentally. We observe that stability of H2 enclathrated in the hydrate structure more affected by the occupancy of $SF_6$ molecules and temperature than pressure, which ranges from 1 to 100 bar.

Impact of Cassia acutifolia Infusion on Glucose Levels in Obesity and Diabetes Rat Model

  • Objectives: The aim of this study was to investigate the effects of Cassia acutifolia on the obesity and the glucose levels in a rat model of obesity and diabetes. Methods: By random selection, 36 Wistar male rats were divided in two control groups, the positive and the negative control groups, and into four experimental groups receiving different infusions of Cassia acutifolia in water ad libitum. Results: The results revealed a statistically significant anti-obesogenic effect (P = 0.02), although this was not considered clinically significant. Additionally, Cassia acutifolia lowered the glucose levels by 30 mg/dL to 90 mg/dL (P = 0.05). However, we observed adverse effects in the liver, a two-fold increase in transaminase levels (P = 0.002), and in the kidneys, decreased creatinine levels (P = 0.001), and these adverse effects had no viable explanation. Conclusion: Cassia acutifolia has anti-hyperglycemic effects in obese diabetic rats. However, Cassia acutifolia also has adverse effects, so it should not be administered to patients.

Comparing the Performance of TCP Algorithms (TCP 알고리즘의 성능 비교)

  • TCP has improved by many papers which suggest the new algorithms and modify the previous algorithms. This paper compares Tahoe, Reno, New-Reno, Vegas, and SACK. The first version is Tahoe and is globally used. Reno has optimal performance during occurring one packet loss within a window of data, but can suffer from performance when multiple packets are dropped from a window of data. New-Reno avoids some of the performance problems of Reno TCP when multiple packets are dropped from a window of data. but is occurring the problem of the necessary retransmission. SACK resolves the all above problems and is used in bandwidth delay product environment. Vegas uses network bandwidth more efficiently and is a new implementation of TCP that achieves between 40 and 70 better throughput, with one-fifth to one-half the losses, as compared to the implementation of Reno TCP.

Evaluation of Microbial Load in Oropharyngeal Mucosa from Tannery Workers

  • Background: Animal skin provides an ideal medium for the propagation of microorganisms and it is used like raw material in the tannery and footware industry. The aim of this study was to evaluate and identify the microbial load in oropharyngeal mucosa of tannery employees. Methods: The health risk was estimated based on the identification of microorganisms found in the oropharyngeal mucosa samples. The study was conducted in a tanners group and a control group. Samples were taken from oropharyngeal mucosa and inoculated on plates with selective medium. In the samples, bacteria were identified by 16S ribosomal DNA analysis and the yeasts through a presumptive method. In addition, the sensitivity of these microorganisms to antibiotics/antifungals was evaluated. Results: The identified bacteria belonged to the families Enterobacteriaceae, Pseudomonadaceae, Neisseriaceae, Alcaligenaceae, Moraxellaceae, and Xanthomonadaceae, of which some species are considered as pathogenic or opportunistic microorganisms; these bacteria were not present in the control group. Forty-two percent of bacteria identified in the tanners group are correlated with respiratory diseases. Yeasts were also identified, including the following species: Candida glabrata, Candida tropicalis, Candida albicans, and Candida krusei. Regarding the sensitivity test of bacteria identified in the tanners group, 90% showed sensitivity to piperacillin/tazobactam, 87% showed sensitivity to ticarcillin/clavulanic acid, 74% showed sensitivity to ampicillin/sulbactam, and 58% showed sensitivity to amoxicillin/clavulanic acid. Conclusion: Several of the bacteria and yeast identified in the oropharyngeal mucosa of tanners have been correlated with infections in humans and have already been reported as airborne microorganisms in this working environment, representing a health risk for workers.

A SCANNING ELECTRON MICROSCOPIC STUDY OF HUMAN DENTIN SURFACE WITH THE CO2 LASER IRRADIATION (CO2레이저 조사후 상아질 조직변화에 대한 주사전자혐기경적 연구)

  • The main purpose of this study was to investigate the possibility of obstruction of dentinal tubules through irradiation of the $CO_2$ laser on the dentin surface. For this study, 104 extracted caries - free premolars and molars were grinded their mesial or distal surface to expose dentin completly. And these specimens were divided into three Groups. Group I : those with their smear layer not removed and Group IT : those with their smear layer removed, were irradiated by the $CO_2$ laser(SPACE LASER - $CO_2$ VEGA 25) at five different energy levels ranging 4 - 20J/$cm^2$, and all specimens were observed with scanning eletron microscope (Hitachi S - 450). Group m was divided into 2 sub - groups: one with smear layer, another one without and those were irradiated with $CO_2$ laser of the same condition as Group I & II. All specimens were immersed in, $37^{\circ}C$, 0.5% methylene blue solution for 48 hours after 100 times of thermocycling at $4^{\circ}C$ and $60^{\circ}C$, and were observed with stereo -light microscope (Olympus SZH - ILLD) to evaluate dye penetration. The following results were obtained; 1. The dentinal tubules were obstructed through laser irradiation. 2. The dentinal tubules were still obstructed after treatment of 10% HCl/2sec., then it was proved that acid resistance of detin surface was also increased. 3. The Group of laser irradiation showed less dye penetration than control Group. 4. The smear layer hardly affected obstruction of the dentinal tubules through laser irradiation.
