Intraocular Foreign Body Entering the Anterior Chamber Through the Mouth: A Case Report

  • An 11-year-old, castrated Maltese dog presented with a 3-week history of periocular swelling, epiphora, and intermittent strabismus. On examination, a foreign body was observed in the anterior chamber, along with orbital cellulitis. Severe gingivitis and plaque accumulation were also diagnosed. The foreign body was surgically removed, and dental prophylaxis and dental extraction were performed. The foreign body entrance could not be found intraoperatively, and the foreign body, later identified as a feather, was removed through a clear corneal incision. The right maxillary molar, which had periodontal inflammation, was also extracted. One day postoperatively, severe hypopyon developed, although the periocular swelling was reduced. These signs persisted despite topical and systemic antibiotic and anti-inflammatory therapy; therefore, the right eye was enucleated 1 week later. Intraoperatively, a fistula was found connecting the orbital medial wall, right maxillary molar root, and sclera. The fistula entered the dorsomedial sclera approximately 7 mm behind the limbus. Enterobacteria were cultured from the area. Foreign bodies can enter the anterior chamber not only through the cornea, but also through the mouth. Therefore, when the entry point cannot be found in the cornea, a careful dental examination is required, and the foreign body must be removed through the sclera rather than the cornea.

베체트병의 동물모형에 대한 사심탕류 투여 효과에 관한 연구 (Experimental Studies on the Kinds of Sasim-tang In Behcet's Disease Symptoms in ICR Mice)

  • Chronic oral aphthae, recurrent ulcer and uveitis are the three main festations of Behcet's disease(BD). The aetiopathogenesis of Behcet's disease is still obscure, but herpes simplex virus is one of the possible casual factors. Gamchosasim-tang (Gancaoxiexin-tang), Banhasasim-tang(Banxiaxiexin-tang) and Saenggangsasim-tang( Shengjiangxiexin-tang) are traditional medication in Oriental medicine, that has been used to treat inflammatory disease. Especially, Gamchosasim-tang used to treat Behcet's disease like symptoms. ICR mice were used for this study. The earlobe of the mice were scratched with a needle, then inoculation with 1.0×10/sup 6/ plaque forming units/㎖ of HSV type I. Virus inoculation was performed twice with 10 day interval, followed by 16 weeks of observation. Using the HSV-induced Behcet's disease mouse model, kinds of Sasim-tang were administered variously before and after inoculation. In order to. classify the symptomatic mice as having Behcet's disease like symptoms. We followed the revised Japanese classification with minor modifications. Ulceration of the mice were monitored. In addition, spleen cytokine expression were measured by polymerase chain reaction, ELISA. HSV DNA was detected in HSV inoculation mice. HSV-induced mice treated with kinds of Sasim-tang showed improvement in symptom. In RT-PCR results, IFN-γ was expressed for all groups, IL-2 was expressed for the treated groups, and IL-10 was also expressed. IL-4 was expressed nothing. In ELISA, IL-2 was increased for GSST 2, BSST 2, GSST 2, GSST3 and INF-γ was increased for GSST 2, BSST 2, SSST 2, SSST 3. This model suggest the possible role of immune response to viral infection in the development and activation of Behcet's disease.

Clinical Features and Extraintestinal Manifestations of Crohn Disease in Children

  • Purpose: The aim of this study was to investigate the clinical features and extraintestinal manifestations (EIMs) of Crohn disease (CD) in Korean pediatric patients. Methods: The medical records of 73 children diagnosed with CD were retrospectively reviewed. Data regarding baseline demographic and clinical characteristics, including CD phenotype at diagnosis based on the Montreal classification, and clinical features and course of EIMs were investigated. Results: Fifty-two (71.2%) of the patients were males. The mean age of the patients was 12.5 years. The mean follow-up period was 3.4 years. The disease location was ileal in 3 (4.1%) of the patients, colonic in 13 (17.8%), ileocolonic in 56 (76.7%). The clinical behavior was inflammatory in 62 (84.9%) of the patients, stricturing in 8 (11.0%), and penetrating in 3 (4.1%). Perianal abscesses or fistulas were found in 37 (50.7%) of the patients. EIMs observed during the study period were anal skin tag in 25 patients (34.2%), hypertransaminasemia in 20 (27.4%), peripheral arthritis in 2 (2.7%), erythema nodosum in 2 (2.7%), vulvitis in 1 (1.4%), uveitis in 1 (1.4%), and pulmonary thromboembolism in 1 (1.4%). Conclusion: Perianal diseases and manifestations were present in more than half of Korean pediatric CD patients at diagnosis. Inspection of the anus should be mandatory in Korean children with suspicious CD, as perianal fistulas, abscesses, and anal skin tags may be the first clue to the diagnosis of CD.

The Effect of Bojungykgitang-Chunbang on Activity of CD4+ T cell

  • BJYGC is often clinically used as a treatment of allergic rhinitis. This study was aimed to find out the effect BJYGC would have on the helper T cell, and how it can promote the subsets of helper T cells to regain their balance that they lost due to immunological diseases. Splenocytes were prepared from BALB/c mice was cultured without stimulation in the presence of BJYGC for 48 hr. The viability of CD4 T cells from Balb/c mouse were measured at various concentrations of BJYGC using the MTS assay. It was somewhat increased up to concentration of 400 ㎍/ml, but did not show any significant difference. Proliferation was measured using the MTS assay, CD4 Th cells were stimulated with anti-CD3/28 in the presence of BJYGC for 48 hr. As evidence for rapid T cell activation, CD25 expression by flow cytometry was evaluated at 10, 50, 100 and 200 ㎍/㎖ of BJYGC. Th cell differentiation experiments were performed to examine whether BJYGC can affect the Th polarization process. CD4 T cells were activated in culture under neutral, Th1-polarized or Th2-polarized conditions in the presence of BJYGC at 10, 100 and 200 ㎍/㎖. Cytokine production was measured by ELISA. This experiment proved that BJYGC could inhibit the secretion of both IL-4 and IFN-γ in neutral condition and polarized condition, too. Considering that BJYGC shows an excellent effect on treating allergies, the author can conclude that its pharmacological action may be associated with decreased IL-4 and, it may also regulate IFN-γ depending the host's need. Also, it was discovered that Th1 cell was pathologic in chronic inflammatory tissue specific diseases, such as insulin dependent diabetes mellitus, multiple sclerosis, RA, and uveitis. We are counting on the BJYGC to be able to control the tendency of Th1 cell predominancy in an immune reaction.

베체트병 환자에서 저용량 스테로이드 사용과 관련하여 발생한 중심성장액맥락망막병증 (Central serous chorioretinopathy associated with low dose systemic corticosteroid treatment of Behcet's disease)

  • Central serous chorioretinopathy may induce poor eyesight and serous retinal detachment. However, its exact cause has not been well established thus far. It can be associated with systemic high-dose corticosteroid treatment mainly for young and middle-aged men and may spontaneously regress or recur after withdrawal from corticosteroid. After corticosteroid administration for Behcet's disease, it is necessary to identify any ocular symptoms. Behcet's disease can lead to the development of ocular complications, such as uveitis, hypopyon, retinal vasculitis, optic neuritis, angiogenesis, secondary cataract, and glaucoma. It is possible to diagnose any of these complications via optical coherence tomography and digital indocyanine green angiography. It is easy to neglect an ocular symptom that may appear after a low-dose corticosteroid treatment as an ocular complication in patients with Behcet's disease. Thus, we report on a case concerning high-dose corticosteroid treatment with a literature review.

결막 충혈도 측정을 위한 공막 영상 분할 (Sclera Segmentation for the Measurement of Conjunctival Injection)

  • 결막 충혈은 결막염, 각막염, 포도막염 등의 안과질환의 초기 증세로서 정량적으로 평가할 수 있다면 진단과 경과 관찰에 도움이 된다. 충혈의 정량화에서 공막의 크기는 중요한 지표이지만 기존의 공막 분할 방법이 정확하지 않기 때문에 수동으로 분할하고 있다. 본 논문에서는 충혈의 정량화를 위하여 level set 방법을 이용한 공막 분할 알고리즘을 제안한다. Level set의 초기 모델은 Lab 색상 모드와 k-means 알고리즘, 기하학적인 정보를 이용하여 지정된다. 헤이시안(hessian) 분석으로 공막과 피부 사이의 골을 향상시킨 영상에 level set을 적용하였다. 제안 방법의 성능 측정을 위하여 52개의 전안부 영상에 대하여 실험하였다. 실험 결과, 제안 방법이 화소값만 이용하는 region growing이나 level set의 초기 모델로 임의의 원을 이용하는 방법보다 성능이 우수하였다. 이 논문에서 제안한 공막 분할 방법은 객관적인 충혈도 측정에서 중요한 요소 기술의 역할을 할 것이다.

비전리방사선의 생물학적 작용 (Biologic Effect of Non-ionizing Radiation)

  • 비전리 방사선의 인체에 대한 작용은 최근 우리 주위에서 많이 사용되는 microwave, radio wave, laser, ultrasoundwave, ultravioletradiation 등의 문제가 될 수 있으며 본고에서는 주로 Radio Frequency Electromagnetic (RFEM) spectrum의 shortwave와 microwave의 생물학적인 작용을 정리 발표하고자 한다. RFEM에 의한 carcinogenesis에 관해서는 아직 RFEM이 인체에서는 발암물질로서 작용한다는 보고는 없으나, cancer promotion에는 작용할 것이라는 보고는 있다. chromosomal effect에 관해서는 체외실험에서 microwave에 의한 chromosome의 stickiness의 증가와 mitotic index의 변화에 관한 보고가 있다. 인체에서는 RFEM에 많이 피폭되었다고 알려진 Moscow의 미 대사관 직원 5000명을 대상으로 한 역학조사에서는 특별한 염색채 이상은 발견되지 않았다. 동구에서의 보고에 의하면 RFEM에 의한 선천성 기형아의 출산이 보고된바 있으나 출산시의 통증을 줄이기 위한 2450 MHz의 microwave를 조사한 산모 2000명 출산아에서 기형은 없었다고 한다. 인간의 행동과 정서적인 측면에서 RFEM에 의한 변화는 1974년 Sadcikoda에 의해 기술된바 있는 µwave sickness&는 irritability, 두통, 전신쇄약, 불면증, 성욕 감퇴 및 발기부전 등을 호소하는 증후군으로 주로 동유럽의 저자들에 의하여 보고되고 있으며 구 소련에서는 microwave sickness 또는 radio wave sickness는 확실한 하나의 임상병명으로 간주되고 있다. cataractogenesis에 관해서는 실험 동물에서 잘 연구되고 있으며, 사람에서도 1952년 레이다 작업종사자 에서 보고된 이후 50여례 이상의 보고가 있으나 백내장 자체가 그 발생 빈도가 높은 질환이고 많은 원인이 있으므로 아직 인간에서의 확실한 cataractogenesis에 관해서는 더욱 연구가 필요한 분야이다. radar wave에 노출된 적이 있는 군인을 대상으로 한 미 해군의 역학조사에 의하여 radar wave에 의한 uveitis, retinitis와 비진행성 백내장등이 보고되고 있으나 딴 연구에서는 입증된바 없다.

  • 저자들은 31세 남자환자에서 고열, 흉막성 흉통 및 흉막 유출액의 임상증상을 보이며 검사상 다발성 혈관염 소견을 보였으며, 결핵성 흉막염 및 불명열로 오인되었던 베체트병을 부신피질 호르몬제 및 colchicine, 비스테로이드성 항염증약물을 이용하여 치료한 1예를 경험하였기에 이에 보고하는 바이다.

Multilevel Precision-Based Rational Design of Chemical Inhibitors Targeting the Hydrophobic Cleft of Toxoplasma gondii Apical Membrane Antigen 1 (AMA1)

  • Toxoplasma gondii is an intracellular Apicomplexan parasite and a causative agent of toxoplasmosis in human. It causes encephalitis, uveitis, chorioretinitis, and congenital infection. T. gondii invades the host cell by forming a moving junction (MJ) complex. This complex formation is initiated by intermolecular interactions between the two secretory parasitic proteins-namely, apical membrane antigen 1 (AMA1) and rhoptry neck protein 2 (RON2) and is critically essential for the host invasion process. By this study, we propose two potential leads, NSC95522 and NSC179676 that can efficiently target the AMA1 hydrophobic cleft, which is a hotspot for targeting MJ complex formation. The proposed leads are the result of an exhaustive conformational search-based virtual screen with multilevel precision scoring of the docking affinities. These two compounds surpassed all the precision levels of docking and also the stringent post docking and cumulative molecular dynamics evaluations. Moreover, the backbone flexibility of hotspot residues in the hydrophobic cleft, which has been previously reported to be essential for accommodative binding of RON2 to AMA1, was also highly perturbed by these compounds. Furthermore, binding free energy calculations of these two compounds also revealed a significant affinity to AMA1. Machine learning approaches also predicted these two compounds to possess more relevant activities. Hence, these two leads, NSC95522 and NSC179676, may prove to be potential inhibitors targeting AMA1-RON2 complex formation towards combating toxoplasmosis.
