  • In the past it was thought that autoimmunity is mediated by antibodies and immune complexes. It has now become clear that many diseases, especially tissue specific, are T cell mediated or at least T cell dependent. The pathogenesis of cell-mediated autoimmune diseases, such as multiple sclerosis, uveitis, diabetes, arthritis, and others, is thought to be in a large measure driven by interferon-gamma-producing antigen-specific T cells polarized toward the Th1 phenotype. Toothache associated with ankylosing spondylitis

  • Ankylosing spondylitis (AS) is a chronic inflammatory joint disorder characterized by sacroiliitis, spondylitis and enthesitis. Patients suffering from AS may also have extra-articular symptoms, such as uveitis, bowl disease, heart, lung, skin, bone and kidney involvement, but vary widely in severity and prevalence. Facial manifestation of AS include eye involvement and temporomandibular joint involvement. In this study, a case of an AS that mimicked dental pain was presented.

  • These studies were performed to investigate the complications affecting the vision after extracapsular lens extraction(ECE), the effects of an I/A (irrigation & aspiration)device and a viscoelastic material used on the vision, the occurrence of complications and the effective corneal incision method to reduce the corneal opacity in dogs. ECE was performed bilaterally with 3 different methods using clinically normal twele mixed dogs; the method in which I/A device and viscoelastic material were not used, the method in which I/A device was used but viscoelastic material not, and the method in which I/A device and viscoelastic material were used. Postoperative complications were observed as followed; conjunctival injection, uveitis, corneal opacity due to endothelial cell loss, hyphemia, remnants of lens cortex, vitreous loss, synechia and capsular opacity. Preservation rate of vision was lower significantly in the cases showing signs of synechia, capsular opacity, or remnants of lens cortex than the cases not showing the above signs(p<0.01). There were significant reduction of the complications such as corneal opacity, clot in anterior chamber in the group using I/A device compared to the group in which I/A device was not used(p<0.01). Groups using I/A device showed slightly higher vision than the group not using I/A device (75%; 42%). There were no significant differences in the occurrence rate of complications and the preservative rate of vision between the groups with and without viscoelastic material. The present study indicated that the postoperative complications of posterior synechia, capsular opacity, uveitis and vistreous loss were important factors affecting the vision and that I/A device was applicable to extract the lens cortex and effective to elevate the success rate after ECE in dogs.

  • Infliximab, a TNF-${\alpha}$ antagonist, has been used to treat refractory rheumatoid arthritis, ankylosing spondylitis, Crohn's disease and Behcet's disease. Tuberculosis (TB) is a well-known opportunistic infection in patients receiving infliximab. Therefore, patients should be screened and treated for latent or active TB infection before being administered infliximab. Recently, we encountered a case of military TB during infliximab therapy in a patient suffering from Behcet's disease and uveitis. We report this case with a review of the relevant literature.

  • 블라우증후군(Blau syndrome)은 nucleotide-binding oligomerization domain protein 2 (NOD2) 유전자의변이에 의해 발생하는 질환으로 육아종성 피부염 및 만성 포도막염, 관절염을 특징으로 한다. 증상이 비특이적이고 동시다발적으로 발생하지 않아 진단이 어려운 경우가 많다. 반복되는 피부 발진 및 발열에 대해 두 차례 가와사키병으로 오진되어 면역글로불린과 전신 스테로이드로 치료받은 바 있는 13개월 남자 환자에서 블라우 증후군을 진단한 증례를 보고하고자 한다.

  • A 8 months old, female domestic Shorthair cat with long-term signalment of anorexia, lacrimation, uveitis and coughing was submitted to the Pathology and Diagnosis Reference Division, NVRQS, Korea, for necropsy. Main gross lesions were characterized by ascities, some grayish-white nodular formation and fibrous adhesion on the surface of visceral organs including liver and kidney. Principle histopathological findings were fibrinous serositis, multifocal granuloma and necrosis, vasculitis, perivasculitis in various pharenchymal organs. Paraffin-embedded tissue sections taken from most of organs with granulomatous lesions were confirmed specific reaction to the monoclonal antibody of feline infectious peritonitis virus in the cytoplasm of many infiltrating macrophages by immunohistochemistry. The report was to describe the pathological lesions of the first naturally-occuring FIP case in companion cat of Korea.

  • Behcet syndrome is a multisystem disease complex of unknown cause. It is usually manifested by aphthous oral and genital ulcers, uveitis and skin eruption. Less frequently, CNS involvement, colitis, large vessel vasculitis, and myocarditis occur. Recently, several studies have reported renal manifestations of amyloidosis and focal necrotizing glomerulonephritis in Behcet syndrome. We describe a patient with Behcet syndrome who experienced nephrotic syndrome with focal effacement of epithelial foot process in glomeruli. A brief review of literature ensues.

  • Vogt-Koyanagi-Harada(VKH) Disease is a bilateral granulomatous uveitis often associated with exudative retinal detachment and extraocular manifestations, such as pleocytosis in the cerebrospinal fluid and, in some case, vitiligo, poliosis, alopecia, and dysacusis. There has been no previous report for the disease in the field of Korean traditional medicine. The case was treated with conventional medical intervention and Korean traditional treatment including acupuncture and herbal medicine. The case was discharged with improvement in fundus photography and optical coherence tomography. The authors report the case to suggest that Korean medical approach combined with conventional treatment can be helpful for the disease.

Goniodysgenesis-associated glaucoma in a Jindo dog

  • A 10-year-old intact female Jindo dog was presented with a 1-week history of conjunctival redness and ocular discharge in the left eye. There was an absence of menace response, dazzle reflex, and direct pupillary light reflex. Slit-lamp biomicroscopy revealed corneal edema, ciliary flush, and aqueous flare. Intraocular pressure was 68 mmHg. Based on the information available, a diagnosis of glaucoma and uveitis was made. Subsequent histopathologic examination showed the glaucoma was produced by the effects of goniodysgenesis, posterior synechia, and pigment dispersion in the trabecular meshwork. This is the first report of primary glaucoma caused by goniodysgenesis in Jindo dogs.
