  • Life-threatening cardiac arrhythmia is a frequent complication of open heart surgery. There are many causes of postoperative cardiac arrhythmias. Electrolyte imbalance such as hypokalemia and acidemia are major causes of ventricular arrhythmias. Infrequently, however, antiarrhythmic agents and /or hypomagnesemia induce[s] a ventricular arrhythmia such as "torsade de pointes" by increasing the repolarization time of myocardium, Recently, we have experienced two cases of "Torsade de pointes" associated with hyp-omagnesemia after replacement of mitral valve and one of whom after use of procainamide. Intravenous infusion of magnesium immediately and successfully abolished the torsade de pointes in both cases.intes in both cases.

외안근 전침 자극을 이용한 마비성 사시 환자의 치험 3례 (Three cases of paralytic strabismus which were treated electra-acupuncture at extraocular muscles)

  • Objectives : Strabismus refers to the muscle imbalanced that results from improper alignment of the visual axes of the two eyes. It may be divided into paralytic and nonparalytic strabismus. Paralytic strabismus is primarily result from neurologic problem and nonparalytic strabismus is more strictly opthalmologic problem. We try to improve the remedial value of acute paralytic strabismus. Methods : We treated the cases by use of electra-acupuncture at paralytic extraocular muscles. The cases were treated almost daily and every treatment were enforced 10 minutes. We use the PG-306 electra-acupuncture products(Suzuki Iryoki Co. Japan) and apply the low consequence wave of 1-8Hz Results : We found that two cases were recovered almost three months and one case is treated till now and improved. So we try to apply this treatment in many cases and make the treating protocol of acute paralytic strabismus.

  • Many business forecasting problems are characterized by infrequent occurences, a large number of variables, presence of error, and great complexity. Because no forecasting models and tools are effective in handing these problems, managers often use the outcomes of past analogous cases to predict the outcome of the current one. They (1) observe significant attributes in describing a case, (2) identify the past cases similar in these attributes to the current case, and (3) predict the outcome of the current case based on those of the analogous cases identified through some mental simulation and adjustment. This process of forecasting can be termed forecasting-by-analogy. In spite of fairly frequent use of this forecasting process in practice, however, if has not been recognized as a primary forecasting tool, nor applied on a regular basis. In this paper, by automatizing this process using computer models, we develop a case-based forecasting system(CBFS), which identifies relevant cases and applies their outcomes to generate a forecast. We demonstrate the effectiveness of the CBFS in terms of its accuracy in predicting the outcome of the current problem based on the similar cases identified. We compare the forecasting accuracy of the CBFS with that of regression models developed by stepwise procedure under varied simulated problem conditions. The CBFS outperforms regression models in most comparisons. The CBFS could be used as an effective forecasting tool.

무기체계 소프트웨어의 모델 기반 테스트 케이스 생성 방법 (Model-based Test Cases Generation Method for Weapons System Software)

  • Test cases in the existing weapon system software were created manually by the tester analyzing the test items defined in the software integration test procedure. However, existing test case generation method has two limitations. First, the quality of test cases can vary depending on the tester's ability to analyze the test items. Second, excessive time and cost may be incurred in writing test cases. This paper proposes a method to automatically generate test cases based on the requirements model and specifications to overcome the limitations of the existing weapon system software test case generation. Generate test sequences and test data based on the use case event model, a model representing the requirements of the weapon system software, and the use case specification specifying the requirements. The proposed method was applied to 8 target models constituting the avionics control system, producing 30 test sequences and 8 test data.

Occupationally Acquired Plasmodium knowlesi Malaria in Brunei Darussalam

  • Simian malaria is a zoonotic disease caused by Plasmodium knowlesi infection. The common natural reservoir of the parasite is the macaque monkey and the vector is the Anopheles mosquito. Human cases of P. knowlesi infection has been reported in all South East Asian countries in the last decade, and it is currently the most common type of malaria seen in Malaysia and Brunei. Between 2007-2017, 73 cases of P. knowlesi infection were notified and confirmed to the Ministry of Health in Brunei. Of these, 15 cases (21%) were documented as work-related, and 28 other cases (38%) were classified as probably related to work (due to incomplete history). The occupations of those with probable and confirmed work related infections were border patrol officers, Armed Forces and security personnel, Department of Forestry officers, boatmen and researchers. The remaining cases classified as most likely not related to work were possibly acquired via peri-domestic transmission. The risk of this zoonotic infection extends to tourists and overseas visitors who have to travel to the jungle in the course of their work. It can be minimised with the recommended use of prophylaxis for those going on duty into the jungles, application of mosquito/insect repellants, and use of repellant impregnated uniforms and bed nets in jungle camp sites.

초고층 주거복합건물의 기단부 계획특성 분석에 관한 연구 (A Study on Characteristic of Super-Tall Mixed-Use Residential Building in the Outdoor Transition Zone)

  • The purpose of this study is to analyze architectural plans of lower part of super tall mixed-use residential building for understanding its functions for urban. The area of this research are limited to super tall mixed-use residential building which were built in Seoul, Kyoung-gi area, and new towns. For this purpose, theories for super tall mixed-use residential building and non-housing parts were researched, and characteristics of selected cases for field research were analyzed by studying drawing materials. The research focused on scale, type of building, included programs for public spaces, and characteristics of location. As a result, every cases were designed as 'outdoor transition zone-centered type', and for urban function, non-housing facilities were condensed to increase convenience. In addition, ratio of non-housing part were incresesed with public spaces design for residents, and various programs were inserted.

상악 구치 부에서 골계생술시 협지방체를 이용한 연조직 피개 : 문헌 고찰 및 증례보고 (The use of the buccal fat pad for guided bone regeneration in posterior maxilla: Review of the literature and report of 2 cases)

  • For the successful guided bone regeneration(GBR) of maxillary bony defect, proper soft tissue coverage is one of the most important things. Soft tissue dehiscence can be most common reason of osseous reconstruction failure. If a vascular supply to the graft should not develop from the host tissue, then the graft may also foil. Both of these prerequisites can be aided by judicious use of the buccal fat pad(BFP). Many methods for adequate soft tissue coverage have been proposed and the use of the BFP is one of them. BFP is useful in posterior maxillary area, can cover larger area and have higher blood flow than other methods. so the use of the BFP may offer protection and early blood supply to maxillary bone graft. This report describes the history, anatomy, blood flow, and clinical usefulness with two clinical cases.

KNE: An Automatic Dictionary Expansion Method Using Use-cases for Morphological Analysis

  • Morphological analysis is used for searching sentences and understanding context. As most morpheme analysis methods are based on predefined dictionaries, the problem of a target word not being registered in the given morpheme dictionary, the so-called unregistered word problem, can be a major cause of reduced performance. The current practical solution of such unregistered word problem is to add them by hand-write into the given dictionary. This method is a limitation that restricts the scalability and expandability of dictionaries. In order to overcome this limitation, we propose a novel method to automatically expand a dictionary by means of use-case analysis, which checks the validity of the unregistered word by exploring the use-cases through web crawling. The results show that the proposed method is a feasible one in terms of the accuracy of the validation process, the expandability of the dictionary and, after registration, the fast extraction time of morphemes.

한 농촌지역에서 실시한 소아 급성호흡기감염 관리사업의 평가 -항생제 사용을 중심으로- (Evaluation of Acute Respiratory Infections(ARI) Control Programme in a Korean Rural Community -The Patterns of Antibiotic Prescription-)

  • The purpose of this study was to evaluate the program for the control of acute respiratory infections(ARI) in children in a Korean rural area(Yonchon county). Evaluating the program, we focused on the pattern of prescription and appropriateness of antibiotics prescribed by the health personnel who had participated in the ARI Control Program. It was implemented at the primary health care setting in rural area, such as district hospital, health subcenters, and health posts. During six-months programme monitoring period, medical records were reviewed and collected data were analysed by the pediatrician, research coordinator of this study. The baseline data were collected from medical records of the same period(six months) of one year before the implementation of the ARI programme. The study results were as follow : 1. Common cold was the most prevalent disease(78.7%. 594 cases) among the all ARI cases (755 cases). The less frequent cases were bronchitis(11.9%), acute pharyngitis(5.2%), and pneumonia(1.8%). 2. Significant reduction in the use of antibiotics was observed after the programme implementation. Ninety three(15.7%) of 594 common cold cases were received antibiotics compared with 282(35.2%) of 802 in the baseline period. In the cases of bronchitis and acute pharyngitis, the reduction rates were 15.1% and 23.2% respectively compared to the baseline period. 3. Mean duration of antibiotics prescription was 1.81-1.75 days, similar to the baseline data. 4. The appropriateness rate of antibiotics prescriptions were 84.3%(common cold), 35.6% (bronchitis) and 28.2%(acute pharyngitis). In the case of pneumonia, the antibiotics prescription was compatible to the criteria developed. 5. Pediatrician prescribed antibiotics more appropriately for all cases than general practitioners in health sub-center, and nurse practitioners in health posts. 6. Antibiotics therapy was shown to be of no effect in the treatment of the all ARI cases. At the 5 and 10 days check-up of common cold cases after visits, proportion of improved patients were 58.3% in the antibiotics-used group and 51.4% in the control group. In the other cases of ARI, the patterns of response were similar to common cold. None of the differences in outcome between the antibiotics-used and control group was statistically significant. This ARI programme may have substantial a substantial impact on antibiotics use at the public health institutions(district hospital, health subcenters, health posts) which are of major domain for primary health care in Korean rural areas.
