Protocol for Physical Restraints of Patients in Nursing Homes (노인요양시설의 신체적 억제대 프로토콜 개발)

  • Purpose: The purpose of this study was to develop a protocol that would help prevent accidents, apply physical restraints properly, and reduce the use of physical restraints in nursing homes. Method: A review of the literature and analysis of existing statutes and regulations were used to develop the preliminary protocol. To test the validity of this preliminary protocol, ten experts were selected from academia and clinical practice to review the protocol. The initial protocol was finalized after it had been reviewed by experts and tested for clinical validity in five different nursing homes. Result: The protocol consists of objectives, definitions and accident probability assessment, principle of using physical restraints, monitoring and documentation of physical restraints. Conclusion: The findings of this study can be used as guidelines to focus on preventing accidents arising out of use of physical restraints, assessing the probability of accidents, and reducing the use of physical restraints through preventive interventions. This will be helpful to prevent ethical, physical, or psychological problems arising from use of physical restraints and to protect the rights of elderly people in nursing homes.

  • Purpose: This study aimed to explore the rationality for and countermeasures against the use of prehospital patient restraint (PPR) techniques in efforts to limit violent behavior toward 119 emergency medical technicians (EMTs). Methods: Previous countermeasures to limit violent behavior toward 119 EMTs and medical personnel were focused on strict reactive and passive proactive responses. However, those in support of the countermeasures do not believe that violent and criminal behavior can be limited or extinguished by strengthening the punishment unconditionally. Results: When it comes to the far-reaching effects of stigmatization on people who engage in violent and criminal behavior, it is possible that unconditional punishment leads to more crime, increases the costs of imprisonment, and consequently, adds to the financial burden of the government. Conclusion: Thus, we are faced with an urgent need to prepare legal grounds for the use of PPR techniques by 119 EMTs for agitated or combative patients only, with direct medical oversight. Moreover, the legal foundation for the use of PPR techniques also needs to be established for emergency medical personnel. The use of PPR techniques not only ensures the safety of emergency medical services personnel, but also protects patients from injuring themselves and others.

An Analysis of Car Seat Use from 2015 to 2019 among Children Ages under 6: Using Korea National Health and Nutrition Examination (최근 5년간 6세 미만 아동 카시트 사용 분석: 국민건강영양조사 자료(2015~2019년)를 바탕으로)

  • Purpose: Traffic accidents are the major cause of death in children. Car seat usage in children is important to reduce the risk of death or injury caused by traffic accidents. This study aims to analyze the trends of car seat use among children ages under 6. Methods: This study adopted a cross-sectional study utilizing the Korea National Health and Nutrition Examination Survey from 2015 to 2019. In total, 1,999 responses were included for the analysis. Factors associated with car seat use were age, sex, body weight, height, family type, household income, and monthly household income. Regression analysis was used to compare the car seat use before and after the revision of the Road Traffic Act (2015~2018 vs. 2019). Results: We found that car seat use was increasing by year, age, region, and household income. Car seat use was higher after fines increased in 2019. Age and body weight had negative association with car seat use, while height and household income had positive association with car seat use. Conclusion: Car seat use increased from 2015 to 2019 and is associated with different ages among children and household income. Further education and promotion that describes the safety benefits to children from car seat use are needed.

  • If the medical staff privileges, which mean the eligibility to practice at open hospitals, are excluded in the United States, antitrust claims based on the violation of the Sherman Act have been raised a lot. The proliferation of these lawsuits in the United States, which are characterized as antitrust lawsuits, can be understandable situation. The reason is because doctors who don't belong to specific hospitals are seriously damaged, if the medical staff privileges are excluded and doctors cannot use facilities of open hospitals. In order to decide to allow the privileges of certain doctors, hospitals have to rely on peer review to maintain high quality of medical services, and it is not easy to find alternative of peer review in the professional areas like healthcare. However, there are possibilities that members of the peer review can abuse power to unfairly exclude privileges of potential competitors. In this sense, it is asserted in the U.S. antitrust lawsuits that the restraint of medical staff privilege can be the illegal restraint of trade in violation of section 1 of Sherman Act and can be monopolization or an attempt to monopoly by hospitals in violation of section 2 of Sherman Act. As Korea adopted open hospital system quite recently, there is still no case related with the exclusion of medical staff privileges. However, medical staff privilege system of Korea is not different from that of the United States in principle. Thus, the U.S. jurisprudence on the exclusion of medical staff privileges can be referred in the interpretation of "practice that interferes with or restricts the activities or contents of the business" based on Article 19.1.9 of Monopoly Regulation and Fair Trade Law of Korea.

  • Although the research of high strength materials is in process briskly in the inside and outside of the country, in south korea, the research of high strength materials is insufficiency. Generally shear resistance of RC beam is influenced dominantly by amount of shear-reinforcing bars($p_w$) and yied strength($f_{wy}$). Therefore, I come to the conclusion that if use shear reinforced bar with shear reinforced bar, it leads to decrease of the quantity of shear reinforced bar and effects on the security of shear-restraint force of member. This study experimented with not only the mixture of high strength-reinforced bar and U-shaped reinforcement normal strength -reinforced bar devises efficient improvement, but also it incites improvement of bond capacity and carries out an experimental study for improvement of member resisting force, finally it evaluates bond capacity quantitatively on the lines of main reinforcement the restraint method.

Pressure Ulcers caused by Equipment and Supplies in Intensive Care Unit (중환자실 환경에서 의료기기 사용과 욕창발생 관련성)

  • Purpose: This study was conducted to assess the incidence and characteristics of pressure ulcer in relation to the type of equipment and supplies(EnS) in intensive care unit(ICU). Method: By using secondary data analysis and descriptive investigation, 29 patients of pressure ulcers caused by EnS were examined from January 2013 to December 2015, at the advanced general hospital located in Gyeongsangnam-do. The subjects were aged 19 years and above and had been in the ICU for more than 3 days. The data were analyzed using the SPSS WIN 23.0 with frequency, percentage, and mean and standard deviation. Result: The incidence of EnS-related pressure ulcers was 3.71%. The EnS-related causes for pressure ulcers were endotracheal tube(31.03%), restraint(20.69%), and neck brace(10.34%). The longest use of EnS was oxygen tube(29 days). EnS that were possibly causing pressure ulcer for one day were restraint(upper extremities), endotracheal tube(oral and face), and oxygen mask(ear and nose). Conclusion: This study has been limited to secondary data analysis that may result in missing records of pressure ulcers related to EnS in ICU. However, the analysis of this research is expected to contribute to the development of nursing interventions to clarify the guidelines for the prevention of pressure ulcers related to EnS in ICUs.

  • A deterioration of typical reinforced concrete (RC) bridge deck is due to the use of calcium chloride, cracks and water penetration inside of the deck slab with steel reinforcement. In order to eliminate the defects of RC decks in terms of material, therefore, the steel-strapped deck system is studied and developed by maximizing the arching effect while the girders are restrained using straps in lateral direction to the bridge. This parametric study was performed to analyze the structural characteristics of steel-strapped deck, and to identify the factors of the thickness, span length and lateral restraint stiffness of deck slab considering the concrete non-linearity. Finally, a design equation, which is adequate to South Korea, is suggested.

Use of child safety seats during transportation of newborns

  • Purpose: Child safety seats (CSS) are critical for the protection of children, in case of motor vehicle accidents. Although the national legislation mandates that all newborns must be placed in an appropriately installed CSS during transportation, people often do not perceive the importance of CSS and do not use it as recommended. The purpose of this survey was to understand the use of CSS for the safe transport of newborns from hospital to home. Methods: We interviewed parents of newborn infants, using a structured questionnaire, at the time of their discharge from Cheil General Hospital & Women's Health Care Center, between May 2014 and July 2014. Results: A total of 403 participants were interviewed. The rate of CSS use was only 14.9%. Overall, 76.4% of the families interviewed were not aware about the recommendations on CSS use for newborns when travelling in a car. The provision of education on using CSS significantly influenced their rate of use. Parents who were educated about mounting the CSS in a car used it more as compared with others (25.7% vs. 12.2%) (P=0.002). Furthermore, if parents had heard about the importance or necessity of CSS, they used it more than others did (19.5% vs. 10.6%, P=0.032). Conclusion: Despite the legal regulation, most parents transport their newborn infants without a CSS while traveling from hospital to their home. The rate of CSS use was influenced by parental education and their knowledge about its necessity. Education programs for parents must be reinforced to increase the CSS use.

Perceptions of Family Care-givers toward Use of Physical Restraints: An Application of Q-methodology (중환자 가족의 신체억제대 사용에 대한 인식: Q방법론적 접근)

  • Purpose: The purpose of this study was to identify on the perceptions of family care-givers toward use of physical restraints according to their values, beliefs, and perceptions using Q methodology. Methods: Thirty-three family care-givers classified 41 selected Q-statements into 9 points standard. The obtained data were analyzed by using a pc QUANL program. Results: Principal component analysis identified 4types of the perceptions of family care-givers toward the use of physical restraints. Type I is 'Rational accepted', which means that they perceived the restraints are essential therapeutic devices and had cooperative attitude to use of medical staffs' restraints. Type II is 'Sardonic sensibility', which means that they have a negative and a cynical attitude to use of physical restraints. Type III is 'Ambivalent', which means that they have conflicts between rationality and emotion, and type IV is 'Practical claim of a right', which means they insist that patients and their family members must be provided with a detailed explanation regarding the application of physical restraints. Conclusion: The findings of this study suggest that perceptions toward the use of physical restraints among family care-givers should be understood for patients' safety and dignity in medical circumstance. Based on the results, this study will be useful in developing the customized nursing intervention for supporting family care-givers' subjectivity considering the Korean context.

  • A percentage current differential relay is widely used for transformer protection. Because many percentage current differential relays recently use modified methods instead of conventional methods for deciding the operating characteristics of the large current region, in this paper, the operating region of a percentage current differential relay is analyzed in input-output current domain instead of operating-restraint current domain. An effective method to set the operating region when a CT is saturated is proposed with a series of investigations comparing a conventional method with the proposed modified method. The performance of the proposed method is evaluated for internal and external faults of a power transformer having the voltage rating of 345/154kV. EMTP-RV is used for the relaying data collection.
