The highly contoured nature of the nose and the abundant free margin makes it especially difficult to reconstruct. In this report, we describe the use of a new helical rim free flap technique for the reconstruction of full-thickness nasal alar defects via supermicrosurgery. Briefly, after a wide excision with a margin of 0.7 cm, an alar defect with a size of $1{\times}1{\times}0.5cm$ was obtained, which included the full thickness of the skin, mucosa, and lower lateral cartilage. Vessel dissection was performed in a straightforward manner, starting from the incision margin for flap harvest, without any further dissection for reach the greater trunk of the superficial temporal artery. The flap was inset in order to match the contour of the contralateral ala. We closed the donor site via rotation and advancement. No donor site morbidity was observed, despite the presence of a small scar that could easily be covered with hair. The alar contour was satisfactory, and the patient was satisfied with the results. The supermicrosurgical technique did not require further dissection to identify the vessels for anastomosis, leading to better cosmetic outcomes and a reduced operating time.
Purpose: Because of good blood supply of the pedicled perforator flap and its advantage of not requiring vessel anastomosis compared to pedicled flap, it is widely used recently. The authors intended to report the results of various pedicled perforator flaps which have been performed to reconstruct the soft tissue defects and the utility of the flap. Methods: The study was conducted for 12 cases of pedicled perforator flap which were performed at the plastic surgery department of the current hospital from the period of June, 2005 to August, 2008. Four patients were male and eight patients were female and their age was ranged from 22 to 74 years old with mean age of 42.6 years old. The sites were 1 case on face, 3 cases on chest, 3 cases on back, 4 cases on coccyx, and 1 case on foot. Results: The defect sites of all patients were successfully reconstructed by using the pedicled perforator flap. Although most of the flaps revealed congestion at the early stage after the surgery, they were recuperated within few days. One case of skin flap was reported to be partially necrotized in old age woman who has no reliable perforator. Other than that, all defects were covered successfully and acceptable aethetically. Conclusion: As stated in above, the pedicled perforator flap has many useful advantages than the conventional pedicled flap and various free flap. Unless free flap must be required, the use of pedicled perforator flap is recommended by first choice for soft tissue coverage.
As Programmable Logic Controllers (PLCs), popular components in industrial control systems (ICS), are incorporated with the technologies such as micro-controllers, real-time operating systems, and communication capabilities. As the latest PLCs have been connected to the Internet, they are becoming a main target of cyber threats. This paper proposes two sanitizers that improve the security of uC/OS-II based firmware for a PLC. That is, we devise BU sanitizer for detecting out-of-bounds accesses to buffers and UaF sanitizer for fixing use-after-free bugs in the firmware. They can sanitize the binary firmware image generated in a desktop PC before downloading it to the PLC. The BU sanitizer can also detect the violation of control flow integrity using both call graph and symbols of functions in the firmware image. We have implemented the proposed two sanitizers as a prototype system on a PLC running uC/OS-II and demonstrated the effectiveness of them by performing experiments as well as comparing them with the existing sanitizers. These findings can be used to detect and mitigate unintended vulnerabilities during the firmware development phase.
The most important issue of modern cities is a crime. Crime prevention through environmental planning is needed in the urban mixed-use commercial facilities which have an important role of modern urban life, and have a positive elements for local society. The purpose of this study is to propose a direction of crime prevention through environmental planning for the urban mixed-use commercial facilities. In order to accomplish this purpose, a questionnaire survey is performed on users of urban mixed-use commercial facilities to evaluate the importance and satisfaction. The conclusions are as follows; 1) Crime prevention through environmental planning is needed in the public space which forms boundary between urban mixed-use commercial facilities and other urban facilities. 2) Safety support planning is necessary for women and children for crime prevention of urban mixed-use commercial facilities. 3) Crime prevention planning is needed in urban mixed-use commercial facilities of the various isolation space. 4) Crime prevention through environmental planning is needed for facilities of after business hours. 5) Various security alarm system considering the characteristics of the space is needed for crime prevention. 6) Community program is necessary in order to increase the utilization of external public space. 7) A sign is necessary to prevent a criminal act in urban mixed-use commercial facilities. 8) The connection of commercial facilities and activity inducing factors is needed.
Background: The limitations of total serum PSA values remain problematic, especially after an initial negative prostate biopsy. In this prospective study of Chilean men with a continued suspicion of prostate cancer due to a persistently elevated total serum PSA, abnormal digital rectal examination and initial negative prostate biopsy were compared with the use of the on-line Chun nomagram, detection of primary malignant circulating prostate cells (CPCs) and free percent PSA to predict a positive second prostate biopsy. We hypothesized that men negative for circulating prostate cells have a small risk of clinically significant prostate cancer and thus may be conservatively observed. Men positive for circulating prostate cells should undergo biopsy to confirm prostate cancer. Materials and Methods: Consecutive men with a continued suspicion of prostate cancer underwent 12 core TRUS prostate biopsy; age, total serum PSA and percentage free PSA and Chun nomagram scores were registered. Immediately before biopsy an 8ml blood simple was taken to detect primary mCPCs. Mononuclear cells were obtained by differential gel centrifugation and identified using double immunostaining with anti-PSA and anti-P504S. Biopsies were classifed as cancer/no-cancer, mCPC detecton test as negative/positive and the total number of cells/8ml registered. Areas under the curve (AUC) for percentage free PSA, Chun score and CPCs were calculated and compared. Diagnostic yields were calculated with reference to the number of possible biopsies that could be avoided and the number of clinically significant cancers that would be missed. Results: A total of 164 men underwent a second biopsy; 41 (25%) had cancer; the AUCs were 0.65 for free PSA, 0.76 for the Chun score and 0.87 for CPC detection, the last having a significantly superior prediction value (p=0.01). Using cut off values of free PSA <10%, Chun score >50% and ${\geq}1$ CPC detected, CPC detection had a higher diagnostic yield. Some 4/41 cancers complied with the criteria for active surveillance, free PSA and the Chun score missed a higher number of significant cancers when compared with CPC detection. Conclusions: Primary CPC detection outperformed the use of free PSA and the Chun nomagram in predicting clinically significant prostate cancer at repeat prostate biopsy.
Background Thoracodorsal vessels (TDVs) and internal mammary vessels (IMVs) have both been widely employed as recipient vessels for use in free muscle-sparing transverse rectus abdominis myocutaneous (MS-TRAM) flaps. However, whether TDVs or IMVs are preferable as recipient vessels for autologous breast reconstruction with a free MS-TRAM flap remains controversial. The purpose of this study was to compare the clinical outcomes when TDVs were used as recipient vessels to those obtained when IMVs were used as recipient vessels for autologous breast reconstruction with a free MS-TRAM flap. Methods A retrospective matched-cohort study was performed. We retrospectively reviewed data collected from patients who underwent a free MS-TRAM flap for autologous breast reconstructions after mastectomy between March 2003 and June 2013. After a one-to-one matching using age, 100 autologous breast reconstructions were selected in this study. Of the 100 breast reconstructions, 50 flaps were anastomosed to TDVs and 50 to IMVs. Patient demographics and clinical outcomes including operation time, length of hospital stay, postoperative complications, and aesthetic score were compared between the two groups. Results No statistically significant differences were found between the two groups in patient demographics and clinical outcomes, including the complication rates and aesthetic scores. There were no major complications such as total or partial flap loss in either group. Conclusions The results of our study demonstrate that both TDVs and IMVs were safe and efficient as recipient vessels in terms of the complication rates and aesthetic outcomes.

Dorsal aorta cannulated rainbow trout averaging 505 $\pm$ 6.5g (Mean $\pm$ SD) were divided into 7 groups (5 fish per group). Twenty four hour post feeding, cannulated fish were intubated with one of seven L-amino acid diets containing graded levels of Valine (Val. : 0.45, 0.95, 1.20, 1.45, 1.70, 1.95 or 2.45%) at 1% of body weight. Blood samples were taken at 0, 5 and 24hr after feeding the experimental diets. Post-prandial (5h after feeding) plasma-free Valine concentrations (PPval.) increased linearly from fish fed diets containing Valine between 0.45 and 1.45%, but the concentration remained constant from fish fed diets containing valine between 1.45 and 2.45%. Post-absorptive (24h after feeding) plasma free valine concentrations (PAval) increased linearly from fish fed diets containing valine between 0.45 and 1.45%, but the concentration remained constant from fish fed diets containing valine between 1.45 and 1.95%. Using the broken-line model analysis, the dietary valine requirement by PPval and PAval could be 1.44 and 1.50% in rainbow trout, respectively. These results supported that the estimated dietary valine requirement by PP(val.) and PA(val.) could be in close agreement with the values obtained from the previous conventional feeding method. Thus, the use of PFAA concentrations for determining essential amino acid requirements could be possible in rainbow trout with dorsal aorta cannulation.
Background: Radiation therapy (RT) is frequently used for supportive treatment and management of advanced head and neck cancers. This study performed a retrospective review of the treatment methods that were used for intractable draining fistulas in seven patients who had received RT for head and neck cancers. Treatment methods used for two of the seven patients are presented in detail. Methods: From 2009 to 2020, seven patients underwent reconstructive surgery for intractable fistulas which occurred after RT for head and neck cancers. Patient characteristics, medical history, treatment method, and treatment outcome were reviewed for each case. The type of surgery performed, failure rate, and treatment period were also analyzed. Results: In this study, a total of seven patients received additional management for radiation-induced fistulas. Patients underwent a mean of 3.3±1.4 surgeries (maximum: six surgeries) to resolve their fistulas. The mean time interval from the first surgery to the last surgery for the patients to achieve resolution of the fistula was 8.7 months. Loco-regional flaps have performed an average of 1.9±1.5 times. However, all loco-regional flaps failed. Instead, the patients' intractable fistulas were resolved with the use of distant flaps or free tissue transfers. Conclusion: Fistulas that develop after head and neck cancer treatment following RT are difficult to treat with simple loco-regional flap procedures. Therefore, more aggressive treatment techniques, such as distant flap or free tissue transfer, may be needed to shorten patients' treatment periods and avoid unnecessary surgeries.

Transfer printing is a method to combine printing and dyeing technology by the use of sublimation. It is an environmentally-friendly printing method that saves costs, reduces the production processes by the omission of the washing process, and saves time by maintaining quality. Due to the development of transfer printing, a high value added printing technology is available now but color fastness to sublimation of the printing products is still low since there are few dyes that have an affinity to the fabrics and the application technology is still inadequate. Specially, in case of high concentration black dyes, eco-label type black dyes, which is a substitution for general dispersal dyes, have been developed while general dispersal black dyes are still used, creating issues such as color differences on the surface and back side of the fabrics and contamination by friction after transfer printing. There are also some restricted substances such as allergens. To address these issues, high concentration black dyes and application technology that are environmentally-friendly and that have over 16 K/S through the use of single dyes with excellent color fastness, fixation ability, and similar melting temperature were developed for this study.
In this paper, the FREECAD 0.14 was used for modeling conventional Selfie-stick and the newly proposed Selfie-stick design. The purpose of this paper is to demonstrate the utility of FREECAD 0.14, which is open-source and still in development for further use. After modeling the conventional Selfie-stick, CatiyaV5 was used to assemble FREECAD 0.14 drawn elements. Main advance in newly designed Selfie-stick is the portability. To improve portability of the Selfie-stick, folding mechanism was adopted from folding LED stands. Several mechanisms were adopted to improve user convenience as well. Ansys 14.0 was used for structural analyses of conventional Selfie-stick model and the newly designed model as well. Several simplifications for the models were needed to process the analyses. When analyzing the newly designed model various materials were used one by one to find compatible composition. Using Aluminum alloys for the stick and the hand grip was found to be compatible. FREECAD was useful for suggestion of the newly designed model but not so much useful to design an actual product. Various efforts would make FREECAD the best choice for industrial use for free as it is named.
