폐암에서 혈장 Urokinase-Type Plasminogen Activator 및 Type 1 Plasminogen Activator Inhibitor의 의의 (The Significance of Plasma Urokinase-type Plasminogen Activator and Type 1 Plasminogen Activator Inhibitor in Lung Cancer)

  • 목 적 : 악성 종양에 있어서 단백질 분해 효소에 의한 세포외기질의 분해는 종양 침습 및 전이에 있어서 중요한 역할을 한다. Urokinase-type plasminogen activator(u-PA)는 이러한 단백질 분해효소 중 하나이며, u-PA의 길항제인 plasminogen activator inhibitor(PAI-1, PAI-2)도 u-PA에 의한 종양조직 자체의 분해를 방어하여 종양의 성장, 침습 및 혈관신생을 촉진하는 역할을 한다. u-PA와 PAI-1은 폐암 등의 종양 조직내의 양이 증가하며, 침습도 및 예후 둥과 유의하게 상관되는 것으로 보고되고 있다. 저자 등은 폐암환자의 혈장 u-PA와 PAI-1의 농도를 측정하여 조직형 및 병기와의 관련성을 조사해 보고자 한다. 방 법 : 폐암 환자 37예, 여러 가지 양성 폐질환 환자 21예 및 유사연령의 정상 대조군 24예에서 혈장 u-PA 및 PAI-1 항원의 농도를 ELISA법으로 측정하였다. 결 과 : 혈장 u-PA 항원 농도는 정상 대조군에서 $1.0{\pm}0.3ng/mL$, 양성 폐질환군에서 $1.0{\pm}0.3ng/mL$, 폐암환자군에서 $0.9{\pm}0.3ng/mL$로 유의한 차이가 없었다. 혈장 PAI-1 항원 농도는 정상 대조군에서 $14.2{\pm}6.7ng/mL$, 양성 폐질환군에서 $14.9{\pm}6.3ng/mL$, 폐암 환자군에서 $22.1{\pm}9.8ng/mL$로 폐암 환자에서 정상 대조군 및 양성 폐질환군에 비하여 유의하게 높았다. 혈장 u-PA 항원 농도는 편평상피암에서 $0.7{\pm}0.4ng/mL$, 선암에서 $0.8{\pm}0.3ng/mL$, 대세포암에서 0.9ng/mL, 소세포암에서 $1.1{\pm}0.7ng/mL$로 유의한 차이가 없었다. 혈장 PAI-1 항원 농도는 편평상피암에서 $22.3{\pm}7.2ng/mL$, 선암에서 $22.6{\pm}9.9ng/mL$, 대세포암에서 42ng/mL, 소세포암에서 $16.0{\pm}14.2ng/mL$로 유의한 차이가 없었다. u-PA 항원 농도는 stage I에서 0.74ng/mL, stage II에서 $1.2{\pm}0.6ng/mL$, stage IIIA에서 $0.7{\pm}0.4ng/mL$, stage IIIB에서 $0.7{\pm}0.4ng/mL$, stage IV에서 $0.7{\pm}0.3ng/mL$였고, 혈장 PAI-1 항원 농도는 stage I에서 21.8ng/mL, stage II에서 $22.7{\pm}8.7ng/mL$, stage IIIA에서 $18.4{\pm}4.9ng/mL$, stage IIIB에서 $25.3{\pm}9.0ng/mL$, stage IV에서 $21.5{\pm}10.8ng/mL$로 유의한 차이가 없었다. 폐암 환자에서 T기를 T1-3 와 T4로 나누어서 비교를 한 결과, 혈장 u-PA 항원 농도는 T1-3에서 $0.8{\pm}0.4ng/mL$였고 T4에서 $0.7{\pm}0.4ng/mL$로 차이가 없었으나, 혈장 PAI-1 항원 농도는 T1-3에서 $17.9{\pm}5.6ng/mL$였고 T4에서 $26.1{\pm}9.1ng/mL$로 T4에서 유의하게 높았다. 혈장 u-PA 항원 농도는 M0에서 $0.8{\pm}0.4ng/mL$, M1에서 $0.7{\pm}0.3ng/mL$였고, 혈장 PAI-1 항원 농도는 M0에서 $23.6{\pm}8.3ng/mL$, M1에서 $21.5{\pm}10.8ng/mL$로 유의한 차이가 없었다. 결 론 : 이상의 결과에서 폐암 환자에서 혈장 PAI-1은 정상대조군 및 양성 폐질환에 비하여 특이적으로 증가하였고, 또한 폐암의 국소적 침습 정도와의 관련성을 보여 주었다.

Lower limb ischemia after bee sting

  • Bee sting causes mild symptoms such as urticaria and localized pain, and severe symptoms including anaphylaxis, cardiovascular collapse, and death. We reported on a patient with arterial thrombotic occlusion and severe ischemia in the lower limb after multiple bee stings. The patient was stung 5 times and complained of pallor, pain, and coldness in the left toe, and did not have dorsalis pedis pulsation. Computed tomography angiography showed multiple thrombotic occlusion of the anterior and posterial tibial artery below the knee. Local thrombolytic therapy using urokinase was administered and the occluded arteries were successfully recanalized.

  • Protease nexin-1 (PN-1)은 활성화 자리에 serine기를 갖는 단백질 분해 효소 즉, 트롬빈, 트립신, 플라스미노겐 활성화 효소 등의 작용을 억제한다. 본 연구에서는 흰쥐의 Sprague-Dawley계통을 이용하여 조직별 mRNA발현여부 및 정도를 조사하였다. PN-1의 발현이 나타난 조직은 뇌 (전뇌, 후뇌), 심장, 간, 폐, 난소, 난관 등이다. 이들 중 유전자 발현이 가장 높은 조직은 암컷의 전뇌(forebrain) 였다. 특히, 생식기관들 중에서는 암컷의 난소와 난관에서만 발현이 관찰되는 등 PN-1 유전자는 성별에 따라 서로 다르게 발현됨이 확인되었다 이러한 결과들로 미루어 PN-1은 여포 형성과정과 초기배 형성과정 등의 생식 및 발생작용과 관련이 있을 것으로 생각된다.

Ultrasound-guided Evacuation of Spontaneous Intracerebral Hemorrhage in Basal Ganglia

  • Objective: Ultrasound can be used in the treatment of large intracerebral hematoma. The authors present our experiences with Ultrasound-guided catheter placement for lysis and drainage of ganglionic hematoma, with emphasis on technical aspects. Methods: The authors applied real-time ultrasonography for the aspiration of intracerebral hematoma in 6cases. Ultrasound-guided aspiration via a burrhole was performed under local anesthesia. We selected a temporal entry point instead of the frequently used precoronal approach in ganglionic hematoma. A burrhole was made 4 to 6cm posterior from posterior border of frontal process of the zygomatic bone at the level of 4 to 5cm above the external auditory meatus. Results: In all patients, the catheter was placed accurately into the hematoma target. All patients were irrigated with urokinase once to three times a day. The catheter could be removed within two or three days. The mean hematoma volume was reduced from initially 32mL to 5mL in an average of two days. There were no intraoperative complications related to the use of real-time ultrasonography and no postoperative infections were noted. Conclusion: Ultrasound allows an easy and precise localization of the hematoma and the distance from the surface to the target can be calculated. Ultrasound-guided catheter placement for fibrinolysis and hematoma drainage is a simple and safe procedure.

Epigallocatechin-3-gallate의 사람 비점막 섬유아세포 케모카인발현에 대한 효과 (Effect of Epigallocatechin-3-gallate on Expression of Chemokines in Human Nasal Mucosal Fibroblasts)

  • Epigallocathechin-3-gallate (EGCG), the main polyphenol component in green tea, inhibits angiogenesis, urokinase, and matalloproteinases, and EGCG also has the antioxidative property. Recent reports proposed that EGCG may modulate the immune response on allergy or asthma. Human nasal mucosal fibroblasts are a rich source of cytokines, inflammatory mediators, and chemokines. Chemokines are important for the recruitment of leukocytes to sites of infection, which is essential in host defense. The objective of this study was to investigate the effect of EGCG on the expression of the chemokines such as RANTES (regulated upon activation, normal T cell expressed and presumably secreted), eotaxin, and interleukin-8 (IL-8) in human nasal mucosal fibroblasts after stimulation with cytokines like IL-4, tumor necrosis $factor-{\alpha}\;(TNF-{\alpha})$, and $interferon-{\gamma}\;(IFN-{\gamma})$. To detect the expression of chemokine genes, RT-PCR was performed. Expressions of RANTES, eotaxin, and IL-8 mRNA stimulated with IL-4 and $TNF-{\alpha}$ were increased, respectively, while the expression of those genes incubated with $IFN-{\gamma}$ was similar pattern compared to control group. Analyses of chemokine genes of cells pretreated with EGCG showed that the expressions of eotaxin, and IL-8 genes stimulated $IFN-{\gamma}$ were higher compared with those not pretreated with EGCG.

Production of Plasminogen Activators during In Vitro Maturation of Fresh or Frozen- Thawed Oocytes in the Pig

  • This study were examined whether plasminogen activators (PAs) are produced by porcine fresh or frozen-thawed cumulus-oocytes complexes (COCs) and cumulus cell free-oocytes. In fresh or frozen-thawed COCs and oocytes for 0 hour cultured, no activity of PAs was detected. However, at 24 hours of culture urokinase-type plasminogen activator (uPA) was detected in COCs and denuded oocytes. In the frozen-thawed COCs and cumulus cell free-oocytes cultured for 24 hours, no PAs were observed. After COCs were cultured for 48 hours, tissue-type plasminogen activator (tPA) and tPA-PAI were observed in COCs only. In the frozen-thawed COCs and cumulus cell free-oocytes cultured for 48 hours, no PAs were observed. These results suggest that uPA, tPA and tPA-PAI are produced by porcine COCs, but only uPA by oocytes during maturation for 24 hours. Only tPA, and tPA-PAI are produced by COCs cultured for 48 hours, and no PAs are produced by denuded-oocytes cultured for 48 hours. In all of the frozen-thawed groups, no PAs are observed by COCs and denuded-oocytes.

  • Plasminogen activators (PAs) are serine proteases that convert plasminogen to plasmin. Two type of PAs are urokinase-type PA (uPA) and tissue-type PA (tPA). Plasminogen is present in most extracellular fluids. PAs play in various reproductive processes including implantation, ovulation and fertilization. In the spermatozoa, PAs and PAIs play a role in sperm motility and fertilization. PAs in the sertoli cell are stimulated spermatozoa maturation and sperm activation through the phospholipase A2. The oocyte maturation is the process for fertilization and implantation. PAs in cumulus-oocyte complexes (COCs) are related to oocyte maturation by protein kinase A and C. In the ovulatory process, PAs activity are changed and it are related to reducing the tensile strength of ovarian follicle wall. The uterine environment is important for reproduction and the uterus undergo tissue remodeling. In the uterus and oviduct of mammals, expression and activity of PAs are changed during estrous cycle. Thus, expression and activity of PAs are concerned to many reproductive functions. Therefore, PAs seem to important factor of regulator in reproductive events.

Remote Cerebellar Hemorrhage after Supratentorial Aneurysmal Surgery : Report of Six Cases

  • The case of postoperative hemorrhage occurring apart from the operative site as a complication of intracranial surgery is a rare malady, especially when it involves the cerebellum after supratentorial aneurysm surgery. In a review of the literature, the possible etiologies for cerebellar hemorrhage are: coagulopathy, intraoperative urokinase irrigation, excessive head rotation on positioning, brain shift due to excessive cerebrospinal fluid[CSF] and epidural hemovac drainage. We experienced six cases of cerebellar hemorrhage after supratentorial aneurysm surgery, and all of the patients were improved by instituting conservative medical treatment. The possible mechanism for the remote cerebellar hemorrhages seen in our series is probably a multifactorial effect, such as excessive epidural hemovac and CSF drainage, and jugular venous compression due to the operative position. The purpose of this report is to alert neurosurgeons to the existence of this syndrome and to suggest several ways of minimizing the possibility of their patients developing remote cerebellar hemorrhage.

Peroxiredoxin 6 Promotes Lung Cancer Cell Invasion by Inducing Urokinase-Type Plasminogen Activator via p38 Kinase, Phosphoinositide 3-Kinase, and Akt

  • The peroxiredoxin family of peroxidase has six mammalian members (Prx 1-6). Considering their frequent up-regulation in cancer cells, Prxs may contribute to cancer cells' survival in face of oxidative stress. Here, we show that Prx 6 promotes the invasiveness of lung cancer cells, accompanied by an increase in the activity of phosphoinositide 3-kinase (PI3K), the phosphorylation of p38 kinase and Akt, and the protein levels of uPA. Functional studies reveal that these components support Prx 6-induced invasion in the sequence p38 kinase/PI3K, Akt, and uPA. The findings provide a new understanding of the action of Prx 6 in cancer.

Anti-thrombosis of Chungpesagan-tang is Activated by Human Intestinal Bacteria

  • The possibility of Chungpesagan-tang, which has been recommended on the stroke patients with constipation in Korean traditional clinic, and its ingredients as a novel antithrombotic agent was evaluated. Most of its ingredients except Puerariae Radix exhibited in vitro antiplatelet aggregation activity. However, Puerariae Radix was significantly effective on ex vivo anti-platelet aggregation activity, whereas Angelicae Tenuissimae Radix, Raphani Semen and Angelicae Dahuricae Radix was not effective. Plasma recalcification was potently inhibited only by Puerariae Radix and Rhei Rhizoma treated with intestinal bacteria. Urokinase was also activated only by Chungpesagan-tang, Angelicae Tenuissimae Radix and Puerariae Radix treated with intestinal bacteria. Chungpesagan-tang exhibited the potent anti-thromboembolic activity activity in vitro. These results suggest that anti-thrombotic activity of Chungpesagan-tang should be activated by intestinal bacteria and may be important in the prevention of thrombosis and cardiovascular diseases, such as myocardial infraction stroke and arteriosclerosis.
