Mini-review; Melamine-related Urinary Stone Disease (Mini-review: 멜라민에 의한 신요로계 결석)

  • Last year, an epidemic of infantile urinary stone disease developed in China. Investigation revealed that melamine-tainted diary product caused urinary stone in these infants. Young infants were susceptible to the melamine toxicity and dehydration or other stone-prone factors aggravated the toxicity. Melamine-related urinary stones were small, multiple, and mainly composed of uric acid, thus conservative treatment of hydration and urine alkalinization worked well in majority of the patients.

A Study on the Diagnosis of Urinary Stone Location by Abdominal Positioning Variations (요로결석 위치 진단에 대한 복부자세 변화에 따른 연구)

  • Patients who visit the emergency room with urinary stones have difficulty lying down in a supine position due to severe pain when performing the KUB test. The purpose of this study was to find methods to reduce the patients' pain and image distortion, and obtain medical images with high diagnostic values. After checking the standard classification of disease and cause of death, the target group consisted of 121 patients who had clearly distinguished stones from computed tomography. Patients with stones in the ureteralvesical junction were excluded. Qualitative image evaluation was performed by confirming the location of the stone in the computed tomography images. and evaluated the rate of visual discrimination of stones possible through KUB and abdominal plain X-ray. Quantitative image evaluation was performed on the KUB, abdominal plain X-ray images. The transverse process of the first lumbar vertebrae served as the standard point, and the length from this point to the lower part of the stone was measured. Results from looking at the rate of visual discrimination of stones possible through KUB and abdominal plain X-ray showed: 94 patients (77.6%) for KUB images and 91 patients (75.2%) for computed tomography images. The standard deviation for KUB and abdominal X-ray was 3 (2.4%). Comparing and analyzing the location from KUB images and abdominal plain X-ray images, the stone position was 10.1 mm in the kidney, 10.5 mm in the ureteropelvic junction, and 9.7 mm in the ureters. It was shown that the stone moved 10 mm on average with significant statistical difference (P<0.05). In cases where the pain is so severe that it is impossible to perform the test in the supine position, an alternative may be to check the stone position by performing a modified KUB test by having the patient stand in a vertical position. In the future, this will provide convenience to both the examiner and the patient when performing the examination, and it will contribute with its reproducibility.

  • Acute flank pain from urolithiasis is the most common condition in people visiting emergency rooms. This study is to evaluate the usefulness of color Doppler in detecting ureteral stones in patients without hydronephrosis. We performed ultrasonography and retrospective analysis on 161 patients who were suspected of urinary stones through plain radiography of the kidney, ureter, and bladder examination or urine tests that showed positive signs of hematuria. In ultrasonography, a total of 154 (95.6%) cases from the 161 patients were diagnosed with ureteral stones. In color Doppler, ureteral stones with twinkling artifact (TA) in the absence of hydronephrosis was shown in 18 (85.7%) cases of the 21 patients(p<0.001). The use of color Doppler has a high diagnostic efficacy for the detection of indistinguishable stones, non-dilatational ureteral stones, and middle ureter stones.

The Optimal Shock Intensity and C-arm Total Running Time in Patients with Inferior Ureter Stones during Electromagnetic of ESWL (전자기식 체외충격파쇄석술시 하부요관 결석 환자에 대해 적합한 충격강도와 C-arm 총 가동시간)

  • The advantages and disadvantages of the various models of crushed ESWL (Extracorporeal Shock Wave Lithotripsy) and their various side effects due to pre-treatment have been reported. We look for the appropriate intensity of the shock wave in the electromagnetic ESWL treatment of patients with lower Inferior Ureter Stones and measure the total running time of C-arm saw. This study is based on the January-June 2014 launch of 65 patients of C university hospital located in Gyeongbuk, who conducted ESWL without pre-treatment. Patients are composed of 48 male and 17 female which were more common in men, while the most common age is 50s. The occurrence of lower urinary tract stones were left and they were more absent than 5mm in size in 39 of the most common. Optimal intensity is one of the suitable intensity of pain, and it is possible to switch the strength of impact to C if one appeals pain. In addition, the C-arm of the total operating time showed $241.73{\pm}30.37$ seconds, which is the size of the lower urinary tract that showed a significant difference (p <0.05). Therefore, ESWL treatment without pre-enforce treatment, the standard for the impact frequency and impact strength depending on the site of ureteral stones is required. The standard for total operating time of C-arm generated for ureteral stones is needed in order to reduce radiation exposure, and the standard for the total operating time for the criteria is needed depending on the site in order to minimize the exposure.

A Case of Cystinuria with Multiple Renal Stones in an 8-month-old Girl (8개월 여아에서 다발성 결석으로 진단된 시스틴뇨증 1례)

  • Cystinuria is an autosomal recessive disease characterized by impaired transport of cystine and dibasic amino acids in the proximal renal tubule, resulting in the formation of cystine stones. It is believed to account for about 1% of all kidney stones and up to 10% of pediatric stones. Here we report a case of cystinuria with multiple renal stones confirmed by genetic mutational analysis. An 8-month-old girl was admitted to AMC with persistent fever and multiple renal stones. A renal sonogram showed multiple stones at the right renal pelvis, right distal ureter, and left renal medullary portion. An approximately 1 cm renal stone was extracted spontaneously, and stone analysis revealed it to be composed entirely of cystine. Cystinuria was confirmed by increased urine dibasic amino acid levels, including cysteine, and genetic mutational analysis showed the patient to be a homozygote for the pathogenic c. 1820del (p.L607fs) of SLC3A1. Despite treatment with oral hydration and urinary alkalinization, and restricted intake of animal protein, the stones increased in size and number. The patient has since been treated with tiopronin.

Calcium Oxalate Stone in Urinary Bladder of Maned Wolf (갈기늑대의 수산칼슘 방광결석)

  • Urolithiasis (UL) is a common disease of canine. Analysis of urinary stone is an essential step in the examination and initial treatment of the patient with UL. The dead maned wolf (Chrysocyon brachyurus), male, was introduced to the Veterinary Teaching Hospital, Chungbuk National University. The maned wolf was imported from USA to Cheong-ju zoo. Postmortem sign of maned wolf was hemorrhage of bladder with a many stones. No bacteria was shown with bacterial culture of urine. But in feces, Clostridium perfringens were overgrown. Bladder stone from maned wolf was composed of calcium oxalate. Calcium oxalate was formed with excess calcium from diet mainly meat base.

A Study on Relationship between Urinary Stone Patients and Dietary Intake in Korean People (한국인의 요로결석과 식이의 상관성 고찰)

  • The study analyzes relationship between Increasing tendency of urinary stone patients and changes of food intake through statistical data during recent 10 years(1982∼1991) in Korea. In four university hospitals, the number of urinary stone patients increased 2.5 times during the period. Study statistics showed sex ratio of male and female patient as 2.1 : 1.The types of the urinary stone of each patient were broad and mostly Ca and others(85.8%), and Ca oxalate stones were 65.2%. The incidence of patients with hypercalciuria and hyperuricosuria was observed in the Ca stone patients than in normal person. The same period showed increase of total monthly household expenditure as 3.2 times, food as 2.5 times and medical expenses as 2.4 times : The food expenditure decreased relatively In the expenditure composition. Daily hood consumption (g/person) increased as 3 times in meat, 4 times in milk, 2.4 times in seaweeds. Daily total Ca nutrition consumption was undulant and became 1.1 times during the same period. Daily food supply (g/person) was increased 1.9 times in meat, 1.4 times in spinach. However, anchovy, laver, brown seaweed, herring and makerel showed undulant tendency. Relationship (correlation coefficient) between the increase of urinary stone patients and of food intake were studied as follows. In the case of food intake, the relative index was 0.923 in seaweeds, 0.865 in meat and 0.대27 In milk And In food supply, the relative index was 0.855 in purine group which constitute meat, anchovy, herring and makerel. The relative index of Ca components which constitute anchovy, laver, brown seaweed and shrimp was 0.270. In the case of nutrition intake, the relative index was medium in protein(r: 0.443) and In animal protein (r: 0.488) but was negative in Ca(r: -0.028).

Dolichos biflorus Linn attenuate progression of renal damage in alloxan-induced diabetic rats

  • Dolichos biflorus Linn. (Fabaceae), commonly known as Horse gram is a medicinal plant, used in folk medicine for treating kidney stones and diabetes mellitus. The purpose of the present study was to investigate the effects of daily oral feeding of various doses of methanolic extract of Dolichos biflorus seeds (DB) for 42 days on blood glucose concentrations and kidney functions in Alloxan-diabetic rats. Plasma glucose levels, body weight, serum creatinine, and urinary albumin levels were monitored on $15^{th}$, $29^{th}$, $43^{rd}$ day. Renal hypertrophy was assessed as the ratio between the kidney weight and body weight of the rats. Plasma glucose concentrations in Alloxan-diabetic rats were significantly reduced by the administration of DB (350 mg/kg) and DB (700 mg/kg) on day 15 and onwards (P < 0.01). After 15 days of Alloxan administration urinary albumin levels (UAE) were over 5 fold higher in diabetic controls as compared to normal controls. Treatment with DB significantly prevented the rise in UAE levels from day 15 to 43 in comparison to diabetic controls (P < 0.01). Renal hypertrophy was significantly higher in diabetic controls as compared to non-diabetic controls. Treatments with DB (350 mg/kg) and DB (700 mg/kg) significantly prevented renal hypertrophy (P < 0.01) as compared to diabetic controls. DB (175 mg/kg) failed to modify renal hypertrophy. Thus the present study indicates that methanolic extract of Dolichos biflorus may be useful in management of hyperglycemia and kidney functions in Alloxan-diabetic rats.

Pediatric Urolithiasis: Our 22-year Experience at a Single Center (소아에서의 요로결석: 단일기관에서 22년간의 경험)

  • Purpose: Pediatric urolithiasis is uncommon in children but is a cause of significant morbidity and damage to the kidney. Although much information on adult urolithiasis is available in the literature, large studies on the pediatric population are still scarce. In this report, we review our experience with pediatric urolithiasis over 22 years at a tertiary referral center. Method: We retrospectively reviewed the records of children with newly diagnosed urolithiasis between January 1991 and May 2013. We assessed the age, sex, family history, initial symptoms, location of stones, underlying cause, stone analysis, treatment, and recurrence among the patients. Results: In total, 137 patients (96 male, 41 female) were assessed. The age range was 0-17 years (mean age, 6.0 years). Forty-three (31%) children were aged <1 year, and 37% (16/43) had a history of intensive care unit (ICU) admission. Thirteen patients (9.5%) had a family history of stones. The most common symptoms at presentation among the patients were gross hematuria (56/137, 41%) and flank or abdominal pain (46/137, 34%). The stones were located in the kidney (85/137, 62%), ureter (29/137, 21%), bladder (2/137, 1.4%), and multiple locations (20/137, 15 %). Congenital abnormalities of the genitourinary (G-U) tract, with or without metabolic abnormality, or urinary tract infection (UTI) was detected in 26 children (19%). Ninety-one patients (66%) underwent metabolic examination, and 38% of these patients exhibited an abnormality. UTI, with or without abnormalities of the G-U tract, or metabolic abnormality was detected in 26 children (19%). Of the 35 stones analyzed, the majority were calcium stones (20/35, 57%), followed by infected stones (5/35, 14%), uric acid stones (4/35, 11%), carbonate apatite stones (3/35, 7%), cystine stones (2/35, 6%), and phosphate stones (1/35, 3%). Five patients (4%) required open procedures, with or without non-open procedures, whereas 77 patients (56%) were managed conservatively; the remaining 55 patients (40%) received some other form of intervention. Eighteen patients (13%) had stone recurrence during the follow-up period. Conclusions: Pediatric urolithiasis is commonly associated with abnormalities of the G-U tract and/or metabolic disorders and/or UTI. Half of the patients will pass their stones spontaneously, and all the techniques of minimally invasive surgery are applicable in the treatment of children with stones. As the recurrence rates are high among this population, long-term follow-up is recommended and the complete clearance of stones is important.

Urinary Stones Segmentation Model and AI Web Application Development in Abdominal CT Images Through Machine Learning (기계학습을 통한 복부 CT영상에서 요로결석 분할 모델 및 AI 웹 애플리케이션 개발)

  • Artificial intelligence technology in the medical field initially focused on analysis and algorithm development, but it is gradually changing to web application development for service as a product. This paper describes a Urinary Stone segmentation model in abdominal CT images and an artificial intelligence web application based on it. To implement this, a model was developed using U-Net, a fully-convolutional network-based model of the end-to-end method proposed for the purpose of image segmentation in the medical imaging field. And for web service development, it was developed based on AWS cloud using a Python-based micro web framework called Flask. Finally, the result predicted by the urolithiasis segmentation model by model serving is shown as the result of performing the AI web application service. We expect that our proposed AI web application service will be utilized for screening test.
