Recognition and Performance on Management for Nosocomial Infections among Nursing Students (병원감염관리에 대한 간호학생의 인지도와 수행도)

  • Purpose: This study was designed to identify recognition and the performance level for nosocomial infections amongst student nurses and to provide data for preparing on efficient policy and control program for nosocomial infections. Method: 191 senior nursing student participated in this research from 5 nursing college which allowed data collection, in the city of Seoul. The questionnaire was composed of 73 items(likert scale) about eight areas; aseptic technique, disinfection, precaution, hand washing, urinary tract infection, respiratory infection, catheter related infection, and self care about nosocomial infections. Results: The mean score of recognition and performance level for management of nosocomial infection were 4.29 and 3.41 respectively. The mean score of the recognition level was significantly lower than the performance level in the eight areas. The mean score of both recognition and performance were highest in the area of disinfection. However, the mean score of recognition was lowest in the area of aseptic technique and the mean score of performance was lowest in the area of catheter related infections. The correlation of recognition and performance level was statistically significant. Conclusions: These research findings should be useful in promoting an intensive and continuous educational program on nosocomial infection for nursing students and to establish an efficient policy for preventing nosocomial infections.

Group B Streptococcal Renal Abscess in a 17-Year-Old Girl with Type 1 Diabetes Mellitus

  • Streptococcus agalactiae or group B streptococcus (GBS) is associated with infections in neonates and pregnant women. Herein, we describe a rare case of GBS renal abscess with peritonitis and pleural effusion in a 17-year-old girl with type 1 diabetes mellitus. The girl was admitted due to fever and right flank pain. Laboratory findings included leukocytosis and increased C-reactive protein level and erythrocyte sedimentation rate. Her serum glucose level was 484 mg/dL. Urinalysis showed no pyuria. Renal sonography revealed parenchymal swelling in the right kidney. The patient was administered intravenous cefotaxime. Urine and blood cultures were negative. Fever seemed to improve, but the following day, she complained of abdominal pain and fever. Antibiotic was switched to imipenem, and abdominal and pelvic CT revealed a ruptured right renal abscess, peritonitis, and bilateral pleural effusion with atelectasis. Pigtail catheter drainage of the abscess was performed. Culture from the abscess was positive for GBS, and fever subsided 2 days after the drainage. She was discharged with oral cefixime. The clinical course of urinary tract infections (UTIs) can be atypical in patients with diabetes, and GBS can be a cause of UTIs. Prompt diagnosis and management are necessary to prevent complications in patients showing atypical courses.

Removal of Urinary Calculi by Laparoscopic-Assisted Cystoscopy in Five Dogs (다섯 마리의 개에서 복강경 보조 방광경을 이용한 요로결석 제거)

  • This article describes the use of laparoscopic-assisted cystoscopy for removal of urinary calculi in five dogs. All dogs had micturition disorder due to urinary calculi. The surgical technique used was same in all cases. A urethral catheter passed into the urinary bladder through the urethra preoperatively. A 5-mm diameter cannula was placed in the ventral midline, 1 to 2 cm cranial to the umbilicus, and the 5-mm laparoscope was introduced via the cannula. A 10-mm diameter cannula was placed adjacent to the apex of the bladder under the visual guidance of laparoscopy. The bladder was then partially exteriorized through the 10-mm portal site, and a stab incision was performed on the bladder wall. The incisional margin of the bladder was sutured to the skin of the second portal site in $360^{\circ}$ simple continuous suture. A 2.7-mm diameter cystoscope with a sheath was introduced into the bladder lumen. The cystic and urethral calculi were removed under the visual guidance of cystoscopy with continuous fluid flushing. No major postoperative complications were identified. During the follow up period (range 7 to 21 months), no episodes of urinary dysfunction or recurrence of clinical signs were observed.

Anterior Vaginal Wall Sling for Female Stress Urinary Incontinence (여성 복압성 요실금에서 질전벽슬링수술의 효과)

  • Background: The purpose of this study was to determine the efficacy and safety of the anterior vaginal wall sling in the management of women with stress urinary incontinence. Materials and Methods: From January 1998 to December 1999, 42 patients(31 with genuine stress urinary incontinence and 11 with mixed urinary incontinence, 38 with anatomical incontinence and 4 with intrinsic sphincteric deficiency) underwent anterior vaginal wall sling at Yeungnam University Hospital were studied retrospectively. The mean age was 49.3 years(ranging from 34 to 66 years of age) and the mean follow-up period was 29.4 months(ranging from 16 to 40 months). Intra- and postoperative complication. success rate and patient's satisfaction were evaluated. Results: The mean operation time was 79 minutes(ranging from 65 to 124 minutes) and the mean hospital stay was 5.1 days(ranging from 4 to 10 days). Mean postoperative Foley catheter drainage was 2.1 days(ranging from 1 to 5 days). As a complication. bladder perforation occurred in one patient(2.4%). residual urine sensation developed in seven patients(16.7%), and suprapubic pain was complained in five patients(11.9%), which improved gradually. Vaginal epithelial inclusion cyst occurred in one patient(2.4%) at postoperative 31 months. Four(9.4%) patients with de novo instability were improved by anticholinergics medication. The success rate was 92.9% and 38 patients(90.5%) were satisfied with this procedure. Conclusion: We consider that the anterior vaginal wall sling to be a safe and effective surgical procedure for the treatment of female stress urinary incontinence, but a longer follow-up is necessary to determine long term effect.

  • Postoperative pain relief and the side effects of epidurally injected morphine were investigated in 10 patients who received thoracotomy. Epidural morphine injection was given via an epidural catheter after thoracotomy. The pain score[VAS] and repiratory rate were decreased and the SaO2, tidal volume and vital capacity were increased significantly after epidural morphine injection. The analgesia of epidural morphine lasted for 13 hours with average. The side effects of epidural morphine were few and mild, but urinary retention was in 10%[1/10] of total patients.

  • Autonomic dysreflexia is a clinical emergency syndrome of uncontrolled sympathetic output that can occur in patients who have a history of spinal cord injury. Despite its frequency in spinal cord injury patients, central nervous system complications are very rare. We report a man with traumatic high level incomplete spinal cord injury who suffered hypertensive right thalamic hemorrhage secondary to an episode of autonomic dysreflexia. Prompt recognition and removal of the triggering factor, the suprapubic catheter obstruction which led to hypertensive attack, the patient had a favorable functional outcome after the resorption of the hematoma and effective rehabilitation programme.

  • The placement of epidural catheter may cause complications such as epidural hematoma, epidural abscess and neural damage. Among the above complications, epidural abscess is a rare but serious complication. This report pertains to a diabetic metlitus patient who developed spinal epidural and subdural abscess after continuous epidural catheterization for management of pain caused by reflex sympathetic dystrophy. The patient experienced urinary incontinence, as a neurologic sign, 8 days after epidural catherization. In was considered that the poor prognosis was due to a combination effects of a delayed visit to the hospital for treatment, rapid progression of abscess and uncontrolled blood sugar level. We therefore recommend aseptic technique and proper control of blood sugar level to prevent infection during and after epidural catheterization for diabetic patients. Early diagnosis of epidural abscess following surgical procedure must be required to avoid sequelae.

A survey on Hospital Infection (병원 감염관리에 대한 실태조사 -간호수기를 중심으로 -)

  • This study was conducted to evaluate the methods used in nursing procedures for infection control. Skilled nursing procedures are related to prevention of hospital infection. The sample consisted of 301 nurses’ response to the questionnaire. Data were collected from Feb. to March, 1993 from 35 hospitals located in five major cities. The findings of the study are as follows : 1) Rotatively unsafe nursing procedures were reported for preperation of IV therapy, change of IV site and aseptic dressing on IV site, hand wash-ing and use of paper towels, use of sterile urine bottle with indwelling urinary catheter, management of dressing cart, disinfection of transfer forcep, ambu respirator, laryngoscope, humidifier and handling of incubator. 2) Relatively safe nursing procedures were reported for management of suction tube, marking for contaminated materials and waste collection. The rate of participation in education programs for infection control by the nurses was higher than in other research results. 3) Further study on procedures for IV site in-fusion and care of dressing cart are recommended.

  • The purpose of urodynamic investigation is to determine information on the function of the urinary system. One of the most frequently used measurement procedures in urodynamics is filling and voiding cystometry using invasive method. But in this method transurethral catheter is use and it makes patients uncomfortable. The aim of this study was to implement the system that could evaluate the function of urinary tract with noninvasive and comfortable method. Therefor in this study, a sensor and measuring system were implemented to measure uroflow, urophonography and noninvasive bladder pressure signal during urination for diagnosing the LUTS(lower urinary tract symptoms) using noninvasive method. The implemented system compose of the sensor parts, signal conditioning parts, system control parts using FPGA and PC monitoring program. For the evaluation of the implemented system, the simulation of system's control part was performed and the model system for the lower urinary system was designed. From the evaluation of the model system, the mean error rate of the uroflow measurement part was 1.08% and coefficient of variation was 1,48. And the mean error rate of the noninvasive bladder pressure measurement part was 2.41% and coefficient of variation was 2.81. urophongraphy signal analysis was accomplished in a time domain and frequency domain. Average RMS power was used in a time domain analysis, and MF was used in a frequency domain analysis. From the evaluation of the model system average RMS power and MF was dependent on the occlusion degree significantly and median frequency range of $60{\sim}160Hz$ was correlated with the occlusion.

  • Renal arterial infusion of renotropic agents has been a very useful technique in the renal function studies. This type of experiments have usually been conducted in the large animals such as dogs and sheep. In these animals a catheter can be placed in the site without much disturbances of renal blood flow. Rabbits as an experimental model, however, caused a disturbances of renal blood flow by a catheterization of renal artery by its properties. Therefore we have developed a new technique that allows a simple and selective access to one side of renal arteries and the other as a control, without any disturbances of renal function. The distance between the both bifurcations of renal arteries on abdominal aorta is about 7 mm. To locate the tip of catheter on one side renal artery, ascending cannulation performed via femoral artery was done. We did an experiment with the technique to clarify the effect of calmodulin inhibitor on the renal function. One of the phenothiazine derivatives, trifluoperazine known as a powerful calmodulin inhibitor. Trifluoperazine, actual dose ranges of $2.76-5.20\;ug\;{\cdot}\;kg^{-1}\;{\cdot}\;min^{-1}$, increased urine volume and glomerular filtration rate significantly. Significant increases in urinary excretion of sodium, chloride and potassium were found. Fractional excretion of sodium and free water clearance increased significantly. These data suggest that this new technique is very useful in field of renal physiology and that striking effect of trifluoperazine on the renal function may be caused by increasing the renal hemodynamics, and by the inhibition of sodium, chloride and potassium reabsorption in the renal tubules.
