A Case Study of Overactive Bladder Patient Treated with Soeumin Sibimigwanjung-tang (소음인 십이미관중탕으로 호전된 과민성 방광 환자 치험 1례)

  • This study was aimed to report an significant improvement of urinary disturbance in overactive bladder patient using Soeumin's Sibimigwanjung-tang based on Sasang constitutional medicine. Methods A female overactive bladder patient with urinary disturbance including urgency, frequency, nocturia, tenesmus, weak stream was enrolled in this study. The patient was diagnosed as Soeum type greater-yin pattern and medicated with Sibimigwanjung-tang three times a day. Clinical improvement was evaluated with urination diary, International Prostate Symptom Score, Overactive Bladder Symptom Score, Numeric Rating Scale, and percentage of improvement compared to the day of hospitalization. Results Significant improvement was seen in urinary disturbance after 26 days of admission treatment. Also, symptoms of dyspepsia, coldness of hands and feet, and facial edema were improved. Conclusions This clinical case suggests that Sibimigwanjung-tang can be valuable option in treating urinary disturbance in overactive bladder within Soeum type greater-yin pattern.

Cytodiagnosis of Primary Small Cell Carcinoma of the Urinary Bladder - A Case Report - (뇨세포진 검사로 진단된 방광의 원발성 소세포암 - 1예 보고 -)

  • Small cell carcinoma of the urinary bladder is a rare tumor which occurs in about 0.48 % of all bladder tumors. We report cytologic features of small cell carcinoma of the urinary bladder in a 66-year-old man who had painless total gross hematuria, which was confirmed by partial cystectomy. In urine cytology, abundant tumor cells appeared in scattered and clustered forms in a bloody background. The tumor cells were small and uniform in size with a high nuclear/cytoplasmic ratio. The nuclei of the tumor cells were hyperchromatic, characteristically molded and showed inconspicuous nucleoli. The cytoplasms were scanty and pale blue.

  • Primary small cell carcinoma of the urinary bladder is an extremely rare but important entity. We experienced a case of small cell carcinoma of the urinary bladder diagnosed by urine cytology. A 59-year-old man presented with gross hematuria and dysuria, and a calcified mass was detected at the left ureterovesical junction by cystoscopy. Abdominal ultrasonography revealed focal wall thickening at the left lateral side of the urinary bladder, and urine cytology findings were of an inflammatory background and atypical small round cells with minute hyperchromatic or pyknotic nuclei, scant cytoplasm, and rare nucleoli. In addition, atypical cells were scattered in an isolated single cell pattern or in small loose clusters with prominent nuclear molding. Subsequent histological and immunohistochemical examinations confirmed a diagnosis of small cell carcinoma.

  • Objectives : The paper's objective is to study into the books dealing in the debate surrounding the existence, or nonexistence, of urinary bladder's upper gate and lower gate, a debate that ensured since the publication of Huangdineijing. Methods : Sikuquanshu and Zhongyishijia databases were searched to collect related materials, and these materials were reviewed to get an understanding of the historical development of the debate. Results : The upper gates of urinary bladder was first mentioned in Nanjing. Since then, Wanglu's Yijingsuhuiji asserted that the upper gate exists while the lower didn't, but many argued after the dawning of Ming Dynasty that the upper doesn't exist while the lower does. Additionally, some urged in relation to the assertion of the nonexistence of upper gate that water liquid pervades into the urinary bladder through sebaceous membrane or oil net. Conclusions : Behind the debate between the existence or nonexistence of the upper and lower gates in urinary bladder is the theory of qi transformation. Even the anatomical knowledge of urinary bladder was submitted as an evidence. In general, the debate developed depending on how the differences between Huangdineijing's osmotic opinion and Nanjing's existence of the upper gate were perceived.

Leiomyosarcoma of Urinary Bladder in a Yorkshire Terrier Dog

  • A 13-year-old, intact male, Yorkshire terrier dog with oliguria and hematuria was presented to the Small Animal Clinic at the Gyeongsang National University animal medical center. There were no remarkable findings on serum chemistry values and complete blood counts. Abdominal radiographs revealed mild elevation of the descending colon at the pelvic level. On the excretory urographic images, a large filling defect was identified in urinary bladder with narrow urinary bladder wall contact and there were no remarkable findings in kidneys and ureters. The abdominal ultrasonography revealed that an irregularly marginated, heterogeneous mass in urinary bladder, central cavitary lesion and mild vascular flow were also observed. Computed tomography showed a heterogeneously attenuated mass within urinary bladder. The mass was surgically excised, and leiomyosarcoma was confirmed by histopathological examination. The patient was followed up for two months and there were no complications. This case is of interest because of the unusual findings of leiomyosarcoma of urinary bladder in a small breed dog.

Hematological differentiation of bladder rupture and complete/partial urethral obstruction in castrated Hanwoo (Korean indigenous cattle) with urolithiasis

  • This case report describes the hematological and radiological examination of urinary bladder rupture and complete urethral obstruction. associated with urolithiasis in Hanwoo. Hyponatremia, hypochloremia, azotemia, and hyperglycemia were observed in both urethral obstruction and urinary bladder rupture. However, cattle with urethral obstruction showed hyperkalemia and mild hyperglycemia, whereas cattle with bladder rupture showed marked hyperglycemia and normal potassium levels. In ultrasonography, the urethral obstruction showed a dilated bladder with a thick bladder wall. In contrast to previous literature, in this study, severe electrolyte changes such as severe hyponatremia, hypochloremia, and hyperkalemia occurred in a case of complete urethral obstruction.

Abattoir survey of bladders lesions in Korean cattle (도축 한우의 방광에 대한 병리학적 연구)

  • To examine the prevalence and type of lesions in urinary bladder of Korean cattle, a random sampling survey was performed at a Jeonju abattoir in September 2000 and January 2001. Collected urinary bladder were examined grossly and histopathologically and the patterns of disease were investigated with season and sex. Of 735 cattle(99 bulls and 246 cows in September and 141 bulls, 3 steers, and 246 cows in January) surveyed, $26.3\%$ cattle with evident lesions were found. The most common gross finding was urolithiasis of urinary bladder($23.8\%$). Other lesions included hemorrhage($5.9\%$), congestion($4.1\%$), hematuria($2.7\%$), pyuria($0.7\%$), hyperplasia($0.5\%$). Urinary calculi collected in this study contained 80 to $90\%$ of calcium oxalate and 10 to $20\%$ of struvite, with or without $20\%$ of cystine and $10\%$ of uric acid. This study on urolithiasis and pathology of urinary bladder of slaughtered Korean cattle revealed subclinical aspects and management problem in Korean beef product.

Action of Autonomic Drugs on the Urinary Bladder of Ditrema temmincki Bleeker (망상어 (Ditrema temmincki Bleeker)의 방광(膀胱)에 대(對)한 자율신경제(自律神經劑)의 작용(作用))

  • The author studied the action of autonomic drugs on the urinary bladder isolated from Ditrema temmincki Bleaker and the results obtained were summarized as follows: 1) The motility of the urinary bladder of the fish was stimulated by acetylcholine and physostigmine. The stimulating action of these drugs was antagonized by atropine. 2) The motility of the fish bladder was stimulated by epinephrine, nor-epinephrine and phenylephrine, but inhibited by isoproterenol. 3) The inhibitory effects of isoproterenol on the fish bladder was not affected by phenoxybenzamine, but blocked by propranolol. 4) The excitatory effects of phenylephrine on the fish bladder were blocked by phenoxybenzamine, but augmented by propranolol. 5) The excitatory effects of epinephrine and nor-epinephrine were reversed by phenoxybenzamine and augmented by propranolol. 6) The motility of the fish bladder pretreated with phenoxybenzamine and propranolol was not affected by isoproterenol, phenylephrine, epinephrine and nor-epinephrine. 7) It seemed that the bladder muscle of the fish had both alpha excitatory and beta inhibitory receptors. 8) The motility of the fish bladder was stimulated by nicotine and DMPP. The excitatory effects of these drugs were abolished by pretreatment with hexamethonium or atropine. 9) It is, therefore, concluded that there are ganglion cells furnished with cholinergic fiber in the bladder wall of the fish.

Cystopexy to Treat Urinary Incontinence Due to Urethral Sphincter Mechanism Incompetence in a Male Dog (수컷 개에서 발생한 요도 조임근 기능 부전에 의한 배뇨실금의 방광 고정술을 이용한 외과적 치료 증례)

  • A castrated male Yorkshire Terrier dog was presented for urinary incontinence and constipation. On physical examination, the dog showed difficult urination. There were no neurological abnormalities and no bacterial detection on urinalysis. Rectal examination revealed a regular, normal-sized prostate. Urethral catheterization was performed easily. Excretory urography and retrograde positive contrast urethrocystography showed displacement of the urinary bladder to the intrapelvic region. There was no evidence of an ectopic ureter. A tentative diagnosis of urethral sphincter mechanism incompetence accompanied with a pelvic bladder was made. Cystopexy was decided to place the urinary bladder to its normal position. The neck of the urinary bladder was anchored to the body wall and prepubic tendon using mattress sutures. Additional sutures were placed to appose the lateral part of the urinary bladder and abdominal wall. A simple interrupted suture was placed to tack the apex of the urinary bladder on the abdominal incision line. A urinary catheter was placed in the urinary bladder to provide post-operative evacuation. The catheter was removed when the dog was able to urinate with minimal straining at 3 days post-operatively. The owner reported that the dogs showed normal urination without straining at 3 days after the catheter was removed. Excretory urography revealed that the urinary bladder was located on its normal position at 2 months post-operatively. Subsequent communication with the owner by telephone revealed that the dog was in good urination at 3 years 11 months post-operatively.

Treatment of Canine Urinary Bladder Mass Using Piroxicam (Piroxicam을 이용한 개의 방광 내 종괴 치료 1례)

  • A 7-year-old, female Shih-tzu dog with clinical signs of hematuria, oliguria, stranguria was brought to the Veterinary Teaching Hospital, Chonnam National University. Ultrasonography revealed the left hydroureter, hydronephrosis and urinary bladder mass ($12.1\;{\times}\;22.0\;mm$). The dog was finally diagnosed as the secondary unilateral obstructive uropathy by urinary bladder mass. The treatment such as unilateral nephroureterectomy and chemotherapy using piroxicam were performed to the dog. After 8 months of treatment, the polyp was not detected by ultrasonography. We concluded that urinary bladder mass was cured completely.
