지역사회 성인남성에서의 하부요로증상 및 전립선비대증 의료이용과 사회경제적 요인의 관련성 (Relationship of Socioeconomic Factors with Medical Utilization for Lower Urinary Tract Symptoms and Benign Prostatic Hyperplasia in a South Korean Community)

  • Objectives : We wanted to evaluate the medical underutilization for benign prostatic hyperplasia (BPH) and lower urinary tract symptoms (LUTS) among Korean elderly men and we wanted to determine their associated factors. Methods : This study was conducted on 239 men with LUTS and 116 men with BPH who were compatible with the diagnostic criteria from a total of 641 participants. These participants were over 50 years old and they were randomly chosen in a community-based study for estimating the prevalence of BPH. Using a self-reported questionnaire, we surveyed the sociodemographics, health status, quality of life, lower urinary tract symptoms, medical utilization and reasons for not seeking treatment. Results : Only 27.6% of the men with LUTS and 31.0% of the men with BPH reported having visited a doctor for urinary symptoms. The reasons for not visiting a doctor were, in order of responses from the group with LUTS: 'considered the symptoms as a part of the normal ageing process', 'not enough time to visit a doctor', 'financial difficulty' and 'the symptoms were not severe or bothersome'. Regarding BPH, the responses were the same as those of the group with LUTS however, 'financial difficulty' placed second. Among the men with experience of visiting a doctor for urinary symptoms, 33.3% of those with LUTS and 28.1% of those with BPH were not treated. The most common reason in both groups was 'the symptoms were not severe to be treated'. On a multiple logistic regression analysis, the larger size household (odds ratio (OR) 3.03, 95% confidence interval (CI)=1.40-6.54) and an unsatisfactory quality of life related with urinary symptoms (OR 2.98, 95% CI=1.23-7.21) were associated with medical utilization in the group of LUTS. For BPH, the current employment status was related with the medical utilization (OR 2.80, 95% CI=1.10-7.11), in addition to the larger size household (OR 3.24, 95% CI=1.14-9.21). Conclusions : Many men with urinary symptoms do not visit a doctor. This medical underutilization for people with LUTS and BPH may be associated with economic status in Korea.

Ca 보충이 폐경이후 여성의 지질, Na, K 대사 및 혈압에 미치는 영향 (The Effect of Ca Supplementation on the Metabolism of Lipid, Na and K and on Blood Pressure in Postmenopausal Women)

  • This study was designed to investigate the effect of Ca supplementation of 1,000mg per day for 53 weeks on lipid, Na, and K metabolism and on blood pressure in postmenopausal women. The subjects were 12 healthy women aged from 60 to 70 years. They were divided into two groups : the placebo(control group) and the Ca supplemented(1,000 mg/day) group(Ca group). Metabolic studies were conducted twice in the 1st and the 53rd weeks. The results were as follows : Serum triglyceride, total cholesterol and LDL-choesterol levels tended to be decreased after the experiment. Serum VLDL-cholesterol lowering effect was observed with Ca supplementation(p<0.05), and also the significantly elevated HDL/(LDL+VLDL) ratio in Ca supplemented subjects whose average Na intake was as high as 4.9g per day. This phenomena was accompanied with increased Na retention and increased Na excretion in feces, but with decreased urinary Na in Ca supplemented group. However, considering much higher Na reteniton in the control group at the end of experiment(control va Ca ; 1272.3mg vs 732.9mg), Ca supplementation may have some beneficial effects on Na blance. Serum aldosterone level increased significantly in the Ca group after the exsperiment(p<0.05). With these normotensive subjects, there were no level increased significantly in the Ca group after the experiment(p<0.05). With these normotensive subjects, there were no pronounced effect of Ca supplementation on blood pressure, however, decrease in diastolic blood pressure were observed at the 14th week and end of the experiment(p<0.05). In summary, the Ca supplementation on postmenopausal Koran women appears to exert a desirables effect on blood lipid patterns related to the coronary heart diseases and to be beneficial in controlling diastolic blood pressure. Further studies with hypertensive or/and hyperlipidemic subjects are required to clarify the effect of Ca supplementation in Koreans.

Determination of Carnitine Renal Threshold and Effect of Medium-Chain Triglycerides on Carnitine Profiles in Newborn Pigs

  • Colostrum deprived, newborn pigs (N=12, $1.64{\pm}0.05kg$) were used to study the renal threshold of carnitine, and effects of emulsified medium-chain triglyceride (MCT, tri-8:0) feeding on kinetics of plasma carnitine and urinary carnitine excretion. An arterial catheter was inserted through an umbilical artery, and a bladder catheter was inserted via the urachus. Piglets were oro-gastrically gavaged with one of six carnitine levels (0, 60, 120, 180, 240, $480{\mu}mol/kg\;W^{0.75}$) with (+MCT) or without medium-chain triglycerides (-MCT) in 0.9% NaCl solution. Blood was sampled into heparinized tubes at 0, 1, 2, 4, 6, 8, 14, and 20 h after gavage, and urine was collected and pooled into 1 h or 2 h composite samples to determine free- and short-chain carnitine concentrations. Plasma from the 12 newborn piglets before gavage contained $10.6{\pm}1.2{\mu}mol/L$ free carnitine and $7.2{\pm}0.6{\mu}mol/L$ acid-soluble acyl carnitine. The renal threshold for carnitine was similar between the MCT and the +MCT group (42.6 13.1 and $46.4{\pm}2.0{\mu}mol/L$, respectively), but the correlation between plasma free carnitine and urinary excretion was altered. Plasma free carnitine linearly increased with increasing carnitine dosage (-MCT group, $R^2=0.95$, p<0.001; +MCT group, $R^2=0.91$, p<0.001), but was decreased by 50% when medium-chain triglycerides were fed. The peak in plasma free carnitine concentration was depressed by medium-chain triglycerides feeding also. Therefore, the plasma and urinary short-chain/free carnitine ratio of the +MCT group was increased by 100% and 40%, respectively (p<0.01). Feeding of medium-chain triglycerides may delay plasma carnitine elevation via altering the kinetics of absorption. Similarly, the plasma and urinary short-chain/free carnitine ratio were affected by interaction between medium-chain triglycerides and time (p<0.01). The present study suggests that an oral carnitine dose over $480{\mu}mol/kg\;W^{0.75}$ may be needed to reach the free carnitine renal threshold within a short period, especially when provided together with medium-chain triglyceride.

광주지역 성인의 나트륨 배설량과 비만의 관계 (Association between Sodium Excretion and Obesity of Adults in Gwangju)

  • Purpose: The aim of this study was to analyze the association between sodium excretion and obesity for healthy adults in the Gwangju area. Methods: The participants included 80 healthy adults aged 19 to 69 years in Gwangju. The dietary intake and sodium excretion were obtained using the 24-hour recall method and 24 hour urine collection. The participants were classified into two groups according to the amount of urinary sodium excretion: (${\leq}141.75mmol/dL$, > 141.75 mmol/dL). Results: After adjusting for sex, age, smoking history, and income, the high excretion of sodium group was significantly higher for weight, body mass index, body fat mass, percent body fat, visceral fat area (VFA), waist circumference, hip circumference, and WHR. The energy and nutrients intake were significant after adjusting for sex, age, smoking history, and income. The LSE group had a significantly higher fat intake and Na/K intake ratio. The HSE group had significantly higher fiber intake, and K intake. As the amount of urinary sodium excretion increased, the risk of obesity before correction was 3.57 (95% CI: 1.13-11.25) times greater, and the risk of obesity of T3 increased significantly by 3.33 times (95% CI: 1.05-10.59). After correcting for sex and age, the obesity risk of T2 increased significantly by 4.23 times (95% CI: 1.11-16.06), and after correcting for sex, age, smoking history, and income, the obesity risk of T2 increased significantly by 6.81 times (95% CI: 1.44-32.19) the risk of obesity. Conclusions: An association exists between sodium excretion and obesity in Korean adults. In this study, the high excretion of sodium group was obese and the risk of obesity was higher than the low excretion of sodium group.

한국 미취학 아동의 Sodium과 Potassium의 계절별 영양대사에 관한 연구 -기타 전해질 : Calcium, Phosphorus 및 Magnesium을 첨가하여- (The Seasonal on Sodium and Potassium Intakes, and Their Metabolisms of Preschool Children in Korea -Add Other Electrolyes : Calcium, Phosphours and Magnesium-)

  • The purpose of this research was to determine the relationship of the dietary nutrients to blood pressure among preschool children in Seoul and to concurrently study the effect of seasonal variance on the aforementioned relationship. The subjects of the study consisted of 203 preschool children aged four to six years. Anthropometric measurements of height, weight, pulse rate and blood pressure, urinary excretion of five cations(Na, K, Ca, P, Mg), creatinine and urea nitrogen and dietary questionaires concerning sodium, potassium calcium and phosphorus were taken during the two periods of summer(Aug. 1986)and winter(Feb, 1987). The results obtained are summarized as follows: 1) The daily urinary excretion of five cations, creatinine and urea nitrogen is summer and winter was as follows; The sodium content was 57.8 mEq in the summer and 59.4 mEq in the winter ; potassium 20.4 mEq and 23.0 mEq, respectively ; calcium, 5.5 mEq and 3.6 mEq, respectively ; and phosphorus, 27.4 mEq and 19.9 mEq, respectively. Only calcium and phosphours excretions in the urine showed significant differences per season(p<0.05). 2) The average dietary intake per day of sodium was 2349mg in the summer and 2155mg in the winter ; potassium consumption was 1425mg in the summer and 1448mg in the winter ; intake of calcium was 472mg in the summer and 500mg in the winter ; and phosphours consumption was 642mg in the summer and 634mg in hte winter. The sodium-to-potassium consumption ratio 1.6 and 1.5, respectively, in the summer and in the winter and the calcium-to-phosphorus ration was 0.7 in the summer and 0.8 in the winter. The dietary calcium intake showed significant differences between the seasons. 3) The principal source of sodium consumption among preschool children was from seasoning-including talbe salt, soy sauce and instant sauce-which accounted for higher then 45% of the sodium intake in both seasons. The main source of potassium was frutis and vegetables which accounted for 29.6% of the potassium intake in the summer and 25.7% in the winter. 한국인임신부(韓國人姙娠婦)의 Sodium과 단백질섭취량(蛋白質攝取量) 및 대사(代謝)에 관(關)한 연구(硏究) (A Study on the Sodium and Potassium Intakes and Their Metabolism of the Pregnant Women in Korea)

  • The purpose of this study was to investigate sodium intake and protein metabolism for pregnant women in Korea. The relationship between salt consumption and hypertension, electrolyte changes in pregnancy, and the tendency of weight gain of pregnant women were also examined. The dietary survey and twelve or twenty-four hour's urine collection was performed in this study. The subjects were pregnant women who came to gynecology clinic for prenatal care from July to August in 1984. The results were as follows: 1) Intakes of calorie and iron of Korean pregnant women were 2,036 Kcal and 16mg per day per person. The calorie composition consists of 65% carbohydrate, 16% protein, and 19% lipids. The average level of all nutrients was about 87% of Korean Recommended Dietary Allowances. 2) The sodium intakes in pregnant women estimated by dietary survey was 318.6 mEq Na (7,966 mg Na), which is equivalent to 20.2 g NaCl. 3) Daily sodium excretion was 112.5 mEq per liter, that is equal to 147.4 mEq. Potassium excretion was 24.95 mEq per liter and creatinine excretion was 594.0 mg per day. 4) There were statistically significant correlations between sodium intake and the excretions of sodium, potassium and creatinine. The blood pressure was positively correlated with sodium intakes. As sodium and potassium ratio was increased, systolic blood pressure and diastolic blood pressure were also significantly increased 5) Urinary nitrogen and creatinine ratio known to be reliable index for nutritional status of protein was 5.5 : 1. These values were comparable to the values in American pregnant women. 6) The weight gain during the pregnancy was examined. There was little weight gain during the first trimester, because of nausea, vomit and decrease of appetite. The weight gain during the second trimester was 4.9Kg, and 11.1Kg for third trimester.

3개월 미만 요로감염 영아에서 중증 방광 요관 역류의 예측인자 (Prediction of High Grade Vesicoureteral Reflux in Infants Less than 3 Months with Urinary Tract Infection)

  • 목 적 : 최근 I, II 등급 방광 요관 역류의 임상적 중요성이 적어지면서 요로감염 영아 모두에게 침습적인 VCUG를 시행하여야 하는지에 대한 의문이 제기되어 있다. 요로감염이 있는 3개월 미만의 영아를 대상으로 III 등급 이상의 중증 방광 요관 역류에 대한 예측인자를 알아보고 VCUG의 시행 여부에 대한 임상적인 근거를 제시하고자 하였다. 방 법: 2004년 1월부터 2007년 9월까지 가천의과대학교 병원에 입원해서 요로감염으로 치료받은 3개월 미만의 환아 중 초음파 검사와 VCUG를 시행받은 환아를 대상으로 하였다. 후향적인 의무기록의 검토를 통하여 I 군(III-V 등급의 방광 요관 역류)과 II 군 (정상 혹은 I, II 등급의 방광 요관 역류) 간의 임상적 지표와 영상 검사 결과를 비교 검토하였다. 초음파의 민감도와 특이도 및 양성예측도와 음성예측도, 그리고 위험도와 우도비를 구하여 중증 방광 요관 역류에 대한 진단적 가치를 알아보았다. 결 과: 총 54명(남아 41명, 여아 13명) 의 대상아 중에서 I군이 14명, II군이 40명이였다. I 군에서 CRP가 높았고(6.11$\pm$5.18와 3.27$\pm$3.45, P=0.025) 비정상적인 초음파 소견이 많았지만(71.4%와 22.5%, P=0.002) 관련 인자를 보정한 후에는 초음파만이 통계적인 유의성이 있었다(P=0.002). 원인균, 균혈증 및 DMSA 이상소견은 두 군간에 유의한 차이가 없었다. 중증 방광 요관 역류에 대한 초음파의 민감도는 71.4%(41.9-91.4), 특이도는 77.5%(61.5-89.1), 위험도는 6.9(1.58-30.41)이었고 음성예측도는 88.6%(73.2-96.7), 음성우도비는 0.37(0.16-0.86)을 나타내었으나($\alpha$=0.05, 검정력 94%) III 등급의 방광 요관 역류4명은(28.6%)은 정상적인 초음파 소견을 보였다. 결 론 : 3개월 미만의 요로감염이 있는 영아에서 중증 방광 요관 역류 에 대해 VCUG를 대체할만한 예측인자를 규명하지 못했다. 따라서 모든 요로감염 영아(3개월 미만)에서 VCUG의 시행이 필요할 것으로 생각된다.

정상 한국인 및 저식염식인에서 본 이뇨제투여후의 Renin-Angiotensin-Aldosterone 계 (Acute Changes of R-A-A system following Lasix Administration in Normal Korean and Subjects with Low Sodium Intake)

  • This study was carried out to investigate the acute changes in R-A-A system following lasix administration, and to evaluate the materials in plasma R-A-A system and electrolytic excretion every 30 minutes for 2 hours after lasix administration with normal high sodium Korean food, moderate sodium restriction, and severe sodium restriction, and it was concluded as followed; 1. Plasma renin activity, angiotensin II concentration, and aldosterone concentration elevated in course of time after lasix administration with high sodium Korean food, but the R-A-A system takes insignificant part because of the increasing rate was so slight. 2. Although the increasing rate of plasma renin activity reached lower levels, angiotensin II and aldosterone concentration were significantly increased after lasix administration with moderate sodium restriction. 3. It was observed that higher rise in aldosterone concentration following lasix administration during severe sodium restriction than when moderate sodium restriction. 4. Urinary sodium and potassium excretion during two hours after lasix administration showed decrease as little as the amount of sodium intake, but K/Na excretion ratio showed increase with small amount of sodium intake because of the decreasing rate of potassium was low value. 5. Sodium excretion after lasix administration reached more than 1.5 times of sodium intake, even though R-A-A reaction showed significantly. 6. As our results showed, R-A-A reaction following acute diuresis was insignificant with high sodium Intake, the increasing ratio of aldosterone concentration showed high rise compare with of plasma renin activity as little as the amount of sodium intake, and the participated rate in sodium reabsorption of R-A-A system was increased.

일차성 단일증상성 야뇨증 환아에서 소변내로의 요칼슘배설에 대한 연구 (Urinary Calcium Excretion in Children with Primary Monosymptomatic Nocturnal Enuresis)

  • 목적 : 야간다뇨증과 요실금을 가진 아동에서 고칼슘뇨증이 원인적 요인의 하나로 관여할 수 있다는 보고가 있으며, 일부 연구자들은 야뇨증을 가진 아동들을 진단함에 있어서 소변내로의 칼슘배설의 정도를 측정할 것을 권유하였다. 그러나 다른 연구자들은 야뇨증과 소변내로의 칼슘배설 사이에 의미있는 연관성을 입증하지 못했다. 이에 저자들은 일차성 단일증상성 야뇨증을 가진 환아들을 대상으로 소변내로의 칼슘 배설을 측정하여 야뇨증과의 관련성을 조사하였다. 방법:야뇨증으로 처음 진단 받은 5세 이상의 환자 36명을 대상으로 하였다. 혈장 칼슘농도를 측정하였으며, 단회뇨를 이용한 소변검사를 통해서 소변 삼투압, Na, 요소 및 Ca/Cr 농도비를 동시에 측정하였다. 24시간 소변의 채집은 오전 8시부터-오후 8시, 오후 8시부터-다음날 오전 8시로 두 번에 나누어 채집하였으며 각각의 소변 20-40 mL를 이용하여 삼투압과 Ca/Cr농도 비를 산출하였다. 고칼슘뇨증의 정의는 야뇨증 진단 당시에 주간(오전 8시-오후 8시)과 야간(오후 8시-오전 8시)으로 나누어 채집한 단회성 소변검사상에서 칼슘/크레아티닌 비가>0.2 이상이면서 24시간뇨 검사상 칼슘의 배설량이 ${\geq}4mg/kg$ 이상인 경우로 정의하였다. 결과: 36명의 환자 중에서 고칼슘뇨증에 해당하는 경우는 6명으로 16.7%의 빈도를 보였다. 고칼슘 뇨군에서의 단회뇨를 이용한 칼슘/크레아티닌 비는 주간뇨 검사에서는 $0.23{\pm}0.10$, 야간뇨 검사에서는 $0.33{\pm}0.10$로 야간뇨 검사에서 높았으나, 통계적인 유의성은 없었다. 24시간뇨 검사상의 평균 칼슘 배설량은 체중 (kg)당 8.95 mg이었다. 혈청 칼슘 농도와 주간/야간 소변량의 비도 고칼슘뇨군과 정상 칼슘군 사이에서 차이가 없었다. 또한 소변의 osmolality, Na 농도, 요소 농도 등도 양군사이에 유의한 차이가 없었다. 또한 주간이나 야간에 채집된 뇨에서 각각의 해당 기간 동안의 소변량과 칼슘/크레아티닌 비 그리고 칼슘/크레아티닌 비와 요중 Na농도, 요소농도, osmolality 간에는 유의한 상관관계는 없었다. 결론 : 야뇨증 환자의 일부에서 관찰되는 고칼슘 뇨증이 야간 다뇨에 의한 일차성 단일증상성 야뇨증의 직접적인 원인이 아니라고 생각한다.

철 보충이 흰쥐의 무기질 이용에 미치는 영향 (Effect of Iron Supplementation on Mineral Utilization in Rats)

  • 본 연구에서는 철의 섭취수준이 증가함에 따라 다양한 무기질의 이용변화를 알아보기 위하여 흰쥐를 대상으로 적정수준, 적정수준의 2배와 4배의 철을 6주간 공급한 후 칼슘, 마그네슘, 나트륨, 칼륨, 철, 구리, 아연의 혈청 수준 및 섭취량과 배설량에 의한 평형상태를 비교, 분석하였으며, 그 결과를 요약하면 다음과 같다. 사료 섭취량, 체중 증가량 및 사료효율은 철 보충에 따른 유의한 차이가 없었다. 철 보충에 따른 혈청 철과 아연 함량은 각 군별 차이를 보여 철 함량은 2배 공급군이 유의하게 낮았고 아연 함량은 4배 공급군이 다른 두군에 비해 유의적으로 높았다(p<0.05, p<0.05). 철 보충에 따라 철 섭취량이 유의적으로 증가한 것(p<0.01)을 제외하고 그 밖의 무기질 섭취량은 유의한 차이가 없었다. 소변 중 무기질 배설량은 철 보충에 따라 칼슘, 나트륨, 철, 구리, 아연이 각 군별 유의한 차이를 보였다(p<0.01, p<0.01, p<0.001, p<0.05, p<0.01). 대변 중 무기질 배설량은 철 보충에 따른 유의한 차이가 없었다. 1일 무기질 보유량은 철 보충에 따라 나트륨과 철이 유의한 차이를 보여(p<0.05, p<0.01) 철 보충이 증가할수록 이들 무기질의 보유량이 높게 나타났다. 그러나 무기질 흡수율은 철 보충에 따라 유의한 차이가 없었다. 이상의 연구결과를 종합할 때 요구량의 2배와 4배의 철 보충에 따라 무기질의 겉보기 흡수율은 유의한 차이가 없었다. 그러나 칼슘, 철, 구리, 아연의 소변중 배설량은 철 보충에 따라 유의적으로 감소함으로써 장기적인 철 보충이 이루어진다면 이와 같은 무기질의 체내 보유량이 감소함에 따라 영양상태가 저하될 수 있을 것으로 보여진다. 이와 같은 결과는 다양한 무기질의 평형상태를 유지하기 위 해서는 무기질 보충제를 이용하는 것보다 균형 잡힌 영양섭취나 일상식사를 통해 적정한 섭취수준을 유지하는 것이 중요하다는 것을 제시해주며, 앞으로 이에 대한 영양지도와 지속적 인 연구가 요구된다.
