뇌졸중 환자의 물리치료 결과에 관한 연구 (A Study on Results of Physical Therapy of Storke Patients)

  • The authors studied 74 patients with cal therapy Kosin University Medical March 1994 through February 1995. The results were, as follows: 1. Average age of the subject patients was 59.3 years old, their age ranged from 20 to 84 years old, and the incidence between male and female was 1:1.24 2. In respect to the economy level the middle class revealed the highest incidence, 40(54%) in 74cases. 3. The cause of injury revealed the highest incidence in hypertension 52 patients(72 %), more than all revealed the highest incidence 23 patients(44 %) in 60th years old. 4. The form of injury revealed the highest incidence in infarction 36 patients(49 %), more then all revealed the highest incidence 13 patients (36 %) in 60th years old. 5. As for the region cerebral artery of infarction patients, middle cerebral artery was the highest region of injury 72 %. 6. As for the physical paralysis side, the left side showed higher incidence 40 patients(54 %) compared to the right side 31 patients (42 %). 7. The period of therapy was 56 days, in average extending from 7 days to II months, nore then all there were the most numerous patients within 3 weeks 28 patients(38 %). 8. The upper side of the upper lims had improved 67 patients(91 %), the lower side 48(65 %). In the cases of the lower limb, the upper side had improved 70(95 %), the lower side 58(78 %). In short, patients had improved more the lower limb than the upper limb, the upper side(elbow and shoulder part) then the lower side(wrist and finger) of the upper limb. In the cases of the lower limb, patients had improved more the upper side (knee and hip part) then the lower side(foot and ankle) 9. In the cases of patients can be walking among subject patients 55(74 %) and patients cannot be walking 19 (26 %), In the cases of patients utilized assistant apparatus 34 (62 %) among self-walking, completely patients can be self-walking 21(28 %).

대뇌 피질 경색으로 인한 하지 단마비 환자 한방치험 2례 (2 Cases of Lower Limb Monoplegia due to Brain Cortical Infarction)

  • Monoplegia is the paralysis of either the upper or lower limb. Monoplegia is commonly caused by an injury to the cerebral cortex; it is rarely caused by an injury to the internal capsule, brain stem, or spinal cord. Most cerebral cortex is derived from the occlusion of a brain cortex blood vessel due to thrombus or embolus. According to motor homunculus, lower limb monoplegia occurs from limited damage to the most upper part of the primary motor area(Brodmann's area 4, located in precentral gyrus). Clinically, lower limb monoplegia due to brain cortical infarction is commonly misunderstood as monoplegia due to spinal injury because the lesion is situated at the most upper part of precentral gyrus. We had many difficulties in finding lesion on brain CT, but we diagnosed two patients correctly by using an MRI, who have lower limb monoplegia due to brain cortical infarction oriental treatment.

Effect of Motor Imagery Training on Somatosensory Evoked Potentials and Upper Limb Function in Stroke Patients

  • Background: Motor imagery is the mental representation of an action without overt movement or muscle activation. However, few previous studies have demonstrated motor imagery training effects as an objective assessment tool in patients with early stroke. Objective: To investigate the effect of motor imagery training on Somatosensory Evoked Potentials (SSEP) and upper limb function of stroke patients. Design: A quasi-experimental study. Methods: Twenty-four patients with stroke were enrolled in this study. All subjects were assigned to the experimental or control group. All participants received traditional occupational therapy for 30 minutes, 5 times a week. The experimental group performed an additional task of motor imagery training (MIT) 20 minutes per day, 5 days a week, for 4 weeks. Both groups were assessed using the SSEP amplitude, Fugl-Meyer assessment of upper extremity (FMA UE) and Wolf motor function test. Results: After the intervention, the experimental group showed significant improvement in SSEP amplitude and FMA UE than did the control group. Conclusion: These findings suggest that the MIT effectively improve the SSEP and upper limb function of stroke patients.

Effect of Paretic Side and Non-paretic Side Arm Training on Trunk Control and Upper Limb Functions in Stroke Patients

  • Background: Stroke patients usually have arm weakness, which affects trunks and arms. Objective: To investigate the effects of paretic side and non-paretic side arm training on trunk control and upper limb functions. Design: Randomized Controlled Trial (single blind). Methods: Twenty patients with stroke in hospital were enrolled in the study. Twenty subjects were randomly assigned to paretic side arm training group (PATG, n = 10) or non-paretic side arm training group (NATG, n = 10). Trunk impairment scale (TIS) was used for trunk control, and box and block test (BBT) was used for upper limb function. Training was conducted for 4 weeks. Results: PATG showed significant difference in TIS (static balance, dynamic balance, coordination, total score) and BBT. NATG showed significant differences in static balance, and dynamic balance and total score except for coordination and BBT. PATG also showed a more significant difference in BBT and coordination and total score than NATG. Conclusions: The arm training performed on the paretic side are more effective than those performed on the non-paretic side in improving both upper limb function and trunk control in stroke patients.

어깨의 움직임을 중심으로 한 상지재활로봇 NREX의 착용감 개선 (Improved Wearability of the Upper Limb Rehabilitation Robot NREX with respect to Shoulder Motion)

  • NREX, an upper limb exoskeleton robot, was developed at the National Rehabilitation Center to assist in the upper limb movements of subjects with weak muscular strength and control ability of the upper limbs, such as those with hemiplegia. For the free movement of the shoulder of the existing NREX, three passive joints were added, which improved its wearability. For the flexion/extension movement and internal/external rotation movement of the shoulder of the robot, the ball lock pin is used to fix or rotate the passive joint. The force and torque between a human and a robot were measured and analyzed in a reaching movement for four targets using a six-axis force/torque sensor for 20 able-bodied subjects. The addition of two passive joints to allow the user to rotate the shoulder can confirm that the average force of the upper limb must be 31.6% less and the torque must be 48.9% less to perform the movement related to the axis of rotation.

Electromyographic Analysis of Upper and Lower Limb Muscles during Gardening Tasks

  • Movements of the upper and lower limb muscles during five common gardening tasks were analyzed by using electromyography (EMG). Twenty adults aged in their twenties (mean age, $24.8{\pm}2.4$ years) were recruited. On two separate occasions, subjects visited a garden plot to perform digging, raking, troweling, weeding, and hoeing; all tasks were performed three times with 20 s intervals for each trial. To measure muscle activation during the five gardening tasks, surface EMG was used. Bipolar surface EMG electrodes were attached to eight upper limb muscles (bilateral anterior deltoid, biceps brachialis, brachioradialis, and flexor carpi ulnaris) or eight lower limb muscles (bilateral vastus lateralis, vastus medialis, biceps femoris, and gastrocnemius) on both sides of the body, for a total of 16 muscles. During the five tasks, photographs were taken of movement phases using a digital video camera. The right flexor carpi ulnaris and brachioradialis showed higher activation than the other upper and lower limb muscles measured during the tasks. All 16 upper and lower limb muscles were actively used only during digging. According to movement analysis of each activity, digging was classified into four movement phases, whereas raking, troweling, weeding, and hoeing each were divided into three movement phases. In each activity, there were high-impact phases in terms of muscle activation; the flexor carpi ulnaris and brachioradialis were identified as major muscles in each impact phase. This analysis may be used to generate biomechanical profiles of gardening tasks for practitioners when designing efficient gardening interventions for physical health or rehabilitation.

팔걸이가 편마비 환자의 보행에 미치는 영향 (The effect of arm sling during hemiplegic gait)

  • ■ Objectives The aim of study is to analyze the change of gait pattern by arm sling in a hemiplegic patient. ■ Methods We analyzed the change of gait pattern under three conditions using Treadmill Gait analysis equipment(Zebris Co.Ltd FDM-T) First, the patient didn't have arm sling on her upper limb, second, the patient have arm sling on her affected upper limb, third, patient have arm sling on her unaffected upper limb. ■ Results In terms of spatiotemporal gait values, swing phase, step time, step length of unaffected lower limb increased. Furthermore, stride time and stride length also increased when the patient had arm sling on her unaffected upper limb. In terms of displacement of Center of pressure(CoP), anterior/posterior position and lateral symmetry of CoP increased. Furthermore, lateral symmetry of decreased when the patient had arm sling on her affected upper limb. ■ Conclusion Arm sling applied on affected side would be advantage to gait improvement in hemiplegic patients.

작업기반 양측성 상지 훈련과 경 두개 직류 전류 자극이 뇌졸중 환자의 상지 기능에 미치는 영향 (Effects of Occupation-Based Bilateral Upper Extremity Training and Transcranial Direct Current Stimulation Upper Limb Function in Stroke Patients)

  • 본 연구의 목적은 작업기반 양측성 상지 훈련과 경 두개 직류 전류 자극이 뇌졸중 환자들의 상지기능에 미치는 효과를 알아보는 것이었다. 만성 뇌졸중 환자를 대상으로 작업기반 양측성 상지훈련과 경두개 직류전류자극을 실시한 실험군 13명, 작업기반 양측성 상지훈련만 실시한 대조군 13명으로 나누어 총 4주간, 50분씩, 주 5회 실시하였으며, 캐나다 작업수행 평가(Canadian Occupational Performance Measure;COPM), 가속도계 (Accelerometer), 퍼글 마이어 검사(Fugle-Meyer Assessment;FMA), 운동 홛동 척도(Motor Activity Log;MAL)를 사용하여 평가하였다. 연구 결과, 실험군과 대조군은 작업 만족도와 수행도, 환측과 건측의 사용량, 상지 기능의 회복과 움직임의 질에서 모두 유의미한 개선을 보였으며, 특히, 환측 사용량에서 실험군은 대조군 보다 유의한 차이를 나타냈다. 따라서 작업기반 양측성 상지 훈련과 경 두개 직류 전류 자극의 결합 중재가 뇌졸중 환자의 상지기능회복에 긍정적인 효과를 보임을 알 수 있었다.

뇌졸중 환자의 견관절 통증과 운동 기능 및 삶의 만족도와의 관계 (The Shoulder Pain after Stroke and the relationship with Motor Function, and Quality of Life)

  • Purpose : To assess the relationship between post-stroke shoulder pain, motor function, and pain-related quality of life(QOL) Methods : Volunteer sample of 62 chronic stroke survivors with post-stroke shoulder pain and glenohumeral subluxation. The patients answered the question in shoulder pain with the Brief Pain Inventory question 12 (BP1-12), Pain-related Quality of life(BPI-23). Therapists measured the performance of combined upper-limb movement including the hand-behind-neck(HBN), hand-behind-beck(HBB) maneuver, added passive pain-free shoulder external rotation range of motion, and Modified Ashworth Scale(MAS) score of the elbow flexors. Physical performance assessments were used to measure basic activity daily living(Modified Barthel Index-self care, MBI-S/C), motor function of upper limb(Fugl-Meyer Upper/Lower Extremity, FM-U/E). Results : Stepwise regression analyses indicated that post-stroke shoulder pain is associated with the BPI 23, but not with the FM-U/E, MBI-S/C. Thus, the presence of shoulder pain is more important predicting pain-related QOL than its degree in predicting motor function of upper limb and basic activity daily living. Conclusion : Post-stroke shoulder pain was associated with reduced quality of life related to pain. The pain was not associated with the motor function of upper limb and basic activity daily living. The result imply that management of shoulder pain & anatomical position of shoulder joint after stroke should be emphasized. This provides a further incentive to develop effective rehabilitation prevention and treatment strategies for post-stroke shoulder pain.

거리환경미화원의 근골격계 증상의 특징 및 인간공학적 평가 (Ergonomic Workload Evaluation and Musculo-skeletal Symptomatic Features of Street Cleaners)

  • The aim of this study was to evaluate the musculo-skeletal symptomatic features of municipal sanitation workers and to compare differences of the musculo-skeletal symptoms by work types. We conducted descriptive cross-sectional survey concerning the musculo-skeletal symptomatic features of 315 street cleaners in Seoul and GyeongGi Province, Korea, during 2 weeks of September 2006. Questionnaires were consisted of general characteristics, occupational characteristics and musculo-skeletal symptoms. And we observed their works and evaluated their movement and posture by REBA. For work types, tools and subjective physical work loading, there were statistical difference whether or not NIOSH symptom positive on upper limb. Musculo-skeletal symptoms on upper limb were claimed from 43.2% of street cleaners, more than in 32.4% of cleaners. Musculo-skeletal symptoms on upper limb were reported higher in the group who felt their subjective physical work loading severe than in the otherwise group. The REBA results were over 'high risk stage' in street cleaners. Street cleaners had more repetitive motions than solid waste collectors on upper limbs. Works according to the criteria of musculoskeletal burdened work by the Ministry of Labor results were over 'high risk stage' of No. 2 and No. 4 in street cleaners. We found that street cleaners complained musculo-skeletal symptoms on upper limb more than solid waste collectors. This study suggests that it is necessary for street cleaners to carry out the prevention program of musculo-skeletal diseases. For street cleaner, the measure plans such as job rotation, automatic street sweepers, reducing work hours are helpful in preventing musculoskeletal symptoms on the upper limb.
