The Impact of Cervical Stretching Exercise and Cervical Traction on Cervical Pain and Muscle Activity among Patients with Cervical Hypolordosis

  • The purpose of this study is to provide fundamental clinical data for the treatment plan and rehabilitation of patients with cervical hypolordosis by comparing the cervical headache and muscle activity after cervical stretching exercise and cervical traction, which are generally applied to patients with cervical hypolordosis. The research subjects included 20 patients without gender division who were diagnosed with cervical hypolordosis. After applying cervical stretching exercise and cervical traction for six weeks, cervical headache and the activity of the muscles around the cervical vertebra(upper trapezius muscle, sternocleidomastoid muscle, splenius capitis muscle, and anterior temporal muscle) were investigated and the following results were obtained. In a comparison of the within group intervention effects of the two groups, cervical pain statistically significantly decreased in the cervical stretching exercise group. According to the results of analyzing the change of muscle tension in the upper trapezius muscle, both the cervical traction group and showed a statistically significant within group difference in the left and right side. According to the results of analyzing the change in the muscle tension of the splenius capitis muscle, both groups showed a statistically significant within group difference in the left and right side. In a between-group comparison, a statistically significant difference in the right side was observed. These results confirm that cervical vertebra traction and cervical stretching exercise decrease the cervical headache and muscle activity of the upper trapezius muscle and the splenius capitis muscle among patients with cervical hypolordosis.

Effects of Hand Positions on Electromyographic Activity in Scapulothoracic Muscles During Push-Up Plus

  • This study was designed to investigate the effect of different hand positions on scapulothorcic muscle activities during push-up plus exercises. Fourteen healthy males performed push-up plus exercises under three conditions (neutral, $90^{\circ}$ internally rotated, and $90^{\circ}$ externally rotated hand positions), during which the activities of the serratus anterior, pectoralis major, and upper trapezius muscles were recorded using surface electromyography. The statistical significance at three different hand positions was tested by repeated one-way ANOVA. The mean activities of the serratus anterior increased and the mean activities of the pectoralis major decreased in the order of neutral hand position, internally rotated hand position, and externally rotated hand position. There was a significant difference during push-up plus between neutral and externally rotated hand positions as well as in the serratus anterior/pectoralis major activity ratio (p<.0.5). However, no significant differences were found in the activity of the upper trapezius muscle or the serratus anterior/upper trapezius activity ratio. We suggest that the push-up plus exercise performed in the externally rotated hand position could a beneficial strategy for selective strengthening of the serratus anterior muscle, while minimizing the activity of the pectoralis major muscle.

Effects of Neck Stabilizing Exercise on Muscle Characteristics, Muscle Activity and Posture in Patients with Cervicogenic Headache (목 안정화 운동이 경추성두통 환자의 근육특성과 근활성도 및 자세에 미치는 영향)

  • Purpose : The purpose of this study was to provide an effective method of exercise therapy for patients with cervicogenic headache. Methods : The subjects were divided into the following two groups according to the intervention received: cervical stretch exercise (n=15, control group) cervix-stabilizing exercise (n=15, experimental group) tone (Hz) and stiffness (N/m) of the suboccipital and upper trapezius muscles were measured. T1 slope angle and neck tilt angle were measured. After the exercise program intervention, a greater amount of change in muscle tone and stiffness of suboccipital and upper trapezius muscles was found in the experimental group, as compared to the control group. Greater amount of change in posture was found in the experimental group, as compared to the control group (p<0.05). Results : After the exercise program intervention, a greater amount of change in muscle tone and stiffness of suboccipital and upper trapezius muscles was found in the experimental group, as compared to the control group. Greater amount of change in posture was found in the experimental group, as compared to the control group (p<0.05). Conclusion : The neck-stabilizing exercise were shown to be effective in decreasing the tone of the cervical muscles by stabilizing the cervical bone and improving muscle activity, and in improving the posture by decreasing muscle tone and stiffness.

A Comparison of Myofascial Release and Ultrasound in Patients with Myofascial Pain Syndrome on Neck (경부근막동통증후군 환자에게 근막이완요법과 초음파치료의 비교)

  • Objectives : The purpose of this study is to compare the effectiveness between myofascial release technique and ultrasound in subjects with myofascial pain syndrome. Method : Twenty subjects with myofascial pain syndrome were randomly assigned to a myofascial release technique group(n=10) or an ultrsound group(n=10). The Cervical Range of Motion(C-ROM) instrument was used to measure cervical range of motion. Electromyography(EMG) was used to measure muscle power sternocleidomastoid and upper trapezius. Result : The result of this study shows there were no significant differences in ROM increase and muscle power increase between the myofascial release technique group and the ultrasound group. In the group with Myofascial Release Technique, the average score was $61.40{\pm}11.12$ in SCM flexion of C-ROM and $127.87{\pm}4.24$ in Rt. upper trapezius extension of EMG. In the group with ultra sound, the average score was $68.60{\pm}10.02$ in SCM flexion of C-ROM and $131.50{\pm}5.45$ in Rt. upper trapezius extension of EMG. Conclusion : The results suggest that there was no significant difference between two therapeutic techniques.

Effect of Head and Leg Positions on Trunk and Upper Trapezius Muscle Activities during Plank Exercise (플랭크 운동 동안에 머리와 다리 위치가 몸통근육과 위등세모근의 근활성도에 미치는 영향)

  • Purpose: This study investigated the muscular activity of abdominal muscles during a variety of plank exercises following changes in the leg and head positions. Methods: Thirty healthy individuals participated in this study. They performed six variations of plank exercises, including three changes in head position and two changes in leg position. Each plank was defined as head neutral-leg neutral, head up-leg neutral, head down-leg neutral, head neutral-leg wide, head up-leg wide, and head down-leg wide. During the plank excises, the muscle activities of the rectus abdominis, internal oblique, erector spinae, and upper trapezius were measured. Results: The head down position significantly increased the rectus abdominis activity compared to other head positions (p<0.05). On the other hand, the upper trapezius muscle activity was significantly higher with the head up position compared to other head positions (p<0.05). Regardless of head positions, both the rectus abdominis and internal oblique muscles were significantly activated with leg wide position compared to the leg neutral position (p<0.05). Conclusion: Head and leg positions could change the muscular activities of abdominal muscles during plank exercises. For example, the head down position is effective for activating the rectus abdominis while the leg wide position could be advantageous for enhancing the internal oblique and rectus abdominis.

Effects of Cranio-Cervical Flexion Exercise with Visual Feedback of Forward Head Posture on Muscle Activity (시각적 피드백을 병행한 머리-목 굽힘 운동이 팔의 움직임 동안 전방머리자세의 근 활성도에 미치는 영향)

  • Purpose : We aimed to investigate the effect of cranio-cervical flexion exercises(CCFE) with visual feedback(VF) on the muscle activity of the upper trapezius in forward head posture (FHP) and whether deficits in proprioception affect the changes in muscle activity. Methods : Twenty subjects with FHP were assigned to one of 2 groups according to deficits in proprioception. The muscle activity of the upper trapezius during arm movement under three exercise conditions (resting, CCFE, and VF + CCFE). Repeated-measures analysis of variance was used to compare differences in muscle activity according to the exercise conditions between the groups and to analyze the interactions between groups and conditions. Results : Significant differences were observed in muscle activity according to the exercise condition (p<.05), with no significant differences between the groups. The muscle activity of the upper trapezius was significantly different between the resting and VF +CCFE conditions (p<.05), with no significant difference between the resting and CCFE conditions (p>.05). Conclusion : The results of this study showed that the CCFE combined with VF are an effective intervention for FHP to train deep muscles selectively. In addition, the loss of proprioceptive sensation is not related to changes in muscle activity during exercises.

Effects of Shoulder Strength Exercise and Cervical Mobilization to Neck and Shoulder Chronic Pain Patients Pain and Muscle Tension (목과 어깨 만성 통증 환자에게 어깨 강화 운동과 목뼈 관절 가동술이 통증 및 근긴장도에 미치는 영향)

  • Background: Approximately 30% of people suffer from chronic neck and shoulder pain. Chronic neck and shoulder pain arise due to the exposure to continuous low loading and monotonous work. This is a common musculoskeletal disorder in a society. As physical therapists, we should give appropriate treatment to these people. Methods: A total of 26 patients with chronic neck and shoulder myalgia were randomly allocated into two experimental groups. First, 13 patients received shoulder strength training, while the other 13 patients received cervical mobilization. Outcome measures included bilateral pressure pain threshold by using an algometer at upper trapezius, visual analogue scale (VAS) to express their pain scale, and muscle tension by using myoton pro device pre-intervention and after the final treatment. Results: The VAS and muscle tension in upper trapezius significantly decreased (p<.05) in both groups; however, no differences between two groups were observed (p>.05). The pressure pain threshold in upper trapezius significantly increased (p<.05) in both groups; yet, again, the differences between the groups did not reach statistical significance (p>.05). Conclusion: As methods of treatment, shoulder-specific strength training and cervical mobilization can be appropriate approaches to the treatment of neck and shoulder musculoskeletal disorder.

The Effects of Muscle Activation of Upper and Lower Serratus Anterior During Scapular Protraction Exercises With Unstable Surface in Sitting Position (앉은 자세에서 불안정한 접촉면을 이용한 어깨뼈 내밈 운동이 위 그리고 아래 앞톱니근 활성화에 미치는 영향)

  • Background: The scapulo-thoracic musculatures including serratus anterior (SA), upper trapezius and lower trapezius can provide shoulder stability and functional shoulder movement. Objects: The muscle activities of upper and lower SA were compared during three different scapular protraction exercises in healthy individuals in sitting position. Methods: Twenty-five healthy subjects were participated. Electromyography device was used to measure muscle activity of upper and lower SA and trapezius muscles. Each subject was asked to perform three different scapular protraction exercises (scapular protraction [SP], SP with self-resistance [SPSR], SPSR with hand-exerciser [SPSRH]) in random order. One-way repeated measures analysis of the variance and a Bonferroni post hoc test were used. Results: The muscle activity of lower SA muscle was significantly different among three conditions (SP vs. SPSR vs. SPSRH) (p < 0.01). The lower SA muscle activity was significantly greater during SPSRH compared to SP and SPSR, which required joint stability more than SP and SPSR (p < 0.01). Conclusion: SPSRH exercise can be recommended to facilitate the muscle activity of lower SA. In addition, the intramuscular variation in the upper and lower SA during scapular protraction exercise is required to consider the effective rehabilitation.

The Effect of Pelvic Tilt on Muscle Activity of Cervical Erector Spinae & Upper Trapezius While Using a Smartphone (골반경사에 따른 스마트폰 사용이 목세움근과 위등세모근의 근활성도에 미치는 영향)

  • The purpose of this study was to investigate muscle activity of cervical erector spinae & upper trapezius while using a smartphone according to anterior pelvic tilt & posterior pelvic tilt in sitting position. & we also want to obtain basic data necessary for development of IoT devices. Fifteen healthy men & women aged 20-30 were enolled, After anterior pelvic tilt & posterior pelvic tilt in sitting position were adjusted, they used a smartphone for 5 minutes & EMG signal was measured simultaneously. We used median 3 minutes of measured EMG signal. The results of this study were as follows: In the sitting position, the use of smartphone in anterior pelvic tilt had significantly lower %RVC values in both cervical erector spinae (p <.001), left upper trapezius (p <.001) & right upper trapezius (p <.002) in comparison with posterior pelvic tilt. This means that anterior pelvic tilt make vertebrae maintain normal curvature & prevent forward head posture occurred while using a smartphone. It reduces loads around neck & shoulders, so that %RVC value is significantly lowered. In the future, new posture corrected IoT devices with an aspect of pelvic tilt should be developed.

Effects of Verbal Cue for Scapular Depression During Scapular Posterior Tilt Exercise on Scapular Muscle Activities and Clavicular Tilt Angle in Subjects With Rounded Shoulder Posture and Upper Trapezius Myofascial Pain

  • Background: Scapular posterior tilt (SPT) is important in the prevention of abnormal scapular movement and pain during elevation of the arm. However, previous studies have overlooked increased upper trapezius (UT) muscle activity interrupting the normal force couple of scapular motion and compensation of levator scapulae (LS) muscle activated simultaneously with UT during SPT exercise. Objects: The purpose of this study was to compare the effects of modified SPT with depression exercise versus SPT exercise on serratus anterior (SA), lower trapezius (LT), UT, and LS muscle activities and the clavicular tilt angle, in subjects with rounded shoulder posture (RSP) and myofascial pain in the UT muscle region. Methods: Eighteen subjects with RSP were recruited and randomly allocated to 2 groups; 9 in the SPT group and 9 in the SPT with depression group. All subjects met the specific RSP criteria and had myofascial pain of UT region. Depending on the allocated group, subjects performed the assigned SPT exercise and EMG data were recorded during the each exercise. Clavicular tilt angle was defined as the angle between the line joining the medial and lateral end of the clavicle and a horizontal line. Results: The SA muscle activity was significantly greater in SPT with depression than with SPT exercise (p<.05). The UT, LS muscle activity and the clavicular tilt angle was significantly lower in SPT with depression than with SPT exercise (p<.05). Conclusion: These findings were insightful because the potential risk of pain from overactivation of the UT and LS was considered, in contrast with SPT exercise. SPT with depression exercise can be implemented as an effective method to facilitate scapular muscle activity for stability and to prevent myofascial pain in the neck and shoulder.
