견(犬)의 주돌기 융합부전증(融合不全症)의 수술치료예(手術治療例) (Ununited Anconeal Process (UAP) in Dog: A Case Report)

  • Ununited Anconeal Process (UAP) is defined as a disease of young dogs due to developmental abnormality in which there is a seperation of the anconeal process from the diaphysis of the ulna, leading to a front leg lameness and secondary osteoarthritis. A six -year-old German Shepherd dog was presented because of intermittent right front leg lameness. Diagnosis was made of ununited anconeal process with moderate osteoarthritis by radiographic examination. Surgical removal of ununited anconeal process and curettage of osteoarthritic growth within the elbow joint relieved significant clinical signs despite of size and age of the dog.

  • A 6 month-old male Labrador retriever was presented for intermittent lameness on the left forelimb after exercise. The dog was suffering progressive lameness that had started two months before. On mediolateral radiographic view of the left elbow, proximal margin of the anconeal process was irregular. A lucent, indistinct line separating the anconeal process from the ulna was observed, when the elbow joint was flexed. The lateral approach to the elbow joint was used and the ununited anconeal process was removed. The limb was bandaged after surgery for 7 days to support soft tissue and exercises were restricted. The dog showed normal limb function 3 weeks after surgery. On a telephone conversation with the owner 18 months after surgery, the dog was reported to maintain normal function of the limb.

  • 비접합성 골절의 치료를 위한 전기적인 방법으로는 관혈적이 방법, 부분적으로 관혈적인 방법, 비관혈적인 방법 들이 있는데, 비관혈적인 방식은 다른 방법들과는 달리 수술 등 외과적 처기 등이 필요하지 않을뿐더러 환자에게도 가장 안전한 방식이다. 본 논문에서는 유도 결합 현상을 이용한 반관혈적인 전기 자기 자극기를 설계, 제작하였으며, 임상 실험 겨과 좋은 결과를 보이고 있다.

Delayed union of a pediatric lunate fracture in the United Kingdom: a case report and a review of current concepts of non-scaphoid pediatric carpal fractures

  • Pediatric carpal fractures are rare and often difficult to detect. This paper reviews the current literature on pediatric non-scaphoid carpal fractures, with a case report of a lunate fracture associated with a distal radius and ulnar styloid fracture, managed nonoperatively in a 12-year-old boy. There is lack of consensus regarding the management of these fractures due to the low number of reported cases. A frequent lack of long-term follow-up limits our understanding of the outcomes, but good outcomes have been reported for both nonoperative and operative management. This case report brings attention to the current time period for the definition of delayed union in pediatric carpal fractures, and emphasizes the need for prolonged follow-up for the detection of delayed complications leading to functional impairment.

  • Atopic dermatitis (AD), a chronic inflammatory skin disease, is characterized by itchy and age-dependent lesions. Previous studies have shown that pulsed electromagnetic field (PEMF) significantly improved chronic ulcers and ununited fractures, providing an evidence for the application of PEMF in resolving inflammation caused by AD. This study investigated the anti-inflammatory effect of PEMF on DNCB-induced AD in animal models. Five male hairless mice (6 weeks old) per group were assigned to a normal group, a sham group, and two PEMF groups (15Hz, 75Hz). Mice were treated with 2,4-Dinitrochlorobenzene (DNCB) to induce uniform AD among all groups excluding a normal group. To examine the inflammatory progress and the improvement of AD after the PEMF stimulation, images are taken with various cameras for non-invasive evaluation and the results are expressed using principal component analysis (PCA) for visualization. The results of this study demonstrated that PEMF effectively improved skin lesions without the use of drugs.

Aseptic Humeral Nonunion: What Went Wrong? What to Do? A Retrospective Analysis of 20 Cases

  • Purpose: Due to recent advances in internal fixation techniques, instrumentation and orthopedic implants there is an increasing number of humeral shaft fracture treated operatively. As a consequence, an increased number nonunion after operative fixation are being referred to our center. The aim of this study is to report the common error during osteosynthesis that may have led to nonunion and present a systematic analytical approach for the management of aseptic humeral shaft nonunion. Methods: In between January 2007 to December 2013, 20 patients with humeral shaft nonunion after operative procedure were treated according to our treatment algorithm. We could analysis x-rays of 12 patients from initial treatment to nonunion. In a subgroup of 12 patients the initial operative procedure were analyzed to determine the error that may have caused nonunion. The following questions were used to examine the cases: 1) Was the fracture biology preserved during the procedure? 2) Does the implant construct have enough stability to allow fracture healing? Results: In 19 out of 20 patients have showed radiographic evidence of union on follow up. One patient has to undergo reoperation because of the technical error with bone graft placement but eventually healed. There were 2 cases wherein the treatment algorithm was not followed. All patients had problems with mechanical stability, and in 13 patients had biologic problems. In the analysis of the initial operative fixation, only one of 12 patients had biologic problems. Conclusion: In our analysis, the common preventable error made during operative fixation of humeral shaft fracture is failure to provide adequate stability for bony union to occur. And with these cases we have demonstrated a systematic analytic management approach that may be used to prevent surgeons from reproducing the same fault and reduce the need for bone grafting.
