This study investigated the prevalence of unmet needs for health care among Korean adults and related factors. Using data from the 2012 Korea Health Panel, 1,896 adults aged 65 and over and two age groups(Young-Old(66-74) and Old-Old($65{\leq}$)) were analyzed to identify these factors. Logistic regression analysis was used to examine the main factors associated with unmet medical needs. According to the results of this study, the factors influencing unmet medical needs with regard to economic factors were Subjective Health Status(young-old), Disability(young-old), Activity Limitation(young-old), Education Level(old) and Economic Activity(old). The factors influencing unmet medical needs were factors of attitude, information, and disease. factors were Disability(young-old), Activity Limitation(young-old), Education Level(old) and Economic Activity(old). Therefore, further research that investigates unmet needs depending on age group in the elderly would suggest helpful policy implications.
Purpose: This study was conducted to identify the factors affecting on unmet healthcare needs of married immigrant women, especially who are working in South Korea. Methods: It is designed as a cross-sectional descriptive study. We analyzed data from 8,142 working married immigrant women to the 'National Survey of Multicultural Families 2015.' Based on Andersen's health behavior model, logistic regression was conducted to determine the predictors of unmet healthcare need. Results: The prevalence of unmet healthcare needs among the subjects was 11.6%. In multivariate analysis, significant predictors of unmet needs included existence of preschooler, country of origin, period of residence in predisposing factors, monthly household income, helpful social relationship, social discrimination, Korean proficiency, working hour per week in enabling factors, and self-rated health, experience of grief or desperation in need factors. Conclusion: The association between labor-related factors and unmet healthcare needs of marriage immigrant women currently working was found from nationally representative sample. Support policies for immigrant women working more than legally defined hours and having preschooler should be supplemented to reduce unmet healthcare needs. In addition, eradicating discrimination in workplace, enlarging social relationship, and developing culturally competent nursing services tailored to health problems caused by labor are needed.
Purpose: The purpose of this study was to identify physiological functional status and unmet care needs among patients with chronic pulmonary disease, colorectal cancer, and strokes after discharge. Methods: A crosssectional study was conducted with 224 patients diagnosed with aforementioned diseases from January to July in 2014 in two different tertiary hospitals in Seoul and its suburban area. Physiological functional status and unmet care needs were collected using Karnofsky Performance Status (KPS) Scale and Problems After Discharge Questionnaire-English version(PADQ-E) respectively. Data were analyzed using SPSS/WIN 21.0 program. Results: Patients with chronic pulmonary disease and colorectal cancer showed a low level of physiological functional status (mean: 77.20 and 77.60 out of 100 respectively) and a high level of unmet care needs (mean 2.23 and 2.63 out of 4 respectively). Stroke patients showed a high level of unmet care needs in the category of 'counseling', 'physical complaints', and 'instructions'. Physiological functional status was significantly associated with unmet care needs in all three patient groups and it showed a significant effect on unmet care needs in patients with stroke. Conclusion: The results showed that patients after discharge were still having insufficient functional status and various unmet care needs. The results of this study suggest a development of nursing care service for patients with chronic diseases after discharge.
Background: The purpose of this study is to provide the data for discussions related to oral health promotion policies for single-person households by analyzing the status of unmet dental needs and related factors in single-person households in Korea, based on the Anderson model. Methods: The data, obtained from 544 single-person households of those over 20 years old who were targeted for the 6th Korea National Health and Nutrition Examination Survey, were analyzed through a complex sample frequency analysis, complex sample cross analysis (Rao-Scott chi-square test), and complex sample binary logistic regression analysis on a complex sampling design. Results: The most frequently given reason for an unmet dental need among single-person households was economic (52.4%). Factors related to the unmet dental needs of single-person households are smoking, which is a predisposing factor; personal income levels, which are an enabling factor; chewing discomfort; and limited daily activities, which are need factors. Smokers, the high-income group, the chewing-discomfort group, and the limited activity group showed high unmet dental care experience. Smokers had a 2.75 times higher rate of unmet dental care than non-smokers, and the high-income group had a 5.29 times higher rate of unmet dental needs than the median group. The rate of unmet dental needs for the chewing discomfort group was 3.27 times higher than the non-chewing discomfort group, and the limited activity group had a 7.87 times higher rate of unmet dental needs than the non-limited activity group. Conclusion: It is necessary to map out policies designed to help maintain and promote met dental needs considered to be internally heterogeneous to single-person householders, based on the Anderson model.
Unmet healthcare is an important indicator for measuring accessibility of healthcare services. We analyzed the following four data from a nationally representative sample of South Korean population: Korea Health and Nutrition Examination Survey (KNHANES, 2007-2021), Community Health Survey (CHS, 2008-2021), Korea Health Panel Survey (KHP, 2011-2019), and Korean Welfare Panel Study (KOWEPS, 2006-2021). The proportion of individuals reporting unmet healthcare needs were 6.0% (KNHANES), 5.1% (CHS), and 13.1% (KHP). Annual percentage change (APC) which identifies trend for the follow-up period was -9.4%, -9.4%, and -5.3%, respectively. The proportion of individuals reporting unmet healthcare needs due to cost were 1.2% (KNAHANES), 0.5% (CHS), 2.7% (KHP), and 0.4% (KOWEPS). The APC was -10.4%, -16.1%, -11.5%, and -19.1%, respectively. Compared to the previous year, the rate of unmet healthcare needs decreased slightly, but the rate of unmet health care needs due to cost tended to increase. Overall, higher rates of unmet healthcare needs were reported in the low-income and the elderly population. Although it was confirmed through the APC that the rate of unmet healthcare experience has decreased over the past decade, it can be seen that there is still a disparity by income level and age. These results suggest the need for an appropriate health benefit coverage policy for the low-income and the elderly.
Purpose: The purposes of this study were to develop and test a prediction model for caregiving experiences including caregiving satisfaction and burden in dementia family caregivers. Methods: The stress process model and a two factor model were used as the conceptual frameworks. Secondary data analysis was done with 320 family caregivers who were selected from the Seoul Dementia Management Survey (2014) data set. In the hypothesis model, the exogenous variable was patient symptomatology which included cognitive impairment, behavioral problems, dependency in activity of daily living and in instrumental activity of daily living. Endogenous variables were caregiver's perception of dementia patient's unmet needs, caregiving satisfaction and caregiving burden. Data were analysed using SPSS/WINdows and AMOS program. Results: Caregiving burden was explained by patient symptomatology and caregiving satisfaction indicating significant direct effects and significant indirect effect from unmet needs. The proposed model explained 37.8% of the variance. Caregiving satisfaction was explained by patient symptomatology and unmet needs. Mediating effect of unmet needs was significant in the relationship between patient symptomatology and caregiving satisfaction. Conclusion: Results indicate that interventions focusing on relieving caregiving burden and enhancing caregiver satisfaction should be provided to caregivers with high levels of dementia patients' unmet needs and low level of caregiving satisfaction.
Unmet healthcare needs lead to increased disease severity, increased likelihood of complications, and worse disease prognosis. To examine the latest status of unmet healthcare needs in South Korea, the four different data configured with nationally representative sample of South Korean population were used: the Korea Health and Nutrition Examination Survey (KNAHANES, 2007-2018), the Community Health Survey (CHS, 2008-2018), the Korea Health Panel Survey (KHP, 2011-2016), and the Korean Welfare Panel Study (KOWEPS, 2006-2018). The proportion of individuals reporting unmet healthcare needs were 7.8% (KNHANES, 2018), 8.8% (CHS), and 10.8% (KHP, 2016). Annual percentage change which characterizes trend for the follow-up period was -9.1%, -3.2%, and -6.8%, respectively. The proportion of individuals reporting unmet healthcare needs due to cost were 1.2% (KNAHANES, 2018), 1.2% (CHS, 2018), 2.5% (KHP, 2016), and 0.5% (KOWEPS, 2018). Annual percentage change which characterizes trend for the follow-up period was -10.3%, -12.0%, -11.3%, and -18.8, respectively. The low-income population and the elderly population were vulnerable groups reporting the highest rate of unmet health care needs. The rate of unmet healthcare needs has been declining since the past decade, still, the disparity between different income groups and age groups suggests that there are many challenges to address.
Objectives: This study aimed to determine and facilitate provisions for the unmet dental needs of Koreans, stratified by time, influencing factors, and research trends, through a systematic review of related published studies since 2006. Methods: this review focused on previous studies published between January 2006 and November 2019 that analyzed influencing the factors underlying the unmet dental needs of koreans. We followed the guidelines set for each phase of research and selected the final 32 studies that met the selection criteria for the analysis. Results: The number of studies has rapidly increased since 2015 (22 studies, 68.7%). the were 68.9% in 2006, 25.9% in 2009, 41.3% in 2010- 2012, and 33.3% in 2013-2015 for adults and 27.9% in 2010, 24.6% in 2015, and 16.1% in 2017 for the rates of older adults. the rates of unmet dental needs related to economic factors, were 38.6% in 2006, 41.4% in 2007-2009, and 35.9% in 2013-2015 for adults and 50.5% in 2010 and 41.2% in 2015 for the older adults. There were common influencing factors for unmet dental needs. the rate of unmet dental needs was increased by with female gender, younger age, single marital status, low family income, low educational level, worsened subjective health condition, and the presence of chronic diseases. Conclusions: Standardized studies with more accurate definitions and assessment tools are required. however, our study emphasizes the need for a policy intervention that accounts for the characteristics of subjects to reduce unmet dental needs.
Objectives: This study investigated the relationship between demographic and socioeconomic characteristics of the Korean elderly and their unmet dental care needs, by using the 2015 data from the $6^{th}$ Korea National Health and Nutrition Examination Survey (NHANES). Methods: In total, 1,372 elderly persons aged 65 and over, who responded to the $6^{th}$ NHANES, were included in the final analysis. Logistic regression analysis was performed in order to identify any relationship between demographic and socioeconomic characteristics and unmet dental care needs. Results: The rate of unmet dental care needs was shown to be less by 0.799 times in the elderly who reside in dong than those who live in eup and myeon (OR: 0.799, 95% CI: 0.679-0.959). Unmet dental care needs were higher in participants with 'low' and 'below average' than 'above average' income, by 1.645 times (OR: 1.645, 95% CI: 1.087-2.366) and 1.172 times (OR: 1.172, 95% CI: 1.108-1.880), respectively. Elderly individuals living alone had a higher rate of unmet dental care needs than those living with their family by 1.157 times (OR: 1.157, 95% CI: 1.084-1.498). Conclusions: Demographic and socioeconomic factors influenced unmet dental care needs, causing inequality. Proper policy support to the vulnerable should be considered in order to enhance the elderly's access to dental care.
Objectives: The purpose of this study was to investigate the influence of demographic characteristics and oral health status on unmet dental needs among preschool children and to provide a basis for improvement of the dental care equality and accessibility using data from the $6^{th}$ National Health and Nutrition Survey. Methods: This study was performed using data collected from the $6^{th}$ National Health and Nutrition Survey. The subjects were 1,472 out of 22,940 people, who participated in the survey and under went oral examination. IBM SPSS Statistics (Version 20.0) was used for statistical analyses based on the complex sampling design. Frequency analysis was performed to determine the distribution of unmet dental needs according to the characteristics of the subjects. The Rao-Scott ${\chi}^2$ test was performed to examine the relationship of unmet dental needs with general characteristics and health- and oral health-related variables. Relevant factors were determined using binary logistic regression analysis. Results: The factors that had statistically significant relations with unmet dental needs included age, medical insurance, household income, limited physical activity, history of dental caries in deciduous teeth, and subjective health status. Logistic regression analysis of complex samples was conducted to determine factors related to unmet dental needs. The results of analysis showed that limited physical activity and history of dental caries in deciduous teeth were related to unmet dental needs. Conclusions: The results show the factors affecting, and the reasons for, the unmet dental needs of preschool children. Future studies are needed to develop national projects and oral health education reforms to address inequalities in preschool children's dental care.
