Unified Multiple System Atrophy Rating Scale(UMSARS)로 평가한 다계통 위축증 환자 1례에 대한 증례 보고 (A Case Report of the Patient with Multiple System Atrophy Evaluated by Unified Multiple System Atrophy Rating Scale (UMSARS))

  • Multiple system atrophy (MSA) is a sporadic progressive neurodegenerative disorder characterized clinically by various combinations of parkinsonian, autonomic, cerebellar, or pyramidal signs and pathologically by cell loss, gliosis, and ${\alpha}$-synuclein-positive glial cytoplasmic inclusions in several brain and spinal cord structures. This is a clinical report about a 69-year-old female who had MSA treated by oriental medical treatment and evaluated by Unified Multiple System Atrophy Rating Scale (UMSARS). The patient was treated with herb medicine Chungsimyeonj-aeumgami(淸心蓮子飮加味), acupuncture, moxibustion and cupping. After treatment, the patient's symptoms improved meaningfully and the score decreased in UMSARS Part I, II. This suggests that oriental medical treatment could be effective to improve MSA patients' symptoms. It is necessary to have more observations and many cases of patients with MSA.

  • Objectives The aim of this study was to examine significant improvement of dizziness, gait disturbance, insomnia and xerostomia after treatment with Jowisengcheong-tang in a Taeeumin patient with olivopontocerebellar atrophy (OPCA). Methods The patient's subjective and objective symptoms were observed daily throughout the hospitalization period using global assessment (G/A), and the Unified Multiple System Atrophy Rating Scale (UMSARS) was used to assess the overall function of patient. Results The symptoms of dizziness and xerostomia decreased from G/A 100 to G/A 10~30 for five days, and the UMSARS score decreased in Part I,II. Conclusions This case showed that Sasang constitutional medicine treatment can be effective treatment method for OPCA. We consider that consistent treatment can contribute to improve the patient's quality of life.

  • Objectives This study aimed to report significant improvement of dizziness, gait disturbance and dysarthria after treatment with Sasang constitutional medicine treatment on a Taeeumin patient with olivopontocerebellar atrophy. Methods A 53-year-old female diagnosed olivopontocerebellar atrophy about one year ago suffered from dizziness, gait disturbance and dysarthria. The patient was identified as Taeeumin Joyeol pattern and treated with Cheongsimyeonja-tang and Taegeuk acupuncture. The patient's main symptoms were observed using Global Assessment Scale (GAS) during the treatment period. In addition, the Unified Multiple System Atrophy Rating Scale (UMSARS) was used to assess the overall function of patient. Results We observed a significant improvement in symptoms of dizziness, gait disturbance and dysarthria using GAS, and UMSARS score decreased in Part I and II. Conclusions This case showed that Sasang constitutional medicine treatment can be effective treatment for Taeeumin patient with olivopontocerebellar atrophy. We consider that consistent treatment can contribute to improve the patient's quality of life.

보행장애를 주소로 하는 소뇌형 다계통 위축증 환자의 한방 치료 1례 (A Case Report of Multiple System Atrophy in a Cerebellar Ataxia Patient Suffering from Gait Disturbance Treated with Korean Medicine)

  • Objective: To describe the effect of Korean medicine on gait disturbance in a patient with multiple system atrophy and cerebellar ataxia. Methods: We observed a 75-year-old female patient who had multiple system atrophy and cerebellar ataxia. The patient was treated with Korean medicine, including acupuncture, moxibustion, and herbal medicine (Yukmijihwang-tang-gamibang and Boyanghwano-tang-gamibang) for 38 days. We evaluated the patient with the Korean version of the Berg Balance Scale (K-BBS), the Unified Multiple System Atrophy Rating Scale (UMSARS), and the Numeric Rating Scale-11 (NRS-11). Results: After treatment, the patient’s symptoms were improved. The K-BBS score increased from 9 to 19, while the UMSARS score decreased from 16 to 12 in Part I, from 15 to 7 in part Ⅱ, and from 4 to 3 in part Ⅳ. The NRS-11 score of gait disturbance decreased from 10 to 5. Conclusions: Korean medicine may be an effective treatment for gait disturbance in multiple system atrophy with cerebellar ataxia.

소음인 태음증으로 진단한 소뇌성 운동실조 환자 1례 (A Case Study of Soeumin Greater Yin Symptomatology Patient Diagnosed as Cerebellar Ataxia)

  • Objectives The aim of this study was to examine significant improvement of gait ataxia, vertigo and tremor after treatment with Sipyimigwanjung-tang, Hyangsayangwi-tang in a Soeumin Greater Yin Symptomatology patient Diagnosed as Cerebellar ataxia. Methods The patient was diagnosed with Soeumin Greater Yin Symptomatology and treated with Soeumin's constitutional medications and acupuncture. The primary outcome measures for this study were the Unified Multiple System Atrophy Rating Scale(UMSARS) and Cerebellar function test to assess the overall function of patient. Secondary outcome assessment included Global Assessment Scale(GAS), change of patient's sleep, stool and digestion. Results The symptoms of gait ataxia, vertigo and tremor decreased from GAS 100 to GAS 0~40 after treatment, and the UMSARS score decreased in Part I, II. Conclusions This case showed that Sasang Constitutional medicine treatment can be effective treatment method for cerebellar ataxia. We consider that consistent treatment can contribute to improve the patient's quality of life.

  • Objectives: This study evaluates the effect of Korean medicine in a patient with multiple system atrophy-C (MSA-C). Methods: A patient diagnosed with MSA-C was treated with herbal medicine (Boyangwhano-tang hap Yeokgan-san), acupuncture, and moxibustion for 8 weeks. Clinical improvements were evaluated by the unified MSA rating scale (UMSARS) and the scale for the assessment and rating of ataxia (SARA). Results: Improvements in the total scores of UMSARS Part I, II, and IV and SARA were observed after Korean medicine treatments. However, there were no improvements to the urinary and autonomic dysfunctions. Conclusions: Korean medicine treatment may be an effective treatment for a degenerative, progressive disease like MSA-C.

  • The purpose of this case report is to describe the effectiveness of Korean medicine in the treatment of gait disturbance in advanced stage multiple system atrophy-cerebellar type (MSA-C). This inpatient was treated with herbal medicine, acupuncture, pharmaco-acupuncture, moxibustion, cupping, and chuna therapy. The gait disturbance was assessed by Unified Multiple System Atrophy Rating Scale(UMSARS), Tinnetti test, and 16m walking spatial features. After treatment, UMSARS Part I score was enhanced, decreasing from 29 to 24, Part II score from 34 to 18. Tinetti test score increased from 7 to 20. Gait stride length increased and width decreased. This study suggests that Korean medical treatment could be an effective treatment for delaying the progress of gait disturbance in advanced-stage MSA-C patients.

  • A 52-year-old female diagnosed with Atypical Parkinsonian Disorders (APD) about one year ago suffered from dizziness and gait disturbance. The patient was identified as Taeeumin esophagus-cold lung-dry symptomatology pattern and was treated with Jowisengcheong-tang. The patient's symptoms of dizziness and night sweats were assessed using Global Assessment Scale (GAS). And, the Unified Multiple System Atrophy Rating Scale (UMSARS) and the Unified Parkinson's Disease Rating Scale (MDS-UPDRS) were used to assess the overall function of the patient. The patient's symptoms of dizziness and night sweats were significantly improved, and UMSARS and MDS-UPDRS scores were decreased. This case showed that Sasang constitutional medicine treatment can improve APD symptoms and improve the quality of life of the patient.

  • This case report describes the effects of Korean Medicine for the gait disturbance of Multiple System Atrophy (MSA). Korean Medicine that included chuna manual therapy, pharmaco-acupuncture, herbal medicine, acupuncture, electro-acupuncture, and moxibustion was administered for 23 days. During the hospitalization period, evaluations were performed using the Unified Multiple System Atrophy Rating Scale (UMSARS), Berg Balance Scale (BBS), and 20 m Round Timed Walking Test on day 1, day 15, and day 23. The UMSARS Part I score decreased from 11 to 5, Part II score from 11 to 9, and total score from 22 to 14, and the BBS score increased from 36 to 48. The 20 m Round Timed Walking Test score also improved. The width of the gait decreased from 65 to 40 cm, and the number of staggering incidents decreased from 6 times to 2 times. The findings of this case study suggest that Korean Medicine, including Chuna manual therapy, has potential benefits in MSA patients with gait disturbance.

  • In this study, a 54-year-old female woman diagnosed as Multiple System Atrophy (MSA) presented diplopia with other symptoms - gait disturbance, ataxia, sleep apnea, dysuria. She had been cared with Korean medical treatment - Herbal medicine, acupuncture, bee venom acupuncture, electroacupuncture, cupping, moxa. Notable improvement was observed in diplopia expressing time and Unified Multiple System Atrophy Rating Scale (UMSARS). For MSA patient with no typical treatment indispensable, Korean medical treatment may be effective.
