오훼돌기에 매복된 과잉치: 증례보고 (Impacted supernumerary tooth in coronoid process: a case report)

  • Impaction of tooth is a situation in which an unerupted tooth is wedged against another tooth or teeth or otherwise located so that it cannot erupt normally. The supernumerary tooth is also called as hyperdontia and defined as the condition of having additional tooth to the regular number of teeth. The most common supernumerary tooth is a mesiodens, which is a mal-formed, peg-like tooth that occurs between the maxillary incisors. The supernumerary tooth is commonly impacted but they are frequently impacted on maxilla. Ectopic impaction of supernumerary tooth on mandibular condyle, coronoid process, ascending ramus, and pterygomandibular space is very rare condition. In this case, we report a case of impacted supernumerary tooth on mandibular sigmoid notch without definite pathologic change.

  • Dentigerous cyst is an odontogenic cyst which occurs in unerupted tooth crown. After the crown formation, enamel epithelium remnants surrounded continuously proliferates and it forms effusionfluid cyst and expands due to increased internal osmotic pressure. Treatments of cysts are mainly enucleation, marsupialization and de-compression. When deciding the way of treatment, the age of a patient, the anatomical circumstances, the region of lesion and the size of cyst should be considered. Marsupialization is that some parts of internal cystic wall would be converted into oral mucosa if the cyst is large size and is concerned about neighboring anatomic structure. It can be accompanied by enucleation later and eruption of related tooth can be possible. If there is a limitation of spontaneous tooth eruption, eruption of tooth can be induced by orthodontic apparatus. There were 3 patients had dentigerous cyst and underwent marsupialization, their impacted teeth had preserved and had induced eruption, all showing satisfactory results.

  • Adenomatoid odontogenic tumor (AOT) is a rare, benign odontogenic tumor that predominantly appears in the second decade of life in female patients. Most AOTs occur in the anterior part of the maxilla and are usually associated with impacted anterior teeth. There are three types of AOT, follicular, extrafollicular, and peripheral, which are classified based on the location of the lesion and its association with the impacted tooth. We report a rare case of AOT associated with an impacted right mandibular lateral incisor in an 11-year-old female patient.

  • Unerupted or erupted supernumerary tooth may cause crowding, diastema, cyst formation, resorption, and displacement or rotation of adjacent teeth. However, there are few studies about prognosis about the pathologic condition and expectation of spontaneous eruption. The object of this study is to evaluate factors predicting the spontaneous eruption of supernumerary tooth. 431 patients (346 males and 85 females, aged from 5 to 29 years) who visited our institution from 2002-2006 and were shown to have 471 mesiodentes was reviewed. Supernumerary teeth were classified inverted and not inverted state. In case of not inverted supernumerary tooth, eruption rate ac cording to length of supernumenary tooth, width of the tooth, angle between the tooth and incisor tooth, location (inside the incisor or not) and shape (conical or tubercle) were investigated. The regression model showed that length, width and angle were all important determinants of influencing the eruption of supernumerary tooth (p < 0.001, Pearson R: 0.619). There is no relation between shape and eruption of supernumerary tooth (p > 0.05). Location of mesiodens has an effect on eruption of supernumerary tooth (p < 0.01).

  • Mucopolysaccharidosis (MPS) is an inherited metabolic disorder caused by a deficiency in enzymes that participate in the degradation of glycosaminoglycans (GAGs) such as heparin sulfate and dermatan sulfate. Left untreated, patients show progressive mental and physical deterioration due to deposition of GAGs in organs. Death often occurs due to cardiac or respiratory failure before patients reach their early twenties. MPS has several oral and dental manifestations. An enlarged head, short neck, and open mouth associated with a large tongue are major characteristics of MPS patients. Dental complications can be severe, including unerupted dentition, dentigerous cyst-like follicles, malocclusions, condylar defects, and gingival hyperplasia. A 21-year-old female patient with MPS was described in this article, with special emphasis on oral manifestations and dental treatment.

예후 불량한 치아를 대체하는 제3대구치를 이용한 자가치아이식술 (Autogenous Transplantation of Third Molar to Replace Hopeless Tooth)

  • 임플란트 술식은 그 동안 많은 발전을 하였고, 자연치를 대체할 훌륭한 치료법임에는 부인할 수 없지만 치주인대가 존재하지 않는 구조적 한계점, 발치 직후 시작되는 bundle bone의 흡수는 자연치와 같은 적절한 emergence profile을 만들어내기 어려울 수 있고 이는 심미적인 결과로 이어질 수 있다. 만약 해당 환자의 구강 내에 건전한 제3대구치가 있고, 결손부위의 염증상태가 심각하지 않으면 건전한 제3대구치를 결손된 부위로 이식하여 치아를 재건할 수 있다. 성공적인 자가치아이이식술은 저작기능을 회복하고, 자가치아를 이용하기에 생체 친화적이며, 치조골의 흡수를 예방하는 훌륭한 치료이기 때문에 가능하기만 하다면 첫번째 치료로 충분히 고려될 수 있을 것이다.

Cystic lesion between a deciduous tooth and the succeeding permanent tooth: a retrospective analysis of 87 cases

  • Objectives: The purpose of this study is to investigate the characteristics of dentigerous and radicular cysts that occur between deciduous and succeeding permanent teeth and to propose considerations for differential diagnosis of cysts at the treatment planning stage in the outpatient clinic. Materials and Methods: A total of 87 patients with a cystic lesion located between a deciduous tooth and the succeeding permanent tooth participated in the study. Twelve variables were analyzed to diagnose such a cyst. For data analysis, Fisher's exact test was used to determine the statistical significance of the variables. Results: Of the total 87 patients who participated in this study, 69 were diagnosed with dentigerous cysts and 18 were diagnosed with radicular cysts. Seven of the 12 differential factors analyzed in this study were statistically significant: age, location, symptoms, dental caries, endodontic treatment, delayed eruption, and size. Conclusion: Several criteria can be considered for diagnosis of dentigerous cysts or radicular cysts. Age, location, presence of symptoms and dental caries, previous endodontic treatment, cystic size, and delayed eruption of impacted permanent teeth are reliable factors that should be considered when diagnosing dentigerous and radicular cysts.

  • The calcifying odontogenic cyst(COC) is considered to occupy a position between a cyst and an odontogenic tumor-having charateristics of both. Gorlin and col. described the COC for first time as an own pathological entity in 1962. Clinically, the COC represents 1% of the odontogenic lesion. It is possible to be found from the first decade to the eight decade but is more frequent during the second decade. It affects in same proportion the maxilla and jaw, being the most in tooth-bearing area of the jaw. This case of COC associated with an unerupted tooth which appeared in the right mandible of 22-year-old woman, was reported. This case report is to present a review of the literature relates to this case of COC and its treatment, discuss clinical, radiographic, histological and therapeutic aspects.

상악 제1대구치의 이소맹출과 골격성 부정교합의 연관성 (Association between Ectopic Eruption of the Maxillary First Permanent Molar and Skeletal Malocclusion)

  • 본 연구는 5 - 10세 아동에서 상악 제1대구치의 이소맹출과 골격적 부정교합 유형 사이의 연관성을 평가하고자 하였다. 상악 제1대구치의 이소맹출이 있는 93명의 아동을 실험군으로, 이소맹출이 없는 693명의 아동을 대조군으로 분류하였다. 제1대구치가 맹출 전이거나 제2대구치가 맹출을 완료한 경우는 연구대상에서 제외하였다. 측모 두부 방사선 사진 계측을 통해 골격적 부정교합의 유형을 조사, 분석하였다. 실험군의 경우, 골격성 3급 부정교합이 57.0% 이었고, 대조군에 비해 골격성 3급 부정교합의 비율이 유의하게 높았다. SNA, ANB, A to N-perpendicular는 실험군에서 작은 경향을 보였고, A-B plane angle과 APDI는 더 크게 나타났다. 반면, SNB와 mandibular plane angle은 두 군 사이에서 유의한 차이가 없었다. 본 연구를 통해 상악의 열성장이 상악 제1대구치의 이소맹출에 영향을 미칠 수 있음을 확인하였다.

환악 결찰술을 이용한 소아 하악 골절의 보존적 치료: 증례보고 (The conservative treatment of mandibular fracture in a child with circummandibular wiring: case report)

  • Maxillofacial injuries are less common in children than in adolescents and adults. This lower incidence is a result of the relatively small size of mandible, the resilient nature of the bones, and a relatively protected environment, which distinguish the treatment principles of pediatric mandibular fractures from those of the adult. The bone of child is malleable, so pediatric fractures tend to be less displaced and rarely comminuted. Moreover, high regeneration potential of the wound allows more conservative treatment modalities for the pediatric mandibular fracture. High risk of damaging unerupted tooth bud renders many clinicians to resort to more conservative treatment modality for the reduction of displaced segments. This case report describes two successful treatment cases using the circummandibular wiring which was applicated to the fracture on parasymphysis of mandible. Circummandibular wiring can protect the tooth buds, and there is no need for intermaxillary fixation so that it prevents the possible complications of intermaxillary fixation such as the temporomandibular joint ankylosis and the facial growth disturbances. The acrylic splint was removed after 3 weeks, which showed clinically good union across the fracture line without complications. They showed complete clinical and radiological bone healing with an optimum occlusion.
